The derangement of glucose metabolism is found frequently in all forms of hyperparathyroidism. Both in primary (PHPT) and secondary hyperparathyroidism (SHPT) PTH excess is thought tobe involved in deteriorating insulin sensitivity and secretion though their different clinical and pathophysiological conditions. In PHPT these abnormalities are related to a high frequency of Type 2 diabetes mellitus and also impaired glucose tolerance according to recent clinicalstudies, without differences between symptomatic and asymptomatic clinical presentation. In chronic renal failure (CRF), the disorders of glucose metabolism due to SHPT do not bear an increased risk for diabetes whereas they seem to be involved in the progression of atherosclerotic vascular damage which connotes CRF. Moreover, clinical and experimental studies have shown that vitamin D deficiency associated with glucosemetabolism abnormalities favors the development of the metabolic syndrome. The potential for metabolic and cardiovascular harm related to hyperparathyroidism, especiallyPHPT, is the most interesting issue for clinical endocrinologists. This short review of the clinical and pathophysiological data of literature on glucose homeostasis disorders in hyperparathyroidism focuses on its potential clinical and therapeutic impact, particularly in the management of PHPT.

...By use of the nationwide Cancer Registry and Causes-of-Death Registry, mortality was analyzed in 10,995 Swedish patients (> 20 years of age) subjected to extirpation of single parathyroid adenomaof PHPT during 1958-1997. The Swedish population standardized for age, sex, and calendar year was used as control. - 102,515 observed person-years in the patients.

Results verify an increased risk of dying after operation for PHPT (standard mortality ratio, 1.2; 95% CI, 1.19-1.27). The increased risk persisted far beyond 15 years postoperatively and occurred in both sexes and in all investigated age groups. Principal causes of excess mortality were cardiovascular diseases, diabetes mellitus, and urogenital diseases in all age groups. However, in patients operated on between 1985 and 1997 (n = 6386), overall mortality did not differfrom that of the normal population, although there was maintained excess death in stroke,diabetes mellitus, and urogenital diseases. These findings infer that modern paradigms of surgical treatment normalize the risk of dying fromPHPT. This improvement may be a late consequence of liberalized calcium screenings that were introduced about 30 years ago and indicate that operation at early disease stages may offer a survival advantage. An association between diabetes mellitus and PHPT is substantiated.

2183 Males; follow-up 18 years. RESULTS: 180 subjects experienced a myocardial infarction (MI). -The serum calcium levels were significantly elevated at the baseline (2.37 +/- 0.09 SD versus 2.35 +/- 0.09 mmol/l, p < 0.03) in the subjects who developed a MI when compared with the rest of the cohort. - Cox's proportional hazard analysis showed that only serum calcium (p < 0.01), BMI (p < 0.0003),diastolic blood pressure (p < 0.0009), and the atherogenic index (p < 0.002) were significantly independent risk factors for MI.

Population-based cross-sectional study. 1459 men and 1753 women. questionari. RESULTS: When stratified for age the rate of CHD was higher in the subjects with serum PTH > 6.8pmol/l than in those with normal or low serum PTH levels [relative risk 1.67, 95% confidence interval(CI) 1.26-2.23 in men and 1.78, 95% CI 1.22-2.57 in women]. The highest PTH quartile (> 3.50pmol/l in men and > 3.30 pmol/l in women) predicted CHD, with odds ratios of 1.70 (95% CI 1.08-2.70) for men and 1.73 (95% CI 1.04-2.88) for women, versus the lowest PTH quartile (< 1.90 pmol/lfor men and <1.80 pmol/l for women).

PATIENTS: 41 postmenopausal women with mild primary hyperparathyroidism and 43 eucalcemic, age-matchedcontrols. MEASUREMENTS: dual-energy x-ray absorptiometry. RESULTS: Women with primary hyperparathyroidism were heavier (75.5 kg compared with 66.3 kg; difference, 9.2 kg [95% CI, 3.7 to 14.7 kg]; P = 0.002), had a higher fat mass (33.3 kg compared with 26.1 kg; difference, 7.2 kg [CI, 3.0 to 11.4 kg]; P = 0.001), and had a more android pattern of fat distribution (android-to-gynoid fat ratio, 1.05 compared with 0.84; difference, 0.21 [CI, 0.1 to 0.32]; P = 0.0004) than the controls.

However, malepatients had significantly lower triglyceride levels at follow-up, 1.16+/-0.09 mmol/L compared to1.57+/-0.14 mmol/L before surgery (P < 0.05). - The serum level of urate decreased in both male and female patients after surgery.

Normalized dyslipidaemia after parathyroidectomy in mild primary hyperparathyroidism: population-based study over five years.Hagstrom E et al. - Sindrome Metabolica ‘has truly come of Age’ (Charles M. Alexander);

- prevalenza ‘non trascurabile’ dell’Iperparatirodismo 1°;

- mortalità CV accomuna MS e pHPT;

- Necessario un approccio ‘endocrino-metabolico’:

sia per l’attenzione nel trt l’pHPT considerando i fdRCV

sia attenzione nella gestione del pz. Diabetico/pz. con SM a rilevare la potenziale presenza di pHPT.
