Question If there is a threshold requirement, is it based on data submitted to CMS or

data accepted by CMS? As you are aware, certain technical problems within the CROWNWeb system cause the submission of certain data to be rejected by the system. Until these issues are addressed, it will not be possible to have 100% completed data.

Answer Facilities meet data submission requirements once data is “accepted” by CROWNWeb. Per the 2008 ESRD Conditions for Coverage, data must be submitted in the format specified by CMS. This requirement includes formatting data correctly so that it is accepted by CROWNWeb. If there are technical issues preventing the successful acceptance of data in CROWNWeb, facilities should contact the QualityNet Help Desk and open a Help Desk Ticket for assistance with the resolving these incidents.

Question What is the consequence of an ESRD facility failing to meet the compliance

threshold? If there is no threshold, would an ESRD facility be deemed to be out of compliance with the Conditions for Coverage if less than 100% of the data submitted to CMS was accepted by CMS?

Answer The successful submission and acceptance of all patient data is critical to the success of CROWNWeb, the Quality Incentive Program, and other programs that rely on a complete body of ESRD data for their operation. Any penalties associated with not achieving compliance with the data submission requirements will be announced in future communications.

Answer When experiencing difficulty submitting data, facilities should request assistance as follows:

• Facilities that submit data via EDI (“batch”) should first contact their Corporate Help Desk. NRAA HIE participants should first contact their HIE software providers. Work with these individuals to ensure that data is correctly formatted in accordance with the XML schema distributed by CMS. If problems continue to arise, contact the QualityNet Help Desk at (866) 288-8912 or [email protected].

• Facilities that submit data manually should contact the QualityNet Help Desk (using the contact information above) only if they are experiencing TECHNICAL DIFFICULTY. This includes non-working passwords, CROWNWeb malfunctions, and non-resolvable technical errors.

• For TRAINING ASSISTANCE (to learn how to complete a task), facilities should contact the Training Team by visiting http://help.mycrownweb.org. Under Training Support, click "I don't know how to do something in CROWNWeb." to submit a training request.

Question My PD patient is coming in center for a short period of backup hemodialysis treatments. Do I

need to enter a new Treatment Record for this? Answer It depends on where the patient is coming from.

• If the patient is receiving 1-2 HD treatments at the same facility that they receive PD: You do NOT need to enter a new treatment record, or alter the admit/discharge record in this instance. This temporary treatment does not require that a new treatment record be created.

• If the patient is receiving extensive HD treatments at the same facility that they receive PD: Consult with the physician to determine if this is going to be a permanent change in treatment. If the physician determines this is no longer a backup course of treatment, then you may need to create a new Add Treatment record for the patient.

• If the patient is coming for HD from a different facility: If the patient is not currently admitted to your facility, the patient must be admitted as a Transient Patient if the number of backup sessions is anticipated to be 13 or less. When the patient returns to their original therapy, discharge the patient with a reason of Transfer.

Answer If data in CROWNWeb is incorrect, you must first fix the data in the originating data source. If you correct it in CROWNWeb only, the value will be overwritten with the incorrect value again when the data is uploaded again.
