ski/snowboard helmet at all times while skiing or snowboarding. Other pieces of added

safety equipment such as mouth guards, wrist guards, body armour and spine protectors

are also now available and may be warn for added protection***

the club, they will be asked to withdraw from the club. Refunds will only be offered IF another student is on the waitlist to take the

CALGARY ACADEMY CERTIFICATION OF CONSENT AND AUTHORITY

RELEASE OF LIABILITY, WAIVER OF CLAIMS,

ASSUMPTION OF RISK AND INDEMNITY AGREEMENT

This Waiver and Acknowledgements Form must be completed by a parent or guardian of the minor registering in the CASSC 2019-

2020 Program.

Part One - WAIVER and RELEASE

I, _________________________________ (Parent or Legal Guardian) hereby for myself, my heirs, executors, administrators and

assigns, waive any and all claims that I have or may have in the future against Calgary Academy and CASSC (Calgary Academy

Ski/Snowboard Club), their executive directors, servants, agents, sponsors, supporters, employees, contractors or volunteers (all of

whom are hereafter the Releasees) and to release the Releasees from any and all liability for any loss, damage, expense or injury

including death that my son/daughter may suffer as a result of their participation in any activity (hereafter the Event), by one or more

of the Releasees, due to any cause whatsoever, including negligence, breach of contract, or breach of any statutory or other duty of

care owed on the part of the Releasees and further including failure on the part of the Releasees to safeguard or protect my

son/daughter from the risks, dangers and hazards associated with the Events scheduled by CASSC.

I further waive and release any and all claims that I may have in the future, on my own behalf, against Lake Louise and Alberta

Freeskiers. I agree that my son/daughter will abide by CASSC regulations and the directions of CASSC personnel. Failure to do so

may result in CASSC terminating my son/daughter from the CASSC activities immediately.

I hold harmless and indemnify the Releasees from any and all liability for any property damage or personal injury to any third party

resulting from the participation of my son/daughter in the CASSC Events. I further waive and release any and all recourse which I

may now have or may in the future have resulting from any decisions of the Releasees in association with the CASSC Events,

including any claims I might have in connection with any cancellation or rescheduling of the Events for whatever reason.

Risks and Dangers: I acknowledge that activities such as skiing/snowboarding or any cross training activities related to CASSC

involves risks and dangers including, but not limited to: accidents that occur traveling to and from the event; slips, trips, falls or

collisions; encounters with other skiers, obstacles (manmade or natural) or wildlife; steep and varied terrain, uneven snow conditions;

falling snow or ice; adverse weather conditions; frostbite; hypothermia; exhaustion. I acknowledge that it is the policy of Calgary

Academy to take all reasonable precautions with respect to such activities and to provide a safe environment. I freely accept and

fully assume all risks, dangers and hazards associated with my child’s participation in CASSC Events and the possibility of personal

injury, death, property damage or loss resulting therefrom.

Health: I declare that my son/daughter’s physical condition, to the best of my knowledge, is adequate to participate safely in the

CASSC Events and that no physician or other medically qualified individual has advised me against participating in the Events. I

further acknowledge and agree that it is my choice whether I will obtain a physical examination for my son/daughter prior to

participating in the Events, and I hereby acknowledge and agree to assume all risks associated with not obtaining such examination,

or if I do obtain an examination and am instructed to not allow my son/daughter to participate in the CASSC Event, I hereby assume

all risks associated with my son/daughter’s participation.

Cancellation or Rescheduling: I acknowledge that CASSC, may choose to cancel or reschedule the Events if travel or Event

conditions are deemed unsafe (e.g., weather, temperature road conditions, snow conditions, health advisory). I accept that these

organizations will not be liable for any costs I may bear as a result of such cancellation or rescheduling.

If my son/daughter becomes ill or incapacitated, I agree to appoint CASSC and its employees to take any action they deem necessary

for my son’s/daughter’s safety and well being, including securing medical treatment at my own expense (ambulance ride costs

approx $950.00 from Lake Louise to Banff) and transporting my son/daughter home.

I understand that I will be required to pay for my son/daughter, any phone calls or incidental personal expenses that my son/daughter

incurs while on the CASSC trips, as well as for any damage my son/daughter may cause while on the CASSC trips.

I understand that CASSC students are under complete supervision while participating in CASSC Events until the scheduled time,

after which time they will remain at the school without supervision until picked up by a parent or guardian. Supervision ends at

5:45pm during the Lake Louise trips, after which time, students will be left unsupervised at Calgary Academy until picked up by a

parent or guardian.

I understand and consent that Calgary Academy may use any film likenesses taken of my son/daughter and any of my

son’s/daughter’s comments while on the CASSC trips for school and club promotion only.

I have read and understand the above acknowledgements and accept the terms and conditions of my son/daughter’s participation in

CASSC (2019-2020) and confirm all information is true and accurate. I understand that the information I provide in regards to the

ability level of my son/daughter is very important for the safety of my child. I verify that the information that I have provided to

CASSC – Calgary Academy Ski/Snowboard Club above is accurate.

Club Cold Weather Policy

1. We will communicate via email and telephone when training needs to be cancelled due to cold weather or hill closures.

2. We ask that you do your best to check the hills website and the team website before coming if you think that they may be closed due to weather.

3. The decision to cancel a training session will be done on a case by case basis, given the weather conditions at the time. 4. It is the policy not to cancel any training sessions unless it is deemed to be dangerous to the athletes and the coaches

to be outside at any given time. 5. Please dress appropriately for any weather that may be encountered outside.

Athlete Code of Conduct Training Policy

1. It is important that you listen to your coach during training sessions for safety. 2. If there is a situation that arises and an athlete does not listen there will be a verbal warning given to the athlete. 3. If another incident is cited during the same training session the athlete will be removed from training for the day and

given a written warning. 4. If another incident is cited during another training session the athlete will be removed from training for 2 weeks and

given another written warning. 5. If a 3rd incident is cited during the season the athlete will be removed from the program for the season with no

refunds given. Club Safety and Helmet Policy

To ensure the commitment to the safety, welfare and wellbeing of athletes, volunteers and parents is maintained at all times during their participation in the activities run by Alberta Freeskiers It is our aim to promote a safe environment for all and to endorse the use of best practices and all safety equipment to better enhance the enjoyment and promotion of our sport.

Application

When members are actively participating in Club activities or promotion of the sport, they must wear all appropriate safety gear including an approved helmet and apparel and practice all appropriate courtesy and etiquette expected in pursuit of sporting excellence and general recreational enjoyment for all.

Protective Mechanisms:

Assess and be aware of course/slope hazards and conditions

Proper use of all club required safety related sporting gear

Demonstrate to and train all participants specifically in the proper care and use of protective helmets

Lead by example for maintenance of sportsman like behaviour and etiquette at all times

Responsibility: Coaches: are responsible to facilitate and/or provide proper instruction to all participants on the personal protection requirements, training, and course condition inspections and enforcement of same. Participants: are responsible to safeguard themselves and others around them by safely participating in all club activities and properly wear all gear required by the club for their personal protection. They are also expected to adhere to all clubs rules, policies and requirements at all times. Parents: are responsible to ensure their children understand and adhere to the rules of the club and that the children are properly and actively using all protective equipment at all times during club activities.

Alberta Freeskiers Release Form

In consideration of Alberta Freeskiers accepting the application of the person identified at the end of this document

as the “Member” to become a member of the Club and permitting that person to participate in one or more Club

programs, the Member covenants and agrees as follows:

1. If the Member is 18 years of age or over (an “Adult”), the Member represents and confirms that the Member

is not legally incapacitated (whether pursuant to the Bankruptcy and Insolvency Act (Canada), the Dependent

Adults Act (Alberta) or other applicable legislation) from granting this liability release.

2. If the Member is under 18 years of age (a “Minor”), the person identified at the end of this document as the

“Guardian” represents and confirms that the Guardian is 18 years of age or over; is not legally incapacitated

(whether pursuant to the Bankruptcy and Insolvency Act (Canada), the Dependent Adults Act (Alberta) or

other legislation) from granting this liability release; has the legal capacity and authority to grant this liability

release on behalf of the Minor; and this liability release binds and is effective against the Minor.

3. The Member (or, if the Member is a Minor, the Guardian) acknowledges that skiing and snowboarding each

involves many risks, dangers and hazards including, but not limited to: boarding, riding and disembarking ski

lifts; changing weather conditions; avalanches; exposed rock, earth, ice, and other natural objects; trees, tree

wells, tree stumps and forest dead fall; the condition of snow or ice on or beneath the surface; changes or

variations in the terrain which may create blind spots or areas of reduced visibility; changes or variations in

the surface or sub-surface, including changes due to man-made or artificial snow; variable or difficult snow

conditions; streams, creeks and exposed holes in the snow pack above streams or creeks; cliffs; crevasses;

snowcat roads; road-banks or cut banks; collision with lift towers; fences, snow making equipment, snow

grooming equipment, snowcats, snowmobiles or other vehicles, equipment or structures; collision with other

skiers or snowboarders; the failure to ski or board safely or within one’s own ability or within designated

areas; negligence of other skiers and snowboarders, and negligence on the part of ski area operators and staff

to safeguard or protect me from the risks, dangers and hazards of skiing or snowboarding. The Member (or,

if the Member is a Minor, the Guardian) is also aware that the risks, dangers and hazards referred to above

exist throughout the ski areas and many are unmarked.

4. The Member (or, if the Member is a Minor, the Guardian) acknowledges that ski or snowboard competitive

training and competitive events each involves many risks, dangers and hazards including, but not limited to

all the risks described in paragraph 3 above, and the additional risks of collision with race start area equipment

and structures, race course gates, finish area equipment and structures, fencing, netting and supporting

structures, spectators and race officials; and the state and changing state of the race course or terrain itself,

including race course jumps and other terrain features.

5. The Member (or, if the Member is a Minor, the Guardian) acknowledges and agrees that, while providing

alpine ski and snowboard programs, the directors, officers, employees, coaches, volunteers, members and

others providing services to the Clubs are not personally liable for any injury, damage or loss the Member

might suffer as a result of the Member’s participation in any such program(s).

6. The Member (or, if the Member is a Minor, the Guardian) is aware of the risks, dangers and hazards associated

with skiing and snowboarding generally, and freely accepts and fully assumes all such risks in respect of the

Member’s participation in all Club activities.

7. The Member (or, if the Member is a Minor, the Guardian) hereby waives any and all claims that the Member

has or may have in the future against the Clubs, their directors, officers, employees, coaches, volunteers,

members and others providing services to them, and their successors and assigns, (all of whom are hereinafter

collectively referred to as "the releases") and hereby releases the releasees (and each of them) from any and

all liability for any loss, damage, expense or injury (including death) that the Member may suffer resulting

from the Member’s participation in any Club program (and participation includes all training, races, travel,

accommodation and other ancillary involvement in any program) due to any cause whatsoever, including

negligence, breach of contract, or breach of any statutory or other duty of care on the part of the releasees or

any or some of them, and also including the failure on the part of the releasees to safeguard or protect the

Member from the risks, dangers and hazards of skiing and snowboarding referred to above.

8. The Member (or, if the Member is a Minor, the Guardian) shall hold harmless and indemnify the releasees

and each of them from any and all costs, claims, actions or proceedings arising in respect of any damage to or

destruction of property of, or expense or personal injury (including death) to, the Member or to any third party,

resulting from the Member’s participation in any Club program (and participation includes all training, races,

travel, accommodation and other ancillary involvement in any program).

9. This agreement shall be effective and binding upon the Member (and, if the Member is a Minor, the Guardian),

and each of the heirs, next of kin, executors, administrators and representatives of the Member (and, if the

Member is a Minor, the Guardian). This agreement is governed by and interpreted in accordance with the laws

of the Province of Alberta and any litigation involving the parties to this agreement shall be brought within

the Province of Alberta. If the Member is a Minor, this agreement is binding upon the Guardian and Minor

even if signed only by the Guardian.

In giving this liability release the Member (or the Guardian, if the Member is a Minor) is not relying upon any oral

or written representations or statements made by the releasees with respect to the safety of skiing or

snowboarding or the nature of the Clubs' programs.

The Member (or the Guardian, if the Member is a Minor) has read and understands this agreement and is aware

that by signing this agreement the Member is waiving certain legal rights which the Member or the Member’s

heirs, next of kin, executors, administrators, and representatives may have against the releasees.

CANADA FREESTYLE SKI ASSOCIATION 2019/2020 Waiver

Injury Waiver

I recognize that skiing entails serious risks. Consequently, I relinquish all rights to an appeal of any kind,

including the right to an appeal for bodily and material damages, regardless of the cause, against the

Canadian Freestyle Ski Association, the Canadian Snowsports Association, its officers, employees,

assignees, agents, representatives, and sponsors, even if such damages result from negligence of the

Canadian Freestyle Ski Association, the Canadian Snowsports Association, its officers, employees,

assignees, agents, representatives, and sponsors.

Without restricting the generality of the preceding, I also relinquish the right to any appeal against the

Canadian Freestyle Ski Association, the Canadian Snowsports Association, its officers, employees,

assignees, agents, representatives, and sponsors resulting from a decision on their part, regardless of

the nature of this decision.

I also recognize that skiing entails serious risks. Considering my participation, I also relinquish all rights

to an appeal of any kind, including the right to an appeal for bodily and material damages, regardless of

the cause, against the Canadian Freestyle Ski Association, the Canadian Snowsports Association, its

officers, employees, assignees, agents, representatives, and sponsors, even if such damages result

from negligence of the Canadian Freestyle Ski Association, its officers, employees, assignees, agents,

representatives, and sponsors. NOTE: Authorization is needed for Aerials/Inverts by parent or Guardian

if registrant is less than 18 years of age.

Photo Consent I authorize the the Canadian Freestyle Ski Association (CFSA) and/or its staff, associates, assistants,

or subcontractors to use photograph(s), video and other media image provided for any CFSA

promotional, educational or other pertinent uses.

I also agree to CFSA using any images of the named minor(s) gathered by photographers, and or

videographers supplying images to CFSA.

These images may include, but are not exclusive to, club, coach, athlete and/or parent, and/or volunteer

submissions for the “Freestyle’s Biggest Fans” Contest, the “Out There” section of freestyleski.com, and

various Club and PSO reports to CFSA.

I authorize the CFSA to permit the use and display of photographs and/or recordings of the named

minors in any CFSA publication, multimedia production, including video and web usage, display, or

advertisement.

I agree that the CFSA may use name, likeness, or information supplied by the undersigned.

The undersigned releases and forever discharges the CFSA, its agents, officers and employees from any

and all claims and demands arising out of or in connection with the use of said photographs / recordings,

including but not limited to, any claims for invasion of privacy or defamation.

Privacy Policy Collection of Information

CFSA collects Personal Information through the Registration Site for the following purposes:

(a) to register a user with CFSA;

(b) to create summary membership reports, including but not limited to location of residence

(city, province, country), gender, age, language, and membership license;

(c) to secure insurance for the benefit of the CFSA and its members.

(d) to respond to inquiries about CFSA and the Registration Site made through the Registration Site;

(e) to troubleshoot problems with the Registration Site and to improve the Registration Site;

(f) to enforce the Registration Site Terms and Conditions of Use;

(g) to conduct marketing, including promotional activities;

(h) to conduct surveys; and

(i) to take any other action required or permitted by law

CFSA also collects non-personal information each time users access or use the Registration Site.

The non-personal information CFSA collects includes the user’s URL, IP address, browser and

operating system information, and the location (country and province) where the user’s server is

located.

CFSA uses “cookie” technology to track user preferences and activities on the Registration Site.

Cookies are small amounts of data that are transferred from CFSA’s server to your computer’s hard

drive for record-keeping purposes. Cookies may store a variety of non-personal information about how

you use the Registration Site. If you do not wish to accept cookies, you have the option of blocking or

disabling cookies. However, please be aware that you may be unable to access certain parts of the

Registration Site if you do block or disable CFSA’s cookies.

Choice and Opt-Out Options CFSA does not collect Personal Information through the Registration Site unless users choose to

provide it. If you do not wish to have CFSA collect your Personal Information through the Registration

Site, you can choose not to do the following:

(a) become a member of the CFSA via the Registration Site;

(b) provide feedback to the CFSA through the Registration Site;

(c) participate in surveys or promotions.

Use of Personal Information CFSA will use the Personal Information it receives through the Registration Site for the various purposes

outlined above under “Collection of Information”.

By providing Personal Information through the Registration Site, users acknowledge and agree that

CFSA may use their Personal Information for the purposes set out above.

Disclosure of Personal Information CFSA may disclose Personal Information it receives through the Registration Site to third party service

providers and other agents if reasonably necessary to further the purpose for which the Personal

Information was collected.

CFSA reserves the right to disclose Personal Information if required to do so by law or legal process or

otherwise requested by any law enforcement officer or agency acting under colour of law. In addition,

CFSA reserves the right to disclose Personal Information in order to (a) enforce the Registration Terms

and Conditions of Use; (b) protect and defend the rights or property of CFSA; or (c) act in urgent

Protection of Personal Information CFSA has implemented reasonable physical and technical measures to protect Personal Information

obtained through the Registration Site against loss or theft, unauthorized access, use or disclosure. [For

example, the Website uses Secure Sockets Layer (“SSL”), an industry standard security protocol which

encrypts information users enter while using the Website.]

Retention of Personal Information CFSA may keep Personal Information it obtains through the Registration Site for so long as is necessary

or appropriate to carry out the purpose(s) for which such information was collected or for such longer

period as may be required or permitted by applicable laws and regulations.

Accuracy CFSA will do its best to ensure that the Personal Information it obtains through the Registration Site is

accurate and current.

RELEASE OF LIABILITY, WAIVER OF CLAIMS,

ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT

(hereinafter referred to as “Release Agreement”)

PLEASE READ CAREFULLY!

TO: CANADIAN SNOWBOARD FEDERATION (doing business as Canada~Snowboard) (“CSF”) and its Provincial/Territorial Snowboard Associations and Member Clubs,

and CANADIAN SNOWSPORTS ASSOCIATION, including their respective directors, officers,

representatives, employees, agents, independent contractors, sponsors, volunteers, successors and

assigns (all of whom are hereinafter referred to as the “Releasees”).

DEFINITIONS In this Release Agreement the terms “Activities” and “Programs” shall be

interchangeable and shall include, without limitation: all activities and events involving the Releasees or

in any way related to snowboard training, competition, orientation, coaching or instruction.

ACKNOWLEDGEMENT – SNOWBOARD SAFETY I acknowledge that at all times while

snowboarding I am required and have been advised to wear a protective snowboarding helmet that is

certified for snow sports in accordance with CSF’s Helmet Policy and Helmet Information Sheet, which

can be found at: www.canadasnowboard.ca.

ASSUMPTION OF RISKS I FREELY ACCEPT AND AM FULLY AWARE of the dangers involved in the performance of the

Activities and Programs, and the dangers inherent to snow sports, including those caused by

gravitational forces or otherwise. I understand and accept that by engaging in the Activities and

Programs, my life and physical safety are endangered. FURTHERMORE, I understand and accept that

the above mentioned dangers exist within the Activities and Program areas, and may include

environmental conditions, technical equipment and atmospheric influences, or natural or manmade

obstacles. I am aware that certain movements or actions cannot always be anticipated or controlled and

therefore cannot be avoided or prevented through safety measures. I am aware of the physical exertion

required to participate in the Activities and Programs, and I am aware that this exertion may activate, re-

activate or aggravate pre-existing injuries, conditions, symptoms or congenital defects.

I ACKNOWLEDGE that it is my sole responsibility to assess whether any Activities or Programs are

too difficult for me. By commencing an Activity or Program, I acknowledge and accept the suitability and

conditions of the Activity or Program. I also agree that I am responsible for the choice of my equipment

and for the selection of my line through the course and my ability to handle this line, inclusive of

inverted air manoeuvres.

BY SIGNING THIS DOCUMENT YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE OR CLAIM COMPENSATION FOLLOWING AN ACCIDENT

FURTHERMORE, I am aware that snowboarding and participation in the Activities and Programs

involves many inherent risks, dangers and hazards including, but not limited to: boarding, riding and

disembarking ski lifts; changing weather conditions; avalanches; exposed rock, earth, ice, or other

natural objects; trees, tree wells, tree stumps, forest dead fall; the condition of snow or ice on or beneath

the snow surface; changes or variations in the terrain used for snowboarding, including the Activities and

Programs, which may create

blind spots or areas of reduced visibility; changes or variations in the snow surface or subsurface,

including changes due to man-made or artificial snow; variable and difficult snow and weather

conditions; streams, creeks and exposed holes in the snow pack above streams or creeks; cliffs;

crevasses; snowcat roads, road banks or cut-banks; impact or collision with lift structures, fences, snow

making equipment, snow grooming equipment, snowcats, snowmobiles and other vehicles, equipment,

structures, or objects or equipment used in connection with skiing or snowboarding and Activities and

Programs of the Releasees; snowcat roads; road-banks or cut banks; impact or collision with other

persons, including other skiers, snowboarders, racers, spectators, participants, competitors or course

officials; the failure to act safely or within one’s own ability or within designated areas; negligence of

other persons, including other skiers, snowboarders, racers, spectators, participants, competitors,

course officials; and NEGLIGENCE ON THE PART OF THE RELEASEES, INCLUDING FAILURE ON THE PART OF THE RELEASEES TO TAKE REASONABLE STEPS TO SAFEGUARD OR PROTECT ME FROM THE RISKS, DANGERS AND HAZARDS ASSOCIATED WITH PARTICIPATION IN THE ACTIVITIES AND

PROGRAMS. I am also aware that the risks, dangers and hazards referred to above exist throughout the area used for skiing, snowboarding and the Activities and Programs of the Releasees, and that many are unmarked. I FREELY ACCEPT AND FULLY ASSUME ALL SUCH RISKS, DANGERS AND HAZARDS ASSOCIATED WITH THE ACTIVITIES AND PROGRAMS AND THE POSSIBILITY OF PERSONAL INJURY, DEATH, PROPERTY DAMAGE AND LOSS RESULTING THEREFROM.

RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT In consideration of the granting of my membership in any or all of the Releasees’ organizations, and in

consideration of permitting me to participate in the Activities and Programs, I HEREBY AGREE AS FOLLOWS:

1. WAIVER TO WAIVE ANY AND ALL CLAIMS that I have or may in the future have against THE

RELEASEES, AND TO RELEASE THE RELEASEES from any and all liability for any loss, damage, expense or injury, including death, that I may suffer or that my next of kin may suffer, as a result of my presence at or participation in the Activities and Programs DUE TO ANY CAUSE WHATSOEVER, INCLUDING NEGLIGENCE, BREACH OF CONTRACT, OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE, INCLUDING ANY DUTY OF CARE OWED UNDER ANY APPLICABLE OCCUPIERS LIABILITY LEGISLATION ON THE PART OF THE RELEASEES. I UNDERSTAND THAT NEGLIGENCE INCLUDES FAILURE ON THE PART OF RELEASEES TO TAKE REASONABLE STEPS TO SAFEGAURD OR PROTECT ME FROM THE RISKS, DANGERS AND HAZARDS ASSOCIATED WITH THE ACTIVITIES AND PROGRAMS;

2. TO HOLD HARMLESS AND INDEMNIFY THE RELEASEES from any and all liability for any damage to property of or personal injury to any third party, resulting from my participation in the Activities or Programs. I understand that this means I will reimburse the Releasees if a claim is made against them in connection with my participation in the Activities or Programs;

3. This Release Agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives, in the event of my death or incapacity;

4. This Release Agreement and any rights, duties and obligations as between the parties to this Release Agreement shall be governed and interpreted solely in accordance with the laws of the Province of British Columbia and no other jurisdiction; and

5. Any litigation involving the parties to this Release Agreement shall be brought solely

within the Province of British Columbia and shall be within the exclusive jurisdiction of the Courts of the Province of British Columbia. PERSONAL LIABILITY

6. I UNDERSTAND that I may be found personally liable to third parties for damages arising from bodily injury or property damage they have suffered as a result of my participation in the Activities or Programs. I agree that the Releasees have no obligation or responsibility for my actions and no obligation to inspect my equipment. I agree that I will make myself familiar with the applicable Activities and Programs regulations before taking part in such Activities or Programs.

MEDICAL COVERAGE 7. I DECLARE that I have current excess medical coverage which must include, without limitation, out-of-country medical coverage, mountain rescue, and repatriation, including my participation the Activities and Programs of the Releasees. I ACKNOWLEDGE AND AGREE that it is my sole responsibility to satisfy myself of the sufficiency of my insurance and that the Releasees bear no responsibility in this regard.

RULES 8. I HEREBY AGREE TO ABIDE by and conduct myself in accordance with all rules, regulations, by-laws and policies of the Releasees.

RELEASE OF MEDICAL INFORMATION 9. I HEREBY AUTHORIZE all doctors, hospitals and medical personnel of any kind whatsoever who may attend to me in connection with the Activities or Programs to release to the Releasees any of my personal health or medical information, insofar as it relates to the issue of my ability or capacity to participate in any Activities or Programs.

PHOTOGRAPHIC MATERIAL 10. I HEREBY AUTHORIZE the Releasees to take my photograph, and to use any and all images of me, including video footage, taken at or in connection with the Activities or Programs. I consent to the use of all such images, photographs and video footage by the Releasees, whether in connection with the Activities or Programs or future similar events, without any compensation to me whatsoever. I HEREBY SURRENDER all rights to any photographs, images or video footage taken of me in connection with the Activities or Programs, which I acknowledge are the property of the Releasees alone.

I ACKNOWLEDGE AND AGREE THAT by entering into this Release Agreement I am not relying

on any oral or written representations made by the Releasees with respect to the safety of the Activities

or Programs other than what is set forth in this Release Agreement.

I DECLARE THAT I am the parent or lawful guardian of the Participant and, as such, I AGREE TO HOLD HARMLESS AND INDEMNIFY THE RELEASEES from any and all

liability for any damage to property of or personal injury to the Participant or any third party resulting

from the Participant’s participation in the Activities or Programs.

I AGREE AND ACKNOWLEDGE that in the event this Release Agreement is not enforceable

against the Participant, I will indemnify the Releasees from and for any and all expenses, including but

not limited to legal costs, disbursements and payments made pursuant to a settlement or judgment,

incurred in connection with the Participant’s participation in the Activities or Programs.

IN CONSIDERATION OF the Participant being accepted as a registered member of the Releasees

and in consideration of the Participant being permitted to participate in the Activities and Programs of the

Releasees, I, being a parent or lawful guardian of the Participant, HEREBY CONSENT to such

participation by the Participant and acknowledge and agree to the terms hereof on my own behalf as

well as that of the Participant to the extent permitted by law.
