School Student (Y4-6) Survey – Non-Consent Form I DO NOT give consent for my child/ren to participate in the St Joseph’s School, Childers student survey.

Name of Child/ren Year _______________________________ _________ _______________________________ _________

School Survey - Tell us what you think about St Joseph’s School, Childers

Here at St Joseph’s School, we are committed to providing the best possible Catholic education for your child. That commitment includes being open to feedback and ideas for improvement — so we're asking our Parents/Carers and Year 4-6 students about their experiences of our school.

As partners in your child's education, what you think matters. Below you will find a link to a short survey that will ask you about different aspects of our school. The survey also has space for you to add your own comments. The survey link is active until Monday 20th August. (Link to survey: https://www.surveygizmo.com/s3/4398090/2018-Parent-Survey?school=sj4660) Computers are available at school for parents to complete the survey at any time between 8:00am and 3:30pm.

We will be surveying each of our students from Year 4 to Year 6 at a convenient time during the week August 13-17. The survey is expected to take between 15-30 minutes. Support will be available for those students who may need help to complete the survey.

All responses will be anonymous, but they will be truly valued in helping us continually improve. We will ensure we provide our community with a summary of the parent and student survey findings later in the year as we will also be asking staff for their feedback.

If you do not wish your child/ren to complete the survey, please complete the non-consent form below and return to the school office by this Friday, 10 August 2018.

If you have any questions about the survey, please feel free to contact our Assistant Principal, Mr Liam Dougherty.

Thank you for your time.

Dear Parents,

School photographs are scheduled to be taken by advancedlife. Whilst an envelope and flyer will be distributed shortly, if possible it is our preference that ordering be completed online to reduce administration and potential security issues related to the return of cash and envelopes on photo day.

Orders for packages and sibling photographs can be placed securely online at www.advancedlife.com.au using our school’s unique 9 digit advancedorder code. Portrait and group package orders are due by photography day.

Should you wish to purchase a sibling photograph online, the order must be placed no later than the day before photography day. Sibling photographs will only be taken if an order has been placed.

Should you have any queries concerning school photographs or online ordering, please direct them via email to [email protected]

ORDER NOW go to www.advancedlife.com.au and enter the code
