Nitratos
- Disminuyen la demanda miocardica de oxigeno al producir vasodilatación sistemica.
- Útiles para episodios agudos, prevenir nuevos episodios o aumentar la capacidad física.
- Requiere de intervalos libres de farmaco 8-12 hrs
- Mejoran la tolerancia al esfuerzo.
- Prolongan el tiempo inicio de angina y el tiempo hasta la depresión del segmento ST.
- Su efecto anti anginoso y anti isquémico se sinergiza al efecto de bloqueadores beta o antagonistas del calcio.

Betabloqueadores
• Alivian la angina al disminuir la FC y contractilidad.
• Retarda o aumenta el umbral de isquemia al ejercicio.
• Disminuyen los episodios de angina y aumentan la tolerancia al ejercicio.
• Previenen el reinfarto y mejorar la sobrevida en pacientes con IM.
• Los betabloquadores cardioselectivos de acción prolongada son los preferidos para el tratamiento de la ACE.
• Metoprolol, Atenolol.
• Meta: 50-60 lpm en reposo y menos de 100 lpm al ejercicio.

Calcioantagonistas
• Actúan causando vasodilatación periférica y coronaria y disminuir la contractilidad.
• Se prefieren el uso de diltiazem, verapamilo, amlodipino o felodipino.
• Suelen usarse en conjunto con los betabloqueadores o cuando estos están contraindicados.

•Incluso con TMO una alta proporción de pacientes persistían con angina residual de 1-5 años
•Es necesario terapias antiaginosas mas efectivas.
