1.0 PURPOSE AND SCOPE

1.1 Purpose

This procedure provides instructions for operation of T-handle operated, motor operated,

and gear-actuated ball valves in the Tank Farm waste transfer system when associated

work documents do not contain the positioning requirements therein.

This procedure also provides instruction for the installation and/or removal of Tank Farm

transfer valve tamper seals. Tamper seals are used when valve positions have been

established to support current and future transfer operations.

HNF-SD-WM-TSR-006, Tank Farms Technical Safety Requirements; AC 5.8.6, Double

Valve Isolation is applicable to the entire procedure.

1.2 Scope

This procedure is applicable for T-handle operated and gear actuated two-way and

three-way valves in the Tank Farm waste transfer system.

2.0 INFORMATION

2.1 Terms and Definitions

Batch documentation - This is when a checklist or table with multiple tasks is

followed but not filled out “AS-YOU-GO” to perform a task.

This is not allowed. Each item on a checklist or table shall

be signed off as it is completed by the performer.

Tamper Seal - Devices used to indicate tampering of a component. These

seals are typically made out of metal with wire which

connected together provide a positive known condition.

Tamper seals must have a unique identification number to

ensure separation and uniqueness at all times.

Transfer Valve Tamper

Seal Log -

Tracking system maintained at the Central Shift Office to

manage tamper seal information, documenting the

installation on or removal from waste transfer valves.

Initial Valve

Positioning -

The process of positioning transfer valves per a work

document or procedure.

Waste Transfer

Activity -

Activities that involve transfer of waste or water through a

waste transfer system (e.g., DST waste transfers, waste

retrievals, flushes, leak checks).

NOTICE In Use Tag - Tag used to designate that a valve is currently in use and

shall not be operated without authorization from the Shift

Manager, Shift Production Team (SPT) OE or Retrieval OE.

2.2 General Information

2.2.1 Although this procedure may be used on its own, it is typically used in

conjunction with other approved procedures. For example, Waste Transfer

procedures incorporate steps from this procedure to ensure the requirements

of Technical Safety Requirements Administrative Control (AC) 5.8.6,

“Double Valve Isolation” are met. If a valve were being positioned for the

purpose of Lockout/Tagout, positioning would be documented on the

Lockout/Tagout Authorization Form. Positioning valves for maintenance or

testing purposes would be controlled by the work package, with this

procedure either referenced or steps incorporated into the work package.

2.2.2 Initial valve position may not be easily determined and may require physical

rotation for determination of initial position.

2.2.3 When positioning valves, the mechanical stop may already be engaged and

valve will not rotate further.

2.2.4 On gear actuated valves the reference and adjustable pointers are aligned to

precisely match the actual valve position when rotated clockwise. If the valve

actuator was rotated counterclockwise the gear actuator would lead the actual

valve position by the amount equal to the gear make-up. The hand-wheel has

a dead spot of approximately one half turn before the ball on the valve starts

to rotate. This is accounted for in the placement of the mechanical stops

during the initial alignment.

2.2.5 Safety Significant valves utilized for DVI must be positioned in accordance

with the safety basis requirements. The valve configuration dictates what

type of method is acceptable for ensuring valve is positioned properly. The

acceptable method for each valve is listed in Table 1.

2.2.6 RPP-34123 has been prepared to evaluate use of a torque wrench adapter for

positioning valves with T-handles. The torque wrench adapter may be

utilized with a torque wrench to help position valves in Table 1 provided the

operating torque values are not exceeded. However, the maximum operating

torque values may be used when authorized by Engineering.

2.2.7 Batch documentation is not allowed at any time during the performance of

this procedure. Each item on a checklist or table shall be signed off as it is

completed by the performer.

2.2.8 NOTICE In Use Tags are installed on valves that are being used for a waste

transfer activity and shall prohibit the use of valves by others without

authorization from the Shift Manager, SPT OE or Retrieval OE. NOTICE In

Use Tags and their use are described further in Attachment 4.

2.2 General Information (Cont.)

2.2.9 The use of NOTICE In Use Tags are only required when establishing routes

for waste transfer activities.

2.2.10 For general information on T-handle valves with manual stops (stop pins on

valve body) see Attachment 1. This information is applicable to

Methods 1, 2, 3 and 4.

2.2.11 For general information on gear actuated valves see Attachment 2.

This information is applicable to Method 6.

3.0 PRECAUTIONS AND LIMITATIONS

3.1 Personnel Safety

3.1.1 Gloves are required to be worn when turning valve T-handles or hand-

wheels.

3.1.2 Hand-wheels are located approximately three feet above the cover blocks.

Due to the amount of stress to back, body, and arms, proper body alignment

and positioning are essential when turning the T-handle or hand-wheels.

3.1.3 In extreme cold weather, a potential for icy conditions exists when accessing

the top of the cover blocks/plates.

3.1.4 Weather conditions (heat, cold, ice, etc.) should be given careful attention

prior to performing this procedure.

3.1.5 Ensure footing is secure and solid in the valve area prior to operating the

valve T-handle or hand-wheel.

3.1.6 Compliance with the DOE-0336, Hanford Site Lockout/Tagout Procedure.

3.2 Equipment Safety

CAUTION - Applying excessive force after mechanical stops are engaged may result in

equipment damage.

3.3 Radiation and Contamination Control

3.3.1 When this procedure is worked in radiological areas, an approved

radiological work permit (RWP) is required. If radiological conditions or

work performed falls outside the scope of the RWP, all work activities must

be discontinued until a new or revised RWP has been issued in accordance

with TFC-ESHQ-RP_RWP-C-03.

4.0 PREREQUISITES

4.1 Special Tools, Equipment, and Supplies

The following supplies may be needed to perform this procedure:

Work gloves for valve T-handle or hand-wheel operation

Appropriate footwear for weather conditions

Torque wrench, sockets, etc. for checking transfer valve torque values

Other tools, equipment, and supplies as identified by Shift Manager/OE/FWS.

NOTICE In Use Tags.

4.2 Performance Documents

The Tank Farm Transfer Valve Tamper seal log may be needed to perform this

procedure.

4.3 Field Preparations

4.3.1 ENSURE two operators are ASSIGNED to perform the “Initial Valve

Positioning”. One to manipulate the valve and one to manage the documents.

4.3.2 OBTAIN Shift Manager/OE authorization to manipulate waste transfer

valves.

4.3.3 CONTACT IH for appropriate sample plan.

5.0 PROCEDURE

NOTE - Sections 5.1 through 5.8 may be performed in any logical order, in parallel, or may be

omitted, as applicable to the valves being positioned.

5.1 Transfer Valve Tamper Seals

5.1.1 IF tamper seal is installed, PERFORM the following;

5.1.1.1 IF transfer valve is in the required position, CONFIRM the

tamper seal number and the position matches the entry in the

Transfer Valve Tamper Seal Log.

a. DOCUMENT tamper seal number in the controlling work

document.

5.1.1.2 IF transfer valve is in the required position, but the tamper seal

number does not match the entry in the Transfer Valve Tamper

Seal Log:

a. VERIFY with Central Shift Manager(s), that removal of

tamper seal(s) does not affect transfer operations.

b. OBTAIN Central Shift Manager approval to remove

tamper seal(s).

c. REMOVE the tamper seal.

d. NOTIFY Central Shift Manager(s) or delegate to update

the Tank Farm Tamper Seal Log.

e. GO TO Section 5.2 to determine appropriate method.

5.1.1.3 IF transfer valve has an installed tamper seal and is not in the

required position:

a. VERIFY with Central Shift Manager(s), that removal of

tamper seal(s) does not affect transfer operations.

b. OBTAIN Central Shift Manager approval to remove

tamper seal(s).

c. REMOVE the tamper seal.

d. NOTIFY Central Shift Manager(s) or delegate to update

the Tank Farm Tamper Seal Log.

e. GO TO Section 5.2 to determine appropriate method.

5.1.2 IF a tamper seal is not installed, and the valve is under Lock and Tag in the

required position, as positioned per TO-020-610 and,

INDEPENDENTLY VERIFIED as indicated on TAF,

INSTALL the tamper seal per Attachment 3.

5.1.3 IF a tamper seal is not installed, VERIFY all of the following criteria are

met:

valve is under Administrative Lock

valve is in the required position

valve is positioned per TO-020-610 and independently verified as

documented on Administrative Lock Establishment/Removal Form

(A-6006-567).

5.1.3.1 INSTALL the tamper seal per Attachment 3.

5.2.1 DETERMINE the transfer valve method(s) as identified in Table 1.

5.2.2 POSITION valve in accordance with Section 5.3 through 5.8.

CAUTION

Applying excessive force after mechanical stops are engaged may result

in equipment damage.

5.3.1 IF use of any torqueing tools (e.g., torque wrench and adapter) is required,

ENSURE torque values provided in Table 1 are not exceeded.

5.3.2 IF tamper seal is installed, GOTO step 5.1.1.

5.3.3 IF placing the valve in the CLOSED position, PERFORM the following:

5.3.3.1 ROTATE the valve fully clockwise until mechanical stop is

engaged.

5.3.3.2 CONFIRM proper indicator disk alignment with routing lines on

cover blocks/plates.

5.3.3.3 IF directed by Shift Manager(s) or OE, INSTALL tamper seal

per Attachment 3 - Installation of Transfer Valve Tamper Seals.

5.3.3.4 IF establishing route for waste transfer activity INSTALL

5.3.4 IF placing the valve in the OPEN position, PERFORM the following:

5.3.4.1 IF valve is in the OPEN position, VERIFY position by rotating

valve clockwise THEN

ROTATE the valve fully counterclockwise until mechanical stop

is engaged.

5.3.4.2 IF valve is in the CLOSED position, ROTATE the valve fully

counterclockwise until mechanical stop is engaged.

5.3.4.3 CONFIRM proper indicator disk alignment with routing lines on

cover blocks/plates.

5.3.4.4 IF directed by Shift Manager(s) or OE, INSTALL tamper seal

per Attachment 3.

5.4.1 IF use of any torqueing tools (e.g., torque wrench and adapter) is required,

5.4.2 IF tamper seal is installed GOTO step 5.1.1.

5.4.3 IF placing valve AN06A-WT-V-103 to BLOCK AN06A-WT-V-102,

PERFORM the following:

5.4.3.1 IF valve is in the BLOCK AN06A-WT-V-102 position,

VERIFY position by rotating valve counterclockwise THEN

ROTATE the valve fully clockwise until mechanical stop is

engaged.

5.4.3.2 IF valve is in the BLOCK AN06A-WT-V-101 position,

ROTATE the valve fully clockwise until mechanical stop is

engaged.

5.4.3.3 CONFIRM proper indicator disk alignment with routing lines on

cover blocks/plates.

PERFORM the following:

AN06A-WT-V-103

5.5.1 IF use of any torqueing tools (e.g., torque wrench and adapter) is required,

ENSURE torque values provided in Table 1 are not exceeded.

5.5.2 IF tamper seal is installed GOTO step 5.1.1.

5.5.3 ROTATE the valve fully counterclockwise until mechanical stop is engaged.

5.5.4 IF placing valve in the full counterclockwise position, PERFORM the

following:

5.5.4.1 ROTATE valve clockwise, THEN

ROTATE the valve fully counterclockwise until mechanical stop

5.6 Method 4: Operate AW05A-WT-V-605

5.6.1 IF use of any torqueing tools (e.g., torque wrench and adapter) is required,

ENSURE torque values provided in Table 1 are not exceeded.

5.6.2 IF tamper seal is installed GOTO step 5.1.1.

5.6.3 IF placing valve AW05A-WT-V-605 to BLOCK TANK RETURN G,

PERFORM the following:

5.6.3.1 IF valve is in the BLOCK TANK RETURN G position,

VERIFY position by rotating valve counterclockwise, THEN

ROTATE the valve fully clockwise until mechanical stop is

engaged.

5.6.3.2 IF valve is in the BLOCK PUMP position, ROTATE the valve

fully clockwise until mechanical stop is engaged.

5.6.3.3 CONFIRM proper indicator disk alignment with routing lines on

cover blocks/plates.

5.6.3.4 IF directed by Shift Manager(s) or OE, INSTALL tamper seal

per Attachment 3.

5.6.3.5 IF establishing route for waste transfer activity INSTALL

5.6 Method 4: Operate AW05A-WT-V-605 (Cont.)

5.6.4 IF placing valve AW05A-WT-V-605 to BLOCK PUMP, PERFORM the

following:

5.6.4.1 IF valve is in the BLOCK PUMP position, VERIFY position by

rotating valve clockwise, THEN

ROTATE the valve fully counterclockwise until mechanical stop

is engaged.

5.7 Method 5: Operate three way T-Handle Valves with No Stops on Valve

Body

5.7.1 IF use of any torqueing tools (e.g., torque wrench and adapter) is required,

ENSURE torque values provided in Table 1 are not exceeded.

5.7.2 IF tamper seal is installed GOTO step 5.1.1.

5.7.3 CHECK indicator alignment with routing lines on cover blocks/plates.

5.7.4 IF valve is in the desired position, ROTATE valve either clockwise or

counterclockwise.

5.7.5 PLACE valve in position by rotating T-handle either clockwise or

counterclockwise, UNTIL valve is correctly positioned.

5.7.6 CONFIRM proper indicator disk alignment with routing lines on cover

blocks/plates.

5.7.7 IF directed by Shift Manager(s) or OE, INSTALL tamper seal per

Attachment 3.

5.7.8 IF establishing route for waste transfer activity INSTALL NOTICE In Use

5.8 Method 6: Operate Gear Actuated Valves

NOTE - When positioning valves, if an adjustable pointer is installed on the required

disk position, the pointer is the required position indicator. If no pointer is

installed, the disk notches are the required position indicator

(e.g., 241-AN-01A) or the narrow black stripe on decal is the required position

indicator. (e.g., 241-AW-B Valve Pit)

5.8.1 IF tamper seal is installed GOTO step 5.1.1.

5.8.2 VERIFY torque paint is present and intact on valve pointers to be used for

desired position.

5.8.3 ROTATE valve counterclockwise a minimum of 2 full turns on the

handwheel past the indicator used for the desired position or to the

mechanical stop.

NOTE - Valve is always positioned while rotating in the clockwise direction.

5.8.4 ROTATE valve clockwise until reference pointer aligns with the adjustable

pointer (if present) or notch/narrow black stripe if adjustable pointer is not

installed.

5.8.4.1 IF valve was rotated past the indicator during positioning,

GO TO Step 5.8.3.

5.8.5 CONFIRM proper indicator disk alignment with routing lines on cover

blocks/plates.

5.8.6 IF directed by Shift Manager(s) or OE, INSTALL tamper seal per

Attachment 3.

5.8.7 IF establishing route for waste transfer activity INSTALL NOTICE In Use

Attachment 1 - T-Handle Valves with Stops on Valve Body

When the valves utilize a valve stop (valve stop consists of a valve stop disk and a valve

stop pin) for valve positioning (e.g., fully closed or block flow), the valve stop disk is

rotated against the valve stop pin. Two-Way T-handle isolation valves with valve stops

and Three-Way T-handle valves with valve stops in the outboard positions (i.e., 0 and

180 degree positions for three position valves and 0 and 90 degree positions for two

position valves) are fully closed by rotating valve stop disk against valve stop pins.

Figure 1 below shows an example of three-way valves (top view) rotated to the stop pin

at the 0 and 180 degree positions. Figure 2 shows an example of a three-way valve in the

90 degree position, which may not be relied upon for DVI.

Attachment 2 - Gear Actuated Valves

Method 6: Align Valve Indicating Disk Markings on Gear-Actuator

The valve indicating disk is attached to the shaft of the gear actuator. The valve

indicating disk has markings (e.g., pointer, painted line, notch, decal) aligned to each of

the ports in the valve ball. The reference pointer is located on the gear actuator housing.

During installation, the reference pointer is adjusted so that when the reference pointer on

the gear actuator housing is lined up with the markings on the indicating disk or

adjustable pointer, the valve is in the designated position. The gear actuator is rotated to

align the reference pointer on the actuator housing with the marking on the indicating

disk or adjustable pointer for all valve positions. Refer to Figure 3 below for example of

configuration.

Installation of transfer valve tamper seals

[1] (Initial Positioner) WHEN valve(s) is/are positioned in accordance with the controlling

work document, INSTALL unique tamper seal on transfer valve and RECORD in the

controlling work document.

NOTE - Tank Farm Transfer Valve Tamper Seal Log signature is required to be a central shift

manager or may be delegated to a FWS or OE by the Central Shift Manager.

[2] REQUEST Central Shift Manager or delegate to PERFORM the following:

[2.1] RECORD tamper seal number(s) in the Tank Farm Transfer Valve Tamper Seal

Log for that facility.

[2.2] CONFIRM initial transfer valve positioning has been completed per work

documents.

[2.3] ENSURE the Tank Farm Tamper Seal Log contains the following information at

Attachment 4 - Notice In Use Tags

NOTICE In Use Tags are installed on valves that are being used for a waste transfer activity and

shall prohibit the use of valves by others without authorization from the Shift Manager, SPT OE

or Retrieval OE.

For Retrieval Operations where installation of individual tags is impractical the access gate to the

enclosed area may be posted instead. However, it is preferable to install individual tags on each

valve. Each access gate/point of the enclosed area shall be tagged with a NOTICE In Use Tag.

The use of NOTICE In Use Tags are only required when establishing routes for waste activities.

The tags are installed during the positioning of the valve. The tags are normally removed as

directed in the work document that controls the waste transfer activity or when authorized

by the Shift Manager, SPT OE or Retrieval OE.

Retrieval Operations and the Shift Production Team shall control the use of NOTICE In Use

Tags for their organization’s waste transfer activity. Each organization maintains similar but

unique tags.
