84.3(4) An application under subsection (3) shall be on a form provided by the Supervisor.

84.3(5) A declaration under paragraph 3i shall be signed by the individual or officer referred to in paragraph (3)(a) or subparagraph 3bii

or (c)(ii), as the case may be.

84.351 Before applying to register under section 84.3, a third party shall appoint a chief financial officer.
