PA Board of Accountancy Allegheny County Robert Alan Benson, license no. CA011209L of Pittsburgh, Allegheny County, was

ordered to pay a civil penalty of $300 and, within six months, complete 12 hours of

continuing professional education because he did not complete the requisite 80 credit

hours of continuing education during the applicable biennial period. (04/19/11)

Bucks County Marc Silverstein, certificate no. CA047082 of Holland, Bucks County, was assessed a

$10,000 civil penalty for practicing public accounting without a current license.

Delaware County David Thomas Shergalis, certificate no. CA011542L of Media, Delaware County, had

his certificate of certified public accountant and current license to practice public

accounting suspended for four days for his late payment of a previously imposed civil

penalty. (03/03/11)

James Patrick Flanagan, license no. CA020184L of Drexel Hill, Delaware County,

was ordered to pay a civil penalty of $2,000 because he engaged in the practice of

public accountancy and used the designation "Certified Public Accountant” and the

abbreviation “CPA” when he did not hold a current license. (04/19/11)

Lackawanna County Bonita & Rainey, license no. AF000729L of Plains, Lackawanna County, was

ordered to pay a civil penalty of $5,000 because it practiced as a certified public

accountancy firm when it was not currently licensed and registered between Jan. 1,

2008 and July 8, 2008. (05/09/11)

Washington County Adam C. Garczewski, license no. CA041640L of Lawrence, Washington County, was

ordered to pay a civil penalty of $300 and, within six months, complete 21 hours of

continuing professional education because he failed to complete at least 80 hours of

continuing education during the relevant reporting period from Aug. 2, 2008, through

Westmoreland County Mark A. Terdle, license no. CA022325L of North Huntingdon, Westmoreland

County, was ordered to pay a civil penalty of $900, had a formal reprimand placed on

his licensure record, and must, within six months, complete 40 hours of continuing

professional education because he did not complete the requisite eight hours of

continuing education in tax subjects during the biennial period, did not complete the

requisite 20 hours of continuing education during calendar year 2008 in the biennial

period, and did not complete the requisite 80 hours of continuing education during the

the requisite 80 hours of continuing education during the applicable biennial period.

Berks County William J. Guidici Jr., license no. BM000387L of Sinking Spring, Berks County, had

his license suspended for failure to pay a previously imposed civil penalty. (02/25/11)

Bucks County Anthony V. Abate, t/d/b/a Tony Abate’s Barber Shop, license no. BO432152 of

Fairless Hills, Bucks County, was assessed a civil penalty of $250 for failing to renew

Out of State Robert L. Thomas, license no. BL050516L of Charlotte, N.C., had his license

suspended for failing to pay a previously imposed civil penalty. (03/04/10)

PA Board of Certified Real Estate Appraisers York County Robert C. Stoner, license no. RL003684 of Dillsburg, York County, had his license

suspended for at least 15 months, with the suspension stayed in favor of probation

after a minimum of three months, subject to terms and conditions, because he failed

to complete an appraisal assignment within the requirements of the Uniform

Standards of Professional Appraisal Practice; failed or refused to exercise reasonable

diligence in developing an appraisal, preparing an appraisal report or communicating

an appraisal; and negligently or incompetently developed an appraisal, prepared an

appraisal report, or communicated an appraisal. He was also ordered to pay a civil

penalty of $3,000, pay costs of investigation in the amount of $1,000, and complete at

least 45 hours of remedial education. (04/14/11)

Bureau of Charitable Organizations Allegheny County Construction Junction Inc., of Pittsburgh, Allegheny County, was ordered to pay an

administrative fine of $2,000 for violating the Solicitation of Funds for Charitable

Purposes Act by failing to keep true and accurate records. (05/10/11)

Indiana County Indiana Lions Club, of Indiana, Indiana County, was ordered to pay an administrative

fine of $4,000 for violating the Solicitation of Funds for Charitable Purposes Act by

making material false statements on applications, statements, or reports required to be

filed with the Bureau of Charitable Organizations for fiscal years ending June 30,

2009, June 30, 2009 and June 30, 2010. (05/10/11)

Philadelphia County Dolce Suono Ensemble Inc., of Philadelphia, Philadelphia County, was ordered to

pay an administrative fine of $1,500 for violating the Solicitation of Funds for

Charitable Purposes Act by soliciting charitable contributions in Pennsylvania

without being properly registered. (05/10/11)

Schultz & Williams Inc., of Philadelphia, Philadelphia County, was ordered to pay an

administrative fine of $1,000 for violating the Solicitation of Funds for Charitable

Purposes Act by failing to file a contract with the Bureau of Charitable Organization

prior to rendering professional fundraising counsel services. (05/11/11)

Out of State Nexus Direct LLC, of Virginia Beach, Va., was ordered to pay an administrative fine

of $3,000 for violating the Solicitation of Funds for Charitable Purposes Act by

providing professional solicitor services prior to being registered with the Bureau of

Charitable Organizations and by failing to file a contract with the bureau prior to

providing professional solicitor services. (05/10/11)

PA Board of Chiropractic Centre County Nellie R. Wortman, license no. DC000900L of State College, Centre County, was

assessed a $1,000 civil penalty for practicing chiropractic without a current license.

PA Board of Cosmetology Allegheny County Ericka Kathleen Gibb, license no. CO230298L of Pittsburgh, Allegheny County, had

ordered to pay a civil penalty of $500 because he practiced as a cosmetologist when

Son Hoang Thai and Hanh T. Vu, t/d/b/a Nails Elegance and Spa, license no.

CY195145 of Pittsburgh, Allegheny County, were ordered to pay a civil penalty of

$5,000 because Ms. Thai and Ms. Vu, while trading and doing business as Nails

Elegance and Spa, had razor blades and razor tools in the salon. (05/02/11)

license suspended for failing to pay a previously imposed civil penalty. (02/18/11) Berks County Impression Nails, license no. CY194036 of Douglassville, Berks County, was ordered

to pay a civil penalty of $2,300 because it practiced nail technology in a grossly

incompetent or unethical manner through the use of razor tools and razor blades,

failed to maintain tools that come into contact with a client in a sanitary condition at

all times, and failed to have a first aid kit with all the required items. (05/02/11)

Bucks County Coette Heard, license no. CO252362 of Bensalem, Bucks County, had her license

suspended for nonpayment of a previously imposed civil penalty. (02/18/11)

Chester County Arthur DiGiacomo, license no. CO153450L of West Chester, Chester County, had his

license suspended for nonpayment of a previously imposed civil penalty. (02/18/11)

Delaware County Taylor Qun Chen, t/d/b/a Q Q Nails, license nos. CY194538 and CZ116226 of Glen

Mills, Delaware County, was assessed a $1,700 civil penalty for having practiced nail

technology in a grossly incompetent manner by having five razor scraper tools and 70

razor blades in the salon. (03/02/11)

Vu Thanh Nguyen, t/d/b/a Tina Nails, license no. CY106565 of Folcroft, Delaware

County, was assessed a $2,900 civil penalty for having practiced in a grossly

incompetent or unethical manner by having a hot wax bowl and instruments used for

facial waxing, multiple razor scraper tools at workstations, no wet sterilizer or

container for sanitized tools, dirty manicure tools at workstations and paper towels

not in a closed container or towel dispenser. (01/20/11)

All in Wrist, license no. CB121341 of Darby, Delaware County, was assessed a

$1,950 civil penalty for engaging in booth rental, maintaining the salon in an

unsanitary condition and failing to have a complete first aid kit. (03/17/11)

Lancaster County Salon 1 Style Inc., t/d/b/a Salon 1 Style, license no. CB121933 of Lancaster,

Lancaster County, was assessed a $1,550 civil penalty for engaging in booth rental,

maintaining the salon in an unsanitary condition and failing to have a complete first

Monroe County Glamor Nail Spa LLC, t/d/b/a Glamor Nail Spa, license no. CY195490 of

Tannersville, Monroe County, was ordered to pay a civil penalty of $1,000 because

Glamor Nail Spa LLC, while trading and doing business as Glamor Nail Spa,

permitted an unlicensed individual to perform cosmetology services in the salon and

maintained an unlicensed cosmetology shop by providing waxing services to a client

on a premises not licensed for the provision of those services. (05/02/11)

Philadelphia County Nicole Hopkins, t/d/b/a Simply Elite Spa, license no. CB119229 of Philadelphia,

Philadelphia County, had her license suspended for failing to pay a previously

imposed civil penalty. (06/08/10)

Phuong Thi Nguyen, t/d/b/a Cheltenham Nails, license no. CY193162 of

Philadelphia, Philadelphia County, was ordered to pay a civil penalty of $500 because

she employed an unlicensed person to practice cosmetology and aided and abetted the

unlicensed practice of cosmetology. (03/16/11)

PA Board of Dentistry Berks County Anthony J. Pichini, license no. DS014935L of Mt. Penn, Berks County, voluntarily

surrendered his dental license because he practiced dentistry from April 1, 1999, until

approximately September 2010, and failed to renew his license during said period.

Delaware County Marela C. Garcia, unlicensed of Newtown Square, Delaware County, was assessed a

$1,000 civil penalty and was ordered to immediately cease and desist from the

unlicensed practice of dentistry based on findings that she practiced dentistry or held

herself out as authorized to practice dentistry without being duly licensed or

authorized to engage in such practice. (03/22/11)

Montgomery County Thomas W. McFarland Jr., license no. DS020097L of Wynnewood, Montgomery

County, was ordered to pay a civil penalty of $5,000 and his license remains

indefinitely suspended under the terms of a previous board order because he engaged

in unprofessional conduct by failing to follow current infection-control

recommendations issued by the Centers for Disease Control and Prevention.

suspended for failing to pay a previously imposed civil penalty. (02/10/11)

Martin Jay Thaler, license no. DS022305L of Monsey, N.Y., received a public

reprimand and was assessed a $1,000 civil penalty based on disciplinary action taken

against his license by the proper licensing authority of another state and having failed

to report this discipline to the board within 90 days. (03/22/11)

Patricia A. Sabers, license no. DS023465L of Sarasota, Fla., received a public

reprimand and was assessed a $1,000 civil penalty based on disciplinary action taken

against her license by the proper licensing authority of Florida. (03/22/11)

Robert J. Winegarden, license no. DS018697L of New York, N.Y., had his license

revoked based on disciplinary action taken against his license by the proper licensing

authority of another state. (03/22/11) Clyde Anthony Maxwell, license no. DS029663L of Wilmington, Del., was ordered

to pay a civil penalty of $1,500 and a public reprimand was placed on his permanent

disciplinary record with the board because he had his license disciplined by the

proper licensing authority of another state and failed to report this action to the board

within 90 days. (04/29/11)

PA Board of Medicine Bucks County Hitesh K. Patel, license no. MD064657L of Newtown, Bucks County, was assessed a

$5,000 civil penalty and issued a formal reprimand based on disciplinary action taken

against his license by the proper licensing authority of New Jersey and failing to

report this disciplinary action to the board within 60 days. (01/11/11)

Montgomery County Arie Oren, license no. MD036293E of Conshohocken, Montgomery County,

voluntarily surrendered his license to practice medicine while his criminal case is

pending in Montgomery County Court of Common Pleas because he has been

charged with a sexual offense relating to his practice of medicine. (04/26/11)

Gary Scheiner, unlicensed of Wynnewood, Montgomery County, was ordered to pay

a civil penalty of $1,000 and to immediately cease and desist from engaging in the

practice of medicine or attempting to offer to practice medicine in Pennsylvania

because he practiced or attempted to offer to practice medicine without possessing a

valid license to practice medicine in Pennsylvania. (04/26/11)

Warren County Elisa Lynn Franklin, license no. YM013188 of Warren, Warren County, was ordered

to pay a civil penalty of $250 because she practiced as a respiratory care practitioner

without a valid current temporary permit or certificate issued by the board. (04/26/11)

Out of State Binod Prasad Sinha, unlicensed of Edison, N.J., had his application for a license

denied based on disciplinary action taken against his license by the proper licensing

authority of New York and New Jersey. (02/18/22)

Gregory E. Furness, unlicensed of Irmo, S.C., had his application for licensure denied

based on disciplinary action taken against his license by the proper licensing authority

of South Carolina. (12/29/10)

Kenneth Zahl, license no. MD028311E of Morristown, N.J., was assessed a $2,000

civil penalty and had his license suspended for five years, with the suspension stayed

in favor of probation, based on disciplinary action taken against his license by the

proper licensing authority of another state. (12/29/10)

Ross E. Levatter, license no. MD435157 of Phoenix, Ariz., had his license revoked

based on his felony conviction in a court of New Mexico. (11/16/10)

Gabriel John Yandam, license no. MD054658L of Coshocton, Ohio, had his license

placed on probation during such time as his license in Ohio is on probation because

he had disciplinary action taken by the proper licensing authority of another state.

temporarily suspended because his continued practice of medicine is a clear and

indefinitely suspended based on disciplinary action taken against his license by the

Daniel Luczkow, license no. MD058942L of Centerville, Mass. and Baltimore, Md.,

had his license placed on probation for a period of one year based on disciplinary

action taken against his license by the proper licensing authority of another state.
