Each step is important because in order to get the end result you must do each step. Omitting a step would change or alter the outcome.

5. What is meant by the activity of priority setting in the planning process? What factors need to be considered when assigning priorities? Nursing diagnoses are assigned high, medium, and low priorities in consultation with the client, if health permits. Prioities are set according to the current state of health of the client. The current situation is addressed by prioritizing the most critical issue down to the other iuuses currently going on

6. What is the purpose of desired goals and or outcomes? Is to observe the patient and see if they have been able to achieve the desired outcomes or goals. The goal is to to prevent, reduce, or eliminate a client’s health problems.
