2018 REPORT
North Country HealthCare is committed to cultural competency. Our second annual report highlights our efforts to provide excellent patient care in an atmosphere of dignity and respect.
In the last year:
• We have increased case management and health education encounters. Our care teams engage with patients beyond the treatment room to increase North Country HealthCare's positive health impact in our patients’ lives.
• Our employees have been trained on a wide variety of cultural competency related topics ranging from transgender health to behavioral health integration. This strengthens shared decision-making.
• North Country HealthCare creates healthier communities by engaging in research projects that improve our clinical practice, especially with special populations. We are one of a small number of health centers engaged in a study to improve the health of sexual and gender minority patients through staff cultural competence and skill training.

As the Cultural Competency Coordinator, I strive to continuously improve our team’s ability to be culturally competent with our co-workers and with their patients. This includes actively ensuring that each one of our staff member functions within a work environment that follows the National Standards for Culturally and Linguistically Appropriate Standards (CLAS), a federal blueprint intended to advance health equity, improve quality, and help eliminate healthcare disparities. We especially want to ensure that we do the following for all our patients:
1. Offer them language assistance services and other communication assistance services, at no cost and in a timely manner
2. Inform them of availability of these services clearly, verbally and in writing, in their preferred language
3. Ensure the competency of all individuals providing these services to our patients
4. Provide print and multi-media materials and signage in the most common languages spoken by our patients.
Cultural competency is a spectrum and it is exciting to see how well North Country HealthCare is progressing along that spectrum. It is even more exciting to know that I have the privilege of having a significant role in that progress, alongside many of my colleagues, and look forward to that continuing to be the case for the foreseeable future.

2017 Patient Visits
North Country HealthCare is the primary community health center in northern Arizona. In 1991, a small
group of doctors and nurses, health and human service representatives and consumers worked together to
establish the Flagstaff Community Free Clinic. In 1996, the clinic transitioned from a volunteer organization
into a state and federally funded community health center. Today, North Country HealthCare serves 14
communities across northern Arizona, providing quality medical care to all. Core services include family
medicine, pediatrics, obstetrics and gynecology, behavioral health, dental care and pharmacy services.
North Country HealthCare also has clinics nested in behavioral health facilities and homeless shelters to
serve special populations.
North Country HealthCare’s service area is largely rural and sparsely populated. Each community in the
service area is considered a Medically Underserved Area or MUA (not enough healthcare providers to
serve the population), with the exception of Flagstaff (a Medically Underserved Population or MUP). Other
barriers to care include reliable and affordable transportation, insurance coverage, employment status,
educational attainment, health literacy and poverty level, among other social determinants of health. North
Country not only provides medical services but also provides enabling services to address these barriers.

ACCOMPLISHMENTS IN 2017North Country HealthCare’s Cultural Competency Advisory Committee (CCAC) understands that culture is more than race and ethnicity. We want to help create an environment where patients and employees feel comfortable disclosing who they are, embracing all their parts. MAKING A MEANINGFUL IMPACT IN 2017
A North Country patient was facing a lease termination as part of the section 8 housing program. It was not easy for a disabled patient with budget restrictions to find housing. His care team gathered the housing resources for the patient and received funding from the Angel Fund to help cover moving expenses and obtained a two-day extension for the patient. The moving company did not complete the job citing safety concerns due to a hoarding disorder. Knowing that they had only hours before the patient would potentially lose everything, his care team (behaviorist, care manager, community health worker) helped move the patient themselves. The behaviorist spoke with the patient about his living situation and the entire care team was allowed into an extremely emotional situation to move all essential items. The patient found housing
with the help of the Care Team but faced homelessness for a few weeks. The Care Team’s job is not done. They will continue
to work with the patient to find in-home care, counsel him to understand the importance of how to maintain a safe, clean
living environment and continue to address his medical needs.
Julio Quezada started working at North Country HealthCare in 2010 right around the same time he became a lawful permanent resident. North Country provided him with multiple training opportunities, which led to him becoming a Certified Medical Assistant. He states “I’ve had the privilege of assisting in piloting and developing multiple projects. In 2016, I started working as a Community Health Worker for the diabetes program and currently I am a Care Manager. North Country has fostered my professional development and my family and I are grateful for that.” His wealth of job and life experiences, and willingness to go above and beyond for his peers and patients alike has gained him well-deserved, deep respect across the organization.
Hired as a Patient Relations Representative in 2015, she utilized her Advanced Education Reimbursement benefit and is pursuing her degree while working full-time. This December, she graduates from Coconino Community College with an Associate’s Degree in Medical Office Management. She has already passed the CMA national certification exam. Today,Tania Munoz Meza is a CMA in our Central Clinic.

INFORME 2018
North Country HealthCare toma en serio su compromiso con la competencia cultural. Nuestro segundo informe anual resalta nuestros esfuerzos de proporcionar cuidados excelentes a nuestros pacientes en un ambiente digno y respetuoso.
Este año pasado:
• Hemos incrementado la cantidad de interacciones con monitoreo de casos y con educación de salud. Nuestros equipos de cuidados conectan con los pacientes más allá de la sala de tratamiento para incrementar el impacto positivo que North Country tiene en las vidas de nuestros pacientes.

North Country HealthCare es un centro de salud comunitario primario en el norte de Arizona. En 1991,
un grupo pequeño de doctores y enfermeras, representantes de salud y servicios humanos y consumidores
trabajaron juntos para establecer Flagstaff Community Free Clinic. En 1996, la clínica dejó de ser una

CAMBIANDO VIDAS EN EL 2017
Un paciente de North Country estaba enfrentando elfin de un contrato de arrendamiento como parte del programa de vivienda de Sección 8 y no sería fácil para un paciente con discapacidad y restricciones presupuestales poder encontrar vivienda. Su equipo de cuidados reunió información sobre viviendas para el paciente y obtuvo fondos del Angel Fund para cubrir los gastos de mudanza. También consiguió una extensión de dos días para el paciente. Sin embargo, la compañía de mudanzas no hizo el trabajo por problemas de seguridad por almacenamiento compulsivo. Como sabían que les quedaba pocas horas antes de que el paciente perdería todo, su equipo de cuidados (conductista, administrados de cuidados, y promotor de salud) ayudaron a mudar al paciente. El conductista habló con el paciente sobre su situación de vivienda y todo el equipo de cuidados pudo entrar a una situación extremadamente sensible para mover todos los
artículos esenciales. El paciente encontró donde vivir con la ayuda del equipo de cuidados, pero se enfrentó con la falta de
vivienda por unas semanas. El trabajo del equipo de cuidados no ha terminado. Ellos continuarán trabajando con el paciente
para encontrar atención en el hogar, aconsejarle para comprender la importancia de mantener un ambiente seguro y limpio y continuar abordando sus necesidades médicas.
Julio Quezada comenzó su trabajo en North Country HealthCare en el 2010 cuandorecibió su residencia permanente. North Countryle dio varias oportunidades de capacitación, lo cual lo llevo a convertirse en Asistente Medico Certificado. Él dijo, “He tenido el privilegio de asistir en programas pilotos y desarrollar múltiples proyectos. En el 2016, comencé a trabajar como promotor de salud para el programa de diabetes y actualmente soy un administrador de cuidados. North Country ha apoyado mi desarrollo profesional y mi familia y yo estamos agradecidos por ello.” Su gran variedad de experiencias laborales y de vida y su disposición de ayudar más allá de sus responsabilidades a sus colegas y sus pacientes por igual le ha conseguido un bien merecido y profundo respeto en la organización.
Contratada como una Representante de Relaciones al Paciente en el 2015, ella utilizó su beneficio de reembolso para avanzar su educación y titularse mientras que trabajaba tiempo completo. Este diciembre, ella se graduó del Colegio Comunitario de Coconino con un titulo de asociado en Administración en Consultorios Médicos. Ella ya pasó su examen de certificación nacional para asistente médicos. Hoy en día, es una Asistente Medica Certificada en nuestra Clínica Central.
