Talipes Support & Education Have your say about your experiences managing CTEV and

the type of support and education you would like to receive

by completing the online CTEV Survey – it will take less than

10 minutes!!

Go to: http://www.surveymonkey.com/s/8HQ7FJL

Congenital Talipes Equino Varus (CTEV) is an area of clinical

practice for which many Physiotherapists have requested

further support and education.

In order to ensure that all Physiotherapists involved in the

treatment of CTEV have a clear understanding of Ponseti

Management, the NSW CTEV Working Party, with the NSW

Child Health Networks, is undertaking a number of

initiatives to provide support and education, including:

CTEV e-learning module (currently under

development)

Talipes workshop/s (to be announced)

Child Health Network Allied Health Secondment

Program.

In order to guide the provision of this education and support,

we need Physiotherapists who work with CTEV across NSW

to complete a brief survey. The survey aims to identify the

following:

your current level of involvement in the management

of clients with CTEV;

your current level of support and education relating

to CTEV management;

the type of support and education that you feel you

require to provide a quality service to clients with

CTEV.

The survey can be completed either:

Education Needs of Health Professionals Working with Children and

Young People in OOHC Questionnaire: Results

An on-line questionnaire was conducted at the end of 2011, to identify the education needs of health

professionals working with children and young people in out-of-home care. Thank you to those of you that took

the time to complete this questionnaire.

Context

The “Model Pathway for the Comprehensive Health and Developmental Assessment for All Children and

Young People Entering Out-of-Home Care” commenced on 1st July 2010 across NSW.

The “Health Pathway” ensures all children and young people entering out-of-home care have a health

assessment and a health management plan developed to ensure their health, psychological and

developmental needs are met in a coordinated way.

A range of health professionals across LHD‟s have been engaged in this work and commonly includes

Medical (Paediatricians, General Practitioners, Specialists), Nursing (Child and Family Nurses, Mental

health Nurses, Other Registered Nurses), Allied Health Professionals (Psychologists, Social workers,

Occupational Therapists, Speech Pathologists, Physiotherapists) and Oral Health Professionals

(Dentists, Dental assistants).

Method and Results

The questionnaire was created via “Zoomerang” and

distributed amongst OOHC Coordinators and across

OOHC networks.

There were a total of 564 respondents across NSW.

88% of respondents worked with children/ YP in OOHC

and 6% currently did not, but would in the future.

An incentive of 2 x $100 gift vouchers were offered, the winners were a nurse located in Bega and a

psychologist based in Ingleburn.

Breakdown of data per LHD has been provided to each of the OOHC Coordinators in the LHD.

Of the respondents, who currently worked in OOHC:

- 74% were Allied Health

- 6% Medical

- 17% Nursing

- 3% other

46% rated their current knowledge of needs of children/ YP in OOHC as “average”

56% rated their confidence in working with children/ YP in OOHC as “pretty confident”

When asked “Do you feel you currently receive adequate education, clinical support and supervision

in working with this population”? 50% indicated “no”.

When asked whether they would like further education and clinical support in working with children/

YP in OHC – 92% indicated yes

Areas for education were listed as: Working with children and young people, Assessment skills,

Professional Skills, Legal issues and Cultural issues (listed in order of popularity).

More specifically within assessment skills- the top 3 areas of need were identified as mental health

assessment and impact of abuse and neglect, social/ emotional well-being screen/ assessment and

trauma assessment/ framework.

Within the area of cultural issues- 52% indicated that they would like education on the health status

of Aboriginal and Torres Strait Islander children in OOHC.

Within legal issues- 37% indicated they would like education on writing reports for children in OOHC

and 35% indicated they would like education on new legislation and requirements of health

professionals.

.

.

Within working with children and young people there was a fairly even split amongst the topics listed-

working with foster carers, attachment theory, impact of trauma and abuse and impact of trauma on childhood development and function were the highest scoring topics.

Summary of findings

There is an identified need for health professionals to receive ongoing education, training and clinical

support when working with children and young people in OOHC (92% of respondents).

75% of questionnaire respondents were from an Allied Health Background.

There is also an identified skill or expertise base which needs to be tapped in to provide ongoing

education and support.

Recommendations:

Program of education to be rolled out, recommendations regarding clinical support to be provided to the

local OOHC Coordinators.

Plan a calendar of events for 2012 targeting education needs of health professionals working in OOHC.

Website to provide up-to-date information, articles, local pathways, processes and contacts.

Quarterly newsletter or publication to distribute to health professionals working in OOHC, which would

include current evidence, change in legislation/ policies, interagency changes etc.

Regular information and education to be included in the Allied 2 Kids Newsletter- Watch this Space!

Thank you to the staff in the Clinical Governance Unit at The Children‟s Hospital at Westmead for their

assistance in setting up and reporting on the questionnaire data.

Limitations of questionnaire

Technical error for those respondents that indicated at

question 1- No- not yet but will do in the future. Key

demographic information was not captured for these 33

respondents (6% of total respondents). Data was captured

separately, however, not entered into overall results as some

components were missing.

Acknowledge that the large percentage of respondents were

psychosocial clinicians (social workers, psychologists,

counsellors) which may have swayed the results in regards to

need for particular topics of education as well as rate of

participation in supervision as this is a requirement of their

profession.

The terms sensory integration and sensory processing often get used when referring to the challenges that

kids experience. These terms can be confusing and are often subject to disapproval, disbelief and

criticism.

Sensory integration has been a term used to describe the theory, the disorder (sensory integration

dysfunction) and the treatment (SI), which was first described by Jean Ayres in the 1960‟s. More recently,

Lucy Miller and colleagues have generated new terminology in order to help the understanding of SI by

clinicians, researchers and the wider community. They have proposed the terms:

Sensory Processing Disorder (SPD) and its subtypes (see diagram below) to explain the

disorder itself.

SI theory to explain the constructs describing the neurological processing of sensation and

subsequent behaviour, emotion, motor and attention responses.

OT-SI to explain occupational therapy intervention using a sensory integrative approach.

More information on this topic in the A2K in April

Prevalence rates of SPD-SMD in the general

population are estimated between 5-16%

(Ahn et al., 2004; Ben-Sasson et al., 2009) and

incidence rates suggest that SPD-SMD is

evidenced in 69-95% of children with ASD

(Baranek et al., 2006; Tomchek & Dunn, 2007) and

56%-69% of children with ADHD (Ahn et al.,

2004; Parush et al., 2007). Therefore, an

understanding of SPD and OT-SI is a vital

foundation of knowledge for assessing and

providing intervention to paediatric

populations.

1. DVD

The new DVD, developed by Siblings Australia, on supporting siblings is now available to preview on YouTube via the

following link:

http://www.youtube.com/user/SibAus

The DVD is designed to be shared with a group of parents and includes discussion notes which can be downloaded from

the DVD. It can be ordered via the website<http://www.siblingsaustralia.org.au/ps_resources.asp>. Older sibs (15+) may

also benefit from viewing the DVD.

2. Sibworks resources

There are only 25 of the Sibworks „special‟ packs left. These include the Facilitator Manual and the Participant booklets for

the children. These resources have been sent out to many organisations around Australia and some, overseas. They are a

great starting point if you would like to consider sibling support. Once these are gone, it is uncertain when we will have the

resources to re-print them. The resources can be ordered via the same link as the DVD order above.

3. Workshops

Workshops for parents and for providers (from disability, health, education and community services) have been run all

around Australia and overseas for nearly 13 years. These are a wonderful way to kick-start sibling support initiatives in your

area, or to help parents feel more confident in supporting their whole family. If you would like to consider one or both of

these workshops please contact me as below. I also offer consulting to organisations on how to best support siblings.

4. Facebook

Please „like‟ us on Facebook – link below. You will find up to date activities from Siblings Australia and other avenues. We

have members from all over Australia, and beyond, including families and agencies.

5. Advocacy

Siblings Australia is continuing to advocate for siblings to be recognised and supported by governments. Families Australia

is working with us to further qualify how the recommendations from the RANZCP Position

Problem-solving in

Paediatric Swallowing

& Feeding

Nutritional needs and pulmonary health always take top priority in therapeutic decision-

making. This workshop will guide participants through the critical decision-making and

problem-solving processes involved in assessment and management of feeding disorders

in medically complex children and children with sensory issues, using an evidence-based

approach, through lecture, videos, and interactive discussions. The multifactorial aspects

of paediatric feeding disorders, including physiologic, sensory-motor and

neurodevelopmental will be discussed in the context of health, medical and developmental

factors. Participants are encouraged to submit case studies one month prior to the

workshop for discussion during the course of the 2 days.

25th October

Facilitating Infant

Feeding: NICU and

Beyond

This workshop focuses on feeding issues in the Neonatal Intensive Care Unit (NICU) with

preterm and term infants. Professionals in this environment are required to have extensive

knowledge about pre-natal development and disorders that can occur in utero and/or the

perinatal period. The interactions of upper airway, cardiopulmonary, gastrointestinal, and

neurologic systems are extraordinarily complex. All have direct bearing on anticipation and

expectations for advancing oral feeding. In addition, parent/infant bonding is critical as is

adequacy of nutrition and hydration. Assessment and management decision making will be

discussed through lecture, video examples, and discussion with active involvement by the

participants.

This workshop focuses on instrumental swallow evaluations with topics to include rationale

for carrying out VFSS, practise identifying anatomic landmarks, interpreting VFSS findings,

and decision-making related to sensorimotor factors in the various stages of swallowing.

Given current concerns about radiation exposure in light of needs to define swallow

physiology, evidence-based best practices will be described and discussed. Case studies

will provide participants with clinically relevant information with active participation in

decision-making. Participants are encouraged to submit video examples of complex

patients one month prior to the workshop.
