ON THE JOB EQUIPMENT TRAINING
DESIGNATED EVALUATOR
Hands-on equipment training is to be conducted by a station “Designated Evaluator”. The
designated evaluator is an experienced Forklift Operator selected by the Station Manager that
has complete documented training records and is fully competent to operate each type of forklift
they will be qualifying new employees to operate.
The “Designated Evaluator” will fully explain all the operating functions, gauges, brakes, daily
preventive maintenance checks, and operational safety requirements of each type of forklift to
every new employee. The “Designated Evaluator” will observe each employee ability to operate
each type of equipment in a practice area well away from any aircraft.
Each different equipment model type can have substantial differences and is to be considered a
different piece of equipment, requiring complete and separate training instructions.
Individual employee evaluations will vary and continue as needed until the “Designated
Evaluator” is confident that the employee has demonstrated complete understanding and
competency in the SAFE operation of each type of forklift. The “Designated Evaluator” must NEVER sign off any employee that they are not fully
confident can and will SAFELY operate the equipment taught.
No employee is allowed to operate any equipment unsupervised without the “Designated
Evaluator’s” completed and signed training acknowledgement on file in their individual training
record.
The “Designated Evaluator” is required to re-evaluate all Forklift Operators every three years
and complete new hands on training record acknowledgements.
An Operator Permit Card is to be issued to each qualifying employee showing the specific
Forklift they are trained to operate. A new Operator Permit Card will be issued every three
years following recurrent qualification.
Prerequisite: Employee must successfully complete the Forklift Safety CBT training modules prior to
OJT practical training.

The maneuvering practice drill illustrations outlined in this packet are to provide the
Evaluator with guidance and practice exercises for the operator trainees’ practical skills
portion of the OSHA requirements for certification.
Instructor’s Note: Please leave adequate time to prepare and train each individual
appropriately.
These tasks shall be demonstrated first by the Evaluator as a minimum standard to assist
in building practical skills for safe forklift operations:
• Demonstrate driving a lift truck in a straight line, in both a forward and backward
direction.
• Demonstrate driving a lift truck in a circle, in both a forward and backward
direction.
• Demonstrate driving a lift truck in tight spots, making smooth turns, in both a
forward and backward direction.
The Trainee shall practice under Evaluator supervision and be able to:
• Drive a lift truck in a straight line, in both a forward and backward direction.
• Drive a lift truck in a circle, in both a forward and backward direction.
• Drive a lift truck in tight spots, making smooth turns, in both a forward and
backward direction.

These tasks shall be demonstrated first by the Evaluator as a minimum standard to assist
in building practical skills:
• Demonstrate driving up and down an incline without a load. (if applicable)
• Demonstrate driving up and down an incline with a load. (if applicable)
• Demonstrate picking up a load from a stack of boxes (3 high) and moving each to a
new area. (Use simulated loading dock set up)
• Demonstrate parking and shutting down a lift truck.
The Trainee’s shall practice under Evaluator supervision and be able to:
• Drive up and down an incline without a load. (if applicable)
• Pick up a load, driving up and down an incline with a load. (if applicable)
• Pick up a load from a stack of boxes (3 high) and move each to a new area in a
simulated loading dock set up.
• Park and shut down a lift truck.

These shall be demonstrated by the Evaluator first as a minimum standard to assist in
building practical skills:
• Demonstrate picking up a load and placing it in a simulated trailer.
The Trainee’s shall practice and be able to:
• Pick up a load and place it in a simulated trailer.
