We want to improve the
quality of care for individuals, carers and families,
empowering and supporting people to maintain independence
and to lead full lives as active participants in their community
“
”
People will be empowered to direct their care and support and to receive the care they need in their homes or local community.
GPs will be at the centre of organising and coordinating people’s care.
Our systems will enable and not hinder the provision of integrated care.

Today
Co-design with delivery partners and people using services
“Co-production means delivering public services in an equal and reciprocal relationship between professionals, people using services, their families and neighbours… both services and neighbourhoods become far more effective agents of change.”
Nesta & new economics foundation
Our commitment to working co-productively in North West London means:
1.Commitment to agreed ways of working – everyone is valued as equal partners, we will capitalise on lived experience as well as professional learning
2.Supporting development and learning
3.Fostering a supportive environment – developing collective resilience and acknowledging that mistakes will be made along the journey
4.Working towards shared goals – promoting local voice and enabling people to be involved in the delivery of their care and support
Collaborative working – lessons learned
• Collaboration requires time to establish and ongoing resource to sustain
• Invest in training and support for both service users and professionals to make engagement as effective as possible
• Co-design not only creates better solutions but also advocates for change
• Effective transformation should combine experiential insight from both people and professionals together with robust quantitative data analysis
Where do we go from here• Tie in with City deals and local growth – as well as better care and troubled
families
• Centre of Excellence on information sharing
• Widen network of people and places
• Website presence and campaigning
• Rolling programme of seminars and workshops
• Mobilise wider leadership
