VaxApp User Guide

Introduction The VaxApp is a web-based application that allows NYP employees to:

Submit proof of COVID-19 Vaccination (if received outside of NYP)

Request Medical Exemption from COVID-19 Vaccine

Request Religious/Deeply Held Belief Exemption from COVID-19 Vaccine

The following User Guide will provide background on how to log in and complete any of the capabilities listed above.

Instructions 1) Log into the VaxApp portal with CWID and password: https://myapps.nyp.org/VaxApp

2) Users will be prompted to provide their contact number and select the appropriate submission type

Submit proof of COVID-19 Vaccination (if received outside of NYP) 1) Once logged in, enter contact number, select submit proof of COVID-19 Vaccination (if received outside of NYP),

and click submit.

2) Select Vaccine Manufacturer, enter date(s) of dose(s) (dependent on vaccine), upload copy of vaccine

document, and click submit.

3) Once submitted, user will see submission confirmation on web app and receive confirmation to NYP email.

4) Users can check status of submission by logging back into the VaxApp.

Request Medical Exemption from COVID-19 Vaccine

1) Once logged in, enter contact number, request medical exemption from COVID-19 vaccine, and click submit.

2) User will receive email with exemption form and additional instructions.

3) Once required form is complete, user can log back into VaxApp, upload document, and click submit.

4) Once submitted, user will see submission confirmation on web app.

5) Users can check status of submission by logging back into the VaxApp.

Request Religious/Deeply Held Belief Exemption from COVID-19 Vaccine

1) Once logged in, enter contact number, request religious/deeply held belief exemption from COVID-19 vaccine,

and click submit.

2) Complete required text field, check required box to electronically sign exemption request form, and click submit.

3) Once submitted, user will see submission confirmation on web app and receive confirmation to NYP email

4) Users can check status of submission by logging back into the VaxApp
