Click on Add an activity…

Select Feedback

Fill in the required information (Name and

Description), and set the time the activity will open

and close (if desired).

Anonymous responses can only be used if the activity appears on the front page

of the course for guests that are not logged-in.

Set the Feedback options.

Decide if administrators will receive a notification of responses via email, if

multiple submissions are allowed (must be set to yes for anonymous surveys),

and if you want to automatically number each question.

Add a message for participants once they have

completed the activity.

The participant will automatically be directed back

to the home page of the course. If you would like to re-direct users, add the URL here.

Set the common modules settings

and click Save and display

Click the Edit questions tab to choose a questions type

Choose a question type, then click Add question to activity.

Or select Templates

You can use previously used questions as a

template, or you can save the current

questions as a template and use them for future

surveys.

When you are ready for the results, click on the Analysis tab. The results will be

displayed. They can also be exported to

Excel.

The Show responses and non-respondents tabs, lets you see

the individual responses or a list of course participants that have

not responded.

When participants click on the activity, they will be directed

to an introductory page. To proceed to the survey, they

need to click on Answer the questions…

What will the students see?

Answer the questions and click Submit your answers

Click Continueand you will be returned to

the class
