It is indeed a great honour to be elected to this position and I pledge to serve to the best of my ability. I must also congratulate Prof. Dr Primuharsa Putra, the president for the year 2012 - 2013 and the executive committee members for 2012 - 2013 on a successful year and for their commitment and dedication over the past twelve months. Over the last 30 years, ORL - HNS has undergone a major expansion. We have improved our teaching of ORL - HNS and our service to the public. Our members have increased tremendously in number. The latest total number of our members is 340. We intend to continue the following activities during the year 2013 - 2014 : the 9 th Malaysia- Singapore joint Scientific Meeting which will be held in Malaysia in 2013 - 2014, MSO-HNS Health Camp 2014, the Annual Scientific Meeting 2014, and the climax will be the 6 th Asia Pacific Otology, Neurotology & Skull Base conference / 5 th ASEAN Academy of Neuro-Otology & Audilogy (AANOA) congress/ Inaugural ASEAN Rhinology Society congress/ 6 th Malaysian International ORL - HNS Congress/ 34 th MSO-HNS AGM. To improve our CPD activities, MSO-HNS will streamline and coordinate the activities with formation of CPD specialty subcommittee for Otology, Rhinology and Head & Neck, Laryngology. The key to the continued success for MSO-HNS is the participation of our members. I urge members to get involved in our organization and stay engaged and united. Give your ideas, time and expertise. It is after all, your society. As an organization, MSO-HNS will only be as strong as the talents and contributions of its collective membership. I would like to extend a special ‘thank you’ in advance to all of the members who will volunteer their time and because of their generosity and expertise, MSO-HNS is a thriving organization. I look forward to an exciting year and interacting with a very special group of individuals! Working together we can make 2013 - 2014 another great year!

It has been an honor and a great pleasure to be able to serve this society for the

past one year. I think the past year has

been a great success.

The8thJointMalaysia-SingaporeAnnual Scientific congress washeld on the 21st of October2012atHotelOasia,Singapore.This eventwas jointly organizedby the Malaysian Society ofOtorhinolaryngologists-Head &NeckSurgeon(MSO-HNS)andtheSocietyofOtolaryngologyHead&NeckSurgerySingapore(SOHNSS)with Singapore being the hostcountryfor2012.

Themeetingwasattended by4EXCOmembersandDrChewYokKuan represented Malaysia fortheresident’spresentation.

Kwai Onn, President, SOHNSSand Prof. Dr Primuharsa Putra,President, MSO-HNS, the guestlectures which followed weregivenbyrenownedspeakersfromMalaysia, Singapore and USAon new techniques in surgeryinvolvingthefrontalsinus,hearingaid implantation and skull basesurgery.Therewasalsoexchangeofthoughtsandideasonmedicaleducation and the direction it isheading.Aftertheteabreak,theresident’s research presentationcompetition commenced. Thishalfdayeventconcludedwiththeprizegivingceremony.

Despite our continuous effortsthere may have been somelapses and shortcomings in themanagement and we could notextend our hospitality to fulfilall expectations. Ithastakenmanyofoursocietymembers to represent thevariouscommitteestotakecareof theneedsof thisassociationover the course of the pastyear and the years before.

Therefore when you cross-pathwithanyofthepastandpresentcommittee members at any oftheconferences,pleasetakethetime to stop and thank them.Thesecommitteemembershavewillinglyvolunteeredthemselvestoensurecontinuedbenefitsforthemembersofoursocietyaswellasthepublic.Theyhavecontributedcountless hours and energy toensure resounding success ofall ourMSO-HNS activities, yearinandyearout. The1stprizewenttoDrLynnKohwhopresentedher research titled‘Comparing Ways of AssessingTongue Base Obstruction InObstructive Sleep Apnoea’. DrLim Mingyann won the secondprizewith his topic on ‘TheMusicListeningPreferencesandHabitsofYoungPeopleinSingaporeanditsRelevancetoLeisureNoiseInducedHearingLoss’whilethe3rdprizewasgiven to Dr Chew Yok Kuan whopresented a paper on ‘ObjectiveAssessment of Hearing ThresholdinDarkandLightEnvironment’.Thiscongresshasservedasaplatformin bringing together current andfutureMalaysianandSingaporeanENTsurgeons.

The ORL - HNS Dept, HUSMKubang Kerian Kelantanin collaboration with DELTAMedisains organized a two dayseminar on the use of laser inthe otorhinolaryngology field on21 - 22 November 2012. Theseminar’s aim was to introducecarbon dioxide laser applicationinto the armamentarium ofotorhinolaryngology surgeons inMalaysia.

ThespeakersfortheeventwerefromthedepartmentsofORLandOpthalmologywitheachspeakerhighlighting their experiences inthe application of CO2 laser intheirrespectivefields.TherewerealsospeakersfromtheRadiologydepartment to help us betterunderstandpre-operativeimagingwhichisverypertinentforproperselectionofpatients.

The speakers included AssociateProfessor Dr Baharudin Abdullah,DrIrfanMohamad,DrNorhafizaMatLazimandDrHazamaMohamad.Twoseniorotorhinolaryngologists,Associate Professor Dr RahmatOmarandDrBalachandranAppo,conductedthelivesurgeriesduringtheevent.Agoodnumberofcaseswere put up for the live surgerysession including tonsillectomy,bilateralturbinoplasty,septoplasty,vocal cord mass excision andlaryngotracheal stenosis withsuccessfuloutcomes.

Theparticipantscamefromacrossthe country including Malacca,Negeri Sembilan and Kedah. Wehad altogether 80 participants.Duetothecontinuingencouragingresponse towards our USM-Laser Seminar, we look forwardto organize similar events in thefuture.

On the 16th of December 2012,MSO-HNS with more than 700doctorsfromaroundthecountry,attended the Grand Dinner withthe Prime Minister of Malaysia.The event was a great successandthedoctorsheardthepolicypointsmadebythePrimeMinister.Oursocietysupportedtheeventbysponsoringonetableandtheorganizingchairmanoftheeventwas Datuk Dr Kuljit Singh, pastpresidentofMSO-HNS.

1st BASIc SURGIcAL SKILLS FOR ENT OFFIcERS WORKSHOP

Attributesofasurgeon;‘He should be youthful or in early middle age, with a strong and steady hand, as expert with the left as with the right, with vision sharp and clear, and spirit undaunted; so far void of pity that while he wishes only to cure his patient, yet is not moved by his cries to go too fast, or cut less than is necessary ’ – Aulus Cornelius Celsus (AD 30)

The first basic surgical skillsworkshop for ENT officers inPerak was held on the 13th ofMarch2013.TwentyENTmedicalofficersfromaroundthecountryparticipated in this hands-on

courseandworkshop.

The workshop incorporated thestandardsurgicalskillsworkshopwith a special focus on ENTinstruments and hemostasis inENT surgery. Specialists fromthe state of Perak shared theirknowledge and experiences withthejuniorsurgeons.

Participants were given abrief history of ENT surgery.Hippocratesdescribeda ‘spongetechnique’ for removal of nasalpolyps 2500 years ago. Celsusadvocated finger dissection oftonsils 2000 years ago ……withoutanaesthesia.

Itwasalsoatimeofinnovation.A rough construct of anoropharynx was designed toteachparticipantshemostasisofthe tonsilar bed with the Negusforcepsandknotpusher.

This inaugural programme waswellreceivedandwillcontinuetobeanannualevent inthePerakENTTeachingProgramme.

GLOBAL POST-L ARYNGEcTOMY REHABILITATION AcADEMY (GPRA) TOURDr Iqbal Farim bin Rizal Wong, University Kebangsaan Malaysia Medical Centre

Laryngectomy is one of themore common major head andneck operations in ENT andthus, though the small detailsmay be rather complex, is welldescribed in Otolaryngologicaltexts.Whatislesswelldescribedisthecomplicatedsequelaethatresults from it, beyond merelylosingyourabilitytospeak–whichin itself is a considerable, if notthemostdistressing,disability inENT.MainstreamOtolaryngologytraining in Malaysia, and Isuspectthistobethecasewithmostpartsoftheworld,focusesmostly on how to perform alaryngectomy and largely simplybrowses through some of thevoice rehabilitation options thatare available. In fact, in lessfortunate parts of world, evenwithinourowncountry,manyofthese options are prohibited byvarious factors – the main onebeingcostconstraint–thusmanyENTtraineesandsurgeonsdonotactuallyhavemuchexperienceinpost-layngectomy rehabilitationand sound understanding ofthe physiological changesthat patients undergo after alarygectomy and the principlesand theories behind eachrehabilitationoption.

Recognising this fact, theGlobalPost-Laryngectomy RehabilitationAcademy (GPRA)was founded in2009.TheGPRA isthebrainchildof the world renowned Prof. Dr

Frans Hilgers and his group attheNetherlandsCancer Institutein Amsterdam and they conductcoursesandworkshopsworldwidetoimproveunderstandingandthemanagementoflaryngectomisedpatients for ENT surgeons andtraineesandalliedprofessionalsalike.

TheKualaLumpurlegoftheGPRAtour underwent its 2nd Edition on the 15-16th March 2013 atUKM Medical Centre attendedby some20participants comingmostly from Malaysia and twofromSpain.ThecoursethistimewasgracedbyProfessorDrIBingTanandAnneKornmanfromtheNetherlands and local expertssuch as Professor Dr AbdullahSaniandAssociateProfessorDrBaharuddinAbdullah.The‘hands-on’ participants were uniquelyprovided with a sample productbag consisting of a range ofproducts available courtesy ofAtos Medical, the makers ofProvox voice prosthesis. Thehighlight of the first day, apartfromtheveryinformativelecturesbytheexperts,wasthehands-ontraining conducted byMr JoakimPersson from AtosMedical. Thisgave the participants an insightintothecomprehensive rangeofproductsavailable to ‘normalise’the lives of laryngectomisedpatients. After the lunch break,the delegates were broughtto the operating theatre to

observe Professor Dr Bing Tanperform twosecondaryTracheo-esophageal punctures (TEP)supported excellently by High-Definitionaudio-videofacilityandrunning expert commentary anddiscussion.

In the evening, after an intensefirst day, the delegates enjoyeddinner at Saloma Bistro, KualaLumpur,andwereentertainedbycolourful cultural performances.Some of the participants hadthe opportunity to display theirdancingtalentsonstagewiththeprofessionaldancers!

Day 2 started with Dr IqbalFarim from UKM sharing UKM’sexperience with the voiceprosthesis over the last 18years. This was later followed

by talks on Adverse Eventsand Trouble shooting in voiceprosthesis by Professor Dr BingTan and Olfactory Rehabilitationby Ms Kornman. The highlightof the second day was the liveprosthesischangedemonstrationon real post-laryngectomypatients as well as a review ofthepatientswhounderwent thesecondaryTEPthepreviousday.

Allinallitwasaverysuccessfullyruncourseandaveryworthwhileone indeed. GPRA is likely to bea fixture in the Malaysian ENTcalendarforyearstocome.

The MSO-HNS annual scientificmeeting (ASM) 2013 was heldin a five star facility at SaujanaHotelKuala Lumpuron the31st March 2013. This successfulevent organized by Dr RevadiGovindaraju drew entries fromtwelve postgraduate students,one of the highest number inthe history of ASM. The eventstartedwithawelcomingaddressby Prof. Dr Primuharsa Putra,presidentofMSO-HNS2012/13followed by the postgraduateoral presentations. The paperspresented were of the highestqualitywithusefulscientificvalue.The trainees gave up-to-datepresentationsontheirtopicswhichwere on par with internationalstandards.Theycertainlydidnotmakeiteasyforjudges,DatinDrSiti SabzahMohd Hashim , Prof.DrPrimuharsaPutraandAssoc.Prof. Dr Mohd Khairi Md Daudto pick the best three. All thepresentationswentsmoothlyandkept to time. The guest lecturethis year was presented by Prof.Dr Prepagaren Narayanan. Hegaveanextremelyenjoyabletalkonallergic rhinitis interlacedwithusefuleyeopeningvideosonthemechanism of allergy and alsoincluded videos of his surgicalexperience.TheeventcontinuedwiththespecialistcategoryaftertheexcellentlunchsponsoredbyGSK. There was cool responsefor the specialist category withonly three entries this year.However the show must go on

andeveryonewashappywiththeoverall quality of presentation.The evening ended with theawards ceremony. UniversitiKebangsaan Malaysia cameout proudest for the traineescategory with Dr MawaddahAzmanwinning first prize for herpresentation titled ‘Randomizedcontrolled trial on effects ofglutamine plus supplementationin surgical patients with headandneckmalignancy’.ThirdplacewenttoanotherUKMtraineeDrSiti Zulaili Zulkepli with her topic‘Insulin resistance in patientswith obstructive sleep apnoea:acrosssectionalstudy’.Secondplace went to Universiti SainsMalaysia’s Dr Shamina SaraMoses Peter who presented‘A study on effectiveness ofautofluoresecence endoscopy indiagnosing upper aerodigestivetract tumours’. Dr SarmadAlazzawi emerged as thewinnerfor the specialist category withhis study titled ‘Radiologicalanalysis of the ethmoid roofin the Malaysian population’.The ASM has been a platformfor otorhinolaryngologists inMalaysia topresent the findingsoftheirmostrecentworktothesociety. It is a place not onlyfor sharing and discovering newknowledge but also for learningand improving our researchskills. Congratulations to all thewinners, theorganizersandalsotoallparticipants.

MSO-HNS HEALTH cAMP AT SG. LEMBING Dr Rekha Balachandran, Hospital Ra ja Permaisuri Bainun, Ipoh, Perak

The annual MSO-HNS healthcampwasheldthisyearinSungaiLembing,Pahang.Thiscampwasconductedwiththecollaborationof the Department of ORL,International Islamic University(IIU)andHospitalTengkuAmpuanAfzan(HTAA),Kuantan.Thiseventwas jointly organized by MSO-HNS and Department of ORL-HNSIIU.TheorganizingchairmanwasDrRekhaBalachandranwithDr Pathma Letchumanan andthe co-organizing chairman wasAssociate Professor Dr ZamzilAmin Ash’ari from IIU. ProfessorDr Primuharsa Putra andDr YapYokeYeowactedasadvisors.

Thehealthcampwasheldonthe7thofAprilattheAnjungKomuniti1 Malaysia in Sungai Lembing.The turnout from the localcommunity was 201 people.ORL surgeons, medical officers,audiologistsandparamedicsfromIIU, HTAA and Hospital Temerlohwere joinedby fourmembersofour EXCO. The ORL team of IIUalso provided hearing screeningviatheirmobileaudiology-testingunit.Thestatehealthdepartmentalso conducted general healthscreening and education for theparticipants.

Thisevent received funding from

the Health Promotion Board ofMalaysia (MySihat). The healthcamp was officiated by the“Penaung Komuniti 1 Malaysia”or ex-ADUN of Sungai Lembing,Dato’Md Sohaimi bin HajiMohdShah.

MALAYSIA’S FIRST ENT SPEcIALIST TRAINING PROGRAMME IN PRIVATE INSTITUTION Prof. Dato’ Dr Lokman Saim, President & Dean School of Medicine, KPJ Healthcare University College.

TheMasterofOtorhinolaryngology-Head and Neck Surgeryprogramme at KPJ HealthcareUniversity College (KPJUC) andKPJ Healthcare Berhad, is thefirst private medical specialisttraining programme in Malaysia.This programme is endorsed bytheOtorhinolaryngology ConjointCommittee, The MalaysianQualification Agency (MQA/PA 2135) and approved by theMinistry of Higher Education(N/721/7/0024). It providescandidates with the opportunitytobetrainedinthemanagementandtreatmentofawidevarietyofENTdiseases in6accreditedKPJSpecialist hospitals.This4yearprogramme follows the UKM/UM/USMconjointcurriculum.Theprogrammeprovidesaninnovativecompetency based education

programme featuring one-to-one, hands-on apprenticeship,supervisedbyagroupofdedicatedand experienced KPJ ENTconsultants. The Faculty ofMedicine Universiti KebangsaanMalaysia is the collaboratorin the curriculum design andassessment to ensure a highquality training programme.Clinical Professor Dr PrimuharsaPutraSabirHusinAtharfromKPJSeremban Specialist Hospital isappointed as the co-ordinatorfor the programme. Currently,3 students are enrolled since1st January 2013 under thesupervision of Clinical AssociateProfessor Dato’ Dr SolahuddinKenali (KPJ Tawakkal SpecialistHospital), Clinical Professor DrAminuddinSaim(KPJAmpangPutriSpecialist Hospital) and Clinical

LecturerDr RoslanMahmud (KPJDamansaraSpecialistHospital).

Apprenticeship training

The hallmark of the trainingprogramme is “one-to-one”,hands-on apprenticeship, whereeach candidate is supervised bya consultant. Candidates willbe supervised by experiencedKPJ ENT consultants on a 3 to6 monthly rotation. In the earlypart of the training, candidatesare introduced tobasic skills foroutpatient assessment, careof inpatients and basic ENTprocedures. Adequate time isgivenforselfstudyinpreparationfor the Part 1 Basic SurgicalSciences examination. As thecandidates gain experience andbecomemoreproficient theywill

beallowedtoprogressinlevelofsupervised clinical skills learningopportunities. Candidateswill begiven increasing responsibility informulating diagnostic workupsand treatment plans as theirexperience and competenceincrease. In the operation room,candidatesarealwayssupervised,but allowed to become moreindependent in decision makingand performance as their skillsevolve, guided by immediateformative feedback by theconsultants. As candidates gainfurther patient care and clinicalskillsexperience,theyprogressesin responsibility to one of“supervisedindependence”,wherethe supervising consultants arealways involved in the care ofevery patientand haveultimateresponsibility.

Intandemwiththegrowingneedtopracticeevidence-basedmedicine,MSO-HNSresearchgrantwassetupthisyeartoprovideaplatformto support research projectscarried out by ENT residents,trainees and qualified surgeonswhichwillresultinpublicationsinInternationalISI/ScopusJournals.The research grant’smainaim istoencourage theENT residents,trainees and qualified surgeonsto conduct study and researchrelatedtootorhinolaryngology.

The grant awards a maximumof RM20,000.00 per researchwith strict adherence to the

guidelines set-up. It is restrictedto ENT residents, trainees andqualified surgeons who aremembersof thesocietyandarenot receiving other grants forthe same research project. Theproposals are to be made bythe 30th of September and thereimbursement begins from 1st January the following year till31stDecemberofthesameyear.

Successful candidates will berequired to present his/herresearch in the Annual ScientificMeetingnotlaterthantwoyearsfromthecommencementdateofthe project with evidence/proof

ofsubmissiontoanInternationalJournal. However failure ofcompletion of research withoutavalidreasonorpresentationattheAnnualScientificMeetingwillresultintheprincipalinvestigatorhaving to return the grant andwill notbe considered for futuregrants. The keynote address of thecongress was delivered byProfessor Naresh Punjabi, fromJohnHopkinsUniversity.ProfessorPunjabi emphasized that sleepmedicine is an archetype of amultidisciplinarymedicalspecialtywith involvement of variousdisciplinesincluding,butnotlimitedto, otolaryngology, pulmonology,neurology,psychiatry,psychology,dentistry, neuroscience, andepidemiology.Theotherprominentspeaker was none other thenthe father of sleepmedicine, DrChristian Guilleminault from theDepartment of Psychiartry ofthe Stanford University MedicalSchool. There were many otherprominent international, regionaland local speakers who wereexperts in their respective fields.Some prominent names includePatrick Bradley, David Terris, RayWeiss, Simon Robinson, GoergSprinzl, Lim Li Ling, NarichaChirakalwasanandSiowJinKeatamongothers.

The meeting was attended byabout 500 delegates from bothsurgical and medical specialties.Concurrent sessions were

Professor PatrickBradley was made an honorarymemberoftheMSO-HNS.

Thecongresswaswellorganisedand it has set the mark forcollabaritveworkinsleepmedicinein the region. The 2nd ASEANSleepCongresswilltakeplaceinBangkok,Thailandfromthe1st to the3rdofAugust2013.

4th MALAYSIAN INTERNATIONAL ORL-HNS cONGRESS AND INAUGURAL ASEAN SLEEP cONGRESS

2012 was a landmark yearfor MSO-HNS. For the first timea collaborative meeting wasorganized between varioussocieties in the field of sleepmedicine. The main congresswascenteredonSleepMedicine,which is in itself notanew fieldbut in recent years has seentremendous developmentswith new thoughts and ideasparalleled with technologicaldevelopments. With the supportof the Ministry of Health ofMalaysia and our partnersfrom the Malaysian ThoracicSociety, Malaysian PsychiatricAssociation, Malaysian Societyof Neurosciences, the NationalHeart Association of MalaysiaandtheMalaysianHealthyAgingSociety,wewereabletogiveanintegrated approach to SleepMedicine - the first time such acollaborationonSleephasbeenbrought together in a congressin this region. We are proud tohave flown in prominent leadersof Sleep Medicine from aroundtheworldtosharetheirexpertisewith our society members andcongressparticipants.Therewereconcurrent sessions on Otology,

The congress venue was atthe Kuala Lumpur ConventionCentre. The opening ceremonywas graced by our HonorableHealth Minister; Liow TiongLai who warmly welcomed allspeakers and participants. Hisspeech touched on variousaspects on the epidemiologicalandeconomical burdenof sleepdisorders including the issuesleep related motor vehicleaccidents.
