About Calvary Aged and Community Care Services

We provide flexible services tailored to suit your lifestyle. Our experienced team of Support Workers are certified in home and community care, first aid and manual handling. They undergo reference and police checks and receive specialist training.

Calvary is a value based, not-for-profit organisation with a rich heritage of caring in the community. Our services are available 24 hours a day, seven days a week. Calvary will always be there with you. It’s our mission.

Home Care Packages

Level One

Support for people with basic care needs

Level Two

Support for people with low level care needs

Level Three

Support for people with intermediate care needs

Level Four

Support for people with high level care needs

A Home Care Package is an allocation of funds enabling you to purchase care and services that support your independence. Packages are flexible so they can meet your requirements today and adapt to the future.

Please check with your Case Manager about which package levels are available in your area.

There are four levels of packages available.

What is Consumer Directed Care?

From July 1, 2015 all Home Care Packages will be delivered on a Consumer Directed Care (CDC) basis.

This means that you have choice and control over how your Home Care Package funding is allocated and how your services are delivered.

Consumer Directed Care is all about choice, options, flexibility and control

With a CDC Home Care Package you can:

• select the services and products that will best support your independence

• plan services to suit your schedule

• decide your level of involvement in the management of your Home Care Package budget and service.

What services and products are available?

Your Home Care Package can be used for services and products that support your independence and your social connections within the community.

These include:

• personal care – showering, grooming and mobility assistance

• social support – companionship

• transport to shops, appointments and events

• nutrition – menu planning, meal preparation and cooking.

• domestic assistance – cleaning, laundry and shopping

• clinical care – nursing and other health support

• house maintenance and modifications

• personal alarms and emergency monitoring systems

• mobility, independent living and continence aids.

Setting your Goals

Managing Your Budget

Your package budget is made up of:

• funds from the government

• your contribution

• income tested fees (determined by the government).

Calvary Aged and Community Care Services is responsible for holding the budget funds on your behalf. You can also “top up” your budget with additional contributions if required to support your services identified during assessment and care planning.

This plan will allocate funds for direct services, products such as personal alarms as well as costs for additional services such as service coordination, administration and management.

Each month we will provide you with an itemised statement which will list your delivered services. It will also display the balance of funds available in your package that will be carried forward into the next month.

Together we will put together a Home Care Agreement and Care Plan in line with your budget.

When designing your package it is useful to think about your needs and goals.

Your Case Manager can work with you on identifying goals and strategies by linking your services to outcomes.

Goals could be related to your mobility, health and well-being or social activities. What do you enjoy doing most? What could improve your daily living? What lifestyle do you want to live?

Understanding your Statement

Your monthly statement outlines:

• Opening balance of the package funds at the beginning of the month

• What care and services have been provided to you throughout the month

• The balance of funds remaining in your package at the end of the month, after the cost of the services provided has been deducted.

Your Case Manager will be available to discuss the contents of your statement with you and provide you with any additional information needed.

Your statement also confirms:

• Government contributions – subsidy provided by the government towards your Home Care Package, including any supplements (if applicable)

• Your contributions – total amount you have agreed to contribute towards the package – including any co-payment and income tested fee

• Calvary Aged and Community Care fortnightly package and care management fees.

Service provision details:

• A list of the services we have provided in the past month, including date and time of the service and the cost of each service

• A list of the services provided by external providers – i.e. – physiotherapy, podiatry or other allied health specialists

• A list of purchases completed throughout the month from your package funds.

Home Care Package Fees

Invoice Charges

Calvary Aged and Community Care Services offers coordination of services if you choose to purchase services from an external provider to support you with services under your package. Calvary will charge a nominal amount (Handling Fee) per each invoice processed for payment.

Staff Travel to Visit Client

This fee covers the cost for a Support Worker to travel to your home. While some providers incorporate this fee into the hourly rate, Calvary only charges once per visit to provide direct service. This fee is a fixed amount for all services and may differ between service centres.

Exit Fee

We charge an exit fee when package funds are available. This fee covers the administration costs associated with leaving (exiting) the package, including reconciliation of package funds.

There are a variety of services and fees covered by your Home Care Package.

Care Management Fee

Together we consider your needs, personal goals, lifestyle and budget. We design a detailed plan of support so you can live in your own home and stay active in your community.

We provide support to navigate the health and community care system by organising referrals, expert advice and advocating on your behalf. We monitor your progress and adjust your plan as your needs change.

This fee also includes coordination and scheduling of appointments, ensuring that care is aligned with other supports and after hours support 24 hours/7 days per week.

Package Management Fee

This fee covers the ongoing organisation activities associated with the delivery and management of a Home Care Package. It includes the cost for preparing monthly statements, managing package funds and compliance and quality assurance activities required for home care. This includes meeting the new Aged Care Standards from July 2019 for service quality, record management and compliance (e.g. police checks) requirements.

Rails and ramps can be installed both inside and outside the house. Personal alarms, fall detectors and motion sensors can also improve your safety. They can be worn around the house or wall mounted in bathrooms or kitchens. Electronically linked to a 24 hour monitoring services these telecare products offer peace of mind and ensure help is just a push of a button away. Calvary can also help source other independent living aids that make living at home easier.

Support Tips from a Calvary Case Manager

Build up a team of Support Workers. This means that when one is unavailable, away or ill you have other Support Workers familiar with your needs available.
