PERSONALISATION Means starting with each person as an

individual with strengths, preferences and aspirations - or hopes and dreams!

Means putting the person at the centre

of the process of identifying their needs

and making choices about how and when

they are supported to live their lives.

PERSONALISATIONACCESS - we need places to meet others, learn with others and act together. The community needs to be accessible and welcoming. CONTROL - is central to our ability to be effective Citizens, and to shape a life around ourselves that makes sense of who we are.

It is about building a society where everyone can find a way of being that makes sense.

OUTCOMESAn outcome is something you want to achieve in your life. It could be something new - such as making new friends, getting a job, taking part in your community. Orsomething you want to keep - such as staying safe or staying healthy.

SELF DIRECTED SUPPORT - now an ACT of Parliament; a 10 year strategy!The person shapes and controls their own support arrangements, and has as much control as they wish over their own budget!Local Authorities have a duty to offer 4 different options to each person.They also have to make sure people have the right advice and support to help them make the best choice for them.

THE 4 OPTIONS1. DIRECT PAYMENT - money is given to the

person to arrange their own support. You can hire your own staff (Personal Assistant) or give the money to a provider organisation.

2. SELECTION OF SUPPORT BY THE PERSON - person chooses the support they want and the LA manages this under the person’s direction, and hands the money over to the provider of their choice.

THE 4 OPTIONS3. SELECTION OF SUPPORT FOR THE PERSON BY THE LOCAL AUTHORITY - the LA arranges for the support to be provided to the person, and sorts out paying for the support for them.

Each Local Authority is doing this slightly differently BUT the usual steps are: 1. Contact your local Social Work Department2. Assessment of needs - are you eligible for support?3. Outcomes focused support planning - what do you want

from your support?4. Discuss the 4 options + decide the one you want.5. Support plan agreed6. Support provided

7. Regular reviews.

Some examples: Buying membership of the local gym so you

can get and stay healthy and fit. Getting a wooden floor in your living room

so you don’t keep falling over the carpet. Having a holiday in Blackpool or London

instead of going into “respite”. Buying a season ticket for your local football

team and one for a friend so s/he can chum you and give the person who ‘cares’ for you a break.
