WORKING TELEPHONE NUMBERS
For your child’s safety, we need to have on record in the office a working telephone number or an
alternate telephone number where the parent or caregiver can be reached during the school day. If your
telephone number changes, notify the office at 542-4390 immediately. We will inform your child’s
teacher and other staff members that interact with your child during the school day.
CHANGE OF ADDRESS AND / OR TELEPHONE
If at any time during the school year a change occurs in your home address or telephone number, please
advise the school immediately. Address verification must be in the parent or guardian’s name and sent to
the main office. A utility bill, lease or mortgage agreement, placement letter, check stub, monthly
medical card or other documentation can be used to verify your new address.
ATTENDANCE PROCEDURES
Daily attendance is essential if all students are to receive the maximum
benefit from schooling. We want all students to be successful. Research
shows that the most successful students are those with good attendance.

Absence: When a student is absent from school, the parent or guardian should call the school (542-
4390) between 8:00 and 8:55 a.m. and state the reason for the absence.
Upon returning to school, the student should present written documentation explaining the reason for the
absence. The note should include date(s) of absence, students’ full name, classroom number and the
reason for absence with the parent/guardian’s signature. All notes must be received within one week of
the student’s return to school.
Absences may be excused because of illness, a death in the family and previously arranged absences
approved by the principal. Any other absences are considered unexcused. You may discuss specific
circumstances with the principal.
Tardy: Students who arrive after 8:55 AM are considered tardy. They must report to the office for an
admission slip before reporting to class.
Early Dismissal: For the safety of our children, there will be no early dismissal after 3:00 pm. We are
sorry for any inconvenience.
Notes explaining the reason for early dismissal are to be given to the teacher upon arrival to school and
will be confirmed by a phone call to parents/guardians. No student will be allowed to leave the building
at the request of anyone other than a parent or guardian without prior notification.
We encourage every student to participate in a full day of activity. But, if it is necessary to leave school
early, parents or authorized persons must come into the main office to sign the child out. A photo ID
may be requested to verify identification. Students will be released only to persons listed on the
Authorization for Pick-Up Form.
A note in your handwriting is the best way to communicate authorizing a one-time pick-up and the
office will confirm the dismissal. All notes are to be given to homeroom teachers upon arrival at school.
Since parents or authorized persons must come to the office and sign out a student who is leaving early,
we discourage any calls for this purpose, as it is difficult to identify the caller.
If you will need an early dismissal for your student every day at a certain time, please complete an Early
Dismissal Form in the main office. This will help us understand the reason for early dismissal each
day. A copy will also be sent to the teacher so that they will know when to expect you each day.
Students may not leave the campus without being signed out by authorized persons. Classroom teachers
will be notified by office staff to release students for dismissal. Under no circumstance is anyone
allowed to go directly to a classroom to remove a child from school.
LUNCH PROCEDURES
There is no cost to any student for breakfast or lunch.
If your child is on a “special” diet, has dietary restrictions, food allergies or
special utensils should be used, please inform your child’s teacher immediately
and complete and return the specific forms included in the Parent Packet.

REPORTING STUDENT PROGRESS TO PARENTS
Students with IEP’s receive a quarterly progress report each nine (9) weeks. The quarterly progress
report includes student progress information from your child’s teacher and each therapist(s) listed in
your child’s IEP. In addition, ECIP teachers send home weekly notes to parents.
Parent conferences are typically scheduled twice a school year. Please contact the teacher or the
principal at any time.
CLASSROOM VISITATION
Parents, legal guardians and caregivers are always welcome at Rosa Parks ELC.
However the classroom staff will not be able to answer questions during school
hours because that would interrupt the classroom instruction. Please report to
the office and sign in before going to any classroom to avoid instructional
interruptions.
If you would like to discuss a specific issue with your child’s teacher, it is best
for you to call ahead and schedule an appointment.
SCHOOL RECORDS
In order to maintain accurate school records, please notify us of any changes that might occur during the
year. Changes to report are changes in address, phone number, father or mother’s place of employment,
guardianship, etc. We would appreciate a week’s notice if a family is moving and/or withdrawing their
children from school. When requesting student records, please allow for a two day turnaround.
BAD WEATHER / SCHOOL CLOSING
Rosa Parks Early Learning Center is within the Dayton Public School district.
When announcements are made regarding school closing, be sure to listen for
Dayton Public Schools.
TRANSPORTATION
Only preschool students with IEPs and some Kindergarten students are transported by Dayton
Public Schools’ buses. Transportation is dependent upon the distance
from the child’s home to the school.

This is the telephone number to call with questions regarding the school
bus. The Transportation Office at the Dayton City Compound can make
radio contact with your driver. If you are moving, be sure to advise them

at least three days in advance; then your child can be assigned to a new bus route without missing any
school days.
* Please Note: If your child is not picked up for three consecutive school days,
bus pickup could stop until you notify the Transportation Department to resume
bus service.
SUPPLIES
School supply lists are located on the Rosa Parks website for your convenience.
Additionally, each teacher will notify parents/caregivers of the supplies that are
requested in the classroom.
Students with IEPs and all children will need to have a complete change of clothing at school. Students
who are not toilet trained will need diapers, wipes and at least two additional changes of clothing. Also,
please label backpacks, lunch boxes, and clothing with your child’s name.
Students riding the school bus to and from school are expected to demonstrate appropriate behavior.
Students who fail to comply with basic safety regulations can be denied the privilege of bus
transportation. Bus riders must observe the following rules:
1. Most students use a safety belt or other safety device restraint.
2. Wheelchair / safety seats must meet district safety standards.
3. Students must ride their assigned bus and load/unload at regular designated stops, unless parents
have arranged with the Department of Transportation and notified the school in writing.
4. Refrain from eating and littering on the bus.
5. Keep noise to a minimum. Students may talk quietly to the person beside them.
6. Be absolutely quiet at places of danger as specified by the driver.
7. Parent or designee must be available to load students on and receive students off the bus at the
designated time. Be prepared 10 minutes before the scheduled arrival time and 10 minutes
after the scheduled departure time.

Student Health Guidelines
Every day counts toward academic success. Students should rarely miss
school. If your child has a chronic health condition, please contact the
school nurse. The nurse may be able to provide help and resources so that
your child can attend school regularly.
Please complete an Emergency Medical Authorization form each year.
Your child’s immunizations must be kept up-to-date according to the
requirements of the Ohio Department of Health. Provide the school nurse
with an updated immunization record whenever your child receives immunizations. Children
whose immunization records are not “complete” will receive an exclusion notice and have two
weeks to comply.
Unless a routine medication is time-specific, it usually can be given at home. We must have
orders from your child’s healthcare provider for any medication to be given at school. This
includes over-the-counter medication. Students are not allowed to self-carry medication without
a healthcare provider order.
Minor injuries will be treated in the health office and the student will return to class. Parents or
designated adults will be contacted for more serious injuries and illnesses. It is best practice for
a parent or designated adult to pick up their child if the school determines that the student should
not remain at school due to illness or injury. We will call 9-1-1 and the parent/guardian for
emergency situations.
Health screening is done for students in preschool, kindergarten, first, third, fifth, seventh and
ninth grades. For most grades this includes vision, hearing, and dental. Scoliosis screening is
done in sixth grade. If you do not want your child to participate in the health screenings, you
must send a written note to the school nurse. Parents will be informed in writing if their child
needs to have a medical evaluation for a possible problem.
Students who are suspected of having a contagious disease will be excluded by the school nurse.
Please do not hesitate to contact the school nurse for question or concerns.
Are we able to reach you in case of emergency? Please be
sure the school has your most current phone numbers and correct
contact information.

STUDENT HEALTH POLICIES
IMMUNIZATIONS
Each student is to be immunized prior to the start of the school year for new
enrollees, or as soon as possible in the case of a student transferring from another
school.
Any student who does not comply with the State of Ohio immunization requirement
shall not be permitted to continue in school until he or she shall so comply. The cost
of the immunization shall be the responsibility of the parent/guardian/caregiver.
EXCLUSION FOR HEALTH REASONS
Policies regarding the exclusions of students for health reasons: (Only the school nurse or designee can
determine if a student will be sent home during the school day/year).
Students suspected by the nurse of carrying a contagious disease will be excluded.
Students with a rash must be excluded until the disappearance of the rash or any other symptoms.
(Unless a physician directs readmission of the student).
No student will be sent home until a parent/guardian or designee has been contacted.
If a child is out of school for 3 days or more, a doctor’s note will be required to return to school.
YEARLY PRESCHOOL PHYSICAL
It is state law that each preschool student have a physical and dental examination every year. Your
physician can fax this information directly to the school at 542-4391. We appreciate your cooperation in
this important manner.
STUDENT MEDICATIONS AND PRESCRIPTIONS
Oral medications for a student brought from home will be administered only when
accompanied with a written prescription from the physician. A request form can be
obtained from the office and completed by both physician and parents.

The Children with Medical Handicaps Program (BCMH) is a health care program in the Ohio
Department of Health (ODH). BCMH links families of children with special health care needs to a
network of quality providers and helps families obtain payment for the services their children need.
FAMILY HOME SERVICES PROGRAM

This program is part of the services provided by the Montgomery County Board of Mental Retardation
and Developmental Disabilities.
Any family residing in Montgomery County who has a family member with a developmental disability
may qualify for services. County Board enrollees are automatically eligible for Family Resources.
Services include: Respite care, both in the home and out-of-home services; training, counseling, and
education for family members; assistance with special diets; purchase or lease of adaptive equipment;
assistance with modifications to the home. The Montgomery County Family Resource Services
Program provides financial reimbursement, based on a sliding fee scale, to eligible families.

THE WIC PROGRAM

The program provides nutritional counsel to Montgomery County women meeting guidelines and free
supplemental foods for infants and children up to age 5 who are at nutritional risk.
