Cardiac Clinic Turns Heart Failure into

Heart Success L ike the 5.1 million Americans who

will develop heart failure in their lifetime, Edward MacDonald

simply wants to feel stronger. The 61-year-old’s symptoms—shortness of breath, heart palpitations, and swollen legs—left him fatigued and unable to complete everyday tasks. The condition had caused his heart muscles to become weak and stop pumping blood to the rest of his body as well as it should.

“It was difficult to walk up the stairs or clean the house,” MacDonald recalls. “I wanted to enjoy life.”

That is why MacDonald came to the Live Well Heart Failure Clinic at Summit Medical Group. Together a team of medical experts partner with patients to strengthen the heart muscles and improve tolerance for activity. Throughout a series of five sessions, physicians assess and diagnose the condition, develop an individualized treatment plan, manage medications, and educate patients about how to protect the heart from damage.

“Heart disease is the number one killer of both men and women,” says Seth Jawetz, MD, a cardiologist in the Heart Failure Clinic at Summit Medical Group. “More than half of these deaths can be prevented with proper precautions, lifestyle changes, and treatments. Our goal at the clinic is to help patients feel stronger

so that they can lead more normal lives.” The key to preventing heart failure,

Dr. Jawetz says, is to report any potential signs to a primary care physician immediately. The most common symptoms include: shortness of breath, chronic cough, swollen legs and feet, fatigue, light-headedness, chest pains, and palpitations.

During the initial visit, the Heart Failure Clinic team focuses on diagnos-ing the condition. A test that monitors the electrical activity of the heart, known as an electrocardiogram (ECG), and other specialized imaging scans that take pictures of the heart are usually ordered.

“Exercise stress tests, in which patients walk on a treadmill at different speeds, can also show how well the heart is handling its workload,” explains Dr. Jawetz.

Treatment usually includes medication and surgical intervention. Physicians also educate patients about lifestyle changes that can keep the heart healthy, such as eating nutritious foods and exercising regularly.

Since MacDonald has other health conditions, he says it is beneficial to be part of an integrated care facility. “Heart issues have a lot of ups and downs, but the physicians and nurses are always on top of it,” says MacDonald. “I know I am in the right place.”

However, it can become problem-atic if left unchecked for too long.

James Korman, PsyD, ACT, director of the Behavioral Health and Cognitive Therapy Center at Summit Medical Group, warns that it’s important to make the distinction between general

worry and true anxiety. It’s normal to feel anxious, nervous, or stressed during

situations such as being late for work, not having studied enough for an upcoming test, or when working on a big project at the office. This type of stress tends to resolve itself once the situation or event is over.

“Anxiety, on the other hand, has two primary symp-toms,” explains Dr. Korman. “One is worry, where people are repeatedly thinking or ruminating about things and the ways to solve it. The other is physical: You can experi-ence muscle tension, your heart races, you have shortness of breath, and you may experience sweating or dizziness, all of which are signs that your ‘fight or flight’ response has been activated.”

Stress and mild anxiety can be helpful in small, limited doses as it encourages you to find solutions for situations and complete tasks. But if it goes on for too long, it can begin to impede and hurt your ability to perform daily activities and can eventually become an anxiety disorder.

If you or someone you know is suffering from signifi-cant anxiety, the psychological stress can begin affecting your physiological and emotional well-being. If this starts to happen, Dr. Korman recommends helping the anxious person, and yourself, by guiding them into treatment. Specifically, he recommends Cognitive Behavioral Therapy (CBT).

In patients with anxiety, CBT helps them to understand that it’s not the events that cause anxiety, but how they think about these events. People with anxiety tend to

spend most of their time thinking about the future and worrying about it.

In cognitive behavioral therapy, you’re taught specific skills that help you think as realistically as possible so that you spend more time in the present than in the future. “Anxious and worried people also tend to only focus on information that supports their thinking, while ignoring information that suggests it isn’t true,” Dr. Korman notes. “This makes the worries seem much more realistic and prevents people from seeing things clearly.”

When it comes to medication, Dr. Korman explains that it can be very helpful to people with a long history of anxiety.

Combined with cognitive behavioral therapy, you can have a powerful treatment plan that’s effective at combating anxiety.

To get yourself started in the right direction with

handling anxiety, Dr. Korman has a simple way to help you manage and cope with stressors. He recommends you ask yourself one big question, “What is the worst thing that could happen in whatever you are dealing with, and if that were to take place, how would you handle it or manage it?” Realizing that in most situations the answer is, yes, that you will be able to manage or handle even the worst outcome, will go a long way toward making you feel less anxious.

You Can Help Prevent Diabetes

A simple blood test can reveal a condition called prediabetes. With prediabetes, your blood sugar levels are higher than normal but not yet high enough for a type 2 diabetes diagno-

sis. Prediabetes can also increase your risk for cardiovascular disease. According to a brand-new recommendation from the

United States Preventive Services Task Force (USPSTF), adults ages 40 to 70 who are overweight or obese should get a screening for prediabetes.

What You Can DoReceiving a prediabetes diagnosis is serious, but there are changes you can make to control your blood glucose levels. These changes include:• Losing weight. If you’re overweight, aim to lose around 7

percent of your total weight. • Exercise. Aim for 30 minutes of moderate-intensity physical

activity, such as brisk walking five days per week. • Diet. Cut back on your daily fat and calorie intake.

The Road AheadIf you have prediabetes, you should have your blood glucose levels checked every year. Work with your health care team and take an active role in lowering your blood glucose levels.
