ESTILL COUNTY DIABETES COALITION
“Working to improve the health of Estill County,
Kentucky”
DIABETES SELF-MANAGEMENT EDUCATION
Coalition offers at least one series of classes per
year
Previous classes have had inconsistent attendance
New approach – 4 different sites, during lunch
Began with Body Fitness group
Average of 17 people per session

TRAVELING DIABETES CLINIC
10 month diabetes clinic initiated at Marcum and Wallace
Memorial Hospital in 2012
Each month the Registered Dietitian traveled to employees of the
hospital and 3 clinics, checking blood sugar and educating on
diabetes
Tracked blood sugars and addressed any issues
A variety of topics on prevention and management were offered to
employees
A “go to the employee” approach worked well in this setting
TRAVELING CLINIC RESULTS…
Two employees were previously unaware they had diabetes
An employee improved their Hemoglobin A1c from 14% to
7.4%
An employee with consistent elevated blood sugars in the
afternoon discovered cause and corrected
An insulin dependent employee was found to consistently be
having blood sugars greater than 300 and was not taking her
insulin as prescribed
DIABETES ALERT DAY – LOCAL GROCERY STORE
Coalition members set up a booth in local Sav-A-
Lot grocery store to distribute information
Free literature and oral care kits
Information on local resources
Over 100 people collected information
Diabetes Alert Day
Education booth at local

BACK TO SCHOOL NIGHT
Coalition members had booths at Back to School
Night at local middle school
Oral care information, toothbrush and toothpaste,
healthy eating and snacking information, community
resource information
Adults could register for a FREE dental visit with
cleaning
Over 125 people visited the booth

COMMUNITY BABY SHOWER
Coalition had previously sponsored community health
fairs, and offered flu shots through the health department
Coalition looked for new focus for large community event
First baby shower held in the county in many years
Great response! 15 local agencies and coalitions
represented; 65 people in attendance
BABY SHOWER CONTINUED…
Open to parents who are expecting or have a child under 2
Provided dinner for families
Lots of great information and giveaways at booths
Great door prizes
2nd annual shower held last week – grown from first year
3 guest speakers were available to talk about car seat safety;
appropriate use of the Emergency Department; what to expect
with a newborn

WALKING TRAILS
Partnership with local Development Alliance and
Chamber of Commerce
1,2 and 3 mile city trails
County is currently partnering with surrounding
counties to form a rural trail system for pedestrians,
bikes and ATV’s
BODY FITNESS CLASS
Interfaith Wellness Ministry
2 class series per year / meet 2 times per week
Target elderly and those with chronic conditions
Improves flexibility, muscle tone, strength and
balance
Current health topics discussed at end of class
