Quick Sheet Google Forms

1. Log on to Google Drive (http://www.google.com/drive) or to your school district’s Google Apps account.

2. When you are logged in click on create and select form.

3. Give your form (survey) a title, select a theme, and click ok.

4. Determine which question choice you need:

5. Enter the question Title (Question) and Help Text needed. Fill in the options (answers) and determine if the question is required. Click done.

6. Click on Add Item and select the next Question type needed.

7. Determine if you want to insert Page Breaks or Section Headers.

8. To edit a saved question, hover over the question, and click on the pencil.

9. When all the questions are entered, scroll down to the confirmation page and enter a confirmation message that you want your users to read after they have submitted their survey (Your response has been recorded.)

10. At the top of the page, click choose response destination. Make sure new spreadsheet is selected and click create. Click on view live survey to see the survey. Copy and paste this link in an email or to post on your website, blog, wikispaces, LMS, etc.

11. Return to your Google Drive and you will see two items (one is the response file (spreadsheet) and the other is the actual survey (form). Click on the Responses File.

12. A spreadsheet of responses has now opened. You can manipulate/sort all the information as a regular spreadsheet or even download it to excel if you prefer. In addition, if you want notified of when a user submits the survey, click on Tools and Notification Rules. Check when a user submits a form and select whether you want a daily digest or emailed right away.

13. In order to see summary results in graphs, in the responses spreadsheet, click on Form and select Show Summary of Responses.
