Key Capabilities: PHR is a repository of personal health

information. PHR is a platform for data exchange

among that person’s healthcare providers.− “Tethered” PHRs vs. “Platform” PHRs

PHR is an interactive platform enabling greater patient engagement.

“Tethered” PHRs PHR is integrated with or

“tethered” to the provider’s EHR.

Information added/updated in PHR made available to the provider within their EHR.

PHREHR synchronization can occur automatically or manually.

Data exchange limited to the provider offering the PHR.

EHR is not a prerequisite.− Data can transmit via fax from PHR

to the provider for inclusion in record.

“Platform” PHR:• PHR capable of

exchanging information with multiple EHRs and data sources/ consumers.

PHR adds more value, if connected to all of that patient’s providers.

Patient’s health data follows them for when they & their providers need it.
