Please select the correct answer for all four questions and hit Submit. 4 Create your permanent user ID and password Enter a permanent user ID. User IDs are case-sensitive and must be between six and 19 characters. User IDs cannot contain spaces or special characters. Enter a permanent password. Passwords are case-sensitive and must contain eight to 17 characters. You must use at least six alpha characters and two numeric characters when creating your password. Click the Submit button. Enrollment InstructionsPremier Personal Online Banking

1 Visit www.ColumbiaBank.com and select “Enroll in Premier Personal Online Banking” from the Online Banking box.You will automatically be taken to the enrollment form once you click on Enroll in Premier Personal Online Banking.

5 Set security dataComplete all information on the Set Security Data screen to establish your security profile for Premier Personal Online Banking.

Please be sure to complete all sections including your contact information and three challenge questions with answers.

Select the Submit button.

6 Accept Online Banking Terms and ConditionsRead through the Online Banking Terms and Conditions and select Accept to continue.

7 Choose to enroll in Mobile BankingReceive instant access to your accounts from your cell phone, or other mobile device with Columbia Bank’s free Personal Mobile Banking*.

Personal Mobile Banking provides you with account access through any combination of mobile access types.

• Web-Based – Access Personal Mobile Banking through the browser on your smartphone or mobile device

• SMS Text-Based – Request account details using short text codes from virtually any cell phone

• App Based – Download the Columbia Bank App for your iPhone or Android phone to access your accounts, pay bills, and find a branch or ATM

*Your mobile carrier’s text messaging and web access charges may apply.

• Select to Enroll Now and proceed to the next step.

• Select Ask Me Later to skip to step 13. You may enroll in Mobile Banking at any time by selecting the Options tab from within Premier Personal Online Banking.

• Select Decline to cancel the offer to enroll in Mobile Banking and skip to step 13. You may enroll in Mobile Banking at any time by selecting the Options tab from within Premier Personal Online Banking.

8 Select Mobile Banking terms and conditionsPlease read through the terms and conditions for Mobile Banking and select the box that says I accept these Terms and Conditions.

9 Select your Mobile Banking servicesPersonal Mobile Banking provides you with account access through any combination of mobile access types.

Select one or more of the Mobile Banking services and click Continue.

10 Mobile account selection and configuration Select the time zone you’d prefer.

Select the accounts you wish to access through Mobile Banking. You may also create nicknames for your accounts for easier use when using SMS Text Banking.

Select the Continue button.

11 Enter your mobile phone number Once you enter your mobile phone number the system will send an activation code to your mobile phone. You will need this code in the next step to complete your mobile enrollment.

Select the Continue button after entering your mobile phone number.

12 Enter your activation code Enter the access code that was sent to your mobile phone number to activate Mobile Banking.

Select the Activate button.

Congratulations – You are now enrolled in Mobile Banking!To continue your first-time login to Premier Personal Online Banking please close out of the Mobile Banking main menu and proceed onto eStatement enrollment.

13 Choose to enroll in eStatementsTo enroll in eStatements, please follow these steps:

1. Select the accounts for which you would like to convert to eStatements

2. Confirm your email address by entering it in the empty box

3. Click on the eStatement Disclosure link to view the disclosure. You must click on this link in order to accept the terms and conditions before you can proceed

4. After you’ve viewed the disclosure, click on the box accepting the terms and conditions

5. Select Accept

If you do not wish to enroll in eStatements at this time select Ask Me Later to proceed to the Premier Personal Online Banking dashboard.

If you do not wish to enroll in eStatements select Decline to cancel the offer to enroll in eStatements and proceed to the Premier Personal Online Banking dashboard. You may always sign up for eStatements at any time by selecting the Options tab in Premier Personal Online Banking.

Welcome to Premier Personal Online Banking!You have completed the enrollment process.

If you’d like to enroll in Personal Bill Pay, simply select the Bill Pay tab for easy enrollment.

For future access to Premier Personal Online Banking simply visit www.ColumbiaBank.com and enter the user ID you created during the enrollment process and click the green Login button.
