• Diseño 3+3. SD 25 mg bd po.

• 2 esquemas 2/7 o 3/7+ Taxol 80mg/m2 x 6 sem

• 20 pacientes MTD 50 mg bd 3/7

– DLT (1 ciclo 7 sem) : fatiga, mucositis, toxicidad cutánea (todas grado 3)

• Fase expansión ca ovario y pulmón

• Actividad antitumoral

( pre tto con taxanos)

• Pendiente correlación con PD data

Trabectedina • Trabectedina: RR 7-43.5%

• 0.58 mg/m2/semana (Krasner et al) : RR 6.3%.

Mediana PFS 2 meses

• 1.300-1.650mcg/m2 (Sessa et al): RR 7 % (2 pts, TTP 4 y 4.5 meses). EE: 28.6%.

• “Italian multicenter study” (*): 98 pts (31 resistentes-refractarios a platino)

• Dosis: 1.3 a 1.1 mg/m2 en infusión de 3 horas c/21 d

Mediana de regímenes previos: 4 (rango 1-6).

RR 12.9% (1 RC, 3 RP), mTFI 2 meses (rango 1-9), beneficio clínico (CB) en 17 pts (54.8%)

BRCA mutations. The BRCAness profile

• BRCA mutations carriers hava a more favorable clinical course and higher responsiveness to platinum-based therapies

• BRCA mutations carriers with early relapse occurring < 6 m following primary therapy may have a higher chance of response to platinum based CT *

• The BRCAness phenotype (epigenetic hypermethylation of BRCA 1 promoter, somatic mutations of BRCA 1, amplification of EMSY..) may also be important.
