SWSRA BOARD MEETINGSThe SWSRA Board of Directors Meeting is held on the third Wednesday of each month at 10:00 a.m. The public is invited to attend these meetings. The meeting will always be held at SWSRA’s Worth office location at 10707 Oak Park Ave. Worth, IL 60482. GENERAL INFORMATIONSouth West Special Recreation Association (SWSRA) is a special recreation agency comprised of the Alsip Park District, Blue Island Park District, Justice Park District, Village of Merrionette Park, Midlothian Park District, Palos Heights Recreation Department, Posen Park District, Summit Park District and Worth Park District. SWSRA was formed in 1981 to provide year-round quality recreation programs and services for individuals with disabilities or special needs. SWSRA’s programs are designed to increase independence and enhance the quality of life for each individual.

SWSRA is now in its 31st year of providing recreation for special populations in the south suburban area. A wide variety of programs are available for individuals from early childhood through adulthood. Programs are designed to meet the individual recreation needs of any person who is in special education classes, participates in a sheltered workshop, is referred to SWSRA from a local hospital or rehabilitation center, or whose special needs restrict or prohibit their participation in traditional park district or recreation department programs. This would include individuals who have varying degrees of physical or mental disabilities, learning disabilities, behavior disorders, and hearing or visual impairments.

”ATLANTO-AXIAL INSTABILITY”Individuals with Down Syndrome are at risk of having a con-dition known as Atlanto-Axial Instability (AAI). This condi-tion allows for increased laxity of ligaments involving the top bones of the neck. In a number of individuals with Down Syndrome, the movement between the first and second cervical vertebrae is abnormally increased and may lead to pressure on the spinal cord. Undue pressure on this area may cause severe injury. It is a SWSRA policy that any individual with Down Syndrome is required to have a doctor’s written note on file stating the person is free of AAI if that person wishes to participate in any program with a potential to cause undue stress upon the neck. Programs with the * sign are identified as those requiring the AAI test. SWSRA will also be offer-ing other programs that are not by inherent nature of the activity at risk for undue pressure upon the neck. However, the nature of play is active participation. Please take this into consideration when enrolling your child into a program and inform staff if the condition is present.

BEHAVIOR CODE OF CONDUCTSSWSRA participants are expected to exhibit appropriate behavior at all times. The following guidelines have been developed to help make SWSRA programs safe and enjoy-able for all participants. Additional rules may be developed for particular programs and athletic leagues as deemed necessary by staff.

Participants will:• Show respect to all participants and staff and take

direction from staff.• Refrain from using abusive or foul language.• Refrain from causing bodily harm to self or others.• Show respect to equipment, supplies and facilities.

DISCIPLINEPositive approaches will be used regarding discipline. Staff will periodically review rules with participants during the program session. If inappropriate behavior occurs, prompt resolution will be sought specific to each individual situation. SWSRA reserves the right to dismiss a participant whose behavior endangers the safety of him/herself or others.

South West Special Recreation Association is committed to the safety and well being of staff and patrons. All acts of violence by program participants are taken seriously, and SWSRA will promptly respond to any incident or suggestion of violence. Violation of this policy will result in disciplin-ary action, up to and including suspension or denial of participation, at the discretion of SWSRA.

DISPENSING OF MEDICATIONIf a participant is in need of assistance (other than a reminder) to take a prescription medication, a permission form must be signed by the parent or guardian and returned to SWSRA before the first day of registration. Medication information must also be indicated on the SWSRA Master registration – this assists SWSRA in determining its ability to comply with any request to assist with the administration of medication. Contact SWSRA to obtain a permission form. [PLEASE NOTE THAT SWSRA DOES NOT ADMINISTER RECTAL MEDICATION]

GUIDELINES FOR PARTICIPATION IN PROGRAMS WHEN ILLNESS OCCURSIn consideration of other participants and the SWSRA staff, to prevent the spread of contagious illnesses it is recommended that participants refrain from attending programs when any of the following conditions exist:• Fever of 100 degrees or higher• Vomiting within the last 24 hours• Persistent diarrhea in conjunction with other symptoms• Contagious rash or a rash of unknown origin• Persistent cough and/or cold symptoms• “Pink eye” (conjunctivitis) or discharge from the eye• Symptoms of mumps, measles, chicken pox, strep

throat, flu, impetigo or coxsackie virus• Runny nose with yellow or green discharge (which

indicates infection)• Lice or mites present• Fatigue, due to illness that will hinder participation and

enjoyment in the program

ADA STATEMENTSouth West Special Recreation Associate (SWSRA) will do its best to comply with the guidelines set forth in the Americans with Disabilities Act (ADA). Any questions or concerns in regard to the ADA should be directed to the SWSRA office.

Please notify the SWSRA office if the participant contracts any contagious illness that will affect his/her attendance at the program. Participants should return to programs at the doctor’s recom-mendation, or, if not under a doctor’s care, when the symptoms have clearly passed.

INCLUSION POLICYSWSRA and our member agencies believe that all individuals should be provided with leisure opportunities that allow par-ticipants to perform at their highest level of ability. Although many people achieve this through participation in a SWSRA program, others may have great success in the activities of member agencies. Our member agencies adhere to the fol-lowing basic principles when providing accommodations to allow for successful inclusion:

Principles of Inclusion:1. The interests and needs of an individual participant will be addressed.2. The concerns of the family will be taken into account.3. The member agency staff will be given support by SWSRA.4. Options and choices will be made available to individuals with disabilities.

MANDATED REPORTERSAll SWSRA staff are mandated by the State of Illinois to make a report to DCFS hotline if they observe a situation that provides reasonable cause to suspect child abuse or neglect. Workers in certain professions, including “recreational program” personnel, have this legal mandate. Our staff is trained on signs and symptoms of abuse and neglect, and procedures for making a report.

MASTER REGISTRATION FORMSWSRA requires all participants to complete a Master Regis-tration Form annually in January. The form provides staff with valuable information, which allows SWSRA to conduct its activities in the safest possible manner and to meet the basic needs of participants. If you are new to SWSRA or do not have a current form on file, please contact the office and one will be sent to you. Additionally, if the agency receives a registration form and does not have a Master Registration Form on file, one will be sent to you to complete and return. If at anytime the form needs to be updated (change in medication, behavior, address, house manager, etc.) please call the SWSRA office. All information is kept strictly confidential and enhances successful participation. Thank you for your cooperation.

LATE PICK-UP FEEPlease pick-up participants at the designated times. If parents/guardians are not on time for a participant pick-up/drop-off, a $5 fee will be assessed after 15 minutes late and every 15 minutes thereafter. Thank you for your cooperation.

PROGRAM CANCELLATION DUE TO SEVERE WEATHER GUIDELINESIn the event of severe weather, SWSRA may be forced to cancel programs. The following guidelines will be used to determine cancellation.

Outdoor programs:• 10 degrees or/in combination with wind chill of

zero degrees or less.Programs with transportation:• Temperature range of –10 degrees or/in combination

with a wind chill or –20 degrees or less.• All programs (including indoors): Snowstorm

situations with driving restrictions (winter storm or blizzard warnings) are in effect.

Severe Hot Weather:• Outdoor programs, or indoor sites without air

conditioning: Heat index of 105 degrees or higher.• All programs (including indoors): Weather

situations when a tornado warning is in effect for Cook County.

REFUNDS/CREDITSA refund or credit will be issued to participants if a program, trip, or special event is canceled due to low registration. If a participant cancels out of a program prior to the start and no costs have been incurred, a $5 service fee will be charged and a refund or credit will be issued. There will be no refunds or credits issued if a participant cancels out of a program, trip, or special event and SWSRA has incurred costs due to purchase of tickets, rentals, deposits, etc. If inclem-ent weather or leader absence occurs, SWSRA will try to reschedule the activity or issue a credit or refund.

PLEASE REGISTER EARLY! If the minimum number of participants needed is not reached by deadline date, the trip or special event will have to be canceled. Alternately, if the maximum number of participants is reached before the deadline, unfortunately, we may have to cut off registrations early.

REGISTRATION/FEES AND PAYMENTPROCEDURESAt SWSRA, it is our responsibility to provide adequate staffing and supplies for our programs, special events and trips. Help us by registering before the deadline date(s) so we can make these necessary preparations.

Registration and fees will not be accepted at program sites. Registration and fees will only be accepted by walk-in regis-tration at the SWSRA office or mail-in registration.

Registration fees have been established for each program, trip and special event. These fees must be paid at the time of registration. If you do not live within Alsip, Blue Island, Justice, Merrionette Park, Midlothian, Palos Heights, Posen, Summit or Worth, you are considered as residing “out-of-dis-trict,” and must pay the NR (non-resident) fee. The NR fees are typically 100% higher than the listed fee.

No registration will be processed if there is a balance due from past seasons. Full payment is expected at the time of registration unless special arrangements are made. Call the SWSRA office to discuss a payment plan or eligibility for a scholarship.

NON-SUFFICIENT FUNDS (NSF)A $35.00 Service charge will be assessed for any returned check and any future registration will need to be paid in cash.

TRANSPORTATIONOur vans are scheduled for many different programs, trips and special events throughout the week, so it is very impor-tant that participants are dropped off and picked up at the listed time so we can meet this schedule. Thank you for your cooperation.

SWSRA has 5 pick-up points for weekend special events and trips where transportation is available. This service is pro-vided for you at no additional cost. We also offer one-way (home drop-off) or round-trip (door-to-door) transportation on select events and trips for a minimal fee. The fee infor-mation is located on the seasonal registration page. Please refer to each individual trip or event to see which type of transportation is available to you.

Residents will be charged $6 per round trip; non-residents who reside within 10 miles of the SWSRA office, will be charged $8 per round trip.

PLEASE BE PATIENT. Although SWSRA gives pick up and/or drop off times, these are only estimates. We do our best when scheduling times, however unforseen circumstances may arise such as construction, traffic, trains, etc. Thank you for understanding!

TRANSPORTATION (CONTINUED)TRANSPORTATION IS NOT AVAILABLE FOR NON-RESIDENTS WHO RESIDE OUTSIDE OF A 10-MILE RADIUS OF THE SWSRA OFFICE. These additional fees must be paid at the time of registration, and must be indicated on the seasonal registration form.

SWSRA has implemented the following transportation proce-dures to ensure participants safety in our vehicles:

• All passengers must be sitting in seats or wheelchairs with belts securely fastened. The driver will not proceed until all seat belts are fastened.

• All wheelchairs must have brakes that are in good work-ing order and can stop the chair from moving.

• The SWSRA drivers will not be able to leave the vehicles to assist participants to/from their homes.

• SWSRA staff are responsible for determining whether or not a participant can be transported safely. The driver may refuse to transport any person(s) if it is determined that it would be dangerous to do so.

• Persons riding in Amigo-type wheelchairs must transfer into a van seat and must use a seat belt.

• Persons unable to be in a house alone should have someone meet the van at the designated time of return. SWSRA cannot wait more than 10 minutes due to the demanding van schedule.

• SWSRA reserves the right to refuse to transport individu-als. Please realize that these policies have been devel-oped to ensure the safety of everyone riding in SWSRA vehicles. Your cooperation is appreciated.

• When transportation is included in special events, the use of vans or buses may change as necessary to accommo-date group sizes and needs.

• Most events have centralized meeting locations.

PICK-UP & DROP OFF POLICYSWSRA asks that participants arrive no earlier than 5 minutes prior to the start of a program or slated pick-up time, and be picked up promptly at the end of the pro-gram or slated drop-off time. SWSRA requires you to walk the participant(s) into and out of the building and/or their program room. Please do not drop off or leave anyone early or without a staff member present.

CAR SEATSIn January of 2004, the changes to the Illinois Vehicle Code Child Protection Act went into effect. All children up to and including 7 years old are required to be in a car seat or booster seat. Section 25/4 places the responsibility for providing the seats with the parents. Therefore, if a car seat is required due to a child’s age, SWSRA requests that parents provide the seat and assist with securing the seat in the van or bus. Please indicate on the registration form if a car seat will be used.

ERRORS AND ADJUSTMENTSWhile we try to be accurate in presenting information in our brochure, some unavoidable errors or changes to the programs, fees, schedules and other information may occur. SWSRA apologizes for any inconvenience resulting from these necessary changes.

HOW TO BE INCLUDEDWhen signing up for a Park District Program, just inform the staff of that Park District that you will be requesting additional services. It’s that easy! The Park District will contact us, and our Inclusion Staff will not only find the appropriate assistance but will also continue to monitor progress throughout the program by continuing communication with the Park District staff and theindividual’s family members if necessary. SWSRA provides inclusion services to our Member Districts only and does not include our Co-op agreement with SSSRA.

SWSRA MISSION STATEMENTOur mission is to serve individuals with disabilities in their need for recreation so that all persons in our member communities may experience and enjoy positive leisure opportunities.

SWSRA VISION STATEMENTSWSRA will be the leading community-based provider of quality special recreation programs for individuals with disabilities, and is consistently setting new standards of value to meet the changing needs of our participants and our member communities.

SWSRA-SSSRA COOPERATIVE AGREEMENTSWSRA has a Cooperative Agreement with South Suburban Recreation Association (SSSRA). This agreement enables participants who reside in SWSRA and SSSRA resident com-munities to participate in programs offered by either asso-ciation at resident rates. This agreement is for all regular programs and trips; however, it does not include our sum-mer day camp program. If a program is full, agency resi-dent’s registration takes priority. SSSRA serves the residents of Country Club Hills, Frankfort Square, Hazel Crest, Homewood-Flossmoor, Matteson, Oak Forest, Olympia Fields, Park Forest, Richton Park, Tinley Park and Lan Oak.We hope that through this agreement, each agency’s residents will have expanded opportunities for program participation. For additional information about this agree-ment, please call SWSRA. To receive a copy of the SSSRA brochure, call the SSSRA office at (815) 806-0384. A limited number of copies may be available at the SWSRA office.

VOLUNTEER WITH SWSRAVolunteers play an instrumental role in the development and quality of leisure services for all individuals with disabilities. SWSRA seeks to utilize volunteers talents and expertise to further meet the needs of special individuals in their communities. Volunteers are given opportunities in various recreational programs. INFORMACIÓN GENERAL

¿Qué es SWSRA?Es un Centro Especial de Recreación; Coperativo; Compre-sible compuesto de las comunidades siguientes: Alsip, Blue Island, Justice, Merrionette Park, Midlothian, Palos Heights, Posen, Summit y Worth. SWSRA se fundó en 1981 para proveer programas recreativos para todo el año a los indivi-duos con necesidades especiales. L.P.G.O. 20th Annual Golf Outing FundraiserPlease join us for the 20th anniversary of the Annual LPGO Golf Outing Fundraiser, which will be held at Gleneagles Country Club on Saturday, September 15, 2012. This exciting afternoon of golf, dinner, awards and prizes is organized and hosted by our wonderful volunteers, Laura Kane, Ann Thatcher and Kyle Werhand. This event is to raise money for our agency. Lori Chesna - Executive Director, APRPLori has been serving as the Executive Director of SWSRA since 1998. Prior to that, she worked at the Oak Lawn Park District as a Special Recreation Supervisor. Lori earned an AAS Degree at Moraine Valley Community College in Recreational Therapy. During her career, she has been recognized for several awards, but the one she is most proud of was presented by the Illinois Park and Recreation Association Therapeutic Recreation Section “Special Recognition Award” for her leadership with legislative advocacy, the passing of a bill that was a team effort led by professionals from all over Illinois. This was a bill that benefits Park & Recreation agencies so they can pro-

vide recreation services for individuals with disabilities.

Lori has been married to her husband Robert for 33 years. She has three children and two grandchildren. Her passion is her family and her job, where she always boasts “she is so blessed to work at a job she loves!”

As a parent herself, Lori realizes the fact that safety is our number one priority here at SWSRA. “If a parent is going to trust us with their loved one, we must make sure we provide a safe, fun environment.”

Marge Cozzone - Business ManagerMarge graduated from Roosevelt University with a degree in Business Administration. Marge has been in the business world for over 35 years and has been SWSRA’s Business Manager for eleven years. She has been active in the South Suburban Parks & Recreation Professional Association and has served as an officer. Marge has also been a member of the Administration & Finance Division of the Illinois Parks & Recreation Association. She thinks special recreation is very special, and enjoys working with all of our wonderful participants and their families.

Early in her career, Marge worked for both advertising and public relations firms in Chicago as well as on the West Coast. Over the years, she has worked for a leading architect & engineer and has been a partner in two businesses that she also managed. For the majority of her career, Marge was a business agent and representative for pharmacists at a major drug chain.

Marge is married and has two married children. Her daughter has one son with another son due any day now. She loves being a grandmother!

Erin Lynch - Superintendent, CTRSErin has worked at SWSRA for 12 years and was a part-time staff member for 2 years while in college. She want-ed to be a teacher since she was little and always played ‘School’ with her sisters. While in college, she volun-teered at SWSRA. She loved it and thought, ‘I could do this for a living,!” then changed her major and graduated from University of St. Francis with a degree in Therapeutic Recreation and received her CTRS accreditation. After graduating, she worked in the Clinical side of the field, running activities on the Alzheimer’s Unit at a Veteran’s Hospital, running a Youth-at-Risk program for The Community House and working at Christ Hospital on the Mental

Health & Substance Abuse floor. It didn’t take long though to come back where she started, at SWSRA. It is different as a full-time staff now though, and, she loves a chance to get out of the office and see everyone!

She is very close with her family, especially her 15 nieces and nephews. Erin spends the majority of her weekends being involved in their lives, everything from sports, school activities and sleep-overs. She grew up being an athlete and playing games, and still plays volleyball every week and loves having game nights with friends or the kids.

William Helling - Recreation Specialist, CTRSNearly three years ago, Will made the move from the great state of Iowa over to Chicagoland and has been en-joying it every day since. He obtained a bachelor’s degree in Therapeutic Recreation at Western Illinois University. Upon graduation, Will did an internship at the Rehabilitation Institute of Chicago. After the internship was com-plete, Will found part time work with Western Dupage Special Recreation Association (WDSRA) where he was a program leader and inclusion aide for the agency. In December of 2009, he was offered the Recreation Specialist position at SWSRA and has been here ever since!

Will has worked numerous camps and has had countless ‘hands- on’ time with individuals of all special needs populations. His favor-ite experience was the summer he spent at the Timber Pointe Outdoor Education Center in Bloomington. Will worked seven days a week at this overnight summer camp. This was by far the hardest summer of his life, and yet, he can’t think of any other summer that can even compare to the fond memories he has of all his campers and co-staff that he shared it with.

Justin Waters - PR/Marketing CoordinatorJustin started his career with SWSRA in March of this year after working three years in minor league baseball. He’s a 2009 graduate of University of Wisconsin-Stevens Point with a degree in Communication. During college, he interned in the Sports Department at WAOW-TV in Wausau, WI while being a member of the baseball team for three seasons. From 2009-11, he worked as the Assistant General Manager of the Springfield Sliders baseball team in Springfield, IL. Last year, he was the Director of Media & Community Relations/Marketing for the Beloit Snappers baseball team, a Class-A affiliate of the Minnesota Twins in Beloit, WI.

Justin is from Huntley, IL and currently lives in Chicago Ridge. He enjoys spending time with his family at his cabin in Wisconsin, spending time with his friends and his dog, Rowdy.

RIP CALLING ALL VACATION ADVENTURERS!!

Did you know SWSRA plans new and exciting trips for our vacation goers every year? Vacations vary by season and last anywhere from 3 – 8 days. The vacation trips are designed for individuals who are at a high level of functioning, can participate in the various activities planned for each day and are independent with all daily living skills. SWSRA staff do all of the planning of the activities in advance, however, there are opportunities on each trip for every person attending to be able to vote on meals, activities, etc. SWSRA has even let everyone come

up with and vote on entire vacation destinations and activities in the past. SWSRA employees staff and participate in every aspect of the trips, providing a fun and safe atmosphere.

SWSRA’s vacations provide a great and unique opportunity to increase or enhance all participants’ independence and social skills through spending days with their peers and friends.

UNIFORM POLICYSWSRA is proud to provide athletes the opportunity to participate in Special Olympic Sports. We also are pleased to be able to provide players with uniforms that they can wear proudly as Coyote athletes. Unfortunately, some of our uniforms are not being returned at the end of the season. As a result, SWSRA established a policy for uniform use. All uniforms MUST be returned, washed and in similar condition within 1 week of the season ending. If this does not occur, athletes will not be given the opportunity to sign up for further Special Olympics activities, including participation in the Annual Sports Banquet and receiving their trophy until

SWSRA is reimbursed for the cost of the uniform.

The National Children’s Study (NCS) Greater Chicago Study Center would like to thank you for your participation in the NCS Vanguard Study. The NCS Vanguard Study is a phase of the overall NCS Study. SWSRA is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. SWSRA continually strives to reduce such risks and insists that all participants fol-low safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for the programs/activities listed above must recognize that there is an inherent risk of injury when choosing to participate in recreational programs/activities. You are solely responsible for determining if you or your minor/child/ward are physically fit and/or skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or re-cently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activ-ity. Recreational programs/activities are intended to challenge and engage the physical, mental and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, condition-ing and equipment, there is still a risk of serious injury when participating in any recreational program/activity. Understandably, not all hazards and dangers can be foreseen. Depending on the particular activity, participants must understand that certain risks, dangers and injuries due to inclement weather, slipping, falling, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises, defects, inadequate or defec-tive equipment, inadequate supervision, instruction or officiating and all other circumstance inherent to indoor and outdoor recreational activities/programs exist. In this regard, it must be recognized that it is impossible for SWSRA to guarantee absolute safety.
