CHAPTER 4

ACTIVE DUTY OFFICER PROGRAMS PROCESSING

SECTION 1 – APPLICATION PREPARATION

040101. OVERVIEW. This section provides specific guidance for preparing an officer programs application for all Active Duty Officer Programs.

040102. RESPONSIBILITIES

a. Unless specifically directed otherwise by the Officer Programs Officer (OPO) or personnel with “By direction” authority, the Officer Processing Lead (OPL) shall:

(1) Ensure that all officer application paperwork (to include ADDOCS and FINDOCS,) is managed, monitored and completed by the applicant and the Processing Division within the Officer Programs Department. Unless specifically directed otherwise by the OPO, officer recruiters (ORs) primary duties are to recruit applicants and Officer Processors (OPs) primary duties are to process the applications.

(2) Conduct a thorough review of each application kit (ADDOCS and FINDOCS) to be forwarded for consideration.

(3) Ensure at a minimum the following:

(a) All required application documents are complete, accurate and included as listed in the appropriate checklist. (b) Any discrepancies between documents are resolved prior to submission to COMNAVCRUITCOM.

(c) The OR and OPO have completed their review and made all necessary comments on the Application Processing and Summary Record.

b. Prior to submitting an application to COMNAVCRUITCOM, the OP, the OR, the OPL and the OPO or personnel with “By direction” authority file shall thoroughly review the file for completeness and accuracy. Each individual reviewing the application shall make any necessary comments in the

appropriate sections on the Application Processing and Summary Record (NAVCRUIT 1131/1), sign and date where indicated, confirming their individual review.

c. During the course of the review, the OR shall evaluate the information in the application, and using this information and his/her personal knowledge of the applicant, make an initial recommendation of the applicant's suitability for appointment. The OR shall record in writing his/her evaluation and recommendation on the Application Processing and Summary Record (NAVCRUIT 1131/1) together with all other items of information required therein.

d. When the complete application is forwarded to the OPO or XO/CO for signature (or personnel with “By direction” authority), it carries the OPO’s, OR’s and OPL/OP’s implicit assurance, by virtue of the initialed application checklist and written evaluations, that the application is in compliance with policy and processing regulations, and that the suitability recommendation is consistent with the factual material therein.

040103. FORMS, DIRECTIVES AND GUIDANCE. The following list of additional resources and directives shall be utilized when additional guidance is desired and/or required:

040104. APPLICATION PROCESSING AND SUMMARY RECORD. The Application Processing and Summary Record (NAVCRUIT 1131/1) shall be completed prior to submission for all applicants. Instructions for completing this form can be found in the status bar within the MS Word ® document.

040105. ACADEMIC DEGREE COMPLETION PLAN

a. The Academic Degree Completion Plan (NAVCRUIT 1131/4) is required for all applicants applying for BDCP, CEC, NUPOC, NPS Instructors, NR Engineers, NCP, and HSCP.

b. All degree plans shall include course number, title and number of hours listed by specific term. Generic lists of courses needed for graduation are not acceptable. The degree plan shall indicate major and expected graduation date. The form must be signed by the applicant's academic advisor or other appropriate school official. It is acceptable to list elective courses without showing specific course number/title.

c. On-line academic degree plans are acceptable, provided they include the specific information listed in items 1 through 3 on page 2 of the NAVCRUIT 1131/4.

040106. NUCLEAR PROGRAMS ACADEMIC HISTORY SUMMARY. The information included on the Nuclear Programs Academic History Summary (NAVCRUIT 1131/7) shall cover the entire period since the applicant graduated from high school. Ensure all periods that the applicant was not in school are included and explain what he/she was doing during that time (i.e., enlisted in military, working to pay for school, etc.). For any period other than summers when applicant was not attending school full-time (typically 12 hours), explain why. Include all schools attended since high school, whether or not the credit counted towards the applicant’s current degree.

040107. ANTHROPOMETRIC MEASUREMENTS. The A nthropometric Measurements (NAVCRUIT 1131/12) form shall be included for all pilot (1395X) and NFO (1375X) applicants. Anthropometric measurements are used to determine which airframes candidates may qualify to fly. These measurements are based on several factors including height, weight, functional reach, buttocks knee length and sitting height, which are used to ensure the aviators safety while sitting in the cockpit as well as emergency ejections.

040108. APPLICATION FOR COMMISSION

a. The Application for Commission or Warrant Rank, U. S. Navy or Navy Reserve (NAVCRUIT 1131/2) shall be completed by all applicants prior to submission. Instructions for completing this form can be found in the status bar within the MS Word ® document.

b. Completion of this form is self-explanatory; however, dependent upon the applicant's answers to individual questions, additional documentation may be required.

040109. APPLICATION FOR NURSE CORPS ACCESSION BONUS PROGRAM LETTER. This letter of application (Exhibit 0401-01) shall be

completed by nurse corps applicants desiring to be considered for the accession bonus program.

040110. APPLICATION FOR RECALL TO EXTENDED ACTIVE DUTY. Applicants for recall to active duty in the Medical Corps, Dental Corps, Nurse Corps, and Medical Service Corps shall complete the Application For Recall to Extended Active Duty (NAVPERS 1331/5).

040111. CERTIFICATE OF RELEASE OR DISCHARGE FROM ACTIVE DUTY/ PRIOR SERVICE RECORD RELEASE REQUEST

a. Certificates of Release or Discharge from Active Duty (DD Form 214)/Prior Service Record Release Request (NAVCRUIT 1070/1) are required whenever an applicant has indicated prior active or inactive duty in another service (including State National Guard and U.S. Public Health Service).

b. The Prior Service Record Release Request (NAVCRUIT 1070/1) shall be completed by the processor and forwarded to the following address:

c. Copies of Certificates of Release or Discharge from Active Duty (DD Form 214, #4 Copy) or Statement of Service from the USPHS, as applicable, shall be forwarded to COMNAVCRUITCOM as directed.

040112. CHANGE OF NAME STATEMENT. When an applicant does not use the name listed on his/her birth certificate or Evidence of Citizenship form (see Article 040121), the following additional information shall be submitted (not required for nicknames):

a. Record of court procedure legally changing name, or

b. Change of Name Statement (NAVCRUIT 1131/14)

040113. CONDITIONAL RELEASE. Members currently serving on inactive or active duty or enrolled in an ROTC program of another service may be considered, provided the individual submits a Conditional Release (DD 368) with the application. Navy recruiting personnel shall not initiate such requests. Verify applicant obtains authorizing signature from appropriate source.

040114. CONTINGENT RESIGNATION LETTER. For applicants holding a commission, see Exhibit 0401-02.

040115. CURRICULUM VITAE. Applicants for all Medical Programs, Chaplain Corps and JAG Corps shall provide a curriculum vitae (if still in school) or a resume.

040116. DISCLOSURE OF SOCIAL SECURITY NUMBER - PRIVACY ACT STATEMENT. The Disclosure of Social Security Number – Privacy Act Statement (NAVCRUIT 5211/1) shall be read and signed by all officer applicants. The original will be retained in the applicant’s residual file and a copy included in the application.

040117. DIVE PHYSICAL READINESS TEST. Applicants for Special Warfare (1185) or Special Operations (1195) shall complete the physical fitness-screening test using this form. Completion of the Dive Physical Readiness Test (NAVCRUIT 1131/6) form is self-explanatory.

040118. DUTY PREFERENCE FOR MEDICAL PROGRAMS. The Duty Preference for Medical Programs (NAVCRUIT 1131/16) sheet shall be completed by all direct appointment Medical Program applicants. All medical programs students (MC HPSP, DC HPSP, DC HSCP, MSC HSCP and NC NCP) shall complete the Duty Preference sheet within six months of graduation and forward it to the respective Program Manager. Completion of the sheet is self-explanatory.

040119. DOCUMENTATION OF REQUEST FOR ECCLESIASTICAL ENDORSEMENT APPROVAL

a. An Ecclesiastical Endorsement (DD 2088) verifies that the applicant is a fully qualified clergy from a recognized religious organization; actively employed in a denominationally approved vocation; and recommended as being spiritually, morally, intellectually and emotionally qualified to represent the applicant’s religious body in the Chaplaincy of the Armed Forces. An ecclesiastical approval attests that a seminary student is considered a potential candidate for ordination. It is the prerogative of the individual church or religious body to establish its own requirements. Each denomination has specific application procedures for endorsement consistent with the policy of that particular church body.

b. The Issuing of an Endorsement. In connection with an application for appointment in the Chaplain Corps/Theological Student Program, a copy of the Ecclesiastical Endorsement

(DD 2088) or Documentation of Request for Ecclesiastical Endorsement (NAVCRUIT 1131/10) shall accompany all applications forwarded by the NAVCRUITDIST. In many instances, the application for the Navy Chaplain Corps Program, and the request for Ecclesiastical endorsement, do not occur simultaneously. Districts shall forward applications if the applicant provides a completed Documentation of Request for Ecclesiastical Endorsement (NAVCRUIT 1131/10) indicating the date of request for ecclesiastical endorsement and the name of the endorsing agent/office contacted. An application will not be considered by the selection board until the Ecclesiastical Endorsement is received in the Office of the Chief of Chaplains. Forwarding an application without ecclesiastical approval can extend the time before an applicant is notified of the final selection decision.

c. Requesting Endorsement or Approval. The point of contact in the endorsement system between individual religious bodies and the Department of Defense is the ecclesiastical endorsing agency. The endorsing agent is a descriptive term used by all three services to identify the committee, commission, board, or diocesan office authorized by each organization and formally recognized by the Department of Defense to issue endorsements or approvals. For information regarding ecclesiastical endorsements, contact the Chaplain Programs Manager, COMNAVCRUITCOM (N342).

040120. ELECTRONIC PERSONNEL SECURITY QUESTIONNAIRE. This form shall be completed in accordance with Article 040201.

040121. EVIDENCE OF CITIZENSHIP. The Evidence of Citizenship (NAVCRUIT 1100/25) form shall be completed by all officer applicants who are naturalized citizens. Certificates of Naturalization shall not be photocopied. Completion of the form is self-explanatory.

040122. FAMILY CARE PLAN CERTIFICATE. Any applicant that is a single parent with physical custody of dependent children or any applicant with a military spouse and dependents shall complete a Family Care Plan Arrangements (NAVPERS 1740/7) for review by the NAVCRUITDIST Commanding/Executive Officer. The CO/XO shall consider the 1740/7 and, based on their review, endorse a Family Care Plan Certificate (NAVPERS 1740/6). Such applicants shall be advised by the OPO that all individuals accepted for naval service are expected to maintain a high degree of commitment to his/her professional responsibilities, and that no exemptions from policies or preferential treatment, to include duty assignments, are extended on the basis of dependency status.

040123. FINGERPRINT CHARTS. Fingerprints shall be submitted in accordance with Section 2 of this chapter.

040124. GPA AND CLASS STANDING. Enter cumulative GPA from most recent college attended. GPAs not computed on a 4.0 scale shall be normalized to a 4.0 scale. For applicants that are enrolled in a Master’s or Doctorate program, the Master’s or Doctorate GPA shall be used. For all medical student and MSC programs, see Article 060160.

040125. HEALTH PROFESSIONAL QUALIFICATIONS SUMMARY

a. Health Care Providers deal in direct patient care and include:

b. As part of the application process, recruiters shall verify license/registration with state and or national licensing boards and forward the Health Professional Qualifications Summary (NAVCRUIT 1131/11) as part of the application.

040126. HSO JACKSONVILLE CREDENTIALING PACKAGE. The Nurse Corps (NC) Individual Credentials File (ICF) Division of HSO Jacksonville reviews credentialing packages for all medical programs officer recall applicants. In addition, all healthcare provider recall credentialing packages are sent to Armed Forces Institute of Pathology (AFIP) for verification prior to forwarding to the professional review boards (see HSO JAX website: https://nhso.med.navy.mil/newimage/Privileging/ResCredForms.htm).

040127. HOLD HARMLESS AGREEMENT AND RELEASE FROM LIABILITY CERTIFICATE. All applicants for OCS shall read and sign the Hold Harmless Agreement and Release from Liability Certificate (NAVCRUIT 1100/27) prior to participating in any physical fitness activities, including testing.

040128. INTERVIEWS. (See appropriate application checklist for communities requiring professional interviews.)

a. The primary objective of the professional interview is to determine the applicant’s potential as a naval officer. The interview has secondary functions to include:

(1) Providing an opportunity to inform the applicant of the details of the program for which he/she is applying;

(2) Informing the applicant about the training he/she will receive; and

(3) Providing the applicant an opportunity to answer questions.

b. The number of interviews required varies among programs. The application checklist shall be reviewed to ensure the correct procedures are followed.

c. The majority of interviews shall be completed by an officer holding a paygrade of 03 or higher.

d. NROTC applicants shall have a minimum of one interview by an officer recruiter with the paygrade of E7 or above.

e. At least one of the interviewers shall be from an officer designator comparable to the program for which applied.

f. Inactive Reserve officers may be requested to give interviewing assistance, where appropriate, such as in the case of NROTC four-year scholarship program applicants.

g. With the exception of all Medical Programs, active duty applicants may be interviewed by personnel assigned to recruiting duty. Personnel assigned to recruiting duty in the Officer Programs Department are not authorized to interview reserve officer program applicants.

h. In no case shall an applicant for an officer program be interviewed by other than as indicated above.

i. In no case shall an interview appraisal form be prepared based on input, either written or oral, from another source.

j. Regardless of the interview method chosen, all interview appraisals submitted with an application shall be the result of

either close association (the one informal interview allowable) or of formal observation (face-to-face personal contact).

k. Interview Procedures

(1) As mutually agreed upon prior to the interview, either the senior interviewing officer or the individual officer most closely associated with the applicant, shall ensure that during the course of the interview, the applicant is advised of the following:

(a) The contents of the applicable service agreement.

(b) The type and duration of training to be received, including Physical Readiness Test (PRT) requirements at OCS and OIS.

(c) That after processing and approval, the application will be forwarded to NAVCRUITCOM for final action.

(d) That written notification to the NAVCRUITDIST is required for withdrawal of an application or declination of selection, stating the reasons for the request.

(e) That marginal applicants should be prepared for possible rejection.

(2) The interview room shall be comfortable and provide sufficient privacy.

(3) Interviewers shall have information concerning the applicant's background, test scores, and physical qualifications prior to the interview. The application folder of each applicant shall be furnished to the interviewers for review prior to commencement of the interview.

l. Recording of Interview Results

(1) Written interview results shall be made using the Interviewer's Appraisal Sheet (NAVCRUIT 1131/5). Interview results shall not be recorded during the course of the interview. However, each interviewer shall evaluate carefully and impartially the characteristics of the applicant and record evaluations while the details of the interview are still fresh in his/her mind.

(2) Any applicant who speaks English as a second language shall complete a portion of the interview telephonically to access the applicant's level of proficiency and clarity using English as their primary means of communication. The interviewer's comments shall include a statement as to the applicant's ability to communicate in English.

(3) Appraisals legibly handwritten in ink are acceptable, but not recommended.

040129. LETTER OF ACCEPTANCE

a. All student programs require letters of acceptance at appropriately accredited institutions. Such letters shall be signed by the school registrar or other similar school official.

b. Letters of acceptance are required for the following applicants:

(1) BDCP and CEC applicants who have most recently attended two-year schools shall have a letter of acceptance from a regionally accredited four-year institution.

(2) Applicants for four-year Medical and Dental Student programs shall provide a letter of acceptance to an accredited medical school.

(3) Applicants for the two and three year Medical and Dental student programs shall provide a letter of enrollment/good standing. Applicants for FAP shall have a letter of acceptance for residency.

(4) NCP shall have a Letter of Acceptance from an accredited college/university.

(5) JAG and Chaplain Students shall provide a letter of acceptance to an accredited law school or divinity school as appropriate. JAG Students shall provide class standing.

040130. LETTER OF GOOD STANDING. A letter of good standing with the school/residency program is required for HPSP and FAP applicants who are currently enrolled in school or residency program.

040131. LICENSE. Certified copies of licenses are required for officer applicants applying for any direct appointment medical, JAG or Chaplain program.

040132. LICENSURE VERIFICATION FOR MEDICAL PROGRAMS APPLICANTS. The licensing of all health care provider applicants shall be verified with the appropriate state/national licensing board using the Licensure Verification for Medical Programs Applicants (NAVCRUIT 1131/13). All Nurse Corps applications shall include a report of inquiry with a licensing agency from all states in which the applicant has ever held a license.

040133. VERIFICATION OF NURSING EDUCATION. All Nurse Corps applications shall include a report of inquiry of their nursing school(s) using the Verification Of NLN/CCNE Nursing Education (NAVCRUIT 1131/17). This information can be located on the internet at the NLN and CCNE websites.

040134. PROOF OF GOOD STANDING. JAG workforce applicants must provide proof of good standing with their BAR.

040135. MAXIMUM AGE STATEMENT OF UNDERSTANDING. All Medical Corps (MC) and Dental Corps (DC) officers who will be beyond their 40th birthday, and all Medical Service Corps (MSC) and Nurse Corps (NC) officers who will be beyond their 35th birthday on the day of commissioning, are required to acknowledge the potential limitations on their career created by their age by reading and signing the Maximum Age Statement of Understanding contained within the respective Service Agreement.

040136. MILITARY SERVICE SUMMARY. Section V of the Application for Commission (NAVCRUIT 1131/2 - see Article 040108), Military Service Summary, summarizes information contained in an applicant’s Prior Service Record or in reference to current military service for use in ADDOCS. All applicants shall complete this section. In lieu of completing an entire Application for Commission, Recall applicants shall complete this section. The blocks in item 1 refer to the associated blocks on DD Form 214 and shall be filled in with the exact same information. Net Active Service should be computed using only information included on DD Form 214 and other official documentation and filled in at the top of this section. Performance Evaluations should be submitted from the three most recent regular reporting periods. Frocking and Transfer evaluations may be included, but shall not count as one of the three required unless submitted concurrently with regular reporting requirements.

040137. MOTIVATIONAL STATEMENT. Section VIII of the Application for Commission (NAVCRUIT 1131/2 - see Article 040108),

Motivational Statement, shall be completed by all applicants and included in the ADDOCS. In lieu of completing an entire Application for Commission, Recall applicants shall complete the Motivational Statement.

040138. NUCLEAR PROPULSION PROGRAM SECURITY SCREEN. All nuclear officer program applicants who are not native-born U.S. citizens, or who have any immediate relatives who are not native born U.S. citizens, shall complete the Nuclear Propulsion Program Security Screen (NAVCRUIT 1131/9) as part of their application. Completion of the statement is self-explanatory.

040139. NUCLEAR PROPULSION OFFICER CANDIDATE LETTER OF INTENT TO ENLIST. All nuclear officer programs participants shall indicate their intent to enlist by submitting a signed NUPOC Letter Of Intent To Enlist (Exhibit 0401-03) if selected by the Deputy Assistant Secretary of Naval Reactors. The original letter shall be included in the application.

040140. NUCLEAR PROPULSION PROGRAM PRE-SERVICE DRUG ABUSE STATEMENT. All nuclear officer program applicants, regardless of admitting to drug use or not, shall complete the Nuclear Propulsion Program Pre-Service Drug Abuse Statement (NAVCRUIT 1131/8) as part of their application. Completion of the statement is self-explanatory.

040141. OFFICER CANDIDATE SCHOOL COMMITMENT TO SUCCESS.All applicants for OCS shall view the video “Commitment to Excellence”, read and complete the OCS Commitment to Success Statement (NAVCRUIT 1131/15). Completion of the statement is self-explanatory.

040142. PHOTOGRAPH. All JAG direct appointment or JAG Student applicants shall submit a photograph with their application. The photograph shall be full-length, either 5X7 or 8X10, and the applicant shall be in business attire.

040143. PHYSICAL READINESS TEST. All applicants for OCS shall complete the Navy Physical Readiness Test as outlined in OPNAVINST 6110.1. All selectees will be informed of the requirements to pass the PRT at OCS and OIS. Completion of the Physical Fitness Test for OCS Programs (Greater than 5000 ft) (NAVCRUIT 1131/28) form or the Physical Fitness Test for OCS Programs (Less than 5000 ft) (NAVCRUIT 1131/29) form, depending on the geographical location the test is being performed in, is self-explanatory. The resultant printout shall be forwarded with the application.

040144. POLICE RECORD CHECKS

a. Police Record Checks are required on all applicants who self-admit to criminal or juvenile offenses. A written statement with detailed information on the offense and lessons learned is required for any offense (see Article 020105).

b. The application may be forwarded prior to completion of the PRCs (DD Form 369). However, PRCs must be completed prior to enlistment/appointment. If, during the investigation, a PRC reveals any police or juvenile involvement not listed by the applicant, a statement from the applicant regarding the offense, and the reason for its omission, shall be included in the application.

c. PRCs shall not be forwarded to law enforcement agencies of foreign countries. If the applicant has indicated that a foreign record exists for any offense other than minor traffic violations, a request for a foreign police check shall be made to the Director of Naval Intelligence (DNI) for action. Where there is clear indication that rejection will ultimately occur, based on locally available information, rejection shall be effected without requesting a foreign police check from DNI. The results of foreign police checks shall be forwarded directly to COMNAVCRUITCOM by DNI.

d. All recruiting activities that originate out of town PRCs are authorized to utilize law enforcement telecopy/ teletype facilities where access is feasible and permitted in place of the DD Form 369. The hard copy of the request and response message are acceptable for verification of completion of the record check.

040145. PRIOR SERVICE RECORD. Applicants with prior or active military service shall submit the #4 copy of their DD Form 214, all fitness reports/evaluations, page 4’s, page 5’s, page 9’s and any derogatory information with their officer application.

040146. PROFESSIONAL CERTIFICATES/CERTIFICATION

c. The purpose of gathering personal data regarding an applicant is to provide information about the applicant's leadership potential, character, and integrity from sources other than the applicant. Any unauthorized disclosure of information provided in response to requests for personal data not only betrays the trust that an individual has placed in the Navy, but jeopardizes the reliability of future responses. No applicant shall be told the nature of any reply to an inquiry, nor shall he be told he has been disapproved for reasons of an unfavorable investigative report.

d. Employer references. Each employer listed on the Application for Commission and the Electronic Personnel Security Questionnaire (EPSQ) including part-time employers, for three years immediately preceding the date of application shall be queried using the DD Form 370. The following guidance applies:

(1) If requested by the applicant, a DD Form 370 shall not be forwarded to the applicant's current employer (e.g., the applicant feels it might jeopardize their job). If the current employer is not queried, it shall be annotated on the Application Processing and Summary Record (NAVCRUIT 1131/1).

(2) Officer recruiting processors shall complete the front of the employment reference and mail it to the appropriate employer.

e. Military References. The Commanding Officer of all applicants who are currently serving on active or inactive duty in non-Navy branches of the Armed Forces shall be requested to provide a reference utilizing DD Form 370.

f. Peer References. One peer reference is required for officer applicants applying for the Chaplain program, utilizing DD Form 370.

g. Character References. Only when employer references are not available shall the applicant be required to submit character reference(s) utilizing DD Form 370.

h. Congressional/Political References. Applicants inquiring about such endorsements or reference shall be advised that they are not necessary and will be favorably considered by the selection board only when the individual providing the reference has had personal knowledge of the applicant prior to the time of application.

In lieu of completing an entire Application for Commission, Recall applicants shall complete Section IV of the Application for Commission (NAVCRUIT 1131/2 - see Article 040108), Security Information, summarizes information contained on the EPSQ and prior drug use. This section shall be completed by recall applicants for use in ADDOCS. Completion of this section of NAVCRUIT 1131/2 is self-explanatory. Additional comments may be required, depending on given responses. A “Yes” response in block 1b or any responses in Column II (right column) of the Yes/No questions require expanded explanations on section VI, Remarks. If explanation was required, Section VI shall be attached to section IV and forwarded with the ADDOCS.

040151. SF86 AGENCY USE FORM. Information for completion of the appropriate SF86 Agency Use Form is contained in the SECNAV 5510 instruction. See Article 040201 for further instructions.

040152. TRANSCRIPTS

a. Accredited Institutions. Officer candidates for all programs shall, prior to reporting for officer candidate training or prior to acceptance of commission, possess a baccalaureate degree from a college or university that is listed in the Directory of Accredited Institutions of Post Secondary Education or the Higher Education Directory, published annually by Higher Education Publishing Incorporated. The college or university shall be accredited by the New York State Board of Regents or the agencies listed above. Exceptions to this will be reviewed on a case-by-case basis by COMNAVCRUITCOM. Completion of all work for a degree at such institutions, together with written certification by the college/university that the degree is to be conferred later, is also acceptable.

b. For all medical programs, contact respective Program Manager (PM) for lists of accredited colleges/universities.

c. Non-accredited Institutions

(1) Officer program applicants from colleges/ universities not accredited by one of the seven regional accrediting agencies may be considered for Navy officer programs if the non-accredited institution's credits are fully accepted by at least three regionally accredited institutions. The following example is an acceptable statement that may be signed by the registrar, admissions officer, or any other appropriate official

of a regionally accredited institution as long as it contains the school seal:

"This is to certify that (name of regionally accredited institution) accepts all credits by transfer from (name of non-accredited institution) in the same manner that it accepts credits from institutions accredited by regional associations. (Name of regionally accredited institution) also admits graduates of (name of non-accredited institution) to graduate study if they meet the same standards that graduates of regionally accredited colleges/universities meet."

(2) On a case-by-case basis, an applicant who has received a baccalaureate degree from a non-accredited college/university may be processed if he/she has been accepted by a regionally accredited institution as a candidate for a master's or higher-level program of study.

d. Graduates of Foreign Institutions. Foreign college or university graduate applicants shall furnish an official translated transcript indicating that he/she has been awarded a baccalaureate degree. The degree must be certified in writing by an international consulting firm, the NAVCRUITDIST Education Specialist, or a COMNAVCRUITCOM Education Specialists (N35). A written statement regarding accreditation of the foreign institution compared to a degree granted by an accredited college and university in the United States is required. Applications receiving positive certification shall be submitted. Questions concerning international consulting firms shall be directed to the COMNAVCRUITCOM (N35).

e. For graduates of foreign medical, dental, medical service corps programs, or nurse corps programs, contact the appropriate COMNAVCRUITCOM program manager regarding accreditation or reciprocal agreements.

f. Official Transcripts. A transcript is considered "official" only if issued by the original source, sent directly from the educational institution in question, and clearly identified as being an official document of that institution; i.e., marked by a raised or colored seal of the institution of higher learning.

g. All transcripts shall be official transcripts. Transcripts stamped "issued to students" are not acceptable unless they have been sent directly from the school to the

NAVCRUITDIST in a sealed envelope. In this case, the OR shall annotate on the Officer Summary Sheet the process by which the transcripts were obtained.

h. Transcripts shall be submitted for each post secondary institution the applicant has attended.

i. Applicants for Medical programs (less students) shall include a statement regarding their participation in continuing education and statements from appropriate training program officials regarding the completion of professional or certification training.

j. All Dental Corps applicants shall provide one certified copy of their dental school diploma.

040153. VERIFICATION OF BIRTH/BIRTH CERTIFICATE

a. All applicants for officer programs shall furnish a birth certificate or provide official verification of birth.

(1) U.S. Birth Certificate. Enclose “Certified to be a true copy” signed and dated by the person verifying authenticity in the application.

(2) Non-U.S. Birth Certificate. Enclose “Certified to be a true copy” signed and dated by the person verifying authenticity in the application along with an English translation of birth certificate. The translation shall be certified by the translator and shall include the translator’s printed name, signature and date. This translation cannot be completed by the applicant.

b. If the birth certificate cannot be produced, the Request for Verification of Birth (DD Form 372) can be used as primary evidence by completing the form per the instructions contained thereon and forwarding it to one of the agencies listed below. A copy of the DD Form 372 shall be retained in the applicant's residual file pending response. Upon receipt of the agency verified DD Form 372, replace the unverified copy with the agency verified DD Form 372 in the residual file and enclose the original in the application.

(1) State Bureau of Vital Statistics or State Health Department.

(2) County Department of Public Health, County Clerk's Office, or County Registrar's Office.

(3) City Department of Public Health, City Clerk's Office, or City Registrar's Office.

(4) In the event the agency states that no birth record exists, a combination of secondary evidence, such as a U.S. passport (or one in which the individual was included), baptismal certificate, certificate of circumcision, hospital record, separation documents of prior service personnel, affidavits of persons having personal knowledge of the facts of birth, primary school records, family Bible entries, early census, newspaper files or insurance papers, any of which must have been created as close to the time of birth as possible, and which may be used to verify the applicant's date and place of birth shall be reviewed by the NAVCRUITDIST Commanding Officer, or an officer designated in writing by the Commanding Officer, for determination of enlistment eligibility. Upon verification, the officer shall prepare a DD Form 372 and indicate in Section II the circumstances of how the verification was made (e.g., "Determination based on review of hospital records and DD Form 372 by ." Such entry shall be signed and dated by that officer in the "Verified by" block.

c. The DD Form 372 is not to be used to obtain birth record information from the U.S. Citizenship and Immigration Services (USCIS), formerly the Immigration and Naturalization Service (INS).

040154. EVIDENCE OF CITIZENSHIP

a. Applicants for all officer programs shall be citizens of the United States and shall submit positive proof of citizenship at the time of application.

b. Native born citizens. Applicants born in the geographical United States (50 states, Guam, Puerto Rico, Virgin Islands and the Northern Mariana Islands) are considered native born citizens.

c. Applicants born outside the United States of U.S. citizen parents. One of the following verification forms can be sighted and a DD Form 372 completed citing circumstances of how the verification was made.

Applicants who were born in the Republic of Panama of U.S. citizen parent(s) claiming U.S. citizenship shall have citizenship verified in the same manner as any other applicant born abroad of U.S. citizen parent(s).

(7) Applicants unable to produce one of the above, or other documentation indicating naturalized U.S. citizenship, shall not be processed.

d. Derived citizenship or naturalized citizens

(1) Applicants who have either completed naturalization proceedings on their own behalf or claim to derive citizenship, though alien-born, from the citizenship/naturalization of their parent(s), must present a Certificate of Citizenship, a Certificate of Naturalization (both of which are issued by USCIS, formerly INS, or a U.S. passport, or one in which the individual was included, as proof of citizenship. The processing office will sight one of these forms and complete the Evidence of Citizenship (NAVCRUIT 1100/25) form and return the certificate to the applicant.

(2) In the case of those applicants who claim to derive citizenship from the status of their parent(s), and who can offer no proof other than documentary evidence of their parent(s)' status, the following can be used:

(a) Document Verification Request (INS G-845) can be used by the applicant to request verification of naturalization or certificate of citizenship, or

SECTION 3 - APPLICANT TESTING

040301. CONTROL OF TESTING MATERIAL

a. Test Control Officer (TCO). To ensure the proper level of accountability for testing materials, the Officer Programs Officer (OPO) shall be designated as the TCO in writing by the NAVCRUITDIST Commanding Officer (see Exhibit 0403-01, Test Control Officer Designation Letter. When Navy Officer Programs testing materials are to be maintained at a Navy Officer Recruiting Station (NORS), the NORS Officer-In-Charge (OIC) (normally a commissioned officer) shall be designated as the Testing Materials Sub-Custodian in writing by the NAVCRUITDIST Commanding Officer (see Exhibit 0403-02, Testing Material Sub-Custody Letter). (2) Ensure the requisition, storage, issuance, safeguarding and accounting of all officer programs testing materials within the District.

(3) Ensure compliance with security and handling regulations at outlying recruiting offices prior to recommending that the NAVCRUITDIST CO authorize sub-custody of testing material.

(4) Ensure all sub-custodians and Test Administrators are thoroughly familiar with security and handling regulations prior to recommending their appointment by the CO as custodians and/or test administrators.

(5) Be thoroughly familiar with the references for administration, handling, accountability, scoring, and safeguards of the Aviation Selection Test Battery (ASTB). (6) Conduct quarterly inventory and page check of all test booklets assigned by sub-custody. This inventory shall be documented in the Test Control Log. This inventory meets the requirements of the sub-custodian monthly inventory and page check.

(7) Conduct an inventory of all testing materials prior to any changes in custodians. This inventory shall be documented in the Test Control Log.

(8) Conduct an additional inventory of all testing materials at any time a suspected compromise of the security container may have occurred. This inventory shall be documented in the Test Control Log.

c. Sub-Custodian Responsibilities. The sub-custodian shall:

(1) Maintain adequate test security provisions, and for the storage, issuance, safeguarding and accounting of all officer programs testing materials assigned to that NORS.

(2) Conduct monthly inventory and page check of all test booklets assigned by sub-custody. This inventory shall be documented in the Test Control Log.

(3) Be thoroughly familiar with the references for administration, handling, accountability, and safeguards of the ASTB.

040302. AVIATION SELECTION TEST BATTERY

a. Testing Overview. The Aviation Selection Test Battery (ASTB) was developed by statistical methods to predict the success of students in aviation officer training programs. The Navy, Marine Corps, and Coast Guard use the complete test battery as a primary selection instrument for aviation programs. The ASTB is available both in hardcopy and online formats. The online version is available via the APEX.NET platform and is only for use at NAVCRUITDIST and NORS locations meeting minimum connectivity requirements.

(1) Applicants for Officer Candidate School, as well as for various direct appointment programs as specified elsewhere in this manual, will be administered applicable portions of the ASTB either via hardcopy testing or online testing through APEX.NET.

(2) The ASTB should be given in its entirety to all applicants who indicate an interest in aviation programs. The ASTB is divided into six parts: (1) Math Skills Test (MST); (2) Reading Comprehension Test (RCT); (3) Mechanical Comprehension Test (MCT); (4) Spatial Apperception Test (SAT); (5) Aviation & Nautical Information Test (ANIT); and (6) Aviation Supplemental Test (AST). From these six parts, four scores are derived, (1)

Academic Qualification Rating (AQR), (2) Pilot Flight Aptitude Rating (PFAR), (3) Flight Officer Aptitude Rating (FOFAR), and (4) Officer Aptitude Rating (OAR).

(3) The Officer Aptitude Rating (OAR) portion of the test, the MST, RCT and MCT sections, may be given to individuals who are not interested/not qualified for aviation programs. This score is applicable only to various non-aviation designators as specified in the Program Authorizations. An applicant who has taken the MST, RCT and MCT only to obtain an OAR score may subsequently take the SAT, ANIT and AST within 90 days of taking the MST, RCT and MCT to obtain a complete set of scores. This is a merged test and is considered to be a single set of ASTB scores.

b. ASTB Administration. The Naval Operational Medicine Institute (NOMI) (Code 341) generates all procedures governing the administration, handling, accountability, scoring and safeguards for the ASTB.

(1) All personnel responsible for ASTB test administration and custody shall have read and be thoroughly familiar with the references for administration, handling, accountability, and safeguards of the ASTB. These references can be obtained from NOMI (Code 0341) at commercial 850-452-2257x1060, or DSN 922-2257x1060. The references for these procedures are:

(a) Examiner’s Manual for The United States Navy and Marine Corps Aviation Selection Tests (NOMI P-5098C)

(b) APEX.NET Version 1.1 Examiner’s Guide (NOMI P-5098C Part II)

(2) Handling of all testing materials, completed answer sheets and APEX.NET Login ID and passwords shall be limited to personnel designated in writing as either Test Control Officers, (Exhibit 0403-01), Officer Programs Test Administrators, (Exhibit 0403-03) or personnel authorized sub-custody of ASTB material, (Exhibit 0403-02). Personnel authorized custody or sub-custody use of APEX.NET by their NAVCRUITDIST Commanding Officer should contact NOMI (Code 0341) at commercial 850-452-2257x1060, or DSN 922-2257x1060 to request Login IDs and passwords for APEX.NET.

c. ASTB Accountability. Those activities authorized by NOMI to hold and administer testing materials have certain specific responsibilities. Any designated individual accepting custody of

the testing materials specifically accepts the responsibility for the following:

(1) Security. Test booklets and filled in test answer sheets are classified "FOR OFFICIAL USE ONLY". These materials shall be handled in all respects as to transmission, use, stowage, and control of dissemination in accordance with current security instructions.

(2) Maintenance of an adequate supply of testing materials and mailing materials.

(3) Exact conformity to the procedures outlined herein, including the above noted references.

(4) Observance of the limitations prescribed herein and the above references concerning discussion of test results with examinees.

d. Sub-Custody of ASTB

(1) Activities authorized sub-custody (i.e. NORS) of ASTB test booklets, answer sheets and/or APEX.NET accounts other than the NAVCRUITDIST Headquarters shall be designated in writing by the NAVCRUITDIST Commanding Officer only after the TCO has verified the adequacy of security provisions, testing facilities, and test administration personnel.

(2) Sub-custody of ASTB testing materials to non-recruiting activities, including ROTC units, is prohibited. These activities shall request ASTB testing materials through NOMI.

e. Handling of ASTB Materials

(1) Only personnel designated in writing by the Commanding Officer are authorized to handle testing materials and completed answer sheets.

(2) At a minimum, all testing materials including APEX.NET Login ID and password (if written down) shall be stowed in a safe or cabinets (preferably metal) secured by combination locks. Where feasible, cabinets should be in secured spaces.

(3) Hard copy ASTB test material may be used during travel, however a system of sub-custody shall be established for all testing materials (see Exhibit 0403-02). Documentation of

testing material taken on road trips shall be made in a Test Control Log, and such material shall be subject to all routine and specific inventory requirements as delineated in the above references. Appropriate security measures shall be taken to preclude loss or compromise. Specifically, testing material shall not be stowed in unattended rooms unless the room is locked and material is placed in a locked container, i.e., luggage, attaché case, etc. Testing material shall not be stowed in unattended automobiles unless the automobile is locked and testing material is placed out of view, preferably in the trunk. Testing material will be kept in a distinctive container or folder to distinguish it from other papers.

(4) Test booklets shall be inventoried as follows:

(a) Test booklets used for a testing session shall be inventoried and page checked by the Test Administrator before and after that testing session.

(b) Test booklets shall be inventoried (but not page checked) when the Test Administrator or TCO opens the safe/cabinet for purposes other than test administration.

(c) A monthly inventory and page check of all test booklets shall be conducted at least once during each calendar month. Inventories of individual testing booklets used for a test session do not meet this requirement. See Article 040301.

(d) Results of the above inventories, including notation of sub-custody material, shall be recorded in a testing materials log and signed by the test administrator conducting the inventory. The monthly inventory and page check shall be specifically marked as such in the Test Control Log.

f. Applicant Counseling Limitations

(1) Officer programs recruiting, processing, and testing personnel are prohibited from disclosing any information concerning the tests other than a general overview.

(2) Recruiters may advise applicants to review standard study guides for military officer candidate programs, which are available in most libraries and commercial bookstores.

(1) The requirements for scoring and mailing of the ASTB are contained in the above references.

(2) Local scoring of the ASTB and OAR is prohibited.

(3) Completed answer sheets shall be forwarded to NOMI per the above references.

(4) For ASTB tests administered via APEX.NET, immediate official scoring is available and can be printed for the applicant and the command.

(5) The official score sheet from NOMI, or the Commanding Officer’s Letter printed from APEX.NET, shall be included in the officer application as required by the Officer Application Checklist.

h. Loss or Compromise of the ASTB

(1) Upon discovery of loss or compromise of ASTB materials or APEX.NET Login IDs or passwords at Recruiting Command activities, NAVCRUITDIST personnel shall immediately contact NOMI (Code 0341), commercial 850-452-2257x1060, or DSN 922-2257x1060, and COMNAVCRUITCOM (N31).

(2) Commanding Officers shall direct an investigation of the circumstances surrounding the loss or compromise of testing material, and upon completion, provide a report to COMNAVCRUITCOM (N31) and COMNAVCRUITCOM (00IG) via the appropriate chain of command.

SECTION 4 - APPLICATION SUBMISSION GUIDELINES

040401. OVERVIEW

a. This section provides specific guidance for submitting an application for Active Duty Officer Programs.

b. All Active Duty officer applications, except for Medical Corps Direct Accession (DA), Medical Corps Recall, Medical Corps FAP and Medical Corps HPSP, are completed in two stages: Advance Documents (ADDOCS) and Final Documents (FINDOCS). Unless otherwise indicated, all ADDOCS and FINDOCS submitted to COMNAVCRUITCOM will be electronic copies (email or fax) of originals to expedite the selection process and minimize costs related to standard mail.

c. Complete Hard-copy application packages for Medical Corps Direct Accession (DA), Medical Corps Recall, Medical Corps FAP and Medical Corps AFHPSP shall be sent to One-Stop at COMNAVCRUITCOM Detachment, Medical Corps Program Office (One-Stop Shop) in Bethesda, MD. There are no ADDOCS for these programs.

040402. PERSONALIZED OFFICER RECRUITING AND TRACKING SYSTEM

a. Upon submission of an application for any program other than Four-Year NROTC, Nurse Option NROTC or Inter-Service Transfer, the OPL/OP shall:

(1) Enter the applicant into the PORT System as a "NEWREC". The PORT USER'S GUIDE provides detailed instructions on the use of this system.

(2) Upon completion of a NEWREC, review, print and staple a PORT SEEREC in the applicant's District file.

b. Upon receipt of the application at COMNAVCRUITCOM, all information concerning the status of the applicant shall be conveyed via PORT and email. It is essential that all data elements contained in a new record are complete and accurate, and that transactions entered at all levels concerning selection, enlistment, appointment, declination, discharge, etc., can be substantiated and are entered into the PORT System in a timely manner.

c. The District shall forward, via email, a PORT Record Action Request (NAVCRUIT 1131/46) to COMNAVCRUITCOM (N36) when a change to an applicant's PORT record is required.

d. Under no circumstances shall an applicant be notified of final selection until he/she appears in the PORT System as a "final select".

040403. DOCUMENT REPRODUCTION/PHOTOCOPY. The following documents are not to be reproduced photographically:

a. Driver’s license and automobile titles in certain states.

b. Passports, immigration papers and certificates of citizenship or naturalization (Evidence of Citizenship Form, shall be completed instead).

c. Birth certificates in certain states (Request for Verification of Birth Form shall be completed in these states).

d. Copyrighted material without permission of the owner of the copyright.

e. Selective Service registration certificates.

f. Merchant Marine License.

040404. TRANSLATION OF FOREIGN DOCUMENTS

a. All foreign documents submitted with ADDOCS/FINDOCS must be translated into English line-by-line prior to COMNAVCRUITCOM submission; a document summary is unacceptable.

b. Foreign documents shall not be translated by the applicant, applicant’s spouse or any family member.

c. Individuals conducting the translation shall verify, by signature, correctness and accuracy of the document. The following statement shall precede the required signature:

“I verify the translation on this document as being correct and accurate to the best of my knowledge.”

040405. SUBMISSION PROCEDURES

a. Advance Documents (ADDOCS). ADDOCS are comprised of the minimum required information on which a community can confer a recommendation for selection to COMNAVCRUITCOM. Refer to the Officer Application Checklist (NAVCRUIT 1131/3) for specific documents required as ADDOCS.

b. After initial contact with an officer recruiter, and indication by the individual that they desire to pursue an opportunity in the United States Navy, it is acceptable for a prospect to download ADDOCS forms or receive them by email or fax. These forms shall be completed and forwarded to the NAVCRUITDIST (by standard mail, email or in person). Multiple faxing of required documents is discouraged as re-faxing typically deteriorates legibility. Utilization of overnight delivery services is not authorized for these purposes. If a document received by COMNAVCRUITCOM is not legible, an error report will be generated requiring the District to provide a legible copy. Processing shall stop until the necessary document is provided. c. Once ADDOCS are received at COMNAVCRUITCOM (N36), the COMNAVCRUITCOM Processor shall:

(1) Review the application for completion and accuracy.

(2) When determined complete, enter a RECAPP date in the applicant’s PORT record.

(3) If ADDOCS are determined to be incomplete, notify the DISTRICT via email with a list of missing items and/or required actions.

d. If a District does not see any action 48 hours after submitting ADDOCS, they should call COMNAVCRUITCOM (N36) for verification of receipt.

e. Incomplete ADDOCS

(1) The District shall submit the necessary information for complete and accurate ADDOCS within 14 days from notification that the ADDOCS are incomplete.

(2) If ADDOCS are incomplete after 14 days, COMNAVCRUITCOM (N36) shall forward the ADDOCS to the Division

Director via the respective Program Manager (PM) recommending RECPRO "X".

f. Not Basic Eligibility Requirements (BERs). When ADDOCS are determined not to meet the BERs, COMNAVCRUITCOM (N36) shall forward the ADDOCS to the Division Director via the respective Program Manager (PM) recommending RECPRO "X".

g. FINDOCS. FINDOCS are the remaining documents required to complete an application that are not included in ADDOCS. FINDOCS are used to make a final selection decision by COMNAVCRUITCOM.

(1) Documents normally submitted as FINDOCS can be requested at anytime during the process at the request of the program manager. These documents are not required to be submitted again with FINDOCS.

(2) Once a professional recommendation for selection is received, the COMNAVCRUITCOM processor shall enter a RECPRO “Y” into the applicant’s PORT record.

(3) The District shall submit FINDOCS when an applicant receives a professional recommendation for selection from any community within 30 days. The time limit may be extended by official request from the NAVCRUITDIST to COMNAVCRUITCOM (N36).

(4) Upon PROREC "Y", the DISTRICT shall submit the PSI in accordance with Section 2. The type of PSI shall be based on the program for which the member has been recommended.

(5) FINDOCS shall not be submitted to COMNAVCRUITCOM prior to an applicant’s PORT record indicating a RECPRO “Y”, unless the COMNAVCRUITCOM Program Manager has requested specific documents.

(6) Once FINDOCS are received at COMNAVCRUITCOM, the COMNAVCRUITCOM Processor assigned shall:

(a) Review the application to ensure it is complete and accurate.

(b) When FINDOCS are determined to be complete and accurate, place ADDOCS and FINDOCS into one electronic folder and forward them to the appropriate program manager for review. There will not be another PORT entry showing RECAPP information. The COMNAVCRUITCOM processor will notify the District via email that FINDOCS are complete.

(c) When FINDOCS are determined to be incomplete, notify the DISTRICT via email with a list of required actions/missing documents.

(7) If the District does not receive verification that FINDOCS were received by COMNAVCRUITCOM after 48 hours from the time FINDOCS were submitted, the NAVCRUITDIST should call their respective COMNAVCRUITCOM processor.

(8) Incomplete FINDOCS

(a) The NAVCRUITDIST has 30 days to submit the necessary information for complete and accurate FINDOCS.

(b) FINDOCS incomplete after 30 days may be granted a total of 2 extensions, not to exceed 90 days.

(c) The NAVCRUITDIST shall request extensions in writing to the respective COMNAVCRUITCOM processor.

(d) FINDOCS that are still incomplete after 90 days shall be forwarded to the appropriate division officer recommending non-selection.

(9) While FINDOCS are not required to be submitted until professional recommendation from the specific community has been made, the timeframe in which they are completed is at the discretion of the NAVCRUITDIST. For instance, if an applicant must make a lengthy trip to meet with a recruiter for ADDOCS, the District may process the applicant's FINDOCS concurrently so that the applicant will not be required to make an additional trip. However, FINDOCS shall not be submitted to COMNAVCRUITCOM until required as indicated by a RECPRO “Y” or as requested by the COMNAVCRUITCOM program manager.

h. Receipt of professional recommendation does not guarantee final selection. Information included in FINDOCS could reveal characteristics about an applicant that are not consistent with final selection. These include applicants who are determined:

(1) Not physically qualified (NPQ) for commissioning.

(2) Not best qualified (NBQ) based on additional information in the FINDOCS.

i. Medical Review by COMNAVCRUITCOM (00M). After receipt of an application by the COMNAVCRUITCOM Processing Division (N36), medical examination documents are forwarded to the COMNAVCRUITCOM Medical Advisor (00M) for a determination of physical qualification. The determinations will be as follows:

(1) Physically Qualified (PQ)

(2) Not Physically Qualified (NPQ)

(3) Temporarily Not Physically Qualified (TNPQ). For TNPQ applicants, the processor will notify the District and the OR of requested documentation by COMNAVCRUITCOM (00M). The NAVCRUITDIST has 30 days to submit the requested information. After 30 days, the record shall be forwarded to the appropriate COMNAVCRUITCOM division officer recommending non-selection.

j. Nuclear and Chaplain Field Interview. A third application phase is required for Nuclear and Chaplain Programs. Once FINDOCS are received and evaluated at COMNAVCRUITCOM, all Nuclear and Chaplain applicants are required to attend an interview with representatives of each community. Upon successful completion of the interview, final selection will be rendered. Applicants who do not successfully complete the interview will be non-selected and processing shall be terminated.

k. Medical Corps Direct Appointment. Applicants shall be interviewed at one-stop shop in Bethesda, Maryland by the professional review board.

040406. APPLICATION FORMS AND CHECKLISTS. Chapter 6 contains additional guidance on all forms and comprehensive checklists. a. The OR and Officer Processor (OP)/Officer Processing Lead (OPL) shall review the kit using the appropriate application checklist. Prior to submitting ADDOCS and FINDOCS, the OR and OP/OPL (or personnel with “by direction” authority) shall initial and date the checklist.

b. Medical Corps and Medical AFHPSP program applications (applications sent to One-Stop) shall be mailed in a standard folder, with the original initialed Medical Corps Officer Programs Checklist (NAVCRUIT 1131/18) or Armed Forces Health Professions Scholarship (AFHPSP) Medical Corps Student Checklist (NAVCRUIT 1131/19) on the left inside cover of the folder.

c. For programs other than Medical Corps and Medical AFHPSP, completed checklists shall be maintained with the applicant's residual file at the District. A copy of the checklist does not need to be submitted to COMNAVCRUITCOM.

d. The first seven columns of the ADDOC/FINDOC checklist specifies required items for each program:

(1) “OCS” is used for all OCS programs, except Nuclear Programs.

(2) “NUC” is used for all Nuclear Programs.

(3) “CHC” is used for all Chaplain programs.

(4) “JAG” is used for all JAG programs.

(5) “DC/MSC/NC” is used for all Dental, Medical Service and Nurse Corps applications.

(6) “Stu/FAP” is used for all Medical Programs Student and FAP applications.

(7) “Rec” is used for all Medical Programs Recall applications.

e. Certain Medical Programs may require use of multiple columns for application completion (i.e., Medical Service Corps HSCP applications must use both the “DC/MSC/NC” and “Stu/FAP” columns).

f. An “A” in these columns indicates that the associated document is required to be submitted as ADDOCS.

g. An “F” indicates that the associated document is required to be submitted as FINDOCS.

h. An asterisk with a number indicates additional instructions that are listed below the table.

i. The “# COPY” column indicates the number of copies of each document that are required to be submitted to COMNAVCRUITCOM.

j. An “O” indicates that an original copy is required.

040407. FORWARDING OF APPLICATIONS

a. Once the NAVCRUITDIST has received the appropriate hardware and software, the following procedures shall be used to forward application documents to COMNAVCRUITCOM utilizing the Visioneer 9650 USB Scanner and Paper Port Deluxe 7.0:

(1) Load documents (no more than 50 pages at a time) face up in the feeder of the scanner.

(2) Double click on the Paper Port icon.

(3) Once in Paper Port, select File and then select Acquire.

(4) After selecting Acquire, set up properties utilizing the three drop down windows. The top drop down window shall be set on ADF for the automatic feeder (Flatbed for flatbed scanning). The second drop down window shall be set to Black and White (JAG photos should be scanned in using 24-bit color on the flatbed). The third drop down window shall be set at 150. Select Scan.

(5) Once all pages are scanned, exit out of the window.

(6) Select the appropriate file by clicking on it once to highlight it in red. Select File and then Export.

(7) When the Export window appears rename the file using the following format: LAST NAME, FIRST NAME MIDDLE INITIAL DDMMMYY.

NOTE: For files containing greater than 50 pages, the files shall be split in equal halves and save using the following format:

b. The OPL/OP shall complete a PORT NEWREC entry into the PORT system when the application is first forwarded to COMNAVCRUITCOM or One-Stop.

c. All correspondence shall be sent electronically or mailed, as appropriate, to the following:

(1) Medical Corps Programs and Medical AFHPSP (1975X): One-Stop Shop, NNMC, Tower #1, Room 4342, 8901 Wisconsin Ave., Bethesda, MD 20889

(2) All other programs shall be sent electronically to: email to [email protected] or faxed to (901) 874-9201.

d. When forwarding additional information:

(1) For COMNAVCRUITCOM submissions, include District name, POC, phone number and email address to ensure proper and timely delivery.

(2) For One-Stop submissions include a Long SEEREC, District name, POC, phone number and email address, and One-Stop code and POC who is to receive the document(s).

e. All documents shall be forwarded in the order as listed on the checklist.

f. Upon forwarding information to COMNAVCRUITCOM, ensure the original documents are placed in the master application file retained at the NAVCRUITDIST.

040408. RECONSIDERATION REQUESTS

a. Any officer program applicant who has been non-selected may request reconsideration of their application by submitting such request in writing to COMNAVCRUITCOM (N36).

b. If the request for reconsideration is submitted within six months of official notification of non-selection, the following information must be included:

(1) A new Application Processing and Summary Record (NAVCRUIT 1131/1), with the “Reconsideration” block marked appropriately.

(2) Applicant’s request for reconsideration, including a letter from the applicant stating the reason he/she should be reconsidered for selection. The reconsideration letter must be signed and dated by the applicant.

(3) Any new or updated information including, but not limited to transcripts, interviews and references.

(4) The field OPL/OP shall make changes to the original PORT record using the CNGREC program. A PORT Action Request (NAVCRUIT 1131/46) shall be completed by the field OPL/OP and forwarded to COMNAVCRUITCOM (N36) when requesting any changes in designator, recruiter’s SSN, FY, etc.

c. Applications will not be reconsidered for the same program within six (6) months of non-selection unless new information is submitted that indicates the applicant is better qualified than when previously non-selected or unless changes occur within the requested program’s status (i.e., requested program was previously closed and re-opens).

d. If more than six months have elapsed since official notification of non-selection, a new application will be submitted via the ADDOCS process.

040409. RECALL PROCESSING PROCEDURES. Personnel who previously held commissions in the U.S Navy or U.S Navy Reserve and are requesting recall to active duty may only be processed through COMNAVCRUITCOM if they are applying to Medical or Chaplain programs. All other personnel must submit their request through the BUPERS Recall Branch.

040410. RECORDS MAINTENANCE. This article is in accordance with SECNAVINST 5212.5 instruction. The OPO shall ensure compliance.

a. Recruiting Operations Records. These are files related to the recruiting of officer and enlisted personnel to include documents on local administration of recruiting programs, recruiting reports not specifically authorized for other disposition, and correspondence with civilian communities or organizations or with other activities or offices. Destroy when 2 years old.

b. Officer Recruiting Records

(1) Control Card Records and Summary Record Forms. These files show status of applications and their depositions. Retain these records until the applicant is commissioned or 5 years after applicant is rejected, whichever is earlier. Note: The Application Control Card and Summary Record have been combined into the Application Control and Summary Record (NAVCRUIT 1131/1).

(2) Withdrawn or Rejected Applications For Commission. These files include those of individuals found disqualified by the local recruiting officer, and those forwarded to the COMNAVCRUITCOM for final action and rejection, and related correspondence and papers.

(a) Return to Applicants: Birth certificates, transcripts of college credit, letters of endorsement, and other material of value to the applicant. Note: Letters of endorsement are distinctly different from letters of reference. Letters of endorsement are generally offered to the applicant for inclusion in their application. They are not required to be submitted directly to the Navy nor do non-disclosure rights apply, unlike letters of reference.

(b) Destroy all other documents after final determination is made by COMNAVCRUITCOM.

c. Correspondence with Individual Applicants for Commissions, Prospective Applicants, and Other Interested Individuals (filed alphabetically by name). Destroy when 2 years old.

d. Officer Recruitment Status and Production Reports. Destroy when 6 months old. [Note: Historical production data may need to be retained longer in order to develop the Marketing Operations Plan.]

e. Officer Recall Records. These are residual files pertaining to an applicant for recall to extended active duty from an office in the inactive Navy reserve. Destroy when 2

years old.

f. Requests for Information. Any requests for any information held in the residual file by anyone other than COMNAVCRUITCOM shall be forwarded to COMNAVCRUITCOM (N36) for review and action.

040411. AUTHORIZATION AND GUIDANCE ON RECORDS DESTRUCTION. All records shall be approved and initialed by the OPO prior to destruction. Destruction of files shall be by burning or shredding. The Residual File Checklist (NAVCRUIT 1131/31) shall be used when preparing records and the original maintained in a Dead File and destroyed when 2 years old.

a. PORT and OAMS Records. Status of applications and their dispositions shall be maintained in the PORT System and the Officer Accessions Management Systems (OAMS)(CIRIMS/O-Tools/NRAMS).

b. Third Party Information. Third party information, (i.e., any written, personal opinions expressed about the applicant, to include references and police checks) shall be destroyed at time of withdrawal, rejection, enlistment/ commissioning, declination, or at the six-month mark from the date of the applicant's first resubmission. Summaries, interviews, FITREPs and EVALs are not third party information. Summaries and interviews, however, shall not be shared with applicants.

SECTION 5 - ENLISTMENT OF OFFICER CANDIDATES

040501. INTRODUCTION. This section contains guidelines for enlistment of officer candidates. Processing activities are authorized to enlist selectees upon receipt of notification through the PORT system of selection and authority to enlist.

040502. PREPARATION FOR ENLISTMENT

a. Prior to an applicant's enlistment, the NAVCRUITDIST OPO, or CO's designee, shall:

(1) Ensure that a review of the applicant's file is completed and that no additional information of a derogatory nature has been received subsequent to the time the application was forwarded to COMNAVCRUITCOM headquarters for selection consideration. Attention shall be paid to any character and employer references or police record checks that may not have been forwarded to COMNAVCRUITCOM prior to the time of selection.

(2) If derogatory information is discovered, ensure the enlistment is not effected. Notify the NAVCRUITDIST CO and appropriate COMNAVCRUITCOM Officer Programs Director (N31, N33, N34) immediately.

b. Prior to an applicant's enlistment, the Officer Processing Lead (OPL)/Officer Processor (OP) shall:

(1) Ensure the physical examination has not expired at time of enlistment. If the physical is older than 24 months at time of enlistment, the member shall not be enlisted until he/she receives a new physical and has been found physically qualified for commission. (2) In case of selectees who are members of the Navy Reserve or another service, review the conditional release to ensure that the release date is still current and in effect and conditions are met prior to the enlistment. If the release date from the Navy Reserve or other service has expired, obtain a new conditional release with a new release date prior to enlisting the officer candidate. Under no circumstances shall the applicant be enlisted prior to obtaining said signed release.

(3) Use the Distribution of Enlistment Documents Checklist (NAVCRUIT 1131/32) to assemble the required enlistment documentation.

(4) Ensure the correct paygrade has been determined. Exhibit 0405-01, Enlistment Paygrade Determination, provides guidance in determining the candidates correct paygrade. NUPOC paygrade determination will be made at the NUPOC Interview. If the applicant is an active duty enlisted member selected for HSCP, consideration shall be given to the applicant re-enlisting at his/her current paygrade. COMNAVCRUITCOM (N34) shall be the determining party as to the member's paygrade.

040503. PREBRIEFING THE SELECTEE

a. The selectee shall complete the following enlistment documents prior to effecting the enlistment:

(1) Change of Status Questionnaire

(2) Program Service Agreement

(3) Annual Certificate of Physical Condition

b. The officer recruiter (OR) and OPL/OP shall review completed documents to ensure no changes or new information has been revealed which would preclude enlistment. The OPO shall notify the NAVCRUITDIST CO and appropriate COMNAVCRUITCOM Officer Programs Director (N31, N33 or N34) immediately should changes or new information preclude enlistment. Particular attention shall be paid to the following:

(1) Change of Status. Any positive answer on the Change of Status Questionnaire (NAVCRUIT 1131/33) form that is inconsistent with previously furnished information. If inconsistencies are found, or new information is discovered, the enlistment shall not to be effected.

(2) Physical Condition. If the selectee's physical condition appears to have changed since the examination was completed at the time of application; or information indicates that a change may have occurred; or the selectee indicates that his physical condition has changed since the latest examination, the enlistment shall not be effected. A new physical examination shall be conducted and forwarded to COMNAVCRUITCOM (00M) for final determination prior to enlistment.

(a) No selectee who has a lawsuit pending against him/her shall be enlisted without the prior approval of COMNAVCRUITCOM.

(b) In order to assist COMNAVCRUITCOM in making a determination in such cases, a description of the nature of the case shall be forwarded along with a statement from the applicant's lawyer as to when the case will be tried and whether or not the applicant's presence in court at the time of trial will be required.

c. Annual Certificate of Physical Condition. The NAVCRUITDIST medical representative shall review the completed Annual Certificate of Physical Condition (NAVMED 6120/3).

d. Sea Duty Assignment. All selectees shall be advised by the OPO/OR that they may be assigned to sea duty during their service in the Navy. If a selectee has not previously received bonuses or stipends from the United States Navy and communicates his/her unwillingness or objections to serve on sea duty, the selectee shall not be enlisted.

040504. EFFECTING THE ENLISTMENT. The OPL/OP shall:

a. Complete the Enlistment/Re-enlistment Document Armed Forces of the United States (DD Form 4) in accordance with Exhibit 0405-02

b. Ensure the Oath of Enlistment is properly administered. Any commissioned officer of any component of a U.S. Armed Force, active, reserve, or retired, may administer the enlistment oath.

040505. DEBRIEFING THE ENLISTEE. At the time of enlistment, the recruiting staff shall ensure the following briefings are conducted:

a. Training Brief. The enlistee shall be provided the appropriate information applicable to the training school to be attended (OCS/OIS), such as OCS/OIS DEP guides, OCS/OIS Website information, etc. The enlistee shall be provided, sign and date the following Page 13s at this time:

(2) Observances of the Religious Faith Practiced (NAVCRUIT 1131/201)

b. Montgomery GI Bill Brief. The enlistee shall be briefed concerning their eligibility for participation in the Montgomery GI Bill, and sign the MGIB Statement of Understanding (NAVCRUIT 1131/34).

(1) The brief is to be an overview of the MGIB. The enlistee will receive a complete briefing at OCS/OIS. Participation decisions on the part of the enlistee shall not be made upon enlistment but after reporting for active duty.

(2) Non-prior service individuals who are assigned full time by the Armed Forces to a civilian institution for a course of education are precluded from receiving Montgomery GI Bill benefits while they are pending receipt of a degree and attendance at OCS/OIS. This includes NUPOC candidates, BDCP, HSCP and CEC active duty enlistees. These individuals may become eligible for benefits three years after commissioning or three years after reporting to recruit training.

(3) Prior service individuals may be eligible to use their MGIB benefits while participating in their collegiate program. Details can be found athttp://www.gibill.va.gov/education/c30pam.htm.

(4) BDCP, NUPOC, HSCP, and CEC Collegiate program participants enlist onto active duty and shall receive a brief upon arriving at OCS/OIS. At that time, they will complete the entire MGIB briefing and DD Form 2366.

c. Tuition Assistance. In accordance with CNETINST 1560.3, personnel in an officer accession program, which involves essentially full-time instruction at a civilian institution, are prohibited from participating in the Tuition Assistance Program.

d. Collegiate Management Brief (active duty only). Active duty enlistees (NUPOC, HSCP, CEC, and BDCP) shall be given a thorough briefing on their benefits and responsibilities. Each active duty enlistee shall be provided a copy of the Collegiate Management Chapter (7), of this manual.

e. Change of Status Brief. Each enlistee shall be apprised of the necessity to immediately inform COMNAVCRUITCOM, via the cognizant District, of any major change in status. Such changes include, but are not necessarily limited to:

(1) Change of address.

(2) Change in name (including by marriage).

(3) Known changes in physical qualifications. Any illness, injuries, surgical procedure or pregnancy shall be reported.

(4) Circumstances that affect the individual's qualifications, such as disbarment, loss of a required license, etc.

(5) Involvement with law enforcement agencies for violation of civil or criminal laws. Any arrest, detainment, posting of bail, court hearings, convictions, etc., excluding minor traffic violations for which a fine of $150 or less is imposed, shall be reported. Failure to report may result in discharge from the officer program. Disposition of such cases shall be as authorized by COMNAVCRUITCOM on a case-by-case basis. The enlistee shall be provided, sign and date, the following Page 13s at this time:

(a) Explanation of UCMJ Article 137 and Discharges (NAVCRUIT 1131/202).

(b) Code of Conduct (NAVCRUIT 1131/203).

(6) Loss of motivation for officer candidate program.

f. Direct Deposit Brief

(1) Inactive enlisted members shall be briefed on the requirements to maintain an active bank account and to initiate direct deposit to be completed at their financial institution.

(2) Active duty members shall be briefed that they must complete Direct Deposit Form (SF Form 1199A) and return the form to the District. The NAVCRUITDIST shall forward the document to the PSD. The enlistee shall be provided, sign and date the Page 13: Direct Deposit Requirement Acknowledgement (NAVCRUIT 1131/204).

040506. DISQUALIFICATION AFTER SELECTION

a. If a selectee becomes physically disqualified prior to accessing onto active duty, the NAVCRUITDIST shall:

(1) Notify COMNAVCRUITCOM (N36) immediately

(2) If the physical disqualification is of a temporary nature resulting from an injury, accident, etc., submit consultations from the attending physician to COMNAVCRUITCOM (N36). The enlistee's file will be held in an inactive status for a limited time pending final recovery. If the enlistee is found physically disqualified for the desired program, the applicant may apply for a designator for which qualified.

(3) Failure to apply or be selected for another designator will result in separation from the Naval Service or reversion to former status, as appropriate, in accordance with the terms of the service agreement.

(4) The above action will be authorized as appropriate only after the candidate has been found not physically qualified by BUMED-25.

(5) For Active Duty Collegiates, see Article 070401.

b. A selectee that fails to earn a degree in the stipulated time may request an extension from COMNAVCRUITCOM (N36), via the NAVCRUITDIST CO, of one semester or one full summer session if circumstances warrant. For students enrolled in internships, the Program Director and/or Specialty Leader will coordinate with COMNAVCRUITCOM (N34) regarding extensions. If, following the extension, the candidate fails to meet educational requirements:

(1) Candidates/selectees procured from civilian/Navy Reserves sources will be discharged (for example, a non-paid 13750-NFO).

(2) For Active Duty Collegiates, see Article 070303.

040507. DOCUMENT DISTRIBUTION. The OPL/OP shall:

a. Use the Distribution of Enlistment Documents Checklist (NAVCRUIT 1131/32) to ensure the enlistment documents are appropriately distributed.

b. Complete appropriate entries in the PORT system and the AAMS (CIRIMS, NRAMS, O-Tools) system.

c. Forward active duty service records to the servicing PSD only upon receipt of active duty orders signed by the applicant.

d. Retain inactive service records at the District until the individual executes travel to OCS/OIS.

e. Mail the officer candidate’s Service and Health records to OCS not less than 1 week and no more than 2 weeks prior to the candidate’s OCS report date.

f. Provide the candidate/appointee with their active duty orders to hand-carry to OCS/OIS.

g. Forward Nurse Candidate Program (NCP) participants' Service Records to Commander, Naval Medical Education and Training Command (NMETC) in Bethesda, Maryland.

h. Use the Residual File Checklist (NAVCRUIT 1131/31) to ensure copies of the appropriate documents are retained. See Article 040411 for document retention and destruction.

040508. ENLISTED FIELD SERVICE RECORD. The OPL/OP shall:

a. Print full last, first, and middle name, and any suffix such as Jr., Sr., III, etc., on the cover of the Enlisted Field Service Record (NAVPERS 1070/600). If the applicant was given initial(s) rather than first and/or middle name, enter such initial(s) in quotations.

b. Insert social security number with hyphens in the space provided. Print an "R" following "USN."

040509. ADMINISTRATIVE REMARKS - PAGE 13. A Page 13 (NAVPERS 1070/613) shall be used to document special circumstances not covered by other forms or directives. Examples of Page 13s required for recruiting and collegiate management processes are at the end of the applicable section within this manual. If necessary, the NAVCRUITDIST shall complete additional Page 13 entries per MILPERSMAN 1070-320.

040510. DEFENSE ELIGIBILITY ENROLLMENT REPORTING SYSTEM ENROLLMENT AND THE MILITARY IDENTIFICATION CARD FOR ACTIVE DUTY COLLEGIATES

a. Upon enlistment of active duty collegiates, DEERS enrollment shall be completed with the assistance of COMNAVCRUITCOM (N36) prior to active duty collegiates going to PSD to obtain a military I.D. card and start BAS and BAH.

b. Obtaining a Military I.D. Card. The OPL/OP shall ensure the application is prepared per current directives. The following documentation is required when applying for the military I.D. card:

(1) Enlistment Contract

(2) Birth Certificate

(3) Blood Type

(4) Active duty orders

(5) Marriage License (if applicable)

(6) SSN Card

(7) One form of picture ID

(8) For members with dependents you must have your dependents birth certificates and SSN Cards.

NOTE: Application For Uniformed Services Identification Card - DEERS Enrollment (DD Form 1172) will be filled out at the PSD. Collegiates may have to fill out a DD Form 1172 at the NAVCRUITDIST to gain access to the local military installation. See subparagraph (a) above for DEERS enrollment.

c. Collegiate Status/Air Transportation/BAH and BAS Eligibility Letter (Example) (Exhibit 0405-03) shall be provided

to each active duty collegiate, by the NAVCRUITDIST, upon enlistment. The letter and its enclosure, Directions for PSD to Place Collegiates on Active Duty, shall be presented to the appropriate authority (i.e., PSD, etc.) by the collegiate along with his/her military identification card, as necessary, to explain their status with respect to military authority. Enclosure (1) of exhibit 0405-04 is a detailed list of required documents to be provided to and activities to be completed by the nearest PSD to establish the collegiate's record, benefits and delivery of the collegiate's military identification card.

040511. COLLEGIATE ORDERS. Enlisted collegiates shall be issue orders by COMNAVCRUITCOM per Article 040701 of this manual.

040512. ANNUAL CERTIFICATE OF PHYSICAL CONDITION. The OPO shall ensure the Annual Certificate of Physical Condition (NAVMED 6120/3) is completed by all non-paid collegiates/selectees assigned to the NAVCRUITDIST. Failure to disclose an illness or disease could be construed as intent to defraud the Government and may be the basis for criminal prosecution, other administrative action under the Uniform Code of Military Justice, dismissal from their program, or loss of disability benefits.

040513. CERTIFICATE OF RELEASE OR DISCHARGE FROM ACTIVE DUTY. The OPL/OP shall ensure all Certificate(s) of Release or Discharge from Active Duty (DD Form 214s) are placed in the field service record for individuals with prior active and/or inactive duty. In order for PERSUPDET personnel at OCS to accurately prepare the DD Form 214 upon commissioning, the service record must contain proper documentation from previous enlistment(s)/periods of prior active/inactive service.

040514. CHANGE OF STATUS QUESTIONNAIRE

a. The Change of Status Questionnaire (NAVCRUIT 1131/33) form shall be used to determine aspects of an applicant's personality and past behavior that indicate the applicant may be unsuitable for Naval Service.

b. The OPL/OP shall ensure the questionnaire is completed by the applicant and then review the applicant's responses prior to enlisting. If derogatory information is discovered, the OPO/OR/OPL/OP shall ensure the enlistment is not effected, and COMNAVCRUITCOM is notified immediately. The following directions apply:

(1) The form shall be completed in applicant’s own handwriting in reproducible black or blue-black ink.

(2) Line-outs are not recommended to avoid any question of what the applicant stated during the enlistment process. Line-outs shall be initialed by the selectee. The OPO/OR/OPL/OP shall ensure all information is clear, concise and full supportive documentation is included if needed.

(3) All "yes" answers must be fully explained.

040515. RECORD OF EMERGENCY DATA. The OPL/OP should assist the enlistee in completing the Record of Emergency Data (DD FORM 93).

040516. ENLISTMENT/REENLISTMENT DOCUMENT

a. The Enlistment/Re-enlistment Document Armed Forces of the United States (DD Form 4) is required to document enlistments in the Armed Forces. DD Form 4, upon execution, is an official legal agreement between the U.S. Government and the enlisted member.

b. General guidance for preparation

(1) The service representative who accepts the applicant for enlistment shall, prior to signing the completed form, verify typed entries for correctness and explain all applicable parts to the applicant.

(2) The OPL/OP shall type the DD Form 4 exactly as explained in Exhibit 0405-02. All required signatures shall be made using reproducible black or blue-black ink. There shall be no typeovers or errors on the DD Form 4. Erasures or corrections in items 5 and 8 are not authorized and shall require replacement of the document. Any changes or corrections in other items shall be initialed by the applicant and the service representative.

(3) Errors discovered subsequent to the member's enlistment shall be corrected by the appropriate PERSUPDET per MILPERSMAN 1070-210.

(4) In administering the oath, the words "So help me God," may be omitted by any person who elects to affirm rather than swear.

040517. FAMILY CARE PLAN CERTIFICATE. This certificate shall be completed by single parents and military couples with dependents. The Family Care Plan Certificate (NAVPERS 1740/6) is used to

b. Prepare the MM33-ENLISTED CONFIRMATION OF ACCESSION transaction to confirm the accession. The MM33 transaction will also correct the dates generated from the E203 transaction.

c. If the MM33 transaction is submitted prior to the posting of the E203 transaction, it will recycle for 27 days or until the E203 transaction is posted. If the 27 days recycle period expires, the MM33 transaction will reject. If the MM33 transaction rejects, prepare and release Exception Gain Message to NPC.

d. If the E203 transaction is posted and the MM33 transaction is not received within 120 days after the Date of Entry (DOE), the MMPA is put into a suspended status and pay is suppressed until the confirmation is received.

e. Prepare a TD Form W-4 for federal tax withholding, DD 2058 State of Legal Residence Certificate, and a TD Form W-4 with “STATE” at the top to declare their state tax withholding factors. Forward these forms to MILPAY for preparing input transactions.

SECTION 6 - COMMISSIONING OF ACTIVE DUTY OFFICER CANDIDATES

040601. GENERAL. This section contains guidelines for commissioning of officer candidates. The authority to appoint a selected applicant for an officer program is inherent in the appointment documents provided by PERS-4802 to the NAVCRUITDIST for delivery, contingent upon compliance with any attendant restrictions.

040602. PREPARATION FOR COMMISSIONING

a. Prior to an applicant's commissioning, the NAVCRUITDIST OPO or CO's designee shall:

(1) Ensure that a review of the applicant's file is completed to ascertain that no additional information of a derogatory nature has been received subsequent to the time the application was forwarded to COMNAVCRUITCOM headquarters for selection consideration. Attention shall be paid to any character and employer references or police record checks that may not have been forwarded to COMNAVCRUITCOM prior to the time of selection.

(2) If unreported derogatory information is discovered, ensure the commissioning is not effected and immediately notify the NAVCRUITDIST CO and appropriate COMNAVCRUITCOM Officer Programs Director (N31, N33, N34).

b. Prior to commissioning an applicant, the Officer Processor (OP)/Officer Processor Lead (OPL) shall:

(1) Ensure the physical examination has not expired (over 24 months old at time of commissioning). If the physical has expired, the member shall not be commissioned until he/she has received a new physical and has been found physically qualified for commission.

(2) In the case of selectees who are members of the Navy Reserve or another service, review the conditional release to ensure it is still current and in effect and conditions are met prior to commissioning. If the release date from the Navy Reserve or other service has expired, obtain a new conditional release with a new release date prior to commissioning the applicant. Under no circumstances will the applicant be commissioned prior to obtaining a signed release.

(3) Assemble the necessary and required commissioning documentation using the Distribution of Commissioning Documents Checklist (NAVCRUIT 1131/35). (4) Review the BUPERS List of Restrictions forwarded with the individual's commissioning documents, to ensure that all restrictions are either complete or scheduled for completion prior to effecting the commissioning.

(5) When commissioning the following direct appointment selectees: physicians, dentists, nurses, Medical Service Corps, JAG or Chaplain, ensure either PCS orders, or a letter stating their future duty station from CHNAVPERS, has been received prior to effecting the commission.

c. Chaplain direct appointment selectees cannot commission until they are within 30 days of class convening date. Chaplain students (1945) may commission up to 12 months prior to reporting to Chaplain School.

d. If not already complied with, applicants drawing or claiming a pension, retired or retainer pay, or disability compensation from the United States Government by virtue of prior military service, shall, by time of commissioning:

(1) Voluntarily agree to waive the pension, retired or retainer pay, or disability compensation being received, or

(2) Voluntarily agree to waive claim for active duty pay and allowances.

040603. PREBRIEFING THE SELECTEE

a. The OP/OPL shall ensure the selectee completes the following commissioning documents prior to effecting the commission:

(1) Change of Status Questionnaire

(2) Annual Certificate of Physical Condition

(3) Family Care Plan and Navy Family Care Certificate (Single Parents and dual military with dependents only)

(4) All statements/agreements on the COMNAVCRUITCOM Information and Restrictions for Person Delivering Appointment Documents Letter.

(5) Program Service Agreement

b. The officer recruiter (OR) and OP/OPL shall review the completed documents to ensure that no changes or new information have been revealed that would preclude commissioning. The OPO shall notify the appropriate COMNAVCRUITCOM Division Officer (N31, N33 or N34) and NAVCRUITDIST CO immediately should changes or new information precludes an appointment. Particular attention shall be paid to the following:

(1) Change of Status. Any positive answer on the Change of Status Questionnaire (NAVCRUIT 1131/33) that is inconsistent with previously furnished information. If inconsistencies are found, or new information is discovered, the applicant shall not be commissioned.

(2) Physical Condition. If the selectee's physical condition appears to have changed since the examination administered at the time of application; or information indicates that a change may have occurred; or the selectee indicates that his physical condition has changed since the latest examination, the applicant shall not be commissioned. A new physical examination shall be conducted and forwarded to COMNAVCRUITCOM (00M) for review prior to commissioning.

(3) Lawsuits

(a) No selectee who has a lawsuit pending against them shall be commissioned without prior approval of the appropriate COMNAVCRUITCOM program manager.

(b) In order to assist COMNAVCRUITCOM in making a determination in such cases, a description of the nature of the case shall be forwarded along with a statement from the applicant's lawyer as to when the case will be tried and whether or not the applicant's presence in court at the time of trial will be required.

(4) Sea Duty Assignment. All selectees shall be advised by the OPO/OR that they may be assigned to sea duty during their service in the Navy. If a selectee has not previously been enlisted into the Navy and/or received bonuses or stipends from the Navy and communicates his/her unwillingness or objections to serve on sea duty, the selectee shall not be commissioned.

040604. EFFECTING THE COMMISSION

a. Any commissioned officer of any component of a U.S. Armed Force, whether or not on active duty, may administer the commissioning oath of office.

b. The official administering the oath of office, the OPL and OP shall ensure that all restrictions appended to the appointment documents have been met prior to the candidate being administered the oath. In the event there are questions regarding the restrictions involved, administration of the oath shall be delayed until all questions have been resolved. Alterations of names, grades or dates of rank, shall not be made on appointment papers before or after execution. Pen and ink changes of birth dates and social security numbers may be made only upon receipt of authority from COMNAVCRUITCOM. Appointment papers will be reissued by PERS-4802 upon notification of an error.

c. All remaining commissioning documents shall be completed by the OPL or OP prior to the selectee's commissioning.

d. After the oath has been administered, the new officer and the officer administering the oath shall sign all copies of the Oath and Acceptance of Office (NAVPERS 1000/20).

e. The OPL/OP shall ensure:

(1) All signatures are original. Use of carbon copies is not authorized except for typing in the date and name/rank of the official who administered the oath.

(2) Appointees serving in enlisted status, and appointees serving in temporary commissioned or warrant grade, whose permanent status is enlisted, must be honorably discharged from their enlisted status by reason of "selected changes in service obligation to accept permanent appointment to officer grade" per MILPERSMAN 1910-102.

(3) A Certificate of Release or Discharge from Active Duty (DD Form 214) is prepared per NAVMILPERSCOMINST 1900.1 series and the enlisted service record closed for members discharged while on active duty.

commission and the enlisted service record is closed. Enlisted record disposition shall be per MILPERSMAN 1070-130.

f. The Appointing Officer shall:

(1) Honorably discharge appointees from their enlisted status by reason of "selected changes in service obligation to accept permanent appointment to officer grade" per MILPERSMAN 1910-102 as of the day immediately preceding the date of acceptance of the permanent appointment as appropriate.

(2) If not the custodian of the enlisted service record, notify the service record custodian regarding the type of appointment accepted and the date it was accepted.

(3) With the assistance of the OPL/OP, request that the member's enlisted status be terminated on the day immediately preceding the date of acceptance of the permanent appointment.

040605. DEBRIEFING THE APPOINTEE

a. At the time of appointment, the OR shall:

(1) Ensure the new officer is given current printed information available for the training school to be attended (e.g., OIS).

(2) Appraise the appointee of the necessity to inform COMNAVCRUITCOM, via the cognizant NAVCRUITDIST, of any major change in status. Such changes include, but are not necessarily limited to:

(a) Change of address

(b) Change in name (including by marriage)

(c) Known changes in physical qualifications. Any illness, injuries, surgical procedure or pregnancy shall be reported

(d) Circumstances that effect the individual's qualifications, e.g. loss of a required license, etc.

(e) Involvement with civil law enforcement agencies for violation of civil or criminal laws. Any arrest, detainment, posting of bail, court hearings, convictions, etc., excluding minor traffic violations for which a fine of $150 or less is

imposed, must be reported. Failure to report may result in discharge from the officer program. Disposition of such cases will be as authorized by COMNAVCRUITCOM on a case-by-case basis. The appointee, if not previously issued, shall be provided, sign and date the following Page 13s at this time:

(f) Failure to continue professional training (student programs). Disposition of students who fail to complete professional training will be as directed by the Chief of Naval Personnel,

(3) Provide the appointee to active duty a copy of Useful Information for Newly Commissioned Officers (NAVEDTRA 12967) and counsel concerning their basic responsibilities as naval officers.

b. The OPL/OP shall brief the appointee concerning their eligibility for participation in the Montgomery GI Bill, view the “Montgomery G.I. Bill Benefits of Enrollment” video, and sign the MGIB Statement of Understanding (NAVCRUIT 1131/34).

(1) The brief will be an overview of the MGIB since the appointee receives a complete briefing at OIS. Participation decisions on the part of the appointee shall be made after reporting for active duty. At that time, they will complete the entire MGIB briefing and DD Form 2366.

(2) Non-prior service individuals who are assigned full time by the Armed Forces to a civilian institution for a course of education are precluded from receiving Montgomery GI Bill benefits while pending receipt of a degree and attendance at OIS. This includes HSCP enlistees who shall receive a brief upon arriving at OIS. These individuals may become eligible for benefits three years after commissioning or three years after reporting to recruit training.

(3) Prior service individuals may be eligible to use their MGIB benefits while participating in their collegiate program. See the following website for further clarification:http://www.gibill.va.gov/education/c30pam.htm.

c. Tuition Assistance. In accordance with CNETINST 1560.3D series, personnel in an officer accession program, which involves essentially full-time instruction at a civilian institution, are prohibited from participating in the Tuition Assistance Program.

d. Upon acceptance of a commission, each officer on active duty shall report to his or her Commanding Officer for duty until orders are received from NMPC. Officers on inactive duty are not required to report until in receipt of orders from NMPC.

e. Upon acceptance of appointment, members on active duty do not accrue entitlement to mileage allowance or any other travel allowance due to termination of their former officer or enlisted status.

040606. DISTRIBUTION OF COMMISSIONING DOCUMENTS CHECKLIST. The Distribution of Commissioning Documents Checklist (NAVCRUIT 1131/35) shall be used to ensure all the commissioning documents are appropriately distributed after the commission has been effected. The OPL/OP shall:

a. Ensure the 1915-Medical Corps Student Records and 1925-Dental Corps Student Records are maintained by the appropriate COMNAVCRUITCOM Program Manager

b. Ensure the 1955-Judge Advocate General’s Corps Records are maintained at the NAVCRUITDIST whose geographic area of responsibility (AOR) includes the law school the JAG student is attending

i. Place all official documentation, to include a copy of the Distribution of Commissioning Documents Checklist (NAVCRUIT 1131/35), in the envelope, seal with adhesive tape, initial and date over the tape, and provide the envelope to the appointee.

NOTE. The social security number shall not be placed on the outside of the envelope.

j. Use the Residual File Checklist (NAVCRUIT 1131/31) to ensure copies of the appropriate documents are retained. See Articles 040411 of this chapter for document retention and destruction directions.

040607. ADMINISTRATIVE REMARKS - PAGE 13. The Page 13 (NAVPERS 1070/613) shall be used to document special circumstances not covered by other forms or directives. 040608. DEFENSE ELIGIBILITY ENROLLMENT REPORTING SYSTEM ENROLLMENT AND THE MILITARY IDENTIFICATION CARD

a. DEERS enrollment of active appointees shall be completed with the assistance of COMNAVCRUITCOM (N36) prior to the active duty appointee going to a PSD to obtain a military I.D. card and start BAS and BAH.

b. Obtaining a Military I.D. Card. The OPL/OP shall ensure the appointee is prepared per current directives. The following documentation is required when applying for the military I.D. card:

(1) Copy of Oath of Office

(2) Service Agreement

(3) Birth Certificate

(4) Blood Type

(5) Active duty orders

(6) Marriage License (if applicable)

(7) SSN Cards

(8) One form of picture ID

(9) For members with dependents you must have your dependents birth certificates and SSN Cards.

NOTE: Application For Uniformed Services Identification Card - DEERS Enrollment (DD Form 1172) will be filled out at the PSD. Active duty appointees may have to fill out a DD Form 1172 at the District to gain access to the local military installation. See subparagraph (a) above for DEERS enrollment.

040609. RECORD OF EMERGENCY DATA

a. Record Of Emergency Data (DD Form 93) shall be completed by the appointee.

b. The OPL or OP shall verify all entries with appointee.

c. If the appointee is married, the appointee shall provide a copy of his/her marriage certificate to the OP or OPL who shall ensure it is included with the officer's documents so it is available when the pay record is opened. These procedures shall also be followed for birth certificates for children. Copies of divorce decrees (including any of spouse) shall also be included.

040610. CERTIFICATE OF RELEASE OR DISCHARGE FROM ACTIVE DUTY. The OPL/OP shall assist officers with prior active and/or inactive duty by ensuring a copy of each Certificate of Release or Discharge from Active Duty (DD Form 214) previously issued is placed in/or present in the officer's official microfiche record.

040611. OFFICER’S REPORT OF HOME OF RECORD. The appointee, following the instructions contained on the form, shall complete the Officer’s Report Of Home Of Record (NAVPERS 1070/74). The OPL/OP shall ensure the date of entry block contains the date the appointee commences travel to OIS or his/her first duty station.

040612. DIRECT DEPOSIT. The OPL/OP shall brief each appointee on their requirements for participating in direct deposit, and completing, signing and dating the Direct Deposit Form (SF FORM 1199A). The appointee shall, if not previously issued, be provided, sign and date the Page 13: Direct Deposit Requirement Acknowledgement (see Exhibit 0405-07).

040613. FAMILY CARE PLAN CERTIFICATE. This certificate shall be completed by single parents and military couples with dependents. The Family Care Plan Certificate, (NAVPERS 1740/6), is used to designate persons who will accept dependent care responsibility (see Article 020106).

040614. STATE OF LEGAL RESIDENCE. The State of Legal Residence form (DD Form 2058) shall be completed by the member and is used to determine the correct State of Legal Residence for purposes of withholding State income taxes from military pay.

040615. PROGRAM SERVICE AGREEMENT

a. The appropriate program Service Agreement shall be attached to the Commissioning Documents (COMMDOCS) as Annex A. Alterations to the Service Agreement are not authorized.

b. No other annexes to COMMDOCS are authorized.

a. The OPL/OP shall review the officer’s BUPERS List of Restrictions to ensure all restrictions have been adhered to prior to allowing the administration of the superceding oath (e.g., physical examination that will not expire prior to completing OIS and proof of graduation from college/university).

b. Administer Superceding Oath to Officer. The OPL/OP shall ensure the necessary sections are complete on the document and distribute them as indicated on the forms. No additional paperwork is required for superceding appointments, including PORT/OTOOLS entries. EXCEPTION: When superceding a Chaplain Candidate to Chaplain Corps or a Dental Student to the Dental Corps, the OPL/OP shall ensure a PORT Record Action Request is submitted to COMNAVCRUITCOM (N3123) with the NAVCRUITDISTs 3-digit Code and the recruiter’s SSN.

SECTION 7 - ORDERS

040701. DISCUSSION.

a. COMNAVCRUITCOM Collegiate Orders. COMNAVCRUITCOM shall fax the Collegiate's Orders (Exhibit 0407-01)(with the exception of Nuclear Propulsion candidates) to the NAVCRUITDIST 24 to 48 hours after the OPL/OP enters the applicant's enlistment into PORT. Nuclear Propulsion candidates shall receive their collegiate orders at the Naval Reactors NUPOC Interview.

b. Active Duty Orders to Officer Candidate (NAVCRUIT 1131/36) shall be used for all officer candidates enlisted for inactive duty.

c. NAVCRUITDIST’s shall not issue active duty orders until the officer candidate concerned has presented evidence of receipt of a baccalaureate degree.

d. Prior to orders being issued, the NAVCRUITDIST shall certify on a Page 13, Evidence of Graduation and Degree Conferred (NAVCRUIT 1131/206) and enter it in the Enlisted Field Service Record (NAVPERS 1070/600).

e. Whenever possible, active duty orders shall be issued at least four to six weeks prior to the class convening date to allow the candidate to make final plans and remove any apprehension about whether or not orders will arrive in time.

f. If less than 30 days remain between presentation of evidence of degree and the class convening date, the active duty orders shall be issued immediately upon the candidate's presentation of said evidence.

g. In those cases where only a very short time span exists between graduation and the class convening date, the NAVCRUITDIST shall advise the candidate that orders with short lead time will be forthcoming immediately upon presentation of evidence of degree, and that plans should be made accordingly.

040702. COLLEGIATE TRANSCRIPTS

a. The OPL/OP shall ensure one complete set of official transcripts indicating degree earned, date conferred, and all credits and grades received are included in the service record. In those cases where, due to short notice, final transcripts are not available, evidence that the transcripts have been requested

from the school (i.e., a transcript release form) shall be placed in the service record. The OPL/OP shall ensure such release forms request the transcripts be forwarded directly to OCS or OIS, as applicable.

b. Active Duty Collegiates: The NAVCRUITDIST shall ensure final transcripts, as well as a copy of the Page 13 advancing member to E5, are forwarded to COMNAVCRUITCOM (N363) immediately upon receipt of transcripts from the officer candidate.

040703. PREPARATION OF OFFICER CANDIDATE'S ENLISTED FIELD SERVICE RECORDS AND OFFICER DOCUMENTS

a. The District OPL/OP shall request PSD transfer the Enlisted Field Service Records (NAVPERS 1070/600) for active duty collegiates to the NAVCRUITDIST for final review by the OPL and OP prior to the officer candidate receiving his/her records.

b. Prior to the officer candidate being issued orders to OCS, the District OPL/OP shall check the candidate's Enlisted Field Service Record (NAVPERS 1070/600) and health records for completeness using the guidelines in Article 040508. The NAVCRUITDIST shall forward the Enlisted Service and health records and any other applicable information to OCS for all candidates no more than 2 weeks and no less than one week prior to the candidate’s class convening date.

c. The OPL/OP shall provide appointees for OIS with their documents to hand carry to OIS. Preparation of these documents shall be in accordance with Article 040606.

040704. ISSUANCE OF ORDERS. When preparing orders for inactive duty personnel going to OCS, the OPL/OP shall ensure the officer candidate is issued orders to depart from one of the following:

a. The officer candidate's home of record, as listed on the Enlistment Contract (DD Form 4).

b. The officer candidate's place of enlistment (i.e. NAVCRUITDIST).

c. A location that is either closer or more cost-effective to OCS Pensacola, FL (cannot exceed the cost of home of record or place of enlistment).

040705. DEPENDENT TRAVEL. All candidates ordered to OCS/OIS under TEMDUINS orders (except NFOs since they attend follow-on training that exceeds 20 weeks in duration at the same location) shall be reminded that, per their Service Agreement, they have been advised that dependent travel is not reimbursable.

040706. SHIPMENT OF HOUSEHOLD GOODS. The OPL/OP shall brief all candidates concerning their entitlement for shipment of household goods.

040707. BUREAU OF NAVY PERSONNEL GENERATED ORDERS

a. Active duty orders for CEC, NUPOC, BDCP and HSCP active duty enlistees, and Direct Appointment Program selectees are issued by BUPERS.

b. Requests for orders shall be coordinated through the appropriate COMNAVCRUITCOM program manager.

c. The OPL/OP shall:

(1) Review the officer candidate's or officer's orders with the member to ensure they understand the orders.

(2) Ensure the appropriate documents are either forwarded to OCS for officer candidates or provided to the officer to hand carry to OIS.

(3) Use the Checklist for Transfer of Officer Candidates /Officers to OCS/OIS (NAVCRUIT 1131/37) for officer candidates transferring to OCS or for officers transferring to OIS.

040708. REPORTING WINDOWS. Active Duty Orders shall stipulate the following reporting windows:

a. OCS - NET 1100 on Saturday (reporting PORT date)

b. NLT 1100 on Sunday (day after PORT date)

c. OIS - NET 1600 two days before convening date

d. NLT 1600 day before convening date

a. To determine the accounting data to use in completing the Active Duty Orders to Officer Candidate (NAVCRUIT 1131/36), the OPL/OP shall refer to COMNAVMILPERSCOMINST 7040.6 series, Chapter II, Section IV tables for "Enlisted Accession - Other Than Recruits -Land".

b. All candidates, with the exception of NFO candidates, are ordered to temporary duty under instruction (TEMDUINS) and the course duration is less than 20 weeks. NFOs are ordered for duty in excess of 20 weeks and will receive PCS orders.

c. For orders issued by BUPERS or COMNAVRESFOR, the OPL/OP shall use accounting data provided by the order writing authority.

040710. OTHER TYPES OF ORDERS

a. Nuclear Officer VIP and Medical Officer VIP Orders

(1) The Nuclear Officer VIP is required for all Nuclear Officer candidates prior to being invited to the Nuclear Officer Interview. The NUCVIP allows applicants the opportunity to experience first-hand the work environment of the Navy Nuclear Officer. Travel arrangements are made through the NAVCRUITREG Nuclear Trained Officer (NTO). The applicant receives applicant travel orders, but does not receive per diem.

(2) The Medical VIP is used for critical Medical Programs. Medical VIPS are approximately one to three days in length and the sites vary according to the type of VIP. NAVCRUITREGs and COMNAVCRUITCOM coordinate the Medical VIPS and prepare the itineraries. The applicant receives applicant travel orders, but does not receive per diem.

b. Nuclear Officer Interview Orders and One-Stop Shop Orders

(1) The Nuclear Officer Interview is a two-day evolution that allows officer applicants to travel to Washington, DC for interview and selection decision by Naval Reactors. Travel arrangements shall be made by the NAVCRUITDIST while lodging and meal accommodations will be made by COMNAVCRUITCOM. The applicant receives applicant travel orders, but does not receive per diem.

(2) One-Stop Shop Interviews are for Medical Corps officer candidates. One-Stop Shop is a two-day evolution where applicants travel to Bethesda, MD to receive a physical and interview by the Professional Review Board. The applicant receives applicant travel orders, but does not receive per diem.

c. Active Duty Recall Appointees Executing Overseas Orders. Recall appointees being ordered overseas (to include family members) are required to complete an overseas screening and apply for a passport(s) early enough to ensure receipt prior to executing orders. Certain overseas commands require Command Sponsorship. Appointees shall work closely with detailers to meet these requirements.

SECTION 8 - DISCHARGE OF OFFICER CANDIDATES

040801. INACTIVE DUTY OFFICER CANDIDATES AWAITING OFFICER CANDIDATE SCHOOL. Upon receipt of a letter requesting discharge from an officer candidate, enlisted for inactive duty, who subsequently declines to attend OCS or when an officer candidate fails to meet any of the requirements for continuation in the officer candidate program in which enlisted, prior to the officer candidate reporting to OCS for officer training, the NAVCRUITDIST Officer Processor Lead (OPL) and Officer Processor (OP) shall:

a. Prepare and have the enlistee execute a Page 13: Discharge (NAVCRUIT 1131/207).

b. Upon receipt of the signed Discharge Page 13 by the enlistee, the CO shall issue an Honorable Discharge Certificate (DD Form 256N) and forward it to the enlistee along with a copy of the signed Page 13. Note: For circumstances that would preclude an Honorable Discharge, contact COMNAVCRUITCOM (N36) for further guidance.

c. When the discharge has been completed, ensure the following documents are forwarded to PERS-832:

040802. ACTIVE DUTY OFFICER CANDIDATES ACCEPTING A COMMISSION

a. Candidates enlisted for active duty shall be discharged for the convenience of the Government upon receipt of authorization to discharge from PERS-32 per MILPERSMAN 1910-102.

b. Prepare Page 13: Discharge (NAVCRUIT 1131/207) and a DD Form 214 Worksheet (NAVCRUIT 1131/38) and forward them to the servicing PSD to ensure that a DD Form 214 is prepared and issued per COMNAVMILPERSCOMINST 1900.1 series.

040803. ACTIVE DUTY OFFICER CANDIDATES. Active Duty Officer Candidates (BDCP, HSCP, CEC and NUPOC) requiring discharge shall be discharged per this manual (Chapter (4), section (8)) and MILPERSMAN 1910-182. 040804. INJURY OR DEATH

a. In the event of the injury or death of an applicant in process, selected, or enlisted for inactive duty, the OPO shall contact the appropriate COMNAVCRUITCOM program manager for guidance.

b. In the case of personnel on or deemed to have been on active duty, active duty for training, inactive duty training, or traveling directly enroute to or from such duty, at the time of injury or death, the OPO shall consult MILPERSMAN 1770-040.

040805. DISCHARGE PHYSICAL FOR ACTIVE DUTY DISCHARGED FOR NON-PERFORMANCE. Officer Candidates enlisted for active duty that have been approved by COMNAVCRUITCOM to be discharged for non-performance shall be directed to a military Medical Treatment Facility (MTF) nearest the NAVCRUITDIST's location for proper pre-separation evaluation.
