Where We WereMoffat Project 2008 – 2010• Partnership with carers centres and NHS• 4 main care groups on 9 hospital sites• Aim : to identify carers in the hospital setting,

involvement of carers in discharge, improving support for carers.

Where We WereWhat we learned• System very dependant on visible presence to

generate response – not sustainable• Need for carer identification to be part of

patient pathway• Identification of carer alone not enough to

provide good outcome for carers• Training needs to directly link to organisational

priorities and outcomes
