Objective1. To determine/ascertain common failure modes on FPF/FPSOs which lead to HCRs.

2. To determine/ascertain the reasons behind why FPF/FPSOs incur disproportionate

HCRs.

Deliverables1. To provide guidance to industry on areas of focus for both brownfield units and new-

build FPF/FPSOs in order to eliminate HCRs.

G-REDL Update

• AAIB report published 24 November.

• A special HSSG meeting was held 5 December to discuss

recommendations, following which a newsletter was issued.

• Six of the 17 recommendations appeared in the interim

reports released in 2009 and were addressed immediately.

• 11 new recommendations are largely technical; responsibility

for implementation oversight is with CAA and HSSG will

monitor.

• CAA were issued 2 recommendations – accepted one and

deferred other to EASA

Progress Highlights• Pilot survey is now complete

– 830 returns

• Practical guide available in Draft form

• Close out workshop held with pilot participants to capture their input

– Input very positive and lessons learned will be incorporated in practical guide

– Participants expressed a desire to attend a follow up work shop

– Subsequent feedback suggests that the survey provides data not available from internal

surveys

• Subsea community will run follow up pilot in Q1 2012. Data will be

integrated with existing data already gathered.

• Focus in 1st half of ‘12 will be the successful launch and roll out of the

engagement survey tool

• Work Group strongly recommends we proceed and is now looking for SCLT

endorsement of the “go forward” plan

Survey Feedback – Top bits of feedback on the Survey

and/or Guide

Positives:

• Main positive factor is that the survey is independent of all other Company surveys;

• The Survey is short, concise and simple to use;

• The Pilot has highlighted the need and usefulness in utilising the ESR’s in the Survey process;

Opportunities for improvement:Opportunities for improvement:

• The Survey was found to be very simple to use, however we need to ensure that there is a

more robust system for the collection and collating of paper copies;

• There was an issue regarding the tracking of Survey responses. This probably has more to do

with the anticipated feedback. There was an expectation amongst some that the location

issues would be reported back on a weekly basis;

• Some locations found that offering small incentives to complete helped with the uptake (i.e.

work break times dedicated to completing the survey; cokes and mars bars issued when

copies were handed in, etc).

Survey Feedback - Tips for anyone who is attempting

to run the survey for the first time

• Ensure thorough preparation has been undertaken

• Test the web link, etc

• Focal Points need to be prepared in advance of the Survey roll out;

• Ensure that they are aware of the difference between this Survey and the multitude of other

Survey’s that are in circulation. Also ensure that all participants are fully aware that the Survey

maintains complete anonymity.

• Need for ESR involvement from the outset and for each location to have clear and •

dedicated Focal Points;

• Before anybody completes the Survey, ensure that they are aware of the purpose

and objectives.

Survey Feedback - Tips on how best to launch the

Survey / Guide

• Decide who will be rolling out the Survey on an Organisational basis. Support

needs to come from the top all the way through the Organisation;

• The message needs to highlight that the Survey / Exercise is ‘solutions based’ and

not just another Survey;

• Set expectations from the outset – What do we want to achieve and clarify the

accountabilities;

• Plan the best time to undertake the Survey – outwith the Survey and Shutdown

season;

• Advertise and market in advance so people know that it is coming;

• Provide Workshop’s for the ESR’s and Focal Points in advance of the roll out.

Survey Feedback – Comments

• A very valuable survey, and going forward my personal opinion is that it will bring what is

required to the industry

• the results are encouraging, as always more to do but the quality and readability are really

good

• We both have enjoyed participating and would like to continue to do so
