Foreword

Asourfirsttwoyearsofmedicalschoolarenearinganend,wecanlookforwardtoputtingourknowledgeandskillstouseintheclinicalsetting.Thisexperienceisanexcitingtimewhichwillgiveusfurtherinsightintoourinterestsandfinalcareerdecisions.

Whilethecorerotationsarethesameforallofus,eachofthesitesisdistinctiveinitsownway.Wehavemanyoptionsavailabletous,rangingfromsmallcommunityhospitalstolargeacademicmedicalcenters.Assuch,wewilleachhaveauniquesetofclinicalexperiencesfromwhichtodrawonfortherestofourcareers.

Thisguidewascreatedtoprovideinformationabouteachaffiliatehospitalandthevariousrotationstheyoffer. Wehave tried tomake this ashelpful and complete aspossible. MidtolateFebruary,studentswillbegintoselecttherotationandlocationofclerkshipswiththeRegistrar'sOffice.SpecificdateswillbeemailedfromtheRegistrar’sOffice.

ApproximatelyApril23rdtheresultsofthelotterywillbeavailabletoallsecondyearstudents.

STUDENTSAREPROHIBITEDFROMHOLDINGMORETHANONESLOTPERCORESUBJECTandtheRegistrarwillautomaticallydeleteextraslotsheldtherebyjeopardizingastudent'sfirstchoice.

Sinceitisrealizedthatattimeschangesarenecessaryoncetheacademicyearbegins,ANYCHANGESmustbemadeunderthefollowingprocedure:

AllchangesmustbemadesixweeksormorebeforethescheduledrotationonlineviaBannerWeborusingthestandardpaperdrop/addform.

Theclerkshipbeingchanged,whetherlocationorrotation,MUSTbearrangedbyandwiththeCoordinatorofthatclerkship(inconsultationwiththeDirector).Iftwoormorestudentsortwoormoreclerkshipsareinvolvedinthechange,allCoordinatorsthatpertainMUSTbeinvolved.TheCoordinatorswilltheninformboththeSITEandtheREGISTRAR.

The Coordinatorwill inform the new site Coordinator and theoriginal siteCoordinatorof the change. ThisinformationwillbebyphoneandbyemailandwillbetotheclerkshipcoordinatorsandtochiefacademicofficeratanyaffiliatedMedicalCenter.TheDeansofAffiliationswillalsobecopied.Studentsmayenlist theadviceorhelpofanyfacultymemberorDean ineffectinga change;however thechangemayonlybemadebyaCLERKSHIPCOORDINATOR.AllchangesmustbereportedtotheRegistrar.

CLINICALCURRICULUMOVERVIEW TheclinicalcurriculumprogramstartsinJulyandconsistsof100weeks,ofwhich: 84weeksarerequiredclerkships, 4weeksofvacationaremandatory(2weeksduringeachWinterHoliday Period), 2weeks’vacationinJunebetweenthethirdandfourthyears, 2weeks’vacationimmediatelypriortograduation,and 8weeksofvacationduring4thYeartobescheduledtofittheneedsofthestudent.***For2018-19,thebasicunitsofthecalendararefive-weekblocks.THIRDYEARRequirements(Classof2020)–Priortobeginning4thYear,each3rdYearstudentmustcompletethefollowing:

Duringthefirst40weeks,thesequenceofcourseswillvary,butbytheendofthe40weeks(correspondingtotheendoftheeighthfive-weekblockofthethirdyearonApril26,2019)allstudentsmusthavecompleted:

• FiveweeksofFamilyMedicine(FMED350)• TenconsecutiveweeksofSurgery/SurgicalSpecialtiesSelectives

Consistingof: -sixweeksofGeneralSurgery(SURG350),and -three-weekrotationofSurgicalSpecialties,and -oneSurgeryAcademicweek

• Tenconsecutiveweeksconsistingof: -sixweeksofInternalMedicine(MED350)and -threeweeksofNeurology(NEUR350)and -oneInternalMedicine/NeurologyAcademicweek• FiveweeksofPediatrics(PED350)• FiveweeksofPsychiatryandHumanBehavior(PSYH350)• FiveweeksofObstetrics/Gynecology(OBGY350)

• AllThirdYearrotations,withtheexceptionoftheNeurologyandSelectiverotations,havea

comprehensivewrittenexaminationattheconclusionoftheteachingblock.

• The3rdYearten-weekcontinuousrotationsstartinblocks1,3,9,or6.GeneralInformationConcerningtheSchedulingProcess

• Themajorobjectiveistogiveeachstudentthehighestorderofcoursesandlocationspreferred,whilealsomeetingJeffersonandaffiliatedhospitalrequirements.Determinationoffinallocationandsequencingofallrotationsisreservedtothecollege.

• Ifthestudent'sfirstpreferenceisnotavailable,duetoenrollmentlimitationsorotherfactors,the

processwillmovetothenexthighestselectiondependingonthepreferenceselectedbythestudentontheschedulerequestscreen.

Thomas Jefferson University’s Sidney Kimmel Medical College (SKMC) Longitudinal Integrated Clerkship (LIC) at Atlantic Health

What is a Longitudinal Integrated Clerkship (LIC)?

An LIC is an innovative model for the clinical clerkship year founded on the organizing principle of continuity—continuity of care, supervision, curriculum, and of relationships with patients, preceptors, and the health care system. You will have the opportunity to follow patients longitudinally throughout the core disciplines, spanning all phases of diagnosis and treatment.

Features of the LIC:

If I choose to participate in the LIC, will I get the same education?

Yes! The LIC delivers an equivalent curriculum to that of the traditional block model, including didactics, patient log requirements, assignments, and NBME exams, longitudinally over the course of the entire clerkship year.

What are the benefits of participating in an LIC?

• Learn through patient-centered care • Develop longitudinal relationships with faculty preceptors and patients in acute and chronic care settings • Perform equivalent to peers on standardized testing • Experience an increased connection with patients • Foster a greater sense of self-awareness and responsibility for your education

To apply, you must:

• Be in good academic standing • Submit a brief, 500-word essay describing your interest in the program • Meet with program faculty

• A nine-week clinical inpatient immersion experience followed by 40 weeks of longitudinal care across the core disciplines in the outpatient setting

• A preceptor in internal medicine, family medicine, pediatrics, obstetrics and gynecology, surgery, neurology, and psychiatry will be assigned to work with you on a weekly basis

• You will gain an understanding of the disease process through involvement of care delivery across all aspects of the health care system

• Opportunities to learn from master SKMC educators in Atlantic Health’s state-of-the-art teaching hospitals, as well as Goryeb Children’s Hospital, Gagnon Cardiovascular Institute and Atlantic Neuroscience Institute

FAMED350 FAMILYMEDICINE

AbingtonMemorialHospitalAsafamilymedicineteachingprogram,wehaveastrongcommitmenttoteachingmedicalstudents.ThirdyearstudentsfrommanylocalmedicalschoolsrotatewithourresidentsontheFamilyMedicineInpatientServiceandin the Family Medicine Center. We also offer four week and two week fourth year elective rotations forinterestedmedicalstudents.StudentsareincorporatedintoallaspectsoftheFamilyMedicineResidencyduringtheirrotationswithus.Themajorityofstudenttimeisspentcaringforpatientsintheoutpatientsettingbutalsomayincludeactivitiessuchasroundinginthehospitalwiththefamilymedicineresidentsandattending’s,geriatricfacilityvisits,andhomevisits. Medicalstudentsare involve inallconferencesanddidacticprogramsat theAbingtonFamilyMedicineduringtheirrotationsandparticipateinaseriesoflecturesonthetop20diagnosesinprimarycarespecificallydesignedforthestudents. There isalsotheopportunity for interestedstudentstoworkwithourresidents inotheractivitiessuchaspreparticipationsportsphysicals,outpatientofficeproceduresandresearchprojectsandpublications.

AriaJeffersonHealthAria Jefferson has a long standing tradition of working with medical students, residents, and alliedmedicalprofessionalsintraining.Thisisourmissionandweenjoyworkingwiththefuturephysiciansandadvancedpracticeprofessionalswhowillservetheirpatientsinthefuture.Our family medicine rotation is integrated into our family medicine residency program. As such, thestudentsworkdirectlywithcommunityfacultyintheirofficesettings.Inthiswaythereal"feel"ofbeinga familyphysicianengaged in their community is experienced.Acute, chronic, preventive andepisodiccare is part of the daily professional life of family physicians, and medical students are immediatelyincludedaspartoftheofficeteam.Therewillbeone-onepreceptorexperience,sometimesincludingafamily medicine resident as well. Additionally, once per week there will be teaching rounds at anextended care facility,workingwith seniors for a geriatric componentof the familymedicine rotation.Wealsooffermedicalstudentstheoptiontoroundwiththeinpatientteachingserviceifthatwouldbean interest. Also, each Thursdaymorning the studentswill join us at our FamilyMedicine conferencefrom7-noonattheAriaJeffersonBuckscampus.Thankyou for considering theFamilyMedicine rotationatAria Jefferson.We look forward toworkingtogether.

BrynMawrHospitalThegoalofthissix-weekrotationisforthestudenttopracticeandimprovehis/herinterviewingandphysicalexaminationskillsandtobeginintegratingpatientdatawithbasicsciencetomakerelevantclinicaldecisions.ThisFamilyPractice rotation is intended toprovide the third-year studentwithabroadclinicalexperience.There is ample time spent in the hospital-based residency practice and in the office of a community-basedfamily practitioner. The array of exposureswill include the family practitioner in the office,making homevisitsandnursinghomevisits. Inaddition, thestudentsmeetwith faculty for seminarsonclinicalproblemsolvingandinterviewingskills. Studentsareresponsibleforreadingnecessaryinformationtoappropriatelylearnaboutthepatientstheyhaveseen.AnevaluationwillbemadebythepreceptorwithwhomthestudentsworkandthefacultyandresidentsatBrynMawrFamilyPractice.

ChristianaCareHealthServicesStudentswillbeexposed toawiderangeof clinicalexperiencesandsettings. Studentsspend twoweeks inaprivatefamilyphysician’soffice,at leastoneweekinourresidencypractice,severalsessionsinanurgentcaresetting, aswell as several varied experiences in communitymedicine, includingHIVClinic, nursinghomeandhomevisits. Aspartoftherotation,allstudentsparticipateinapracticeOSCesessioninourVirtualEducationandSimulationCenter.Lecturesalsoincludeinteractivedidacticsessionsoncommonoutpatienttopicsaswellas an introduction to evidence-based medicine. Students are evaluated by the course director outsidepreceptors, faculty, residentsandstaff. Mid-rotationmeetingsare informaldue to thewiderangeofassignedlocations. Theclerkshipdirectormeetswithall students for several sessionsoforientationandmostFridaysthereafter. Due to thevaried locations towhich thestudentsmust travel,anautomobile is required for thisrotation.

Crozer-KeystoneHealthSystemAttheCrozer-KeystoneCenterforFamilyHealthinSpringfield,weoffer2and4-weekfourthyearelectivesanda6-weekthirdyearclerkshipforinterestedmedicalstudents.Duringyourrotation,youwillspendthemajorityofthetimecaringforpatientsintheoutpatientsettingwhichislocatedinDelawareCounty(20minutesoutsideofPhiladelphia.) Our Springfield office recently received Level 3 recognition from the National Committee forQualityAssuranceforourPatientCenteredMedicalHome.Patientcareactivitieswillroutinelyincludeexposuretoglobalhealth,officeprocedures,cosmeticmedicine,sportsmedicineactivities,medicalinformatics,inpatientservice and behavioral science sessions. In addition, students will also be involved the community throughschoolphysicals,homevisitsandnursinghomevisits.Studentsparticipateintheweeklydidacticsessionswithour residentsaswell asdedicatedweeklymedical student teaching sessions. The student takes an active role incare/managementofpatients,andstudentsparticipateinclinicalanalysisprocesses.Studentsaresupervisedbyboard-certifiedfamilypracticeattendings.ThereisprobablynoothersitewithintheJeffersonsystematwhichthestudentsgetthisopportunitytoactascliniciansandtocontributesodirectlytothecareofactualpatients.Studentsreceive informal feedback fromeverypreceptorwithwhomtheywork.Preceptorsrelay informationverballyorinwritingtotheclerkshipcoordinator,whohasaformalfacetofacediscussionwiththestudentsinmid-courseandat theendof therotation. Theclerkshipcoordinatorcompiles theevaluationsandwrites thenarrative grade report to send back to Jefferson. Students are evaluated compared to the theoreticalexpectationsforastudentattheirleveloftrainingandexperience.Knowledge,professionalism,personalskills,andabilitytothinkinclinicaltermsareallimportantgradingfactors.

FAMED350 FAMILYMEDICINEInspiraMedicalCenterWoodburyOur facultyhas training inWomen’sHealthandWoundCare inadditiontoa focusonGeriatrics.Wehaveourownprenatalclinic,whichissupervisedbystaffobstetricians.Midrotationevaluationsoccursat3weeks,andareintheformofverbalfeedback.Weevaluatestudentsbasedoncompositescorescollectedfromallphysiciansthatworkedwith the student. Our formsmirror the criteria present on the final evaluation forms sent fromJefferson.Allsubjectivecommentsareincorporatedintheevaluations.

ReadingHospitalandMedicalCenterTheReadingHospitalisalarge,communitybasedhospitalwithmanyresources.Studentshavetheopportunitytheopportunitytocustomizepartsoftherotation,dependingupontheirparticularinterest.Students“formally”staywithin the residency program, but have the opportunity to workwith specialists alongwith the FamilyMedicine Residents. Students typically work with 8-10 residents and 8 faculty members throughout theirrotation. The mid-rotation and final evaluations are a composite of input from those who worked with thestudent.Foreachofficehoursession,thestudentwillcompleteaSOAPnotewhichiscritiquedandreturnedtothestudent.StudentswillhavetheopportunitytodohomevisitswithBerksVisitingNurses1dayperrotation,AlverniaUniversityStudentHealthClinic1afternoonperrotation,Woundcareone½dayperrotation,palliativecare1dayperrotation,andotherprocedureandsubspecialtyclinicsthroughouttherotation.SpecialfeaturesofyourclerkshipincludearequiredBiopsychosocialprojectwhichinvolvesinterviewingalong-termpatientandwriting a 3-page paper with a genogram (time built into your schedule to complete). You will have theopportunitytoworkwithskillednursesandgainhands-onexperienceaswellasdocumentdirectlyinEpic.

ThomasJeffersonUniversityHospitalFor clerkship students rotatingat Jefferson, thebulkof clinical time is spent in the JeffersonFamilyMedicineAssociates practice. There, students typicallymeet patients, take the history, conduct the physical exam, andpresent patients to precepting faculty. Students are responsible for formulating a differential diagnosis,developinganassessmentandtherapeuticplan,andperforminghealthmaintenanceactivities.Studentsmayberesponsible for labs, follow-up, notes, and referrals as needed. As a teaching program with over 30 years of experience training residents, we arededicated to ensuring that students get an excellent experience in seeingwhat FamilyMedicine is all about.Studentswillspendthemajorityoftheirtimeatouroutpatientfacility,theVirtua-TatemBrownFamilyPracticeCenter,andalsospend1weekoninpatientroundsonourfamilymedicineserviceatVirtuaVoorhees.Weareabout30minutes fromCenterCityPhiladelphia,makingaccess for Jeffersonstudentsquiteeasy.Ourpracticehas a very diverse patient population,with a broad demographicmix including 30%pediatrics. In the office,studentswill workwith residents and 1 on 1with an attending, andwill see awide variety of patients thatencompass everything from well child and preventive health visits to the most complex chronic diseasemanagement cases, aswell as office procedures.Our practice is recognizedby theNCQA as a level 3medicalhome,andstudentswillgainexperiencelearninginanenvironmentthatemphasizespatientsatisfaction,patientsafety and continuous quality improvement. WellSpanYorkHospitalStudentsseepatientsattheThomasHartFamilyPracticeCenterwhichispartofthefamilymedicineresidencyprogramfoundedin1968.Thefamilypracticecenterisconnectedtothehospitalandhas24examrooms,twoprocedure rooms, a lab, and a conference room as well as offices for the faculty and residents. The averagenumber of outpatient visits per year is approximately 23,000. These visits includewell-child care,maternitycare,adultcare,andnumerousoutpatientprocedures.The familypracticecenterhasbeenusinganelectronichealthrecordsinceSeptember2006.StudentswillspendmostoftheirtimeattheThomasHartFamilyPracticeCenterbutwillalsospendtwohalf-daysperweekataprivatefamilymedicineofficeinthecommunity.Studentswill also spend two weeks on the family medicine inpatient service. For the 2018-19 academic year, all students will start the Internal Medicine Clerkship with a mandatory“AcademicWeek” duringwhich theywill participate in a variety of educational activities thatwill give themfoundationinInternalMedicineandNeurology.Afterthat,studentswillspend3weeksattheThomasJeffersonUniversityHospital,and3weeksatoneoftheacademicaffiliatehospitals.ChristianaBranchCampusstudentsspend entire 6 weeks at Christiana Care Health System. Each site offers unique learning opportunities, butultimately adheres to the unified set of educational objectives. It is a very large,multidisciplinary tertiary-care hospitalwith 600 acute care beds, caring for adiversepatientpopulationwithalargescopeofcomplexmedicalillnesses.DuringInternalMedicineatEinstein,studentswill rotateoneithergeneralmedicineorsubspecialtyservices.Therobustdidacticprogramincludescore conference series, EKG workshops, radiology rounds,subspecialty rounds,noon student reports,and aJeopardy-style review session. Physical diagnosis rounds are held both at bedside and in a simulated setting.During the simulation sessions, students learn IV placement, venous blood draws, and basic review of ACLSalgorithms. Students also have a chance to roundwith thephlebotomy and IV teams to acquire andpracticetheseskills.

ChristianaCareHealthSystem–ChristianaHospitalStudents interactwith consultants fromall subspecialty InternalMedicine services. These includeCardiology,InfectiousDisease, Gastroenterology,Hematology,Nephrology, Pulmonary andRheumatology.On the internalmedicine service, students are evaluated by the intern and resident with whom they worked most closely.Dependingontheservice,anattendinghospitalistevaluatesthemaswell.TheseindividualscompletetheSidneyKimmelMedicalCollegeevaluationinNewInnovations.AworkshopinIVinsertionisprovidedforstudentsandis schedule each month. Students are also required to prepare a one-page response to an evidence-basedquestionassignedtothem.Severalliteraturereferencesareexpected.Abedsidephysicalexaminationsessioniscarried out usually with two students. This may involve performance of an entire physical examination orsectionsbasedonstudentneeds.MandatorysessionsincludeMedicalGrandRoundsandstudentcorelectures.

During the rotation, students will rotate on general internalmedicineservices.Thiscourseemphasizestheintegrationandapplicationofpathophysiologytothediagnosisand management of patients in addition to the skills of history-taking, physical examination, and casepresentation.The course is an apprenticeship focusingon thebedside careof patients. AbingtonJeffersonHealthAs an OB/GYN teaching program, we have a strong commitment to teaching medical students. Third yearstudents from many local medical schools rotate with our residents and faculty on several services. Theseservices include: gynecologiconcology,night float, gynecology, andobstetrics. Students alsoget the chance torotateinourOB/GYNCenterfortheunderserved.Didacticsessionsandareheldweeklyforbothresidentsandstudents,andweeklymorbidityandmortality(M&M)conferencesarealsoheld.Morningconferencesarehelddaily.Students are incorporated into all aspects of the OB/GYN residency during their rotations with us. Studentsreceive training both on the inpatient and outpatient EMR in the hospital. EMR write access is available tostudentsduringthegynecologyservice. AlbertEinsteinMedicalCenterThe clerkship in women’s health, obstetrics & gynecology in the Department of Obstetrics and Gynecology,Albert Einstein Medical Center, is a six-week learning experience designed to provide the basic informationabout women’s health, obstetrics and gynecology needed by medical students to successfully complete theirclerkships and pass national standardized examinations. Students are assigned to rotations in generalobstetrics, gynecology, and night float teams, providing ample opportunity to learn the basic knowledge andskillswhileexperiencinghands-ontrainingindeliveriesandsurgery.StudentsareincludedasactivemembersoftheObGyncareteamsandareallowedandencouragedtodoasmuchastheirknowledgeandskillspermit.

InObstetrics,studentsareinvolvedinlaboranddeliveryactivities,theantepartumandpostpartumfloorandinclinical scenarios such as normal labor and delivery and high-risk pregnancies (i.e., preterm labor, multiplegestation,prematureruptureofmembranes).InGynecology,studentsparticipateinbothgeneralandoncologic

Written evaluations from attending staff are compiled by themedical student clerkship director.Evaluators consider the student’s attitude, conscientiousness, motivation, history taking, patient interaction,medicalknowledge,casepresentationandproblemanalysis.Verbalfeedbackisprovidedonadailybasisbytheattending the studentworkswith. ChristianaCareHealthServices(CCHS) TheOB/GYNstudentsalltrainatChristianaHospitalandWilmingtonHospital.Christianacarehealthsystemdeliversapproximately7000babiesperforms6000gynecologicsurgicalproceduresannually.Allof theobstetricalcareand themajorityof thegynecologicsurgicalcare isprovidedatChristianaHospital inNewarkDelaware. ChristianaHospital is a full service, community based, tertiary care,academicHospital. WilmingtonHospital is an urban communityHospital and is the location of theclinic experience for students rotating and OB/GYN. Students will be exposed to the subspecialtysome maternal fetal medicine, reproductive endocrinology, family planning, urogynecology, andgynecologic oncology. Students are expected to perform or assist with patient admissions,preoperativeandpostoperativechecks,inpatientevaluationsandconsultations,outpatientcare,laborand delivery, surgical assistants, fetal monitoring, and OB/GYN triage. Students are involved incontinuity clinics with residents. Students are expected to participate in Wednesday didacticsincluding noontime Grand Rounds and resident and student lectures on Wednesday afternoons.StudentsreturntoJeffersononFridayafternoonforrequiredidactics.Studentswillspendoneweekworkingnights.LocalhousingmaybeprovidedperChristianapolicies.Studentsareassessedbasedupon evaluation ofworkdone in the clinic, on obstetrical inpatients, on surgical patient services innumbertosedationandgroupdiscussions.Emphasisisplacedonclinicalaspectsofpatientcareandthe ability to interact with patients and staff. VirtuaVoorheesHospital DuringtheclerkshipatVirtuaWestJersey,studentswillhaveexposuretoMaternal-Fetalmedicine,GynecologicOncology,andReproductiveEndocrinology.Thestudentsareevaluatedonhistorytaking,physicalexamination,labresultsreviewandfollow-up,duringtheclerkshipatVirtuaWestJersey.Theyareevaluatedbyattendingsinthedepartment,andresidents.VirtuaWestJerseydoesthelargestnumberofdeliveriesintheSouthJerseyarea(5500+/year)withanumberofhighriskobstetricalcases.TheCenterforWomenisouroutpatientunit–thisisa privately run clinic setting rendering GYN and OB needs for the community. Students will experience allaspectsofofficegynecology(routineannualcare,colposcopies,papsmears,familyplanningcounseling,andSTDscreens.Inaddition,theCenterdealswiththemanagementofthemenopausalstatewithemphasisonhormonereplacement and alternative medicine. PEDIATRICSROTATIONOVERVIEWOver six weeks the Pediatrics clerkship gives students a varied combination of clinical experience in thefollowing areas: hospital based outpatient, private practice outpatient, newborn nursery, transitional andintensivecarenurseries,inpatientunit,emergencyroomandothercommunitysettings.TheclerkshipsitesareDelaware Valley Clerkship (duPont, Christiana, Einstein, Bryn Mawr, Abington and TJUH), Reading andMorristown.Eachoffersvaried,broadandstimulatingclinicalexperienceswithfrequentandextendedcontactwith attending pediatricians. Every site has an affiliated pediatric residency program – either Pediatrics orFamilyMedicine(ReadingandBrynMawr).

Core curriculum is based on a U.S. national curriculum in pediatrics developed to prepare students for thepediatric portion of the USMLE Step 2 examination. Teaching at all sites focuses on small group case-basedlearning inaddition toweb-basedpediatric cases. Studentsat all sitesalsoattend resident lectures,morningreport, and grand rounds. Pediatrics: A Competency Based Approach textbook is loaned to students for useduringtherotation.

Clinical skills are taught on the first day of the clerkship during a “Clinical Skills Day.” History taking,communicationandphysicalexamskillsaredevelopedduringtheclerkshipusingdirectobservation(SCOs)withreviewandfeedbackbyafacultymember.

DelawareValleyClerkship-ThomasJeffersonUniversityHospitalThePediatricDepartment at TJUH is integrated into a full service hospital in the center of Philadelphia. Theprogram is staffed by the duPont general Pediatricians, sub-specialists and the 60+ residents from theduPont/JeffersonResidencyProgram.Theoutpatientexperienceisatalargehospitalbasedprivatepracticeorcommunitybasedprivatepractices.The4bedpediatricCRHinpatientunit,isaregionalmedicalcarefacilityforsick children and childrenwith chronic illnesses. CLINICALASSIGNMENTSSeveral types of clerkship experiences are offered at Jefferson and our affiliates. All of the placementsutilizethe"treatmentteam"approach-amultidisciplinaryteamthatworkstogethertotreatpatients.Asamedicalstudent,youareviewedasanactivememberofthetreatmentteam.Asyourconfidenceandskillsincrease, you can become increasingly involved in teamdecisions and interventions. Wherever you areassigned within our teaching network, you will have exposure to at least two different clinical servicesduringyourworkinthedepartment.

Studentsareevaluatedattheendofeachthreeweekcorerotationbythemainattendingwhosupervisesthem,withinputfromresidentsandotherattendings.Studentsarealsoevaluatedonthebasisofcontentand thoroughness ofwrite-ups and presentations on rounds, basic familiaritywith differential diagnosisand treatment, and empathic interaction with patients. Their formal case write-ups are evaluated forclarity, detail, differential diagnosis, and treatment plan. The criteria used for evaluation include: 1)ProfessionalBehavior,2)CognitiveSkills,and3)ClinicalSkills.DataiscompiledforreviewbytheClerkshipDirectorandAssistantClerkshipDirector,whodeterminefinalgrades.

ALBERTEINSTEINMEDICALCENTERAEMC is a large tertiary teaching center and part of the JeffersonHealth System. Each student spendsthreeweekson twoof the following three services: inpatientpsychiatry (adultorgeriatric); consultationand liaison psychiatry; and emergency psychiatry (Crisis Response Center, for adults, adolescents, andchildren). TheCRC is a free-standing service adjacent to the emergency room. TheCRCevaluatesover6000 patients per year, and over one-third of patients are children. TheDepartmentofPsychiatryencompassesawide rangeof clinical services, includingapsychiatric inpatientunitandconsultation-liaisonservices.Studentswillgainexposuretomanyoftheseservices,workingfourweeks on the consult-liaison service and two weeks on the inpatient unit.While on the inpatient unit,studentswillparticipateinteammeetings,individualmeetingswithpatients,andgrouptherapy;theywillfollow patients across the two weeks and produce a case write-up based on their work. While on theconsult-liaisonservice,theywillalsohaveanopportunitytoworkwithpsychiatristsintheERcrisisserviceand in the outpatient psychiatry, CF, and/orHIV clinics. AdultInpatientPsychiatryTheThomas JeffersonUniversityHospitaloperatesa16bedgeneraladult inpatientunit.Thisunitoffersintensive psychiatric care to individuals, including patients with underlying medical complications. Amultidisciplinary treatment approach is used in which patients receive the benefit of working withpsychiatrists, psychologists, social workers, psychiatric nurses, and occupational/recreational therapists.Extensive evaluation of treatment needs is performed for all individuals and typically includespharmacotherapy,individualandgrouptherapy,andmilieutreatmentinanintensivesetting.

NarcoticAddictionRehabilitationProgram(NARP)TheJefferson Narcotic Addict Rehabilitation Program (NARP)is an outpatient substance abuse recoveryprogram that specializes in providing treatment to peoplewho are addicted to opiates (such as heroin,oxycontin,percoset,etc.).Recoveryisinitiatedandachievedthroughregularpsychotherapyandavailablepsychiatric services, paired with methadone maintenance therapy to suppress symptoms of opiatewithdrawal,decreasecravings,andblocktheeffectsofotheropiates.Furthersupportanddevelopmentisfound through engagement in on-site psychotherapy groups and peer-run self-help groups, as well asthroughinvolvementinthegreatersubstanceabusecommunity.

FamilyCenterThe Family Center is a comprehensive women-centered substance abuse treatment program providingoutpatient and intensive outpatient care for women who are pregnant, parenting, or working towardsreunification. The program is licensed to provide both methadone and buprenorphine outpatientmaintenance as well as non-medication assisted treatment.Further support and development is foundthrough engagement in on-site psychotherapy groups and peer-run self-help groups, aswell as throughinvolvement in the greater substance abuse community. Every Friday, studentswill be reporting to ThePennFoundation,adrugandalcoholrural rehabilitationcenterwithaprimaryfocusonsubstanceabusetreatment. AbingtonJeffersonHealthAsasurgeryteachingprogram,wehaveastrongcommitmenttoteachingmedicalstudents.Thirdyearstudentsfrom many local medical schools rotate with our residents and faculty on several services. These servicesinclude: advanced laparoscopy, colorectal, general surgery, surgical oncology, and vascular. Additionalexperiences are also available such as trauma and subspecialties. Didactic sessions are heldweekly for bothresidents and students, and weekly morbidity and mortality (M&M) conferences are also held. Morningconferences are held daily. On-call time is also incorporated into the rotation; post call days, along with astrategic napping room are available to all students.
