Question What is the minimum clinical data submission requirement in
CROWNWeb to satisfy the ESRD Conditions for Coverage? Answer You must enter all required lab values for all admitted patients in
your facility once per month. Beginning with CROWNWeb 4.4, all lab values that are present on the clinical screens must have a valid entry. Additionally, facilities must enter all required CMS-2728 forms and CMS-2746 forms, report PART data once per month, address all Notifications and Accretions, and maintain updated facility and personnel information in CROWNWeb.

Question If there is a threshold requirement, is it based on data submitted to CMS or
data accepted by CMS? As you are aware, certain technical problems within the CROWNWeb system cause the submission of certain data to be rejected by the system. Until these issues are addressed, it will not be possible to have 100% completed data.
Answer Facilities meet data submission requirements once data is “accepted” by CROWNWeb. Per the 2008 ESRD Conditions for Coverage, data must be submitted in the format specified by CMS. This requirement includes formatting data correctly so that it is accepted by CROWNWeb. If there are technical issues preventing the successful acceptance of data in CROWNWeb, facilities should contact the QualityNet Help Desk and open a Help Desk Ticket for assistance with the resolving these incidents.

Question What is the consequence of an ESRD facility failing to meet the compliance
threshold? If there is no threshold, would an ESRD facility be deemed to be out of compliance with the Conditions for Coverage if less than 100% of the data submitted to CMS was accepted by CMS?
Answer The successful submission and acceptance of all patient data is critical to the success of CROWNWeb, the Quality Incentive Program, and other programs that rely on a complete body of ESRD data for their operation. Any penalties associated with not achieving compliance with the data submission requirements will be announced in future communications.

Question If an ESRD provider is having difficulty completing the data submission
either manually or using the batch mode or NRAA HIE, who can the ESRD provider contact to address the issue?
General
16
Answer When experiencing difficulty submitting data, facilities should request assistance as follows:
• Facilities that submit data via EDI (“batch”) should first contact their Corporate Help Desk. NRAA HIE participants should first contact their HIE software providers. Work with these individuals to ensure that data is correctly formatted in accordance with the XML schema distributed by CMS. If problems continue to arise, contact the QualityNet Help Desk at (866) 288-8912 or [email protected].
• Facilities that submit data manually should contact the QualityNet Help Desk (using the contact information above) only if they are experiencing TECHNICAL DIFFICULTY. This includes non-working passwords, CROWNWeb malfunctions, and non-resolvable technical errors.
• For TRAINING ASSISTANCE (to learn how to complete a task), facilities should contact the Training Team by visiting http://help.mycrownweb.org. Under Training Support, click "I don't know how to do something in CROWNWeb." to submit a training request.

Patients Admit/Discharge
FAQ Number:
00128
Question What is a System Discharge, and how is it different from a Transfer discharge? Answer A System Discharge occurs when another facility admits a patient before the patient's current
facility discharges them. CROWNWeb documents this event with a System Discharge in the patient's Admit/Discharge record. A Transfer discharge indicates that a facility discharged the facility prior to the patient's admission at another facility.

annually to be included on the CMS-2744 annual facility survey. When updating Vocational Rehab, change the status on the Edit Patient Attributes screen, then enter the new effective date for that status in the Effective Date field. Vocational Rehab information is only required for patients aged 18-54.

The CMS-2728 form cannot be submitted without this lab value, as it is used to determine if the patient truly is an ESRD patient. If the patient arrives without this lab, obtain the lab on the date of admission.

Question Will I be able to continue to use ICD-9-CM codes after October 1,
2015 for patients who started at our facility prior to October 1, 2015? If so, how long will CROWNWeb allow users to select ICD-9-CM codes?
Answer The availability of an ICD-9-CM or ICD-10-CM code in CROWNWeb is dependent on the Patient’s Admit Date on the Admission Record associated to the CMS-2728 form. If the patient has an Admit Date prior to 10/1/2015, than the users can only select or submit ICD-9 codes. If the patients Admit Date is on or after 10/1/2015, only ICD-10 codes will be available for selection or submission.

Question My PD patient is coming in center for a short period of backup hemodialysis treatments. Do I
need to enter a new Treatment Record for this? Answer It depends on where the patient is coming from.
• If the patient is receiving 1-2 HD treatments at the same facility that they receive PD: You do NOT need to enter a new treatment record, or alter the admit/discharge record in this instance. This temporary treatment does not require that a new treatment record be created.
• If the patient is receiving extensive HD treatments at the same facility that they receive PD: Consult with the physician to determine if this is going to be a permanent change in treatment. If the physician determines this is no longer a backup course of treatment, then you may need to create a new Add Treatment record for the patient.
• If the patient is coming for HD from a different facility: If the patient is not currently admitted to your facility, the patient must be admitted as a Transient Patient if the number of backup sessions is anticipated to be 13 or less. When the patient returns to their original therapy, discharge the patient with a reason of Transfer.

Question On my Clinical data screens, I do not see patients that I should be seeing. What do I do? Answer Check to see if you have selected the correct treatment type (HD/PD) for the patient you are
trying to locate. Patients will only display for the collection type selected for that clinical month, and only if they were admitted to your facility during the clinical month. If you have the correct treatment type selected, check to see if data has already been submitted for that patient for that month. Patients with data successfully submitted once during the month will no longer appear on the Without Clinical Data search results.

Answer If the error has been identified prior to the clinical month closing, simply resubmit the correct lab value to overwrite the incorrect one. Errors cannot be corrected once a clinical month closes. If the lab data was submitted via EDI (batch), you must correct the lab value in the source system and then re-upload the data to CROWNWeb. Question Do we enter lab values for a month only if the data was obtained during that month? Answer Yes. Beginning with CROWNWeb 4.4, all lab values that are present on the clinical screens must
have a valid entry. You should enter all required lab values for a patient. In the event that you did not obtain a specific lab value for a patient, you can check the “N/A” checkbox for that individual field, as shown in the image below. ALL clinical fields must have a valid lab value OR an “N/A” value checked, and “N/A” must only be used when there is no value present for a patient for a month.

Question If a patient is hospitalized for an entire reporting month, do I enter any clinical data?
Answer If you have labs from the hospital, they can be entered in CROWNWeb. However, if no labs were drawn for the patient during a calendar month, there is no clinical data required for that patient. If the patient appears on your Clinical list for that month, you can use the Master N/A option to indicate that there are no lab values available. Additionally, if a patient is hospitalized for the entire reporting month, you should discharge the patient from CROWNWeb effective on the date the patient left for the hospital.
