The Almost Home Kids model is present in any place there is a need without regard to geographical boundaries. All key community and policy stakeholders are fully committed to the AHK model and recognize it as the premiere standard of care.

As 2016 draws to a close, all of us at Almost Home Kids celebrate another exciting and inspiring year. During the last year, we challenged ourselves to extend our reach to serve more children with complex medical needs. As we plan to bring our program to new communities, we strive to maintain our exemplary clinical standards at each Almost Home Kids. In a time when the healthcare landscape is changing rapidly, AHK has remained steadfast and constant, providing critical services to a growing population of children with complex medical needs. The care doesn’t end with the children; AHK provides the family with support and functions as a safety net within the community.

Our Mission is the driving force behind all of our decisions. Our decisions, then, provide the directions we take; providing transitional care to children with complex health needs, training their families, and delivering respite care. Our fundamental care requires excellence in nursing and clinical protocols. AHK strives to create a place that replicates a cozy, home-like setting and a fun, relaxed, loving atmosphere.

Our wonderful volunteers, committee members, and board members keep us going. We extend our heartfelt appreciation for all you do. We always need more volunteers and engaged committee and board members. We have a multitude of opportunities for you to experience. We would love your input, expertise and guidance. On behalf of the AHK staff and board members, we would like to thank you for your time and wisdom. The Almost Home Kids Model AHK is licensed by the Illinois Department of Health, under the Alternative Healthcare Delivery Act as a Children’s Community Health Care Center (501c3). It is the only community-based facility in a home-like setting in Illinois dedicated exclusively to the care of children with complex medical conditions. The organization is poised to share its expertise with other institutions therefore increasing the capabilities and capacity of the region to care for children with complicated health needs.

How We Do It

Transitional Care — A bridge from hospital to home

AHK provides a 120-day Transitional Care Program for children who are ready to be discharged from a hospital but for whom preparations at home need to be made. In a comforting environment, children receive 24-hour medical and nursing support from skilled pediatric nurses. AHK trains families on how to both care for their child with medical complexities and how to prepare their home.

Respite Care

The Respite Care Program at AHK offers children a comfortable place to stay with access to medical and nursing support while their parents/caregivers tend to personal or urgent family issues. The Respite Transportation Program provides round trip transportation for children from their home to an AHK facility.

Community Support

In addition to training and care, AHK provides community resources and activities that become a support system the family can depend on for years to come.

Quantifying our Outcomes and Developing Programs Based on Need

Almost Home Kids continues to track statistics and data on many aspects at Almost Home Kids to help us improve our own performance and become an even better advocate for children with medical complexities. In 2010, under the guidance of Program Committee Chair at the time, Kathleen Ruroede, PHd, MEd,RN, AHK began tracking demographic and clinical data. This allowed AHK to understand the population changes of the children we care for in a more measured and thoughtful way. As a result we understand the wide range and trends in diagnosis and have developed new programs based on the needs of the children we serve, as well as continuously train and educate our clinical team and help hospitals identify children that would benefit from our unique and innovative model of care. We also use this data to help legislators and regulators understand this population of children as well as educating other medical centers across the country that are looking to bring an Almost Home Kids to their Community.

Families transitioning their child with medical complexities home after what might have been a lengthy stay in the hospital can be a daunting prospect for any parent. Working closely with both the case management team at Ann & Robert H. Lurie Children’s Hospital of Chicago and the case management team from Almost Home Kids, transitioning their child to AHK is introduced early on to families as a benefit that provides a practical and needed bridge from hospital to home. Having the community case management team in the acute care setting helps families understand the importance of going to AHK to insure an effective transition home. Prior to coming to Almost Home Kids, parents are encouraged to tour their local AHK to see firsthand the home-like environment and start to visualize what care will look like outside of the intensive care unit. Being part of the continuum of care at Lurie Children’s, as well as collaborating with multiple other children’s medical centers in Illinois as well as nationally and internationally, provides that necessary safety net for families and a resource they can depend on whenever needed.

Our Financial SummaryOur fiscal year 2016 runs from September 1, 2015 to August 31, 2016. During this time, AHK’s Program Expenses totaled $6.1 million, which includes investments toward expansion initiatives. Our expenses are up 4.7 % over last year and the increase is attributed to clinical programming costs. This year, we enhanced the capabilities for our clinical team and increased efficiency in care coordination by adding traveling nurses that alternate to Chicago or Naperville based on daily census needs.

After receiving $3.9 million fee for service revenue from the State of Illinois, third party insurance companies and cash payments, AHK turned to philanthropy to close the gap for our medical services.

With the generosity of 1,093 donors, AHK’s fundraising contributions for the year tallied $1,037,643.

The organization remains focused on fundraising efficiency and is committed to providing critical healthcare services for children, regardless of the family’s ability to pay. The inherent shortfall resulting from actual cost of clinical care versus reimbursement rates is omnipresent. AHK raises unrestricted dollars to fund the operating shortfalls and provide Respite Care services to ensure that families and caregivers receive a much needed break from 24/7 caregiving.

Your Support Makes an Impact■ Almost Home Kids provided 6,642 total days of care in Fiscal Year 2016, a 9% increase over the previous

year. Whether families need transitional care or respite from the 24/7 care of a child with special healthcare needs, Almost Home Kids is always there.

■ To date, the Keith Veselik, MD, Medical Education Program which launched in 2015 has welcomed more than 150 medical students, residents and fellows from 6 different universities and 5 medical systems!

■ In fiscal year 2016, 58% of monetary donations to Almost Home Kids were made online!

In an effort to better steward the donations entrusted to us, as well as the environment, we have omitted the multiple page listing of our donors this year in the Annual Report.
