Whether you are a competitive athlete or just enjoying your workout, what you eat will affect your overall performance. Give your body the right fuel and you will begin to see results. The following guidelines provided by Christina Bowling, Director of Clinical Nutrition, are based on current scientifi c research focused on the proper nutrition and timing to prepare pre-exercise, during exercise and post-exercise snacks.

Pre-Exercise

Combining protein and carbohydrates 3-4 hours before an intense workout helps with overall muscle growth and provides more energy when exercising.

Carbohydrates sustain our blood sugar as well as make up our body’s glycogen stores, which is what provides us prolonged energy throughout a workout. As glycogen levels decrease, exercise intensity and work output diminish1. Because of this connection, the concept of carbohydrate loading is one of the oldest forms of nutrient timing practices. Glycogen stores are best promoted by following a high-glycemic, high-carbohydrate diet (600-1000 grams or ~8-10 g//kg or body weight/daily)[2, 3, 4]. More specifi cally, studies have shown that a high carbohydrate meal four hours prior to prolonged exercise (around 105 minutes) increased muscle glycogen stores as much as 42%, which resulted in higher utilization of muscle glycogen1.

Researchers have explored the potential of ingesting protein and/or amino acids, either alone or in combination with carbohydrates, in order to enhance training status. Results show that pre-exercise ingestion of essential amino acids or protein alone increases muscle protein synthesis. It has also been shown that combined

protein and carbohydrate consumption pre-exercise produces signifi cantly greater levels of muscle protein synthesis than just protein consumption alone[5, 6]. The current general guidelines recommend 1-2 grams carbohydrate/kg/ of body weight and 0.15-0.25 grams protein/kg/ of body weight 3-4 hours before competition4.

During Exercise

When fueling during your workout, try to limit sup-plements high in fructose to avoid stomach cramps. Ingesting small amounts of protein and carbohydrates during your workout is recommended to assist with endurance, performance and muscle recovery.

Examining optimal carbohydrate delivery in effort to sustain glucose is where the initial research put its focus. For example, Australian researchers had eight highly trained cyclists complete two trials at 70% VO2 max until the point of volitional fatigue (the point at which a person cannot perform a muscular contraction). Before exercise, and every 15 minutes throughout, participants were either given a placebo or an 8% carbohydrate solution to ingest. Ingestion of the carbohydrate solution was associated with a 30% increase in time to reach volitional exhaustion, or a 47 minutes longer period of cycling when compared to placebo7. This study goes along with the carbohydrate recommendation during exercise: 30-60 grams of carbohydrate should be per hour and can typically be delivered by drinking 1-2 cups of a 6-8% carbohydrate solution (8-16 ounces) every 10-15 minutes8. That solution should contain a mixture of carbohydrates like glucose, fructose, sucrose, and maltodextrin.

The addition of protein to carbohydrate during exercise has also been investigated as a means to improve performance and facilitate recovery. A ratio of 3-4:1 (carbohydrate: protein) has been shown to increase endurance performance during both acute exercise and subsequent bouts of endurance exercise[9,

10]. During resistance exercise, a combination of both carbohydrates and protein has been shown to increase muscle glycogen stores11, offset muscle damage12, and

also produces greater training adaptations after acute and prolonged periods of resistance training13.

Post-Exercise

Within 30 minutes after your workout you should re-fuel your body with carbohydrates. There is solid evidence which supports the value of post-exercise nutritional supplementation as a means of recovery for both endurance and resistance athletes. Ath-letes who ingest 1.5 grams carbohydrate/kg of body weight within 30 minutes after exercise have been shown to have a greater rate of muscle glycogen re-synthesis than when nutrient supplementation is delayed by two hours. This is largely due to a greater sensitivity of muscle to insulin14. These carbohydrates can also be consumed in either liquid or solid form and will promote similar levels of glycogen re-syn-thesis4. To ensure complete recovery of muscle and liver glycogen it is also recommended that frequent feedings of carbohydrates in high amounts over the 4-6 hours following intense exercise[4, 8].

Ken was born and raised in Florida and currently re-sides in Jacksonville. After a long career at an insurance company where he constantly sat at his desk, nine hours a day for 40 years, he noticed that his body had become more and more infl exible. Given his love of golf, this concerned him and he wanted to undo the damage from his sedentary lifestyle.

Ken didn’t work out much, but knew that once he re-tired he would return to golf, which he had put on the back burner for so many years. He realized that without some help, his game would not improve. That’s when he decided to become a member of the Fitness Centre, and begin training with Commit 2 Fit Certifi ed Personal Trainer Rayann Cruz.

“I enjoy Ken’s determination to improve his current fi t-ness level and his drive to improve his golf game,” said Rayann. “We focus on core strength, fl exibility and bal-ance exercises and he has increased his fl exibility and improved his joint range of motion. We make a great team because he trusts that I will plan exercise routines that are helpful not hurtful.”

After retiring two years ago, Ken has maintained an es-tablished workout routine with Rayann. Noticing the improvements while training has motivated Ken to continue his workout regimen as well as to seek addi-tional golf-specifi c training at a nearby golf course.

The results from personal training have allowed Ken to golf two to three times a week and he believes working with a personal trainer is important to prevent injury and focus on his personal goals. Just this past month, Ken putted for an Eagle for the fi rst time in a long time. He was also able to break a score of 100.

“At my age [64], it’s amazing how much harder it was to get back into a routine.” said Ken. “I knew how out of shape I was from sitting at a desk all day. I’m not one of those people who enjoys the gym, but the better I got the more I’ve enjoyed it and I have improved my golf swing!”

Neutral or cushioning shoes are designed for those with higher arches and provide the wearer elevated shock absorption. These are not designed for those who overpronate, but for those with a neutral stride or a runner with a foot that supinates. Supination oc-curs when the runner stays more to the outer edge of their foot while running without the standard roll to the interior of the foot. Those with supinating or neutral running styles will want a cushioning shoe in order to help absorb impact and take pressure off of the joints of the body.

How long have you had your shoes?

Clinical Exercise Physiologist & Commit 2 Fit Personal Trainer Bill Kelly specializes in providing metabolism testing and has helped more than 2,000 clients overcome their personal health challenges

Celebration Health’s Bariatric program is one of the few in the country that has a full time, dedicated Exercise Physiol-ogist. And, Bill Kelley serves in that critical role.

“Exercise is a critical component of any long term weight loss program,” Bill said. “especially if undergoing Bariatric surgery. “

Bariatric surgery or weight loss surgery, is a type of pro-cedure performed on people who are dangerously obese, for the purpose of losing weight. This weight loss is usually achieved by reducing the size of the stomach with an im-planted medical device (gastric banding) or through re-moval of a portion of the stomach.

The Total Wellness Approach

Although the surgery will create a calorie defi cit, it will not determine where the weight loss comes from - muscle or fat. Dr. Keith Kim, director for the Metabolic Medicine and Surgery Institute at Celebration Health, recognized this when he started performing bariatric surgeries and embraced the concept of a complete patient care team. A program that emphasized not only the surgery, but also the lifestyle needed to max-imize long term success.

All bariatric patients require a metabolism test prior to surgery. This provides baseline measures and ed-ucation regarding exercise’s role in helping them to achieve long term success. Post surgery patients are re-tested at one month, three months, six months and one year. This helps determine whether the patient is maximizing their fat loss and sparing muscle tissue or if a more specifi c exercise protocol is necessary to maximize the effects of surgery.

The Celebration Health Bariatric Program is nationally

recognized as a Centre of Excellence. This is the highest designation that can be bestowed on a bariat-ric program. The program has to undergo a rigorous review process that demonstrates hospital support, documented clinical outcomes and a dedicated pa-tient care team that includes clinical, nutrition and ex-ercise. As such our program stands as a benchmark in providing metabolism testing, exercise education and post surgery follow-up.

In addition to metabolism testing and patient educa-tion, Bill, who received his Clinical Exercise Physiolo-gist and Exercise Test Technologist certifi cations from American College of Sports Medicine (ACSM), also provides individual exercise prescriptions.

“Over 90% of our bariatric patients have some type of orthopedic constraint that can limit their exercise capacity,” Bill explained. “ The Fitness Centre at Cele-bration provides a unique and ideal environment in which to assess and tailor individualized programs.”The extent of this component has not been highlight-

ed nationally. By bringing attention to our education and post sur-gery programs, Bill explained, we have an incredible opportunity to get more exposure and publicity about the lifestyle changes critical to bariatric success. It is good for our program, patients and for the industry as a whole.

“Exercise is absolutely critical to long term success and is the most neglected compo-nent in most bariatric programs,” he explained. “Our program at Celebration Health embraces exercise and the total lifestyle change component.”

When Dr. Damon Tanton, Endocrinologist and co-di-rector of the Metabolic Medicine and Surgery Institute, came to Celebration Health, he wanted to establish a non-surgical weight loss program that incorporat-ed all facets for long term weight loss success. These components included medical testing, physician care, nutrition, lifestyle counseling, metabolism testing, ex-ercise prescription and spiritual guidance. Bill helped establish the metabolism testing and physical activity protocol for this program.

“The experience I gained from establishing both bar-iatric and endocrinology exercise objectives and mea-sures was critical to developing our current patient metabolism testing and education classes for other physicians to refer their patients,” said Bill, who has been with the Fitness Centre since 2002.

Metabolism Testing

Put simply, metabolism is the amount of energy (cal-ories) your body burns to maintain itself. Your body is constantly burning calories to keep you going. The actual metabolism test is called Indirect Calorimetry. This test measures a person’s resting oxygen utiliza-tion for a ten minute time period. The result pro-vides a very accurate measure of how many calories that the person utilizes at rest during a 24-hour time

period, Bill explained. This is critical in establishing a calorie range that creates a defi cit but does not re-sult in muscle loss.

The test also measures your BMI (body mass index) and body com-position. Once baseline measures are taken short term targets for weight loss, BMI and body fat are established for the person. This

helps provide realistic, measurable and time sensitive results.

Women’s Health

Body fat actually acts as an independent organ in the body infl uencing hormone activity and energy uti-lization. When a person has excess body fat many negative physiological effects can occur including in-creased chance of heart disease, diabetes, high blood pressure and even cancer. In women increased body fat can decrease fertility, increase complications with POS (Polycystic Ovary Syndrome) and Endometriosis, complicate surgical adhesions and increase degenera-tive disease postmenapause.

Bill, who received a B.S. in Exercise Medicine from Marshall University, works with six other Celebration Health physicians and their female patients.

“Bill is an amazing trainer,” said Fitness Centre Opera-tions Manager Laura Jones, “He is as comfortable train-ing power lifters and youth as he is working with dis-ease education and speaking at medical conferences. He has a wonderful con-nection between preventative health and wellness and medical treatments provided by the physicians.”

Celebration Health’s new parking garage is open and operating as of Monday, April 29. “It’s seven levels of pure parking pleasure,” said Vice President of Operations Doug Harcombe (pictured above) smiling in front of the completed project, “With 1,032 parking possibilities.” A sheltered walkway leading to the rear of the main lobby will be completed May 13.

The Member Challenge: ‘I Lost it at the Club’, began on Wednesday, April 3. The goal is to workout and keep each other motivated while losing the most body fat percentage by the fi nal weigh in on Wednesday, May 29. Six teams were created and 46 participants signed up.

Your skin works hard to protect itself from the sun, heat and even the cold weather, but over time the environment’s harmful effects will begin to accelerate the aging process. Microdermabrasion removes this outer-most layer of dry, dead skin cells, revealing a younger looking you.

“A microdermabrasion regimen can help even the most complicated skin,” said Day Spa Esthetician Carrie Reed. “It’s my favorite facial that we offer because clients can see a difference the fi rst time they do the treat-ment and the results keep getting better the more treatments they do.”

The microdermabrasion treatment is done with the USB Aesthetics Power Peel Mi-crodermabrasion machine which precisely exfoliates the surface of the skin reducing fi ne lines, sun damage, and acne scaring. It feels like a delicate exfoliating scrub is being brushed over the surface of the skin, Car-rie explained. The intensity of the treatment can be adjusted depending on the client’s needs. You will notice a difference after the

fi rst treatment but over time this repeated process stimulates the increased production of collagen and elastin, key proteins in your skin that keep it looking fi rm and fl awless.

The ideal regimen is to do a series of six treatments every 10-12 days, take a break for a month, then set up a monthly mainte-nance program. Following a treatment, the

skin must be kept hydrated and of course sun protection is essential. The intensity of the microdermabrasion can be customized to fi t anyone’s skin type.

Here Comes the Sun...

With spring in full swing and summer just around the corner, it is important to talk about the risks of too much sun exposure. Skin Cancer is the most common form of cancer in the United States and more than 3.5 million skin cancers cases are di-agnosed annually. Each year there are more new cases of skin cancer than the combined incidents of cancers of the breast, prostate, lung and colon.

It is important to know what you can do to help prevent these painful situations from effecting you directly. Since it’s introduction in 1979, The Skin Cancer Foundation has always recommended using a sunscreen with a sun protection factor (SPF) of 15 or higher as one important part of a complete sun protection regimen. Statistics show that wear-ing at least 15 SPF should allow the average person to stay in the sun fi ve times longer than they would without any sunscreen on. We would agree to this to an extent if sunscreen was permanently affi xed to our skin but since sweat, water, clothing…etc. can interfere we strongly recommend re-applying every hour to ensure your skin’s safety.

Most people with a darker complexion produces a larger quantity of melanin which has a bit more natural sun protection versus people with “fair” skin. People with “fair” skin tend to produce more red melanin that has less protection against UV light. Sunscreen protects against the total amount of sun exposure and not just the duration of the time you are outside.
