ACCOMPLISHMENT REPORT School Year 2014-2015

DR. NELIA C. RESULTAY Campus Executive Director Lingayen Campus Thru: PROF. RIZZA S. BALDONADO Department Chairman Communication Arts Madam: I have the honor to submit my Accomplishment report for the School Year 2014 2015.

I. INSTRUCTION

i. Distributed the course syllabus to the students in the different designated subjects.

Assigned subjects are as follows:

1. Fil 101 Pagbasa at Pasulat Tungo sa Pananaliksisk 2. Fil 103 Masining na Pagpapahayag 3. Eng 103 Speech and Oral Communication 4. Eng 101 Study and Thinking Skills 5. Lit 101 Philippine Literature 6. Eng 102 Communications for I.T. 7. NSTP 2 CWTS 1

ii. Effectively delivered the different topics of each of the given subjects within the allotted time.

iii. Successfully administered the major exams (midterm and final exams) to the students in all the different assigned subjects.

iv. Meritoriously evaluated the students according to the evaluation scheme written in the course syllabus.

v. Submitted the grade sheets to the office of the registrar.

II. PRODUCTION

1. Triumphantly regained the lost glory of the PSU Football Team as the Head Coach in the 2014 Regional SCUAA 1 Olympics held in University of Northern Luzon, Vigan City, Ilocos Sur last January 20 to 24, 2014 and proclaimed as the Champion.

2. Magnificently conducted the 1st National Summer Football Tournament held in the PSU Football Field last April 19 and 20, 2014 participated by more than 40 teams from different towns of Pangasinan, nearby provinces and regions. Over-all Event Organizer

3. Productively fulfilled the requirements set by the Asian Football Federation (AFC) for the C License and has been awarded on September 6, 2014.

4. Participated and reached quarter finals in the mixed doubles in the 7th Philippine National Open Table Tennis Championships in held in SM Subic, Pampanga last October 24 to 27, 2014. Head Coach

5. Successfully organized the 1st Governors Football Cup. The first ever 11-a-side football tournament participated by 16 teams as far as Tuguegarao City, Baguio City and also by foreign students. It was in cooperation with the Junior Chamber International, the Provincial Government, and the Local Government Unit. Over-all Event Organizer

6. Victoriously brought home the Championship Trophy in the 2014 Regional SCUAA 1 Olympics in the Table Tennis Womens Event as the Head Coach held in Mariano Marcos State University, Batac City, Ilocos Norte last December 1 to 5, 2014. Head Coach

7. Effectively coordinated with the San Beda College Manila Sports Director for the tune up match and training of our womens table tennis team in preparation for the 2015 National SCUAA Olympics.

8. A very successful tune up match with the Pampanga Table Tennis Association players held last January 31, 2015 in Minalin, Pampanga.

9. Provided our PSU Womens Table Tennis Team with their equipment and other stuff for their participation in the 2015 National SCUAA Olympics with the help of my friends and other sponsors.

10. Placed 5th in the 2015 National SCUAA Olympics in the Table Tennis Womens Event held in Cagayan State University, Tuguegarao City last February 8 to 15, 2015. Head Coach

11. Jessica Jacoba, one of the players of PSU Table Tennis Team placed 3rd runner up in the 3rd

GoSSIMA Invitational Table Tennis Tournament held in City Walk, Tarlac City last February 21, 2015. Head Coach

12. Rogielyn Quijana and Shekienna Ugaban, both players of the PSU Womens Table Tennis Team placed 2nd runner up and 3rd runner up in the 1st Laoac Table Tennis Tournament held in Laoac, Pangasinan last March 1, 2015.

13. Participated in the Asian Football Confederation and Philippine Football Federation Womens Football Day 2015 held in Far Eastern University Diliman Campus last March 8, 2015.

14. Thesis adviser of the IV-BSICT under the leadership of Janine F. Ordaez with their title Football Tutorial for Beginners. A software developed in android platform using Corona SDK.

15. Participated and hosted the 2015 PE/NSTP Festival held last March 11, 2015 at the PSU Football

Field, Lingayen, Pangasinan.

III. EXTENTION

i. Won the 2014 Temiong Merit Award for Best Sports Development Program during the JCI Philippines National Convention held in Cagayan De Oro last October 8, 2014 for our 1st National Summer Football Tournament.

ii. JCI Lingayen Bagoong: TRICK OR TREAT. An annual activity of JCI-LB for the children. The

event held last October 31, 2014. Co-chairman

iii. JCI Lingayen Bagoong Christmas Project, "Pamaskong Handog 2014. An annual gift giving to the poor but deserving children of Lingayen in the celebration of Christmas.

iv. Attended the 2015 National Women's Month Celebration - "Juana, Desisyon Mo ay Mahalaga sa Kinabukasan ng Bawat Isa, Ikaw na!" An activity conducted by the PSU Gender and Development last March 13, 2015 at the Sison Auditorium, Lingayen, Pangasinan.
