New Guideline: Management of Infants and Children with Congenital Talipes Equinovarus

In New South Wales (NSW), the assessment and management of children with Congenital Talipes Equinovarus (CTEV) is undertaken by clinicians in both tertiary and non-tertiary hospitals. Referral to one of the three tertiary children’s hospitals is recommended for the management of complex cases. However, there are recognised benefits of providing ongoing services as close as possible to the child’s home.

The experience and expertise of clinicians in the management of CTEV varies. Many physiotherapists working in rural facilities have limited access to both ongoing education and clinical support, and specialist orthopaedic medical services.

The CTEV evidence based practice guideline was developed by the NSW CTEV Working Party in response to identified variability in the management of babies and children born with CTEV in NSW. The working party was comprised of senior physiotherapists from the three tertiary children’s hospitals and a tertiary hospital, the Department Head of Orthopaedics at The Children’s Hospital at Westmead and an Allied Health Educator.

The process undertaken to develop the guideline was evidence-based. The published literature, existing clinical practice guidelines and websites of a variety of professional organisations were searched. Recommendations were based on evidence found, integrated with clinical expertise and parent contribution.

Following completion of a state-wide consultation process, the Management of Infants and Children with Congenital Talipes Equinovarus guideline has been endorsed by the NSW Ministry of Health and is now available on the NSW Health intranet site:

http://www0.health.nsw.gov.au/policies/gl/2014/pdf/GL2014_014.pdf

The guideline will be used to support the existing CTEV education program which includes an e-learning module, a one-day workshop and opportunity for clinicians to attend a CTEV Clinic at one of the tertiary children’s hospitals. This education program is implemented by members of the NSW CTEV Working Party with support from the NSW Child Health Network Allied Health Education & Clinical Support Program.

Initially, the guideline is intended for use by orthopaedic surgeons, paediatricians, physiotherapists and orthotists involved in the care and management of babies and children born with CTEV in NSW. Opportunities for making the guideline accessible interstate, and potentially internationally, will also be considered.

If you would like further details regarding the management of children with CTEV or related education opportunities, please contact the Physiotherapy department at one of the NSW tertiary children’s hospitals or Jenny Nicol (see contact details on page 7).

Apps for Allied Health

Mobile applications (apps) have a lot to offer allied health professionals and are becoming an essential part of a clinician’s toolkit. This section of A2K newsletter would like to offer allied health professionals a forum for sharing the apps that are most useful for them and how they incorporate them into therapy or their day to day work.

Dental Trauma

This app is endorsed by IADT - International Association of Dental Traumatology!

Dental Trauma First Aid - Knowledge is lacking among the general population in how to take care of a dental trauma at the scene of an accident.

For a totally knocked out tooth your first aid could be of vital importance to save the tooth. This app will help parents, teachers, sport coaches and others to make the best first aid. This app shows what to do, and if a visit to a dentist could be delayed or not, when a fragment of the crown is fractured, when there is increased mobility or dislocation of a tooth, or in case of a completely knocked out tooth.

CRANAplus Bullying

Bullying is unacceptable in any workplace and a team of psychologists at CRANAplus Bush Support Services has created this new app to help professionals recognise the signs. The app will help you identify the symptoms of bullying and then create an action plan to deal with the situation. The CRANAplus Bullying app is free to all remote and rural health practitioners and their families.

Please remember to consider the National Physical Activity Guidelines and Sedentary Behaviour Guidelines for Australians when recommending the use of apps to parents. Australia’s Physical Activity and Sedentary Behaviour Guidelines are supported by a rigorous evidence review process that considered:

the relationship between physical activity (including the amount, frequency, intensity and type of

physical activity) and health outcome indicators, including the risk of chronic disease and obesity;

and

the relationship between sedentary behaviour/sitting time and health outcome indicators, including

the risk of chronic disease and obesity.

Information can be found by visiting http://www.health.gov.au/internet/main/publishing.nsf/content/health-pubhlth-strateg-phys-act-guidelines.

Strong Hearing, Strong Start – Care For Kids’ Ears The Australian Government has developed the Care for Kids’ Ears campaign in an effort to support our children having a strong start in life, and good hearing is an important part of this. The campaign includes resources for parents and carers, early childhood groups, teachers and teachers’ aides and health professionals, providing important information on keeping kids’ ears healthy.

The Care for Kid’s Ears campaign aims to increase awareness among Aboriginal and Torres Strait Islander people(s) of ear disease, highlight risk factors for ear disease and promote the importance of seeking and following treatment to prevent hearing loss.

A range of electronic and hard copy resources have been developed to get these important ear health messages out to the community. A number of these resources have been translated to up to 22 Indigenous languages that are spoken across Australia.

A ‘Talking Book’ has been produced which communicates ear health promotion messages in English and a range of Indigenous languages. This engaging resource can be used by health workers in a professional setting or by parents and carers in the home. The ‘Talking Book’ is available for download at the itunes and Google Play store.

These new electronic resources complement the range of popular information resources that are already available through the campaign website. All the Care for Kids’ Ears resources aim to reduce ear disease and keep children’s ears healthy. For health professionals, regular ear checks for otitis media are important.

“Kids’ ears should be checked every time they’re at the clinic. If they’re coming in for an immunisation, a chest infection, or even a splinter on their toe – check their ears,” Sandi Nelson, Youth Hearing Health Coordinator says. “A quick 10 minute test can make all the difference. It’s impossible to underestimate the importance of kids’ hearing.”

The Care for Kids’ Ears resources for parents and carers, early childhood and community groups, teachers and health professionals can be downloaded or ordered from the Care for Kids’ Ears’ website: http://www.careforkidsears.health.gov.au/.

Online videos telling parents, health workers and teachers stories about the impact of ear disease on Aboriginal and Torres Strait Islander children can be viewed at: http://www.careforkidsears.health.gov.au/internet/cfke/publishing.nsf/Content/video
