• The consent form templates on the IRB Forms page have been updated. Please note:– The new ClinicalTrials.gov language has been added
to the templates and is required in your consent form if your study must register with the ClinicalTrials.gov website.
– A new Consent Form INSTRUCTIONS document is available that helps you choose the best template for your study and provides other tips for drafting a good consent form.
