All Passes allow access for member and one free assistant to All County Fitness Centers.

Depending on type of pass gives you access to Indoor Pools, Ice Skating, Outdoor Pools and ALL County Fitness Centers

1. A statement signed by a licensed physician attesting that the

applicant has a permanent physical, mental, or sensory impairment

that severely limits one or more major life activity, and states the

nature of the impairment. 2. A document issued by a federal agency, such as the Veteran's

Administration, which attests that the applicant has been medically

determined to be eligible to receive federal benefits as a result of a

disability. Other acceptable federal agency documents include proof of

receipt of Social Security Disability Income (SSDI) or Supplemental

Security Income (SSI) due to a disability. 3. A document issued by a state agency, such as the vocational

rehabilitation agency, which attests that the applicant has been

medically determined to be eligible to receive that agencies benefits or

services as a result of medically determined permanent disability.

Showing a state motor vehicle department disability sticker, license

plate, or hang tag is not acceptable documentation.

To receive this benefit make an appointment and submit the

documentation listed above in person to Salt Lake County Adaptive

located at the Copperview Recreation Center, 385-468-1515.

A Salt Lake County resident, who is permanently disabled,

and can provide one of the following pieces of

documentation to receive a facility pass that permits the

holder an assistant entrance fee at no cost.

Socialization

Saturday Night Dance A live DJ spins popular favorites at our bi-monthly dances for individuals with disabilities ages 18 years and older.

2.Refund Policy: As per Salt Lake County policy and procedures, the Parks and Recreation Division may withhold 25% of

the refund (program registration fee) for administrative costs. All refunds must be requested in person and accompanied

3.Collections: I agree to pay Salt Lake County all costs incurred, together with reasonable attorney’s fees in the event that

my account is referred to the Salt Lake County Attorney’s Office for collection. I understand that any account delinquent

30 days or more will be turned over to the Salt Lake county Attorney for collection.

4.Emergency Treatment: I hereby authorize Salt Lake County Parks & Recreation program staff to act on my behalf in

accordance with their best judgment in case of an emergency involving myself/my child, and agree to assume full respon-

sibility for all expenses, medical or otherwise, that may arise there from. I understand that my insurance company or I will

Special Olympics

Special Olympics Bowling Who: For ages 8 and older who have intellectual disabilities.

You may choose either Tuesday or Thursday to practice.

Just let the coach know which day you will be practicing

for Special Olympics.

Cost: $85 (regular $55 for session plus additional $30 for Special

Olympic fees) includes practices, shirt and Special Olympic

Indoor rock climbing!!! This program is for all ages and all ability levels.

Trained staff will set-up, provide instruction and adaptive climbing aids.

Wheelchair Rugby A Paralympic high-action and high-contact sport that’s also called

“Murderball”. Lots of fun!

wheelchair.

6:00-9:00pm. Advanced play on Saturday

mornings from 7:00-11:00am.

Ages 15 and up may also be considered, by their individual skill level, for the Adult League. Youth: For ages 5-12 with intellectual disabilities who are learning the basic skills of basketball. Tuesday Adult Basketball League For adults with intellectual disabilities, 18 years and older, that want to play on a team and practice their basketball skills. This is for beginner to advanced skill levels.

Wheelchair Basketball For kids & adults, beginners & advanced players that can independently

Sports/Classes

Boccia Ball

Boccia Ball is one of our newer programs offered by our Rec.

Center. Boccia Ball is one of the largest and fastest-growing

sports in the world and it is a sport recognized by the Paralympics

and Special Olympics. You can play one-on-one or in teams. Lots

of fun.

Who: Individuals with physical and/or intellectual disabilities.

When: TBA Where: Central City Rec. Center

Power Soccer Power Soccer is for anyone that uses a power wheelchair for mobility.

Specialized guards are added to the front of the power chairs to kick

and move the ball without causing damage to the chair. There are a few

extra chairs and guards that can be borrowed, if you don’t want to use

your chair.

Who: For kids that use power wheelchairs and/or for individuals that

need mobility assistance and want to participate in soccer

When: Wednesday evenings starting September 26

Where: Copperview Rec. Center

Cost:$20 per 8-week session or “drop in rate” of $3.50 per night. Price

includes use of power wheelchair, guards and instruction.

Miracle League There are no outs, every player runs the bases and

everyone wins each game! Every player has a “Buddy” to hit and field with. Please register at Gene Fullmer Rec. Center.

Who: Children ages 3-22 with mild to severe mental and/or physical disabilities

The 2012 Season is already under way. If you have questions or would like more

First Fall game is September 8th

Sledge Hockey is a Paralympic sport for anyone that has lower body

mobility impairments. Individuals sit in “ sledge” seat, with ice blades

below the seat and propel themselves across the ice using picks at the end

of their hockey sticks. It’s a high-energy ,contact, and fun sport!!!

Who: Individuals with physical mobility impairments that can push a

sledge or use a wheelchair independently

10 ratio or with their personal assistant and would like to benefit from an

individualized fitness program.

When: Monday, Wednesday, & Friday year round (Closed on public holidays.)

Adaptive Fitness

A fitness attendant will be available to answer questions and offer advice

about using weight and exercise equipment.

Who: Individuals with a physical or intellectual disability that would

like to benefit from an educational fitness night.

Time: 6:00 PM—8:00 PM

League Bowling Practice your bowling skills while socializing with friends. The session lasts 8 weeks held on Thursdays. Each week includes up to two games and shoes.

Who: Individuals 8 years and older. 1 to 10 ratio. Participant can bring someone to assist them. This is a league that will get you ready for Special Olympics .

When: Session 5 starts October 18, 2012 Fee: $55 Session 1 starts January 10, 2013 Fee: $55

There will be a $5 late fee for signing up after the program starts. Times: 4:00 PM- 6:00 PM- Do Not be Late Where: Bonwood Bowl—2500 South Main Street

Kickball There are 2 kickball programs. One is for families and one for adults. The

rules are the same as regular kickball leagues but instead of throwing the

ball at someone to get them out, players must tag runners with the ball or

beat the runner to the plate with ball in-hand

All games will be at the all accessible Field of Dreams at the Gene Fullmer

Rec. Center (8015 S. 2200 W.). For more info call Susie 385-468-1515..

Family Kickball:

Who: Families with members with mild to severe cognitive and/or physi-

cal disabilities.

When: TBA

Adult Kickball:

Who: Men & women, 18 &older with mild to severe cognitive and/

or physical disabilities.

When: TBA

Cost: Costs for both the programs are $15 per individual or $150/team of

ten. Cost includes 6 games, a jersey and a trophy.

Sports/Classes

Adaptive Theatre Come be a star! For three months you will be working with

Miss Wendy on a play and performing it for your family

and friends.

Who: Ages 13 and older

When: Friday evenings

January 11 - April 5, 2013 AND
