Equip Patient with Mobile Health Record

• Solution Requirements– Reduce the patient's need to manage physical paperwork.– Securely transmit and retain EHR information on patient’s

smartphone.– Enable the patient to present his/her mobile health record in a

completely offline environment.

Sample Use Case - Mobile Electronic Health Record 4

Upon initial visit to health care provider’s facility, patient is issued a mobile Electronic Health Record (EHR).

Patient receives notification in smartphone app indicating that his/her EHR is available for download.

Health Service Provider is able to easily scan patient's motionQR Code with a dedicated mobile device or desktop app to access patient's EHR.

Upon subsequent visits to health care provider, patient easily and quickly presents his/her EHR motionQR Code. Furthermore, this presentation can take place in an emergency situation (even when connectivity is unavailable).

Patient receives (wirelessly) his/her EHR in the form of a motionQR Code for secure storage. Updates to patient's EHR are sent wirelessly to patient's smartphone.
