Demolition of the existing two story brick building of Approx. 7250 SQ FT area including basement level, foundation & footing.

Demolition of existing asphalt area of Approx. 35000 SQ FT including removal of 3 existing basketball pole, foundation & net.

Demolition of existing concrete side walks of Approx. 3920 SQ FT area. Demolition of existing SOD area of Approx. 37300 SQ FT including salvage & relocate existing sign,

existing shrub & its roots, existing catch basin & dispose off site, existing chain link fence, posts & foundation, existing benches, existing manhole, its inlet & outlet pipes & to be filled with compacted sand, existing hydro pole, foundation pier & guy wires.

Demolitions of existing play area of Approx. 1100 SQ FT including playground equipment & benches. Load and Transport demolition debris off site from scope of work. Supply labor, equipment and material to complete scope of work.

SCOPE OF WORK: EXCAVATION $________ SCHEDULE B

Excavation for proposed building of Approx. 2670 SQ FT @ average of 4.9LF Depth to U/S footing. Supply of all Ontario One Locates within job envelope. Stockpile excavated earth material on site for reusable back-fill. Load and transport remainder of extra-excavated earth material off site. Supply all labor and equipment for completing scope of work.

SCOPE OF WORK: FORMWORK $_________ SCHEDULE C

Supply and Install 1050 Linear Feet footings & Concrete block pier as per foundation details. Supply and Install 1050 Linear Feet foundation wall @ 4.8FT height 14in thick. Supply and Install rebar and dowels as per structural drawings. Supply and Install waterproofing system. (Liquid rubber, Delta Membrane, 4inch Weeping tile, Sump

Pump) Supply all labor and equipment and material for completing scope of work.

SCOPE OF WORK: BACK-FILL $________ SCHEDULE D Back-fill exterior wall foundations to existing grade. Back-fill of foundation walls using stockpiled excavated earth material.

SCOPE OF WORK: SLAB $_________ SCHEDULE E Supply and Install 4” compacted granular base for Approx. 18630 SQ FT for grade slab. Supply and Install 125 mm concrete grade slab. of Approx. 18630 SQ FT.

NOTES:

Note 1: Work is to be continuous and completed in one mobilization. Additional mobilization is $1850. This includes (1) removal (1) delivery of machine. Any delays or work stoppages in the work described above, caused by no fault of ____________, will be back charged at a rate of $500.00/crew hour (partial hour delays will be accumulated daily). All payments must be paid in full to validate warranty and drawings can be released.

GENERAL EXCLUSIONS Any work not shown as a scope, drawings or agreed upon before commencing of work. Supply of all permits, road closures, road housekeeping, air rights etc. Placement of hording or fencing unless listed on above scope of work. ____________ construction will not be responsible for delays in completion of work arising from
