The Active Female: Maintaining Ethics in Uncharted Territory

Course Description

The work of healing has not had much of a place in the complex of reckonings in the past year that we condense into the hashtag “#MeToo.” But surely what we are naming and wrapping our arms around with the impetus of #MeToo is, at best, an opening to a long-term cultural reckoning to grow up humanity, to make our society more whole. I walked with some trepidation onto this territory, inviting those who navigate this landscape with admirable cause. Drs. Atkins, Sorrentino, Baumgardner and our male perspective, Dr. Bagnell. How can this profession maintain a superior ethical standard and be a model for other professions. Clearly, our “hands on” approach must be executed with the adept social skill, and purest healing intention, since the power of touch is mighty. The ratio of men to women in practice is about 4:1, 60% of our patient demographic is female. It is paramount that we understand The Active Female and delve deeply into Unchartered Territory, so we make expose and maintain the most ETHICAL standards of practice. We explored how they and all the rest of us might build the spaces, the imaginative muscle, and the pragmatic forms to support healing for women and men where it is possible, now and in time. We can entertain a new paradigm of healthcare that goes beyond Evidence Based Medicine. 1993 was the first time the word “Evidence” was introduced into the literature, and the use of the guidelines were remarkably unsuccessful. Intuition-Based Medicine was the predecessor and was most reliable after a lifetime of practice. Today we can merge our Evidence into “Interpersonal Medicine,” a disciplined approach to delivering care that responds to a patient’s circumstances, capabilities, and preferences.

Pink Concussions: Detection/Recovery Principles and Running Biomechanics for the Female Athlete

Course Description

In the first hours of this lecture the attendee will learn the differences between female and male brain activity after TBI opens the way for specific examination and treatment protocols to be delivered to the athlete. Online educational provides part of the solution, whereas hands-on applications bring the clinician to the "field" of play in producing the results with consistency. Dr. Bagnell will review, demonstrate, and lead doctors to experience the hands-on approaches that deliver the most consistent results in this work-shop introduction. In the second hour, the attendees will how pregnancy is the most physically challenging event that the female will experience in her lifetime. Within a short span of a couple months, dramatic physiological, hormonal and biomechanical changes occur. This section will focus on identifying the changes occurring in the female body, evaluating the effects of exercising and developing exercise and rehab programs for these specific female athletes.

Infection Susceptibility and the Influence of Stress

Course Description

This 2-hour presentation will cover the relationship of infection (viral, bacterial, parasitic and fungal), chronic disease, stress and the changes that occur within the HPA access. We will explore the connection of different types of stress: physical as well as emotional that will decrease one’s resistance to infection. The role of neurotransmitters affecting immune cells will affect the HPA axis. In addition, cortisol imbalances and inflammation can be connected with chronic infection. Research around chronic infection and the HPA axis will be presented. Treatment protocols to tackle even difficult cases of these types of conditions will be shared. Case studies on patients seen in practice will demonstrate the success in treating individuals with chronic HPA disfunction both with infection or at risk for infection. Legal and Evidence-Based Clinical Indicators for Chiropractic SMT/Adjustment

Course Description

Validly determining whether a patient requires chiropractic SMT or adjustment is of the utmost importance with respect to ethics, legal liability, patient care, and patient outcomes. Without the ability to validly and reliably determine clinical need for chiropractic SMT/Adjustment there is no ability to validly establish ethical, effective care recommendations or to defend them. It is valid, evidence-based, legally accepted clinical exam findings/diagnosis that MUST determine care recommendations; there simply is no other ethical, legal, or clinically defensible method. Clearly it is of the upmost importance that every chiropractor MUST, for both legal and clinical reasons, be aware of, and be skilled in assessing, the legal and peer-reviewed clinical indications/diagnostic criteria for chiropractic SMT/Adjustment.

Course Description

Proper documentation and proper CPT coding and are co-dependent in today’s chiropractic practice. A thorough discussion of documentation of the doctor’s encounter with the new patient and subsequent follow up visits including establishing medical necessity for all treatment recommended and performed along with the proper use of CPT coding as it pertains to the most common examination and treatment procedure codes in the modern chiropractic practice. This discussion begins with end in mind by starting from the insurance reimbursement check and works backwards through a thorough dissection of the health insurance claim form, daily SOAP notes and the initial report. The entire discussion is centered on medical necessity and the doctors are shown how medical necessity is developed and where it comes into play from the initial visit to the printing of a claim form. We continue with a discussion of various CPT codes used in the chiropractic practice. Compare and contrast therapeutic modalities with physical therapies. Examples are given throughout showing (1) how to choose the appropriate code and (2) how to document the use of that code in the patient’s records.

Documenting Properly: A Conceptual Approach to Reading X-Rays and MRI

Course Description

There are four steps required to properly document the need for diagnostic imaging. In addition, using diagnostic imaging appropriately involves the acquisition of high quality studies and their accurate interpretation. The interpretation is essential to optimizing the benefit of those studies. This requires a solid understanding of pathology and a unique observational skill set. This process takes time to develop but can be enhanced by an approach to image interpretation that uses several basic concepts. This course will review proper documentation, essentials of image quality and a conceptual approach to image interpretation. Cases will be presented to demonstrate these concepts. Participants will never look at diagnostic images in the same way again.

During the past decade an increasing number of sports injury physicians have integrated MSK US into their practices to facilitate the management of athletes presenting with a wide variety of musculoskeletal and neurological complaints. Clearly it is imperative that the most evidence-based interventions with respect to effectiveness, cost-effectiveness, and safety are identified and promoted as we move into the future; about this there is no rational debate. Ethical care is patient centric care and ethical, patient centric care is evidence-based care that provides the best possible chance for patient recovery and satisfaction with the lowest possible risk of adverse effects. The questions being begged are obvious. Of the available interventions which, if any, represents the most evidence-based option with respect to effectiveness, cost-effectiveness, and safety. What represents evidence-based intervention for low back pain and other neuromusculoskeletal health problems? It is not possible to practice ethical, patient-centric, evidence-based care if you are not aware of, and willing to practice based upon, the best available evidence. Further, if we are to integrate and collaborate with other health care professionals, we have a moral obligation to ensure that any referrals or co-management we engage in is also evidence-based. Simply collaborating and integrating in order to engage in a multidisciplinary approach is not, in itself ethical, patient-centric, or evidence-based. Only collaborating and integrating with practitioners that offer evidence-based interventions is ethical and patient centric.

Global Postural Reprogramming

Imbalances in the postural system are the cause of many pathologies. The mechanical component of pain, osteoarthritis, subluxation or vertebral deformities is rarely taken into account by standard treatments. Such pathologies are due to disruption of the postural tonic system that is caused by pathological information received from its sensory inputs: the feet, the eyes, the teeth, the skin, etc. The feet and eyes are the primary points of entry to the system, so any disturbance in how the feet come into contact with the ground or in eye convergence will affect the postural system. A Nutritional and Life-style Approach for Reducing and Eliminating Pain (in the Chiropractic practice)

Course Description

The opioid crisis has caused us to focus on how inadequate we are, as a Nation, in the treatment of chronic pain. The statistics are staggering, and what is being lost in the discussions are the many viable and effective treatment options that are readily available. As a profession, we know that Chiropractic care is a valuable and proven treatment option for chronic pain. However, by incorporating whole food nutrition and life-style modifications into the treatment regime, the reduction and elimination of pain can be greatly enhanced. From a functional and systems biology perspective, if we understand the cause of pain, it can more adequately reduced in therapy. In this lecture Dr. Bump will discuss the various causes of pain which are influenced by diet, nutrition and lifestyle, which, if not resolved, become chronic. He will explain the relation between diet, nutrition and lifestyle and the immune system. It is our immune cells which stimulate the nociceptors and therefore pain. Pain is one of the 5 Cardinal Signs of Inflammation, and therefore a desirable part of both the immune response and the stress response, in the short term. (See Dr. Bump’s Friday morning lecture on Stress and Inflammation.) Further, Dr. Bump will offer an overview of lab testing for assessing inflammation. Also, we will take a deep-dive into nutritional, dietary and life-style therapeutics. Specific nutrients will be discussed which are valuable for reducing both inflammation and pain, and we will look at how unhealthy and imbalanced diets drive the inflammatory process. We will further explore life-style modifications such as meditation, improved sleep and recreation among others.

Neurophysiological Mechanisms That Are Changing Our Understanding of the Chiropractic Adjustment

Course Description

Resent scientific studies are revealing a new understanding about how spinal adjustments work. Heidi Haavik, a chiropractor and PhD trained neurophysiologist has spent the past 16 years studying the changes that occur in the brain when chiropractors adjust chiropractic subluxations. The original theories were based on the idea that dysfunctional spinal segments were ‘out of place’, or misaligned, and that this put pressure on the nerves exiting the spine. We now know that this theory is not really the best way to describe what a chiropractic subluxation is. The original theories were based on the idea that an adjustment relieved pressure off the squashed nerves. We now also know this is not the best way to describe the neurophysiological effects of an adjustment. Today, over a hundred years on from that ‘first’ chiropractic adjustment, we know much more about how the brain and the rest of the central nervous system functions. And it is becoming clear – finally - just how the chiropractic adjustment really work. We have now come to understand is that we don’t really put bones back in place when we adjust the spine. A chiropractic subluxation is not so much the condition of a bone being out of place; it is more that a bone is functioning or moving in a less than ideal way – in a manner that is not ‘normal’ for the body. The spine itself has three basic functions; 1) sometimes to move to dissipate forces for example during running, 2) sometimes to stiffen up – also to protect us for example during heavy lifting, and 3) sometimes to reflexively respond to maintain balance and prevent falls. Thus from a neurophysiological perspective, if vertebral motion segments are not doing one of these three things when it should then we have a Central Segmental Motor Control problem – and from a neurophysiological perspective this is what a chiropractic subluxation is. What is really interesting is how spinal dysfunction impacts our brain’s function, and how this likely translates into clinical symptomatology (or not). Dr Heidi Haavik will share with you a summary of where we are at with the neurophysiological understanding of the impact of spinal function on brain function, and will discuss what future implications this has for us as a profession.

Fables of Foibles: Identifying Fake News in Research Studies

Course Description

For 65 years, the accepted standard by which the usefulness of therapeutic treatment is judged has been the randomized controlled trial (RCT), building from Hippocrates’ premise 2000 years ago that experience combined with reason was the therapy of choice for patients. In other words, any treatment plan should both seem reasonable in theory and then be tested experimentally. Assuming that any threats to both internal and external validity could be ruled out, the RCT became what is commonly regarded as the highest quality of clinical outcome study that could be mounted to allow inferences about cause and effect relationships to be drawn. Even higher in the traditional ordering of quality and rigor has been the systematic review and meta-analysis, amassing all the pertinent literature on a given subject to date and rating the rigors of these studies, both in the aggregate and individually. Thus, RCTs are rated higher than cohort studies or case series or case studies. This is shown in FIGURE 1a. But this totemic relationship ignores numerous sources of important research information, such as basic research, epidemiology, and health services research. In addition, models of evidence-based medicine (EBM) commonly used by third party payers have commonly ignored clinical judgment and patient values and expectations, which together form a tripartite with experimental results. Compounding the issue is that poor systematic reviews which comprise a significant portion of EBM are prone to subjective bias in their inclusion criteria and methodological scoring, shown to skew outcomes. Furthermore, the blinding concept that often elevates the quality score of RCTs is particularly problematic in applications of physical medicine. Finally, several examples of flawed and arguably fraudulent randomized controlled trials have appeared in the literature, becoming an integral part of many systematic reviews and consequently negatively impacting the chiropractic profession. Compounding the problem of the defective RCTs is the fact that they get swept into the mainstream media, seriously misinforming the public and other healthcare professions who have stayed away from chiropractors as a result.

Science Behind Brain-Body Biomechanics: Eliminating Pain through Proper Function

Course Description

Movement through life does not just travel from point A to point B. It also incorporates how fluid and effortless the movement is to the proper destination. It gets more complicated unfortunately, because we are inundated with multiple distractors along the way. As we get older in life, we lose some of the ability to compensate and the aberrant movement patterns that use to cause little concern now start catching up to us. How the brain processes these changes, influences how our body compensates and rewiring of are neural moment pathways are then ingrained. How do we change this process that may have been happening for years and is now causing pain and discomfort? How do we identify these abnormal changes before they affect are daily quality of life? These are the questions I want to answer in this lecture and give tools to set up a proper treatment plan to correct these changes.

• Discuss how treating the body without understanding how it effects the brain is doing our patients a disservice • Discuss why balance is important and how to identify balance deficits quickly • Discuss all the component within the gait cycle and what muscles are firing at each component

Infection Susceptibility and the Influence of Stress

This 2-hour presentation will cover the relationship of infection (viral, bacterial, parasitic and fungal), chronic disease, stress and the changes that occur within the HPA access. We will explore the connection of different types of stress: physical as well as emotional that will decrease one’s resistance to infection. A Functional Approach to the Blood-Brain Barrier in Health and Trauma

Course Description

The blood brain barrier (BBB) is a physical barrier between the brain and the circulating blood, which restricts the movement of all soluble proteins from the blood across to the brain. Within hours of concussion or other traumatic brain injury (TBI), the BBB and intestinal barrier fail. This scenario allows for the invasion of intestinal bacterial toxins, neuronal tissue-binding food proteins and cross-reactive food protein antibodies into the once protected nervous system. Bacterial toxins, some of which cause SIBO (small intestinal bacterial overgrowth) contribute to systemic inflammation that cyclically keep the intestinal and blood-brain barriers open, even years after the trauma. Additionally, specific food lectins and agglutinins bind to myelin tissue. Patients with circulating food antibodies and/or lectins and agglutinins, who experience BBB breakage, are at the risk of reactive antibodies infiltrating the brain and causing neurological tissue damage. Once environmental trigger antibodies cause tissue damage, neurological autoantibodies can be produced, which will further contribute to the neuroautoimmune process. It is therefore important to implement a pro-brain dietary protocol and treatment strategy to heal the broken BBB. By taking proactive strategies, the risk of neurological disease occurring after the concussion or other TBI, can be significantly reduced.
