Your Funding Advantage health plan includes the HealthChoices benefit. HealthChoices rewards you with immediate monthly premium savings and requires you to use the services listed to help you receive the best care and the best benefits from your health plan.

Under the HealthChoices benefit, you have the advantage of having lower or waived out-of-pocket costs when you or other covered family members require medical treatment that falls within one of our specified services and you follow the protocols set out in the plan. For members whose provider did not call 866-317-5273 (option 3) to prenotify, these services may be treated as out-of-network and/or have reduced benefits.

• Diabetes and lifestyle management • Behavioral health management — Both coaching and inpatient stays• Second opinions for high cost diagnoses• Quality providers and bundled pricing for non-emergency outpatient surgery

or imaging• Specialty drug management — Both education and possibility of reduced pricing

As a HealthChoices benefit participant, your provider must prenotify us by calling 866-317-5273 (option 3) anytime they recommend a procedure or anytime you will be receiving any of the following procedures:

• Joint replacements or surgeries (knee, hip, shoulder, etc.)• Spinal surgeries or procedures (laminectomy, spinal fusions, foraminotomy,

diskectomy, disk replacement, implants)• Cardiac interventions (angioplasty, stents,

pacemakers) or surgeries (bypass, valve replacement)

• Other surgeries, gall bladder, digestive system• Complex imaging, like MRIs and CT Scans

The following do not require prenotification but you may benefit from reduced out-of-pocket expenses by calling us and using a quality provider:

• Diagnostic tests, such as upper GI endoscopies, colonoscopies, mammograms, ultrasounds, radiology, x-rays

• Physical therapy
