Have the Soldier remove their ACU top
Make sure the Soldier has an ID card If there is no ID card, verify the ID of
a soldier in another way
Have the soldier look at their personal information on the DD 2624, unit roster, and bottle label
If the information is incorrect, draw a line through the incorrect information, write the correct information, initial, and date
Makes sure the cap is tight Makes sure the specimen has 30ml Makes sure the specimen is not
adulterated If adulteration is suspected, have the
Soldier stand fast and notify the commander.
Puts tape on the bottle Initials the bottle
The Observer signs the ledger The Donor signs the ledger The UPL Returns the ID Card to the
donor.
It is good to go through the box at the end of the process to verify that all the bottles have been taped, are in the correct slot, and that all have been initialed by the donor and UPL
