Most RCFA processes focus on identifying causes and
deliver categorized suggestions for remedial actions.
RCFA Pro offers a quantum leap in capability and
defensibility by first targeting the real business need of
preventing a reoccurrence of the consequence and
secondly by having the built in capability of determining
actual remedial actions for both human and equipment
factors, including maintenance task and intervals. RCFA
Pro also has the capability of performing the HAZOP for
suggested modifications such that undesired
consequences will not occur as a result of the
implementation.

RCFA Pro is based on the fundamental principles
of Reliability Centered Maintenance, which is a best
practice process itself. It is a facilitated process with
input from the people closest to the assets. RCFA
Pro has the capability of identifying both the actual
causes and the potential causes resulting in the
overall prevention of the consequence. This is a
best practice criterion for RCFA processes. The
RCFA pro process has been in use in high-risk
industrial applications for over 10 years. The
process is auditable and defensible.

Consequences, and most importantly serious
consequences, should be prevented due to all known
causes, not just the ones that occurred on the day of
the failure. RCFA Pro provides a sound basis for due
diligence in consequence prevention by developing
remedial actions for both actual and potential
causes.

processes to finalize remedial actions. RCFA Pro,
through the RCM2 logic, determines actual
