Nigel Broad MBE POWYS tHB NEWS Spring 2014 Staff magazine Nigel joined us last year, having worked for ABMU Health Board for 20 years. Nigel is based in Ystradgynlais but works with colleagues across Powys. The honour was given to him for his work improving patient safety and for helping to develop and deliver the 1000 Lives Plus ‘Skin Bundle’ – a project which saw an incredible reduction in pressure sores in patients and has gone on to be used across Wales and has been adopted by other countries as well. Nigel is keen to point out that he wasn’t working alone and said “The work we have done for pressure sores was as part of a team and this honour reflects the Nigel is keen to help keep patients at home and out of hospital. hard work and dedication by everyone involved. If it wasn’t for the great front- line nurses and the support from medical colleagues at all levels of the organisation, we wouldn’t have had the success we had. All of the people that I worked with deserve this honour as well.” Nigel joined Powys because he wants to help to reduce the number of patients being admitted to hospital unnecessarily and to help patients return home as quickly as possible. Nigel saw the work that has already been developed in Powys with the ‘Virtual Ward’ as a real opportunity for him to help and get invovled. Everyone at Powys teaching Health Board is very pleased that Nigel has been honoured in this fashion and is happy to have him working here with us.

To make sure that everyone’s jobs are graded fairly and we are paid what we should be, the NHS introduced ‘Agenda for Change’ pay grades in 2004. The system identifies strict criteria for all jobs to be measured against in order to keep things fair.Everytimeanewjobiscreatedoranexistingjobneedstobere-evaluated,apanelmeetstomatchandevaluatetheroleagainsttheAgendaforChangecriteria.

Lorraine Burroughs is a Speech and Language Therapist in Brecon. She’s married and has a young son. She also developed Bowel Cancer when she was just 28. This is her story.“Bytellingmystory,I’mhopingtohelpraiseawarenessthatpeoplecandevelopbowelcancerregardlessoftheirage.

Ihadmyson,FinnleyinApril2011,andlookedforwardtoanexcitingfutureasanewmum.WhenFinnleywasabout3monthsIstartedgettingabdominalpain,bloatingandabnormalbowelmovements.

IwenttomyGPwhobelievedIhadirritablebowelsyndromeandIwasgiventheappropriatemedicationtohelpmysymptoms.

However,thesymptomspersistedandgraduallygotworse.WhenthevomitingstartedIsawmyGPagainandwassentstraighttomylocalhospital,theRoyalGwent.Theyexaminedmyabdomen,gavemestrongerpainreliefandreferredmeforanultrasound,astheybelievedIcouldhavegallstones.

BowelcanceristhethirdmostcommoncancerintheUKtoday.Althoughmorethan7in10ofthepeoplediagnosedwithbowelcancerareaged65andover,bowelcancercanaffectanyoneatanyage.

Bowelcanceraffectsaround1in20womenand1in16menintheUKeveryyear,andisthesecondbiggestcauseofcancerdeaths.

Despitetheseshockingstatistics,atleastoneinfourpeopleiscompletelyunawareofthedisease,orthesymptomsassociatedwithit.Thereasonsforthisarenotalwaysclear,butwedoknowthatweoftenshyawayfromtalkingopenlyaboutbowelsandbottoms,eventoourdoctor.

Afewweekslater,Ireturnedtohospitalasthevomitinghadreturned.

ThistimeIwasadmittedfor5daysandgivenanti-sicknessinjections,whichworked,andmorepainrelief.Ireceivedthescheduledultrasound,whichfoundnogallstones.ThenextstepwasthereforetorefermetoradiologyforanMRIandendoscopy,butIwastoldthatIwouldhavetowaitfortheseinvestigationsandwasthereforedischargedfromhospital.

Followingmydischarge,IfoundcaringforFinnleyverydifficult.Sowhilemyhusbandworked,mysisterhelpedmewithhim.

ThenonBoxingDay2011everythinghappenedveryquickly.Ifeltveryunwellandmyheartrateandbreathingwereelevated.IthoughtIhadoverdosedon

theOromorphpainreliefthatIhadbeengiven.IwasrushedintoAandEviaambulance.Therestisalittlebitofablur.

IamawareIhadnumerousemergencysurgeriesbutlittleelse.Iwokeupeightdayslaterinintensivecare,onaventilator.

IwaslatertoldthatIhadhadatumourinmybowel,whichhadblockeditandcausedittoperforate.Thecontentofmybowelhadthenpoisonedmybodyandsentmeintosepticshock.Thiswasthecauseofmyelevatedheartrateandbreathing.

Myrecoverywasveryslow.DuetothedamagetothebowelIhadtohaveanileostomybagfitted.Ihadtolearntodoeverythingagain,fromsittinguptowalking.Ispentatotalofsevenweeksinhospital.

Mycancerwasastage3tumourwithlymphnodeinvolvementandatthispointaspreadtotheliverwassuspected,soIalsoIunderwentsixmonthsofchemotherapy.Halfwaythroughthechemomyregimehadtobechangedasmybodywasnotcopingwell.Thesideeffectsfromthechemowerevile.

IreceivedtheallclearinOctober2012andhadtheileostomybagreversedinFebruary2013.

MysonisnowtwoandahalfandIhavebeenabletoreturntowork.Iamverythankfultobeheretodaytowatchmysongrow.

Mostpeoplethinkthatbowelcanceronlyaffectsolderpeople.Butitdoesn’t.I’mluckytostillbehereandIhopethatthisstorywillhelpraiseawarenessofthisdiseaseandsavesomeone’slife.”

High-risk symptoms• Bleedingfromthebottom(rectal

bleeding)orbloodinyourpoowithoutanyobviousreason,suchaslocalsoreness,piles(haemorrhoids),tears(analfissures).

• Anychangeinbowelhabitthatlastsforthreeweeksormore,especiallyifyouaregoingtothetoiletmoreoftenorexperiencingunexplainedlooserpoo.Symptomsmayalsoincludeunexpectedconstipation,andafeelingoffullnessorincompleteemptyingfromyourbackpassage(rectum)afteryouhaveusedthetoilet.

• Anyconstant,unexplainedpainanywhereintheabdomen,especiallyifitissevere.Itmayalsobelinkedtogoingtothetoilet,oritmightcomeandgolikecrampsorcolic.

• Anunexpectedlumpinyourabdomen,especiallyifitisonyourrighthandside.

• Unexpectedweightlossperhapsduetolostappetite,orfeelingbloatedorsick.

• Unexplainedtiredness,dizzinessorbreathlessness(symptomsofanaemia).

Petula Garner, Award winning nurse

Petula Garner is the Respiratory Nurse Specialist for South Powys. Last year she won the Royal College of Nursing’s ‘Clinical Nurse Specialist of the Year’ award and was invited to attend a gala presentation in Cardiff.Petulawasverymodestwheninterviewed,saying“Idon’tknowwhyIgotthisaward,I’mnotoneofthosepeoplethatstandout,Ijustgetonwithmyjob.”However,withalittlediggingitturnsoutthatinherfirstyearasarespiratorynurse,shereducedhospitaladmissionsforAsthmaenormously.

PetulajoinedtheHealthBoardin2002todevelopthespecialistrespiratoryservice.SheworksbyhelpingtoeducateGPs,PracticeNursesandDistrictNursesontheissuesassociatedwithrespiratoryillnesses,andlooksafterpeoplewithcomplexneedsherself.

SheestablishednewpoliciesandwaysofworkingandextendedwhatwasinitiallyaservicejustfortheYstradgynlaisareatoonethatcoversthewholeoftheCounty.WhenshestartedshewastheonlyspecialistinPowys,butfollowingonfromhersuccesstherearenowtwoothersaswell,oneforeachlocality.

Asanex-miningcommunity,YstradgynlaishasahighprevalenceofChronicObstructivePulmonaryDisease(COPD)andotherrespiratoryillness.However,thankstoPetula’swork,theyhavethelowestadmissionrates.

ManyofPetula’spatientsarelookedafterusingthe‘virtualward’modelwhichreallyhelpskeepthemoutofhospital.Somehaveoxygentherapyathomewhichtheyareabletomanagethemselves,withsupportandtrainingfromPetulaandcolleagues.

Onesuchpatient,MrMorris,suffersfromCOPDandhasarangeofcomplexrespiratoryneeds.Petulavisitshimweeklyandisonhandviatelephonetoensurethatifhisconditionflaresup,hecanbelookedafterandmanagedquicklytoallowhimtostayinhisownhome.

MrMorrissaid“Petulagivesmepeaceofmind.ShehelpskeepmeontherightfootandremindsmewhatIneedtodotolookaftermyself.IwasreallypleasedwhenIheardthatshewontheawardasshereallydeservesit.Ihaveallmyfaithinher.”

Anewwaytolookatpatientcare?The All Wales Patient Experience group has been looking at an article by the American College of Healthcare Executives which suggests we should focus on ‘Always Events’ as much, or more than, ‘Never Events’.

‘NeverEvents’havebeendiscussedforanumberofyearsnow;thosethingsthatshouldNEVERhappeninahealthcaresetting,butunfortunatelydooccuroccasionally.Thearticlesuggestsweshouldidentify‘AlwaysEvents’–providingclarityaboutwhatshouldhappenforeveryperson,everytimetheyusetheNHS.

ThearticlegivesanumberofexamplesofhowthisattitudeisbeingputinplacebyhospitalsintheUSA,buttheprinciplescanbeappliedasreadilyintheNHS.

Itconcludesbytalkingabouthowmanagementcantakealead:

‘Too often, healthcare improvement efforts focus exclusively on what is wrong with healthcare rather than building on what is already done well and focusing on how to do it consistently.

Leaders, however, can set the right tone by helping their organizations reframe their discussions about person-centered care in a positive, collaborative way that builds relationships between and among patients, families, staff and providers.

Leaders can use Always Events to send the message that making care more person and family centred is the key to addressing some of today’s most challenging issues in healthcare, including reducing avoidable readmissions, improving care and communication across the continuum and improving patient safety.”

PeoplewithCOPDhavedifficultiesbreathing,primarilyduetothenarrowingoftheirairways;thisiscalledairflowobstruction.

TypicalsymptomsofCOPDinclude:

• increasingbreathlessnesswhenactive

• apersistentcoughwithphlegm

• frequentchestinfections

ThemaincauseofCOPDissmoking;smokingirritatesandinflamesthelungs,whichresultsinscarring.

COPDisoneofthemostcommonrespiratorydiseasesintheUK.

Itisthoughtthereareover3millionpeoplelivingwiththediseaseintheUK,ofwhichonlyabout900,000havebeendiagnosed.ThisisbecausemanypeoplewhodevelopsymptomsofCOPDdonotgetmedicalhelpbecausetheyoftendismisstheirsymptomsasa‘smoker’scough’.

Powys joins national programme to support LGB equality for staff.

Powys teaching Health Board has joined Stonewall’s Diversity Champions programme which promotes and develops good practice for employers. The programme – run by the Lesbian, Gay and Bisexual (LBG) equality charity – promotes a good working environment for all existing and potential employees and commits members to ensuring equal treatment for those who are lesbian, gay or bisexual.TheHealthBoardjoinsotherHealthBoards,PowysCountyCouncilandover600majoremployersontheprogrammeincludingtheNationalAssemblyforWales,SportWales,LloydsBankingGroupandAmgueddfaCymru/NationalMuseumofWales.

Powys teaching Health Board believes that the promotion of health and wellbeing should be at the heart of our work as an organisation. Making Every Contact Count (MECC), is an approach which aims to promote this idea and support NHS staff to help people to make positive changes to their health behaviour.

MakeEveryContactCount,isaboutusingeveryopportunitywehaveasstafftodeliverbriefadvicetoimprovehealthandwellbeing.Itisaboutususingeveryoccasiontoaskourpatientsandserviceuserstherightquestionstofindoutabouttheirunderlyinghealthneeds,sothatwecanthengivethembriefadvicetohelpimprovetheirhealthandwellbeing.Itdoesn’tmatterwhetherweareclinicalstaffornot,everyonecanhelptogivebasicadvice.

OverthenextfewmonthsthePublicHealthTeamwillbedevelopingarangeoftoolsandtrainingtohelpallstafftogetinvolved.

For many years NHS computers have run Microsoft Windows XP, which has worked well with all NHS software. But from this year, Microsoft are no longer supporting Windows XP, which has been replaced by newer systems.ThereforewearenowintheprocessofupgradingallourXPcomputerstoWindows7Enterprise.

Manycomputerswillalreadyhavebeenupgraded,butifyouarestillusingWindowsXPyoucanexpectanupgradeshortly.

SomenewercomputerswereinstalledwithWindows7ProfessionalandthesewillalsoneedupgradingtoEnterpriseoncealltheXPupgradesarecomplete.

TofindoutwhicheditionofWindowsyouarecurrentlyrunningonyourcomputergoto:Start\Control Panel\System and Security\System.IfyouareanXPuseryouwillreceivenotificationclosertothetimeofyourupgradedateexplainingtheprocessinmoredetail.Atthistime,wemayalsorequireinformationfromyouaboutyourcomputer/applicationsinordertocarryouttheupgradeassmoothlyaspossible.

Windows XP upgradeFAQQ: I have an XP machine, what do I do?

A:Nothing,wewillnotifyyouinthe2weekspriortoyourmigrationwithanyinstructions.

Q: I have Win7Pro, what do I do?

A:Windows7ProfessionalmachinesarescheduledforupgradetoEnterpriseinthenextphase.ProbablybetweenApril–June2014.

Q: What will the upgrade mean (will I lose any data, will the computer look any different when it’s done?)

A:Allworkyoupreviouslyhadstoredonthenetworkwillremain.Someinstalledprogramsandappsmayneedtobereinstalledaftermigrationbutwewilltrytoidentifythiswithyoupriortothemigrationdateandarrangeaconvenienttimeforreinstallation(probablythenextdayifyouareavailable).UsersettingsandcacheddataforMicrosoftproductsshouldremain.

Q: How long will I be without a computer (do you take it away to do remotely or come to me?)

A:Theprocessfordesktopmachinesonhospitalsitesisgenerallyremote.SomeexternalsitessuchasGPsurgerieswillrequiremanualmigrationsowillbevisitedbyamemberoftheITteam.Inasuccessfulmigrationtheprocesswouldstartat4:30pmandfinishovernight.WhenyoureturntoworkthenextmorningmigrationtoWindows7willbecomplete(however,someuserswhohavespecialistsoftwareinstalledmayrequireITtore-installitthenextmorning,butthiswouldbearrangedpriortomigration).

Q: When is this process starting?

A:Wehavestartedalready.

Q: Why are we only upgrading to Windows 7 when Windows 8 is available?

A:BecausetheNHSrunssomanyspecialistsystems,ittakesawhileafterthereleaseofanoperatingsystemforthesesystemstobeupgradedtorunonthem.Thereforewearealwayslikelytobeatleastoneversionbehindthelatest.

Last year, the Welsh Government passed a new law on organ donation. The new rules will come into effect on 1 December 2015. After that date, everyone will be assumed to consent to donating their organs after death unless they have specifically opted out.

Whychangethesystem?In2012/13,35peoplediedinWaleswhilstwaitingforanorgantransplant,asadonorcouldnotbefoundtoenabletransplantationtogoahead.

Veryfewpeopledieincircumstanceswhereorgandonationispossible.Soitisimportanttohaveasystemtoensurethatasmanyofusaspossibleareidentifiedaspotentialorgandonors.

Itisrareforfamiliestobeintheawfulsituationwheretheirlovedonecouldbeapotentialdonor.Currentlyin43%ofcaseswhereorgandonationispossible,familiessaynotodonationbecausetheydon’tknowwhethertheirlovedonewantedtobeadonor.

Mostpeoplesupportorgandonation,buthaven’tregisteredtheirdecisiontobeadonor.

Thenewsystemwillmeanifyousupportorgandonation,youwon’thavetoregisterawishtobeadonor.Thenewsystemwillhelptomakeyourdecisiononorgandonationclearertofriendsandfamily.

Sowhodoesthisaffect?Thenewrulesaffecteveryonethatisover18andhaslivedinWalesformorethan12months.Itwillnotaffectthosewhohavenochoicewheretheylive,suchasthoseinthearmedforcesorinprison.

WhatthenewlawmeansinPowysAlthoughPowysiswithinWales,ithasalongborderwithEnglandandmanyofusreceiveourhospitaltreatmentinEngland.ThenewrulesonlyapplytothosethatliveANDdieinWales.Thereforeifyouwouldliketodonateyourorgansafterdeath,itisrecommendedthatyoualsoregisterasadonorontheEnglishsystemtobecoveredintheeventthatyouaretakentoanEnglishhospital.

Whataboutmyfamily?Peopleclosetothedeceasedwillalwayshaveaveryimportantparttoplayinanydiscussionaboutorgandonation.

Inadeemedconsentsituation,familyinvolvementisessential,togiveamedicalhistory,residencyandpossiblyalsoacheckonmentalcapacity.Theymustalsobepresenttobeallowedtoprovideinformationaboutwhetherthedeceasedobjectedtoorgandonation.Iftheyknowthedeceaseddidnotwanttodonatethendonationwillnotgoahead.

SowhatifyouDON’Twanttodonate?Beforetheendofnextyear,theGovernmentwillopenaregisterforpeopletooptout.Thishasnotyetbeenstarted.

Foot CareOne of the most common causes of difficulty for older people is foot care. Simple acts such as cutting toe nails becomes much harder as we get older and simple foot problems can quickly lead to more serious ones which can reduce a person’s ability to get around.Powysishometoalargenumberofolderpeople,andtheproportionofolderpeopleisexpectedtoriseoverthenextfewyears.

Tohelpsolvethis,PowysAssociationofVoluntaryOrganisations(PAVO)havesetuptheSocialFootCarePartnership.

Overthelastyear,theSocialFootCarePartnershiphasgonefromstrengthtostrengthduetothehardworkofthecoordinators,commitmentofvolunteersandtrainingbyourPodiatrystaff.AnincreasingnumberofpeopleinPowysarenowabletoaccessaffordablesocialfootcarelocally.AstrongpartnershiphasbeenestablishedbetweenthePodiatryTeam,theThirdSectorandtheCountyCouncil.InmanytownstheprovisionofsocialfootcareisalsolinkedwithotheractivitiessuchastheLegClub,makingiteasierforpeopletoaccess.

TheprogrammeisdeliveredacrossPowysbyseveralorganisations.

• CAMAD(CommunityActionMachynllethandDistrict)providesaservicetopeopleinMachynlleth2-3timesamonth.

• SimplyNails(AgeCymruPowys)hasestablishednewservicesinKnighton,Presteigne,NewtownandWelshpool,reinvigoratedtheYstradgynlaisgroupandmaintaineditscoregroupsinBuilthandBrecon.

• Theprogrammeisawareofthechallengesofprovidingaserviceinresidentialhomes,theneedfortrainingofhealthcareassistantsonwardsandincommunitysettings,aswellastheneedforservicesinmentalhealthwards.

• Fundingandsustainabilityremainanon-goingconcernparticularlywhenpumpprimingmoniesceaseandcorefundingofexistingserviceprovidersisthreatened.Ittakesseveralmonthsfornewservicestogeneratesufficientincometocovercosts.Jointbulkbuyingmayhelpreducecosts.DespitetheseworriestheSocialFootCareProgrammehopestocontinuetoprovideanexpandingprofessionalservice.

• TheCouncil’sRhayaderHomeSupportwillbelaunchinganewhomebasedserviceinJanuary2014whileEastRadnorDayCentreprovidessocialfootcaretoitsmembers.

SarahRochira,OlderPeople’sCommissionerforWales,visitedtheSimplyNailsserviceandLindsayLegClubinBreconrecently.Sarahsaid:“Theimpactofthesetwoservicescannotbeunderestimated.Manyoftheolderpeoplethatusethemtoldmethatiftheywerenotavailable,theywouldnotleavetheirhouses,wouldbeinpainandwouldnotbeabletodothemostbasicthings,makingitdifficultforthemtomaintaintheirindependence.Manyofthesepeoplewould,therefore,requireformalpackagesofcare.

“Theservicesalsoprovideanopportunityforolderpeopletochat,laughandsocialisewhileaccessingotheressentialhelpandadvice.”

AgeCymruPowys’JillLloyd-Phillipsstartedanailcuttingpilotschemein2011andithasnowgrownmassively,withmonthlySimplyNailsclinicsinBuilthWells,Brecon,Ystradgynlais,Presteigne,Knighton,WelshpoolandNewtown,lookingafter500clients.

Jillsaid:“Itisnotjustaboutthenailcutting,butprovidingsupportandhelpthroughtherelationshipsandfriendshipswebuild.Forexample,onepersonwhowasusingtheservicewassleepingdownstairsashecouldnolongergetupstairsinhishouse.Wegothimsomehelpandhewasrehomedinabungalow.”

Last year Sally Price went on “Brief Intervention Training” (BIT) at the suggestion of her manager. The course aims to help staff to have the confidence to talk to patients about smoking and to sign-post people to the various smoking support services that are available.SallyhasworkedinthePhysiotherapydepartmentinKnightonforfiveyearsandmeetsawiderangeofpeoplethroughherwork.Sheexplainedthat“It’snotaboutforcingpeopletoquit,butaboutkeepingtheideafreshintheirmindsandmakingsurethattheyknowhowtogetsupport.”

Followingthecourse,Sallyputupadisplayboardinherdepartmentthathighlightsthedangersofsmokingandthebenefitsofquitting.Oneposteridentifiesallthedifferentingredientsofacigarette,whichwasquiteaneye-opener!

Whentalkingaboutwhyshequitsmokingherself,Sallysaid“Ihadaclosefamilymemberwhowasaffectedbyasmokingillness.Youalwaysthinkitsgoingtohappentosomeoneelse!”

Overthepastfewyearstherehavebeenmanydifferentanti-smokingmessagesandthenumberofsmokershasdeclined.However,someoftheoldmessagesdon’tseemtobehavingthesameimpactanymore.Sallysaysit’snotjustthefinancialbenefitsanymoreasalotofpeoplesmokerollups.Thankfullyit’sbecomeveryunsociabletosmoke.Butshedescribedhowshockedsheisatthenumberofpeoplethatstillsmokeinfrontofchildren.Smokinginthehome,andevenworse,incarswhenchildrenarepresentcanputthosechildren’shealthatasmuchriskasthesmokerastheydon’thavethebenefitofbreathingthroughthefilter.

Autistic Spectrum Disorder (ASD)Information Day for Parents, Carers and Professionals in Powys

Wednesday 19th March 2014 at 10am-2.30pmThe Welfare, Brecon Road, Ystradgynlais SA9 1JJThe ASD Strategic Action Plan for Wales was launched in 2008, the first of its kind in the world! Powys has its own ASD Plan that is being actioned by multi agencies working together to meet the needs of those with an ASD.

We would like to invite up to 80 parents, carers and professionals working with children and young people with an ASD to an information day. The event will include presentations on the all Wales and local ASD plans and provide the opportunity to attend workshops. Parents can also visit poster presentations and network with fellow carers and professionals during the event.

Around 70 people attended a training and information day in Bronllys in February as Powys joined the ‘Butterfly Scheme’. Attendees included clinical staff from across the whole of Powys, enabling them to learn new techniques for managing the needs of patients with dementia.

Memoryimpairmentcanmakebeinginhospitaldistressing,butitneedn’tbe.Inhospital,dozensofstaffcanpassthroughapatient’slifeeachdayandinordertodeliverappropriatecare,theyneedtoknowthatapatienthasdementiaormemoryimpairmentandhowtosupportthem;thisiswheretheButterflySchemecomesin.
