Mission Statement:

Visiting Nurse Home &

Hospice is a not-for-profit,

independent home

healthcare and hospice

agency providing optimal

health and quality of life

for individuals and

families in Rhode Island

and Southeastern

Massachusetts.

Evelyn Marano, member of the VNH&H Board of

Directors from 2013 until her death in 2018, has been

named posthumously as the recipient of the Mary A.

Dwyer award for outstanding service to Visiting Nurse

Home & Hospice. Her husband, Fred Marano, will

accept the award in her name.

Evelyn moved to Aquidneck Island from New Jersey

in 1990 as a result of Fred’s career move with Pfizer

Pharmaceuticals to its facility in Fall River. Prior to

the move, she had been Director of Admissions at

Fairleigh Dickinson University, but when relocating

to Portsmouth with her toddler son, she put her vast

energies into volunteer ser-

vice rather than employment.

There was also considerable

time to devote to her life-long

passion for horses and hunt-

er-jumper equitation.

Evelyn’s volunteer commitments followed her passions. She became involved on

every committee and the Board at St. Michael’s Country Day School, where Geoffrey

was a student. She was on the Board of Directors and chaired fundraisers for the ALS

Society of Rhode Island after her mother passed from ALS. She taught CCD classes at

St. Barnabas Catholic Church, which was her parish. Most importantly for us, she be-

came a Hospice volunteer and event attendee at Visiting Nurse, where she was tapped

for Board service. Evelyn was active on the Marketing and Development Committee,

where she helped conceive our fundraising events and where she provided invaluable

assistance in the rebranding project. Even in illness, Evelyn remained active in support

of the Marketing Committee and stayed in constant contact.

Evelyn died in 2018 after a courageous battle with cancer. She is survived by her

husband Fred and her son Dr. Geoffrey Marano, an orthopedic surgery resident in

Washington DC. She is deeply missed by Visiting Nurse Home & Hospice.

Dear Fellow Visiting Nurse Home & Hospice

Community members,

Last year I wrote about all the changes that were

taking place to move the agency forward and stay

ahead of the competition. Our staff did a heroic

job implementing new systems and smoothly

bringing ‘West’ into our family. In 2018, our search

began for a new CEO as Candy announced her

retirement, having given us a few more years

than she expected. So, how to replace such an

inspirational and forward-thinking leader? That

was the question the board faced. We hired an

outside search firm and gave them multitudes of

criteria. They met with staff who also gave them a

plethora of concerns. Luckily, we all agreed on the

type of person needed, and we then created an

internal search committee of both board members

and staff. We sent the firm back to the drawing board

once or twice, never wavering on our needs and expectations. They had made

grand promises from the beginning, and they delivered! We are thrilled to have hired

Jennifer Fairbank, RN, who started in mid-December. Jennifer’s expertise in

healthcare, nursing, eldercare, strategic planning, outreach and business development

made her the perfect choice. Jennifer’s warm and inviting personality fits the culture of

our agency, and we hope she will be with us for many years to come.

On behalf of the board, a huge thank you to all staff, volunteers, donors, stakeholders

and fellow board members for always stepping up, supporting us and believing in the

incredible work Visiting Nurse Home & Hospice does!

Welcome to the Visiting Nurse Home & Hospice Annual Meeting.

If I were to coin a phrase to characterize 2018, it

would be “taking stock”. 2018 was an important

year, as it provided the opportunity to reflect

on all the initiatives that we had implemented

in the previous year and to prepare for a change

in leadership.

During those incredible few weeks when I was

able to spend time with outgoing CEO Candace

Sharkey, it became evident to me how passionate

she was about this organization. Now I understand

why. I am humbled and impressed with the team

here at Visiting Nurse Home & Hospice, not only

by their incredible work ethic, but by their passion

for perpetuating the mission, never losing sight of

each individual we have the privilege of caring for

each and every day. I feel so fortunate

to be a part of this organization that values our

staff, volunteers, and patients, strives to provide

the most exceptional care possible, and maintains

a keen focus on building the infrastructure we

need to grow and remain relevant.

As the only free standing not for profit home

healthcare and hospice agency in RI, our future

prosperity is dependent, in large part, on the support we receive from the growing

number of communities we serve. That support comes in many ways. It includes

financial support from grantors and foundations such as the vanBeuren Charitable

Foundation, RI Foundation, Champlin Foundation and others; civic support from

the many cities and towns where we have a presence; and individual contributions

from our wonderful group of donors. Volunteers in 2018 numbered in excess of 56

and provided a myriad of services throughout the Agency-Thank you!!

Last and certainly not least, our Board of Directors represents a remarkable and diverse

group of professionals who provide stewardship as they continuously seek the support

from additional community members who want to contribute their time and abilities. I

want to thank all of you who are currently involved and ask those of you who are not

yet connected to our organization to please consider how you might further support

our mission.

I am confident 2019 will hold challenges but am equally confident that we will

experience successes on many levels. While we continue to make our way in the

Homecare and Hospice arena it is reassuring to know we have a solid team and

an engaged Board of Directors to help chart the course.

Finance

The components of the Finance

Department include billing and

collections, payroll processing, cash

receipts and disbursements, and

financial reporting. Various subcategories

of these areas extend to third party payer

contracting and general compliance

with governmental programs such as

Medicare and Medicaid. As our operation

has become more data driven in recent

times, the Finance Department serves

as a constant internal point of contact for

specific requests for information relative

to the operations of our organization

AdministrationHuman Resources

In 2018, Visiting Nurse Home & Hospice had 295 active employees, and we continue to grow. Turnover remains between 8% and 10%, which is excellent for our industry. Thank you to all of our staff for your hard work, commitment, and invaluable contributions to this organization.

We continue to be privileged to have nursing student interns from Salve Regina University as well as Pharm D students from the University of Rhode Island and therapy assistant students from the Community College of Rhode Island and New England Institute of Technology.

The agency has a robust volunteer program that enlists the help of 17 office and 39 hospice volunteers.

Clinical Reimbursement

During this past year, the clinical reimbursement department has hired additional

staff and streamlined workflow processes in response to our increased workload

and extended operating hours that now include weekends and evenings.

By initiating a new biweekly reporting system, we are now able to identify the

current status of each record in a timely way so that we can clearly identify next steps,

maintain quality care, and expedite the time between admission and billing.

We will continue these efforts over the next year as we also work to reduce the

difference between our charge for service and the insurance company’s

approved payment.

Compliance

January 13, 2018 the Centers for Medicare

& Medicaid Services (CMS) implemented

new Conditions of Participation (CoPs).

These enhanced CoPs focused on:

• Safeguarding Patient Rights.

• Quality Assurance and Performance

Improvement (QAPI).

• Placing emphasis on Patient Care

and Outcomes.

• Interactions of skilled

professionals in meeting

patients’ needs.

The structure of the regulations has

changed. The following are now

separately established:

• Quality Assurance and Performance

Improvement (QAPI)

• Infection Prevention and Control

• Emergency Preparedness

Our Admission Patient Handbook was

expanded to assist both our patients and

clinicians with the tools they need to work

together toward improved outcomes.

Also, patient education enhancements

have been expanded for Infection Preven-

tion and Emergency Preparedness. Our

goal is to continue to look for opportu-

nities for improvement for our patients,

their caregivers and families.

Visiting Nurse Home & Hospice staff embraced an enormous change related to the

expansion of our service area to include most all communities in Rhode Island. The

patient census in the new service area doubled in 2018. Additional staff were hired

and trained to meet the needs of our expanded service area. Visiting Nurse Home &

Hospice opened an office in Warwick to meet the needs of patients. The office is fully

staffed with a scheduler and clinical supervisors for the Adult Nursing and Rehab

Teams. The staff was provided with additional training to enhance their utilization of

Homecare Homebase, streamlining the documentation of care and improving patient

outcomes. Visiting Nurse Home & Hospice maintained our 4 STAR Quality rating in

2018 by focusing on key outcome measures and patient satisfaction target measures.

Visiting Nurse Home & Hospice nurses continue to meet the needs of our patients

with the use of specialty teams, including cardio pulmonary, wound, diabetes,

intravenous, and chronic disease management. Clinicians at Visiting Nurse Home &

Hospice were trained on neurocognitive disorders (delirium, dementia and depres-

sion) and their implications for patients in the home care setting as part of agency

cross-training on behavioral health. Staff has focused on transitions of care and

reduction in avoidable re-hospitalizations. The agency utilizes a Transitional Care

model, which includes nurses visiting our patients in the hospital and nursing

home, to ensure a successful return home.

Maternal & Child Health

In Maternal and Child Health, homecare

visits are provided by pediatric RNs to

preterm, sick, or injured children to

enhance their health and well-being as

they transition from the hospital to the

home environment. Visits are also provid-

ed to families with children, birth to age

18, following illness, injury or premature

birth. Lactation consultants are available

to all new mothers in need of assistance

as they manage breastfeeding a newborn.

SIDS prevention and car seat safety are

reviewed in detail. Community support

information including mom’s groups,

play groups and long term visiting pro-

grams are referred if needed. The PQ9

Depression Scale is accessible for all

parents struggling with signs of anxiety or

depression, and resources specializing in

postpartum depression are available for

referral as needed. Staff have been trained

in Neonatal Abstinence Syndrome, and

appropriate supports for the parent and

child are available. Through the Depart-

ment of Health, staff have recently attend-

ed a Cultural Sensitivity training updated

in safe sleep environments and safety in

the home.

Palliative Care

Palliative care is for patients with serious

advanced illness who are not on hospice,

but who need care in their home to man-

age pain or symptoms. Our palliative care

program focuses on providing relief for

the symptoms and pain related to illness.

We understand that it can be challenging

dealing with illness during the later years

and the associated symptoms.

Our palliative care program has been well

received as a point of difference with re-

ferral sources and our patients. The bridge

style program for palliative and hospice

patients includes our interdisciplinary

team that ‘bridges’ patients who are still

actively seeking treatment for life limiting

illnesses and symptom management. If

the patient declines in health or makes

a choice to no longer seek treatment, a

patient can easily transition from palliative

care to hospice, keeping the same nurse

and team in place. Keeping the same

interdisciplinary team is often comforting

to the patient and family who has already

built a trusting relationship.

Rehabilitation

The concept of Hospice is to allow the whole family to experience quality of life together.

The Department continues to grow

and now has a dedicated team of more

than 80 clinicians with a wide variety

of specialties. Including PT, OT, PT/OT

assistants, Speech, and Dietitians, the

Rehab Department focuses on balance,

home safety, functional mobility, and

most importantly fall prevention using

a multidisciplinary approach. The West

team has settled into the Warwick office

and have become an integral part of the

agency and the Rehab team. The new

group of therapists came with many

years of home care expertise as well

as individual specialties that have al-

lowed the agency to offer high quality

rehabilitation to almost the entire state

of Rhode Island.

At this time, the Rehab department

completes almost 50% of the visits for

the agency. The Rehab Team has initiated

a Pre-Operative home safety evaluation

program to help clients with total joints

prepare for surgery as well as to help ease

surgical anxiety. Specialties of the Rehab

department include: Orthopedics, Stroke

and Neuro, Vestibular, Lymphedema,

LSVT “Big and Loud” for Parkinson’s

Disease, Cardiopulmonary, Diabetes

Management, Aphasia, and Dysphagia.

and at home

Hospice

The hospice team at Visiting Nurse Home

and Hospice continues to grow in both

staff servicing our patients as well as the

areas we serve. We continue to expand

the patients we serve in assisted living

facilities, skilled nursing homes, and

hospitals. The role of our hospice program

is provide physical, social, spiritual, and

emotional support during the last stages

of illness, during the dying process and

during bereavement. Our mission is to

help a family maintain their comfort and

dignity as life nears its end. We put our

patients wishes first by honoring,

respecting and listening to every patient

and family we serve.

Our Medical Director Dr. Rocco and

team physician Dr. Hackett guide our

interdisciplinary team to focus on meeting

the needs of the patient and their family.

The team is comprised of our hospice

director and clinical supervisor, as well as

a nurse practitioner, nurse case manager,

social workers, chaplain, home health

aides, and volunteers.

Patients rate Visiting Nurse Home and

Hospice as a top quality agency over

other hospice agencies many of the

quality measures as illustrated on

Medicare.gov hospice compare as well

as through Strategic Healthcare Programs

metrics. Our hospice team works hard

to exceed patient standards by continuing

to provide staff education and training

on a regular basis.

Community HealthThe Community Clinic Team (CCT) consists of nurses, volunteers and

other professionals within our interdisciplinary team..

Visiting Nurse Home & Hospice offers regular Adult Wellness Clinics in 9 communi-

ties, totaling 263 clinics at 31 sites. The nurses performed over 1700 blood pressures

checks and 150 glucose/cholesterol screenings, documenting and following up on

over 75 abnormal readings.

We held 75 flu clinics and gave over 1800 shots. Nearly 100 of these flu shots were

administered to the most vulnerable individuals who are unable to leave

their homes.

We continue to participate in multiple Health Fairs with hands on educational

demonstrations.

Telehealth ProgramDaily telemonitoring with the Honeywell

HomMed monitors measures patient

vital signs and patient’s weight from the

patient at home and transmits them to

nurses at our Portsmouth central mon-

itoring station. The program provides

support to the patients and to the clini-

cians, which creates a sense of security as

Visiting Nurse Home & Hospice is able to

assess our telehealth patients on a daily

basis. The telehealth program is an essen-

tial piece of the care provided to the car-

diac and respiratory patients at the agency

for further review and monitoring. .

By implementing daily telemonitoring as

the standard of care for cardiac and

respiratory patients, Visiting Nurse Home

& Hospice offers advanced technology

and high quality monitoring of our pa-

tients’ conditions. As a result of their daily

review of patients’ telehealth data, slight

changes in medical conditions are ad-

dressed in a timely manner, which often

prevents the need for trips to the emer-

gency room or acute care hospitalizations.

In 2018 the agency experienced a signifi-

cant amount of growth and change while

taking on territory as a statewide agency

rather than just Aquidneck Island and the

East Bay. With the addition of the new

territory, Visiting Nurse Home & Hospice

began to serve almost 1,000 patients per

month, an all time high. New growth

brought new team members to the busi-

ness development department, including

three Transitions of Care Specialist RNs

working with all of the major hospitals as

well as two liaisons in the East Bay area.

Becoming a strong partner to our referral

sources has been a major focus of our

business development efforts. Visiting

Nurse Home & Hospice Transition of

Care Specialists are involved in all aspects

of a patient’s discharge from a hospital or

skilled nursing facility. In addition, our

business development team has been

working closely with the Palliative and

Hospice team to reach out to families

early with critical conversations about

Business Development

plans of care when faced with critical

illnesses. The goal of these conversations

is to work closely with patients and their

families to help make the appropriate care

decisions and to develop a care plan that

is appropriate with the disease process.

By having these conversations early in the

planning, a patient can access Palliative

or Hospice services sooner, have a more

seamless transition of service, and get

the full benefit of Hospice when the

patient is ready.

This continues to be a strong focus of our

business development and transitions

of care effort moving into 2019, offering

Visiting Nurse Home & Hospice as a

resource for both patients, their families,

and referral sources in the community.

Our goal for 2019 is to continue to

maintain a strong presence in our prior

service area as we have for over 69 years

while expanding our brand of excellence

statewide to our expanded territory.

Communications

The agency’s public face is broadcast

throughout multiple platforms. We con-

sider our best and most effective tool for

communication outside of our “bubble”

to be our top notch field staff, who with

their badges, bags, and professionalism

are seen throughout the community and

maintain the high level of esteem the

Visiting Nurse Home & Hospice brand

has earned.

Fundraising

Charitable fundraising continues to be the

most important source for funding our

mission priorities: offering care to all in

our treatment areas, regardless of ability

to pay, and providing best practices (phar-

macist, dietitians, telemonitoring) even if

they are not covered by insurance.

Grants continue to be our primary

source of charitable funding. We are so

very grateful for support from the van

Beuren Charitable Foundation, Champlin

Foundation, the Prince Charitable Trust,

McBean Foundation, The Thomas A. Jr.

Family Foundation, and Rhode Island

Foundation. In 2018 we received $452,291

in grants under the capable stewardship

of grantwriter Karen Pucciarelli. We were

also recipients of a generous bequest from

the estate of Eleanor Silvia.

We had our most successful Annual Fund

campaign yet at the end of 2018, doubling

the amount raised from previous years.

The number of participants in the Annual

Fund also increased over previous years,

which was one of our goals.

We hosted fewer events in 2018 because

we pushed Chocolate Brunch into spring

of 2019 instead of November in 2018.

The Visiting Nurse/Edna Open Golf

tournament was a huge success in June,

surpassing other golf tournaments in

net proceeds for Hospice. O’Brien’s Pub

Salute to Summer and the Children’s

Christmas Party continue to be important

dates on the calendar for our community.
