Parent Teacher Organization Looking to get involved? Join us each month in the cafeteria. The next meetings are February 6th and March 10th at 6:00 pm. Parents who attend will earn a free dress coupon for their student!
With cold and flu season approaching, please remind your child to practice good hand washing,
and to "cover your cough" to help prevent the
spread of germs. If your student requires any
over-the-counter medications during the school
day, please administer the medication at home if
at all possible. Any medications taken during the
school day require a health care provider's
signature on the School Medication Authorization
form, including cough drops. The medication must be brought to school in the original
container, and is kept locked in the office. If
needed, you may visit the school office to
administer medication to your child.
Our Goal is to read 10,000 Books by the end of the quarter. Keep up the good reading!!

Campfire Reading Night Is being rescheduled for February
Attention parents of students with asthma: New Illinois state law requires school districts to request a copy of the Asthma Action Plan for students diagnosed with asthma each school year. Please bring your copy to the school office as soon as possible. If your child uses an inhaler or nebulizer treatment during school hours, or at school-sponsored activities, we must receive a new School Medication Authorization form each school year. The form is available to print from www.psd150.org; under the red "Students & Parents" tab, drop down to "Forms." For inhalers ONLY, Do not take the form to the doctor's office; a xerox copy of the prescription label from the inhaler box substitutes for the health care provider signature. Please indicate on page two of the form if you wish for your student to be in possession of the inhaler. Please also send an extra peak flow meter to keep with the inhaler, and spacer, if so ordered.
A Sport physical exam is required for students participating in athletic sports. Forms completed on the IESA / IHSA form are valid for 395 days. Approval for sports for one year may also be indicated on the Illinois Certificate of Child Health Examination form ("School" physical form).
Students in 5th & 8th grade are encouraged to obtain a "school" physical exam at the same time as the "sport" exam, which will be valid for entry into 6th and 9th grades. The IESA / IHSA sport physical examination form is NOT accepted as proof of the "school" exam.
Attention parents of kindergarten and new-to-Illinois students: Good vision is critical to student success in school; difficulties with vision can lead to academic and behavior issues. Some conditions are treatable if diagnosed early in life. An eye exam dated 8/17/15 or later was due by October 15. It is not too late to submit the eye exam paper! If your child has an appointment by December 15, please submit proof of the appointment to the school office. Then, submit the eye exam paper when done.
