This form relates to absences of five consecuve days or greater or in situaons where workplace accommodaons are required for non-occupaonal illness or injury. In order to address efficiencies, the following procedure must be followed by employees when responding to a request to submit a Medical Confirmaon Form/Medical Cerficate. Strict adherence to this process is required to ensure confidenality and the opmal management of a claim. Please note: For employees that are absent from work five consecuve days or greater due to personal illness or non- occupaonal injury, the compleon of the Medical Confirmaon Form/ Medical Cerficate is required. This form is sent to you electronically through your TCDSB email and is specific to your union/associaon affiliaon or non-union status. This form is to be completed and returned in a mely manner directly to the Sick Leave and Disability Department through email at mdbenefi[email protected] or by fax at 416-512-3435. This form can also be completed in situaons where workplace accommodaons are required. To iniate this process, please contact the Sick Leave and Disability Department directly at 416-222-8282 extension 2073. We would like to remind employees, given the confidenal nature of the informaon contained in this form/cerficate that copies are not required to be provided to anyone other than the Sick Leave and Disability Department. Medical Confirmation/Certificate for CUPE members

We would like to provide you with further clarification on the submission of the Medical Confirmation Form/Medical Certificate for employees who are CUPE members. This form relates to absences of five consecutive days or greater or in situations where workplace accommodations are required for non-occupational illness or injury.

In order to address efficiencies, the following procedure must be followed by employees when responding to a request to submit a Medical Confirmation Form/Medical Certificate. Strict adherence to this process is required to ensure confidentiality and the optimal management of a claim.

Please note:

For employees that are absent from work five consecutive days or greater due to personal illness or non-occupational injury, the completion of the Medical Confirmation Form/Medical Certificate is required.

This form is sent to you electronically through your TCDSB email and is specific to your union/association affiliation or non-union status.

This form is to be completed and returned in a timely manner directly to the Sick Leave and Disability Department through email at [email protected] or by fax at 416-512-3435.

This form can also be completed in situations where workplace accommodations are required. To initiate this process, please contact the Sick Leave and Disability Department directly at 416-222-8282 extension 2073.

We would like to remind employees, given the confidential nature of the information contained in this form/certificate that copies are not required to be provided to anyone other than the Sick Leave and Disability Department.

New/Important Information ESS—Reporting Absences

Effective January 2019, all Adult Education program employees must report their availability to supply and absences, only through the ESS page. Remember that you can report online even after the day you have been absent.

One of the most common issues users are experiencing is the substitute list not being populated in the Permanent Employee Absence Form. Please make sure when you fill in all the information depicted below, you click the Check Available Substitutes button, in order for you to see the names of the supply instructors. We would also like to remind you that under Action, you need to always select New Absence. The Full and Partial Day Absence functionality has been updated to make it easier to choose between the two of them.

As mentioned in earlier issues of the memorandum, you can always write any other important details under the Note section. If you require assistance, please email Jaime Puentes at [email protected] with a screenshot of the issue you are experiencing at the time you are in your ESS page.

New/Important Information

Employee Portal—Upgrade

Effective February 5, 2019, the Employee Portal (Intranet) has a newly upgraded version that can be found at https://tcdsbcec.sharepoint.com/sites/Home

The updated version of the Intranet has a modern look, which provides easy access to all the information employees need by using a few key drop down menus, and increased search capabilities. For all employees who need to access the ESS page through this portal, here are a few tips on how to navigate it.

How to Login:

Bookmarks/Web Browsers:

Please delete all old bookmarks to the INTRANET The new Employee Portal is supported with Chrome, IE 11, Edge, Firefox, Safari 10+. The best experience will generally be with Chrome.

How to find your applications:

SMARTFIND EXPRESS, ESS, etc are now grouped under one convenient button – EMPLOYEE SERVICES.

Outreach Report for December 2018 Submitted by Kay Ham, Program Consultant

Community Engagement:

1. A meeting with Coordinator of Settlement, Language and Integration Workgroup of Toronto North LIP took place on January 28, 2019. The purpose of this meeting is to find ways to promote our programs in the catchment area and to reach out to organizations and agencies affiliated with the North LIP. The coordinator will promote our programs through Social Media platforms and other promo channels North LIP is using, and Kay will join Fairview Interagency Network group.

2. Kay participated in Vietnamese New Celebration events at James Culnan Catholic School on Saturday, February 2nd and at St. Jane Frances Catholic School on February 3rd. Both events were organized by Heritage Language Program coordinators and drew approximately 400+ people. This was a wonderful opportunity to connect with the staff of Vietnamese Language Program at both locations.

Our Lady of Mount Carmel Adult Education Centre ESL Program:

Long-awaited ESL classes began at the center on February 4, 2019. Prior to the opening, a Canada Post mail campaign was carried out that targeted both Our Lady of Carmel ESL and Victoria Park LINC programs. In addition, outreach was done at local places, such as libraries, a daycare center, and social media postings through Toronto North LIP.

Social Media Posting:

Many thanks to staff who sent pictures and videos for posting. Many of them attracted reactions and followers on our Instagram and Twitter platforms. Your continuous support to promote our program through sharing materials as well as encouraging your learners to “react to and follow” our platforms is much appreciated.

Building and Maintaining Good Credit Webinar

In this live webinar, you will learn about how to build and maintain credit in good standing, what effects credit score, and the Canadian Credit system.

Another topic is the Foreign Credential Recognition Loan program, which offers loans up to $15,000 to internationally trained individuals with credentials obtained outside Canada who want to get licensing/certification in their profes-sional or related field, or to pursue an alternative career not related to their professional qualifications.
