2016 Annual Enrollment TimelineHere are the key dates you need to know:

Update Your Adoption Agreement for 2016Provide Your Eligibility Data (Census File) for Online Enrollment (if Elected)

Communicate Annual Enrollment to Your Employees

Employee Enrollment Period

• You will once again use the Adoption Agreement website to access your enrollment materials and 2016 rate sheet, and to review and make any updates to your Adoption Agreement for 2016.

• Remember to complete your Adoption Agreement in order to elect online enrollment and to display premiums on the enrollment site for your employees (if you offer online enrollment).

To log in to the Adoption Agreement website, visit www.mcdlicenseebenefits.com and click on “Visit the Adoption Agreement Site.” Once on the login page, enter your Login ID and the password you created. If you need help, please contact the Operator Support Line at Mercer at (866) 881-6646.

First-time users should follow the instructions online to create a password and login. You will need to know your Company Number located on your monthly invoices and the access codes provided to you via email. If you cannot find your access codes, please contact the Operator Support Line for help.

Take note: Consider the confidential and sensitive nature of the information available on the Adoption Agreement website before sharing your login information with others in your organization.

• If you are new to the online enrollment, the system will need your eligibility data in order to know which of your employees should be able to access the site and enroll.

• If you already participate in online enrollment, this is the time to make sure your census file is updated for 2016. Whether or not you elect to display your premiums online this fall, there are some required updates to the census file. See page 3 to learn more.

• For instructions on creating and maintaining your census file, including how to provide your files to Mercer, please visit the Online Enrollment Resource page on www.mcdlicenseebenefits.com.

• You can use the Participant Communication Tool available through the Adoption Agreement website to create custom communications for your employees that show their benefits options and rates for 2016.

• In addition, templates for communicating online enrollment with your eligible employees will be available by late September. These templates will be posted on the Online Enrollment Resource page available on www.mcdlicenseebenefits.com.

• Additional communication templates are available on the Health Care Reform site on AccessMcD.

• If you offer online enrollment, your employees will go online to enroll in their benefits you can use the enhanced Administrator/Reporting Site to see if your employees have gone online to enroll or decline coverage.

• If you do not offer online enrollment, you should use the Enrollment/Change Form to make changes to coverage for individual employees or to add new employees. Make sure you use the updated Enrollment/Change Form posted on www.mcdlicenseebenefits.com.

• Please note: Current participants will continue to use their existing ID cards. New participants who enroll by December 4 will be guaranteed ID cards by January 1, 2016. Those who enroll after December 4 will receive their ID cards as close to January 1, 2016, as possible.

n The Plan is continually working to enhance the online enrollment experience for you and your employees. New for this fall’s Annual Enrollment, owner/operators will have the ability to display employee premiums online. To learn how to do this, see page 3.

The Plan is focused on supporting owner/operators in administering the site, as well as creating a simple, easy-to-use platform for your employees.

• This fall, we’re introducing an updated Administrator/Reporting Site for owner/operators, making it easier for you to enroll on behalf of your employees and access reporting about which of your employees have enrolled and the options they selected. A user guide and video demonstrating how to use the site is now available when you visit the Online Enrollment Resource page on www.mcdlicenseebenefits.com.

• We’re also updating the online enrollment site for your employees so it’s even easier to navigate and use. The web address for the site remains www.MyMcDBenefits.com; the enrollment website also includes a video for employees demonstrating how to use the site.

n The Platinum medical plan will no longer be offered.

• If you do not go online and update your Adoption Agreement for 2016 your employees who are enrolled in the Platinum plan (and their covered family members) will automatically default to the Gold Plus option.

• If you update your Adoption Agreement for 2016 but do not elect to offer the Gold Plus plan, your employees who are enrolled in the Platinum plan and their covered family members will automatically have their coverage terminated unless they elect another medical plan during this fall’s Annual Enrollment. In this case, these employees will receive a letter from the Plan informing them that they must actively enroll this fall and select a new coverage option for 2016 or else not have coverage next year.

n If you do not yet offer online enrollment, you still have the opportunity to sign up.

If you do not yet offer online enrollment for your employees, you can sign up by electing it on your Adoption Agreement this fall. Benefits of online enrollment include:

• No more paper forms – Employees will use the simple online enrollment system to enroll and update benefits. You also have the ability to enroll on behalf of your employees.

• Faster benefits administration – A faster process for managing your employees’ Plan enrollment information as they complete the process.

• Enhanced employer support – Access to an Administrator/Reporting Site that provides a variety of benefits reports that can help you track your employees’ enrollment information.

As you review and update your Adoption Agreement for 2016, remember:

Quick tips for paper enrollment

If you’re using paper enrollment for your employees this fall, please be sure to…

• Use the new 2016 Enrollment/Change Form available on www.mcdlicenseebenefits.com. Before sending to Mercer, review the forms to make sure information is complete and legible.

• Obtain the required Proof of Age documentation for all new enrollments. For your convenience, an I-9 Form is available online at www.mcdlicenseebenefits.com.

• Submit completed forms with any Proof of Age documentation to Mercer via mail, email or fax. If emailing or faxing, please keep a copy for your records, and note that you do not need to also mail a hard copy.

New this fall, the Plan has enhanced the online enrollment site with the ability for Owner/Operators to display employees’ benefit costs. If you offer online enrollment, you can decide if you want your employees to see their personalized monthly premium costs on the site. This new feature creates a better enrollment experience for your employees, helping them make more informed decisions about their elections. It also supports an easier communication process for you as an employer. While using this new feature is entirely optional, the Plan encourages you to consider displaying premiums online this fall!

As you complete your Adoption Agreement online for 2016, you will be asked if you would like to display premiums online this fall. Please indicate “yes” or “no.”

If you would like to display premiums on the enrollment site, you will need to do the following:

Important reminder: Dual year processingThere may be some employees who become eligible for benefits for the remainder of 2015, based on your eligibility requirements. As usual, the newly-eligible employees will have the opportunity to sign up for benefits for the remainder of 2015. These employees may also be eligible for 2016 enrollment and should go through the Annual Enrollment process with all other employees. This is why it’s important to keep your census files updated for the remainder of 2015, as well as for 2016 Annual Enrollment.

Online enrollment: How to display premiums online for your employees

Regardless of whether you would like to display premiums online for your employees, you will still need to review and update your eligibility census file as follows:

• Review the revised layout instructions for the 2016 census file, which are available on www.mcdlicenseebenefits.com by clicking on “Online Enrollment Resources.” (This does not apply if you provide your data manually via the Adoption Agreement website.)

• Work with your payroll provider or internal team to update your census file for 2016 Annual Enrollment. As of October 8, you must update your file with a new field for hourly pay rate even if you do not populate the field with data.

• Be sure to carefully review your data and remove any terminated employees from the 2016 census file.

First, select the “yes” option on your Adoption Agreement for 2016.

Next, complete the new Contribution Strategy page on the Adoption

Agreement website to indicate how much your employees pay for

their benefits. A new Adoption Agreement User Guide is available to

help you; find it on the “Online Enrollment Resource” page on

www.mcdlicenseebenefits.com.

Take note: The Adoption Agreement website is open from September 1

through October 1, 2015. During this time, you can visit the site and update

your contribution/subsidy strategy as many times as you would like. If

you need to change your contribution/subsidy strategy after the Adoption

Agreement period has closed, please contact your Account Coordinator at

Mercer through the Operator Support Line.

Finally, you will need to add an hourly rate of pay field to the

eligibility census file you send to Mercer, the administrator of the

employee enrollment website. Keep reading to learn more.
