Although the cardiotoxic effects of anthracyclines have been known for at least 40 years, no evidence-‐based guidelines exist for post-‐treatment monitoring and preven/on of treatment-‐related cardiotoxicity in clinically asymptoma)c adu l t surv i vors o f b reas t cancer . There are currently no consistent recommenda)ons on the frequency and modality with which cardiac imaging should be performed in pa)ents at risk of LV dysfunc)on related to cancer therapy. Exis/ng surveillance protocols are on the basis of methodology from clinical trials and expert opinion.
Owing to the absence of data, recommenda/ons for ini)a)on and frequency of surveillance are largely consensus based. There was a consensus that surveillance should begin no later than 2 years ager comple/on of cardiotoxic therapy and con/nue for a minimum of every 5 years thereager, as pharmacological interven)ons in individuals with asymptoma)c cardiomyopathy can delay the onset of conges)ve heart failure and decrease mortality.
More frequent cardiomiopathy surveillance is reasonable for high risk survivors.

The prevalence of gonadal dysfunction was similar in the three considered periods of pediatric cancer diagnosis (1985–1989, 1990–1999, >2000).
The adjusted risk of gonadal dysfunction was higher in patients treated with radiotherapy (OR = 8.72; 95 % CI 3.94–19.30) and in those exposed to both alkylating and platinum-derived agents (OR = 9.22; 95 % CI 2.17–39.23).
Sarcomas were the cancer diagnosis associated with the higher risk (OR = 3.69; 95 % CI 1.11–12.22).
An extremely high rate of gonadal dysfunction was detected in patients who underwent HSCT and TBI

receiving ima)nib treatment, suggest that con/nuous treatment with ima/nib does not impair male or female gonadal func/on
• Regarding newer addi)ons to the class, both pre-‐clinical and clinical data on nilo/nib and dasa/nib appear to show that gonadal func)on is not altered; however, the number of observa)ons is too low to derive solid conclusions.
$ Angiogenesis inhibitors • About TK-‐I, despite the pivotal role of angiogenesis in gonadal development,
preclinical findings indicate male and female fer/lity may be only mildly compromised by suni/nib.
• Pazopanib with high doses seems to decreased sperm concentra)ons and decreased tes)cular weight
• it has been reported that chemotherapy regimens containing the an)-‐angiogenic agent bevacizumab may cause ovarian failure in up to 34% of female pa/ents with colorectal cancer compared to 2% of pa/ents who received the same regimen (Folfox) without bevacizumab.
$ mTOR-‐Inhibitors • Animal models show a reversible reduc)on of fer)lity parameters when treated with
everolimus at doses higher than those used in clinical seqng. Clinical studies (in male renal transplanted pa/ents, not in cancer pa)ents) seem to show that chronic treatment with everolimus causes a decrease in testosterone levels and disrup/on of spermatogenesis.
$ EGFR-‐Inhibitors • Considering the lack of preclinical and clinical data, the effect of trastuzumab on
spermatogenesis and folliculogenesis is uncertain. • Considering EGFR TKIs, such as erlo)nib and gefi)nib, pre-‐clinical models have shown a
reduc)on in fer)lity parameters, but the effect on humans is unknown. The only clinical study of gefi/nib on fer)lity parameters revealed suppression of androgen levels during treatment.
$ Alk-‐inhibitors • Based on non-‐clinical safety findings, crizo/nib may compromise both male and
female fer/lity. The molecular mechanism of toxicity could have both a central component involving the hypothalamus/pituitary axis, and a peripheral gonadal component.
• Fer)lity issues should be addressed to all pa)ents in reproduc)ve age before cancer treatment.
• In men, cryopreserva)on of sperm should be offered to all cancer pa)ents in reproduc)ve age, regardless of the risk of gonadal failure.
• In women, the recommenda)on of fer)lity preserva)on should be individualized based on mul)ple factors such as the urgency of treatment, the age of the pa)ent, the marital status, the regimen and dosage of cancer treatment
