1.1 INTRODUCTION

1.1.2 Background and Objectives of the Study

Punjab Small Industries Corporation (PSIC) has been working for the development of small-scale

industry and the handicrafts sector in various parts of the Punjab. PSIC intended to carry out a

diagnostic study of Industrial and Handicrafts Clusters in Gujranwala, Gujrat, Mandi Bahauddin and

Sialkot districts of the Gujranwala Division to prepare cluster profiles, diagnosing cluster issues and

making policy recommendations for cluster based development with special focus on export oriented

clusters.

The objective of the consultancy is to prepare a roadmap and policy guidelines for future interventions

in the area to facilitate growth through cluster based industrial development. The major objectives of

the study are:

Profiling and Mapping of Clusters in Punjab

Identification of Issues Confronting these Clusters

Policy Planning of Interventions for Uplift of the Clusters

Consequently, GAT has been hired by PSIC through a competitive bidding process to carry out a

Diagnostic Study of Industrial & Handicrafts Clusters in Sialkot district in the Punjab. During the

inception phase, we maintained active liaison with the PSIC team to develop a shared understanding of

the task requirements. We visited PSIC offices a number of times in order to hold meetings to clarify

certain observations and collect relevant information/data and documents and reports.

1.1.3 Literature Review and Global Experiences

Over the past three decades, development research has focused on the importance of industrial clusters

in the development process. In the United States (US), many states have designed and implemented

cluster based development strategies. In 1999 the National Association of State Development

Agencies and State Science and Technology Institute held a one-day workshop to analyze cluster based

development strategies in the US.

Enright (1996, 1998) and Feser (1998) noted that concept of cluster based survey is widely accepted by

the development planners because it has the benefit of contributing towards understanding regional

economy. There are also questions about the abilities of the policy makers to exploit the strengths of

the clusters to make them a driving force for the economy and turn them into successful industry.

Rabellotti (1995) explained that cluster is a group of firms located in the same geographical area, where

there are a significant number of firms specializing in the production of inputs and for manufacturing

the same type of goods. In an industrial cluster there are numbers of firms that are engaged in the

production process of a commodity. Textile sector is an example of this manufacturing model. The

research goes on to cite the Sinos Valley cluster in Brazil, Agra in India and Guadalajara and Leon in

Mexico for the production of footwear and further mentions the surgical goods cluster in Sialkot,

Pakistan.

The current era of trade liberalization has highlighted the dire need of joint and synchronized action

among the cluster members to address new commercial pressures as the Indian and Mexican clusters

have faced problems in the past decade. Furthermore, Pakistani and Indian clusters have faced issues

related to quality and environmental standards as a result of trade liberalization. Trade liberalization

often leads to increased global competition and loss of traditional markets. In the light of current

threats to the domestic industrial clusters there is a need for joint action to maximize market share.



In the surgical industrial cluster of Sialkot, Pakistan, there are around 220 producers with around 1,500

subcontractors who are producing goods valuing around $124 million for the US and Western

European markets since 2000-2001. This sector came under pressure in the first decade of 21st century

when issue of sterilization of the surgical instruments was raised. Subsequently, there was a major

shift in the US policy to import surgical instruments from Pakistan. They removed third party

purchasing and direct selling to the hospitals evolved.

After the collapse of the Soviet Union, the woollen knitwear cluster in Ludhiana, Tewari lost their

traditional market. However, exploring alternative markets in Europe led to a swift revival. The firms

in the woollen knitwear cluster upgraded their brands to a higher quality for their domestic as well as

foreign buyers. This experience highlighted the importance of the domestic market in the process of

innovation and learning opportunities for any cluster.

Rabellotti (1995, 1999) studied shoe-producing clusters in Mexico and Italy and the initial study

conducted in 1995 compared the Guadalajara and Leon shoe clusters from Mexico and Italy. The study

found that initially the relationship between suppliers and manufacturers was stronger in Italy as

compared to Mexico while shoe clusters in both Italy and Mexico, exhibited weak forward linkages,

i.e., marketing and commercialization. This study highlighted the importance of informal relationships

that was greater in the case of Mexican clusters. Rabellotti (1999) again studied the Guadalajara

cluster and studied the effects of trade liberalization on the inter-firm relationships within the industrial

cluster. The study found that through positive cooperation with their suppliers’ performance improved

for the majority of the firms.

In Sialkot, Pakistan, firms from the surgical instruments cluster prefer to take initiatives in those areas

where they have some prior experience of marketing and lower opportunity cost. Thompson (2005a)

highlighted these firm level characteristics that enhance its inter-cluster cooperation to the market.

Thompson (2005b) discussed relational contraction theory highlighted by Macaulay (1963), North

(1990) Gereffi (1999), and Kranton (1996) and found that informal relationships could substitute for

formal contracts through the judicial system.

In the light of above-mentioned definitions of clusters and case studies it is evident that industrial

clusters have become an important policy tool for development. In developing countries, industrial

clusters provide jobs to a large number of people. Case studies have highlighted the success of these

clusters to become leading exporters of a country. Studies also show that social network based

relationships in clusters hinders growth. Ilias (2001) and Banerjee &Munshi (2000) brought forward a

precautionary note to this discussion that cluster development policies should consider the benefits and

drawbacks of clustering.

1.2 METHODOLOGY

1.2.1 Methodology of the Study

Sampling Design and Approach

The firms in surgical instruments clusters exhibit different properties. The size of firms

differs from each other. There are small, medium and large size firms involved in

manufacturing and export. Their distance from the basic infrastructure might cause

differences in their joint dependence and rivalry. Having this issue at hand, a representative

sample of 57 firms was selected through systematic random sampling technique out of total



17361 small medium and large firms. Total 80 firms were contacted and we have received

response from 57 firms. The response rate was 71%.

In addition to these surveyed firms, 15 other members of the surgical instrument cluster were

also interviewed and their opinion was sought for the purpose of this study.

Development of Questionnaire

With the consent of stakeholders a detailed list of possible issue was developed and then

survey instrument was designed in the light of those issues.

An integral part of the study depends upon a structured survey instrument, which was to be

administered to the firms during the course of this study. Since, the questionnaire was to be

developed by the contractor, we developed the questionnaire in coordination with Ernest and

Young, PSIC and other stakeholders.

We adopted two-step approach to probe into the cluster issues. We designed two separate

survey instruments for firms and for Association of the cluster. Each item in the questionnaire

was designed according to the sensitivity and the nature of information asked. To do this,

questions were designed with structured, semi structured and unstructured manners to obtain

accurate information. The basic implementation of SWOT analysis and Porter's Five Forces

(discussed in next section) requires structured questions while owners of the firm or an expert

opinion will be asked through semi structured and unstructured questions in the questionnaire.

The main issues covered in the questionnaire and group discussions were diagnosis of social,

economic & value chain dynamics of cluster, existing level of factors of production, issues

and knowledge of Value chain diagnosis, existing resources in terms of Tools & Technology,

issues and provision of Raw Materials, Human resource, Institutional support, Marketing

facilities, Infrastructural facilities etc and current & future requirements and policy

interventions required.

Furthermore we have also designed questions on the Assessment of existing skill set of

various agents in the cluster and assess any scope for the up gradation to make it more

competitive in the world market. The information asked through Questionnaire also focused

on the issues related to financial constraints and legal aspects of the cluster.

Training of Enumerators

Two days training at Sialkot was imparted to the field and QA staff, for the field survey phase.

Test was conducted to evaluate the understanding of the staff, at the end of training session.

The project manager and the Team Leader trained the enumerators. The field teams were

trained in the basics of the Survey requirements, the data collection protocols, and simple

problem handling techniques.

After the training session, field test was also carried out in 20 firms of Musical Instrument

cluster to check possible issues with the questionnaire and corrected. The activity also helped

to test the understanding level of enumerators. The post pilot discussion session highlighted

the practical difficulties and the level of preparedness of the staff. It also addressed the

practicality issues with respect to the tools and instruments.

1.3 DATA ANALYSIS:

1.3.1 Data Entry, Data Cleaning and Processing

The process of data entry needs careful assessment of the filled questionnaires. There were three-step

validation processes for the check of validity of the filled questionnaire. The field supervisor

rechecked the collected data from survey in first stage. In the second step desk team re-evaluated

questionnaires and finally sent to data entry team. The Information System was developed to check

any further inconsistency in the data by applying various logical and other checks to maintain the

quality of data. Major part of the questionnaires are closed ended questions, so simple check boxes,

drop down menus were programmed in such a manner that it reduced a great chunk of errors at the

stage of data entry. Thus overall quality of data was maintained at this step.

Using the clean data, the analysis was carried out in the following steps:

Descriptive analysis:

Descriptive analysis enabled us to extract some vital statistics from the survey data to explain

the key issues of the cluster.

SWOT analysis:

SWOT analysis is an important tool to identify firm's strategic position in a business

environment2. SWOT is a combination of four factors Strengths, Weaknesses, Opportunities

and threats. It helps analysts to identify best possible strategies for the firm by using its

resources and capabilities according to prevailing business environment. It covers all factors

(positive and Negative) that can affect firm's success. In this study, we intend to find out

those factors that help firms in an industrial cluster to flourish and with the help of those

factors we forecast the changing trends that would be helpful for the policy makers in the

process of decision making.

1.4 PORTER'S FIVE FORCE MODEL:

Porter's five force model helps the entrepreneur to understand and deal with competition in the market

(Porter, M., 1979 and Porter, M. 2008). Porter argues that factors that affect competition in an industry

are almost common across industries. Porter identified five forces to study and deal with the

competition in an industry. These forces are

1.5 DESCRIPTION OF CLUSTER

1.5.1 History of the Cluster

Sialkot is renowned for its world-class surgical products. The production of surgical instruments from

Pakistan is mainly attributed to Sialkot as almost 99%3 of the production of surgical instruments takes

place in Sialkot. Manufacturing units are spread in and around Sialkot city. This industrial cluster

consists of large, medium and small firms engaged in the production of surgical instruments. Small-

scale producers generally produce for the medium or large size firms or sell their production to

exporters.

Surgical tools can be defined as the devices or tools that are used to perform cutting, holding, grasping,

retracting or suturing. The basic material used in the manufacturing process of surgical instruments is

steel. Along-with steel, radium, titanium, chromium and molybdenum are also used in the production

process of surgical instruments. With the introduction of new medical fields, surgical instruments have

become more complicated. The nature of surgical instruments has advanced to inclusion of

computerized components in them. This hi-tech era and the use of advanced technology have changed

the design and prerequisites of production of surgical equipment.

The annual export value of surgical instruments from Sialkot in 2013-14 was around USD 335

Million4. As this industrial cluster produces surgical instruments for international producers on their

demand with special type of designs so around 99% of its production is exported.

Sialkot is a city of skilled workers. They produce world-class products and receive business share from

the world. The history of surgical instruments spans over 100 years. The journey of this skill started

from the making of traditional knives and other iron traditional weapons to the current modern world

class complicated surgical instruments making industry. 1930s was the time when this city stepped

into international market. To strengthen the institutional setup of this cluster, in 1941, British

government established Metal Industries Development Centre (MIDC). It helped this industry to

upgrade from production of basic metal items to complicated high-class surgical instruments. At the

time of independence there were 17 registered surgical firms operating in Sialkot5.

With the passage of time and increasing worldwide recognitions, it has grown up to around 2500 small,

medium and large size registered enterprises. Surgical Instrument Manufacturing Association of

Pakistan (SIMAP) was formed in 1958 to promote and safeguard the interests of this cluster.

1960s was the period of incentives and government regulated support for the surgical instrument

industry as government provided subsidies and credit incentive schemes to this cluster during this

period. As a result of these incentives, this cluster has seen a technological boost along with increase

in quality and productivity in 1960s. During 1970s, this cluster faced a number of challenges. Labor

laws and related issues caused panic in the labor and industrialists simultaneously. This conflict

resulted in a major policy shift in the production process as firms started outsourcing their work to local

vendors and reduced the number of full time employed labor at firm.

1980's and 1990s was the period of high demand and regulations for surgical instrument industry.

Restrictions were imposed by US Food and Drug Administration (FDA) on Pakistani surgical goods

and made it mandatory to get certification like ISO, CE and GMP6 after improving its management and

manufacturing system. Surgical industry also made necessary arrangements and overcame these

hurdles and continues to show its presence at international markets.

Since then, the Sialkot Surgical goods industry is growing and contributing to the GDP of Pakistan

significantly. In Pakistan, main production area for these surgical instruments is Sialkot where almost

99% production of these surgical instruments is taking place. Currently, there are around 10,000

different types of instruments being produced and exported from Pakistan to different destinations in

the world. There are around 2500 companies operating in the vicinity of Sialkot in which the number

of large scale manufacturers is around 20. These companies have worldwide presence and have their

offices in the importing countries. Around 50 medium size units are also operating in Sialkot and some

of them also have their offices in the importing countries. Remaining number of firms constitute small

scale firms which includes outsourced part of production process, vendors segments etc.

The classification of the market on the basis of number of employees, investment in plant and

machinery etc is very difficult because clear information on these indicators is not available. The

reason for non-availability of this data is high degree of rivalness among cluster members. So the

studies on this cluster generally classify these firms on the basis of sales turnover as the sales volume

more likely indicates the firm size7.

The surgical instruments produced at Sialkot are mainly exported to different countries around the

world. Major importer of surgical instrument from Pakistan is USA that imported 24% of the total

exports of disposable surgical instruments from Pakistan in 2013.

1.5.2 Geographical location

Mainly, this cluster is located in the urban areas of Sialkot city. The major production units are in the

city center with the spread of 0-15 sq Km of range. The main area where the manufacturing is done is

mostly in and around Commissioner Road, Air Port Road, Gohad Pur, Kulluwal Road, Jinnah Park

Colony, Roras road, and Muslim street. A detailed analysis of geographical spread of the firms in

Sialkot is presented in Appendix “A”.

1.5.3 Main Products of the cluster

The majority of surgical companies operating in Sialkot are certified with ISO-9002 standards of

certification. These companies produce world-class surgical instruments but they are not entitled to

design equipment. There are more than 300 companies in Sialkot that have ISO-9002 certification

while around 250 companies have good manufacturing (GM) certification. These standards enable

these manufacturing units to trade worldwide. The production network of this cluster primarily follows

the international brands to produce certain item according to their needs. The first step of designing

takes place at the importers end and according to the nature of the designed product, the order is

placed. These companies have long list of designs of surgical instruments being produced at this

surgical industrial cluster.

Following is the range of major equipment being produced at Sialkot surgical cluster.



The production of these surgical instruments is very complex procedure. Not all the firms produce all

the instruments. It depends on the nature of expertise of the firm that how many instruments are being

produced. The ISO-9002 certification enables this cluster to export its product to international buyer

but restrict them from designing their own models for international market. This aspect bounds the

manufacturing firms to produce for international buyers and only follow the instructions of main

importer. Not all the firms produce all the surgical items. There is need to change this track and adopt

aggressive approach in international market by introducing its own brands and models to trade directly

with end user.

There is variety of surgical instruments being demanded and produced in the field of medical. Main

types of surgical instruments can be outlined as follows8:

1.5.4 Core & other cluster Actors analysis

This industrial cluster is a mix of traditional and dynamic production firms. The actors in the

production process can be traced in four different categories. From production point of view large

scale manufacturing production units, it includes around 10% of the cluster firms and produces the

major share of the total production (50%) and export activity with relative less labor employed (15%).

The medium scale firms constitute 40% of the cluster; have 40% employment share with significant

production activity (35%). Rest of the production and employment is at small scale and vending

sectors of this cluster.

The surgical instrument cluster can be classified on the basis of revenues of the firms. It would result

in four type of firms in the cluster, high sales firms, medium sales firms and low sales firms and

vendors present in the market. These firms can be identified as large size, medium size and small size

firms respectively with vendor segment in the market. This classification provides us with 20 units as

large firms with annual turnover around 60-100 million and with average employment of 250-350

employees in each firm. While medium size segment in the market, there are around 50 firms with Rs.

10-60 million revenues annually giving firm wise average employment opportunities to 100-250

laborers. Third part of this cluster consists of 150 small-scale firms with annual sales between Rs. 1-10

million per annum giving employment opportunities to 30-50 persons.

The fourth part of this cluster consists of vendors. There are around 2000 registered vendors in the

cluster. These vendor firms generally specialize in one or two processes of instrument making. The

revenue of such firms ranges 1-1.5 million annually.

As most of the production process is outsourced to large and medium firms, the role of traders is

widespread in this cluster. Traders support the process of manufacturing in this cluster. These traders

are not part of production process of the cluster but they are active members in the international market

of surgical instruments. In Sialkot there are more than 900 traders involved in the trade of surgical

instruments along with trade of other products being produced at Sialkot9.

Type of Firms/Organizations

There are multiple types of organizations

working in surgical instrument cluster in

Sialkot. Survey indicates that most of the

firms operating in this industry are partnership

owned (52.7%) followed by public company

(Listed)(29.1%).

The organizational structure of the firms regarding international business indicates that

most of the firms at Surgical instrument clusterSialkot

are not registered with ISO certification. Only 47.3%

of of surveyed firms were registered with ISO

certification while around 50.8% firms were not

registered. Rest of the firms did not provided

information regarding this question.

The main trend in this sector to get business or

establish a set up/firm to do business, especially at

small and medium size of the business is shown in

the dig. Majority of the business owners are self-

established and they are in this business since long.

The trend of new entrants who purchased a running

business is negligible. There is also small numbers

of businesses passed on by family members. This

trend of business establishment indicates that this

cluster is operating in a typical enviornment in which

majority of the self established business owners are on the derivig seat to control trends of the business in this

cluster.

Most of the owners of the business are not

technically trained. The survey results indicate

that around 59% owners of the business owners

did not possess any technical training. Out of

41% of the trained owners of the firms, majority

of the owners possess traditional training. To

make these firms upto mark and compatible in

international market, there is dire need to

provide these owners with the training and

awareness regarding trends in international

market and latest technology being used for the

products being produced at international

markets.

1.5.5 Raw Materials and Intermediate

Goods

The quality of the production being produced

at Sialkot surgical cluster is mainly an

outsourced production activity. International

brands have outsourced their production to

standard material by firms in Sialkot. Large-scale firms engaged in the production activities at surgical

instruments, use imported stainless steel and other raw materials to produce surgical instruments.

Some small and medium scale firms use mix of local and imported raw material. Generally firms that

manufacture disposable surgical instruments use mix of imported and local raw material. But large

firms that are engaged with the international brands use only imported raw material10

. According to

industry sources there are around 50 domestic suppliers in Sialkot and allied cities for the supply of

domestic and imported raw material to production firms.

A notable fact is that, in Pakistan no industrial setup is producing stainless steel as raw material to

supply surgical instrument cluster despite the fact that this could be a huge market. Pakistan Steel has

tried to start supply of steel to this cluster in 1990's but due to unknown reason this project failed. Still

this is a huge market for quality stainless steel raw material.

1.5.6 National, International Scenario of Cluster

The main production clusters of surgical instruments in the world started to establish in early 20th

century. These were Solingen and Tuttlingen in Germany, Nogent-sur-Martin in France and Sheffield

in England. Presently, these clusters do not exist in the manufacturing of surgical instruments field

except Tuttlingen in Germany. In recent past Pakistan has emerged as a leading manufacturer of

conventional instruments for surgical and medical field. Presently, there are a number of countries

producing surgical instruments but Pakistan is producing high quality at remarkably low prices in the

world market. Only China is close competitor to Pakistan with lower price than Pakistan11

.

The journey of surgical instrument cluster, from repair of ordinary scalpels and scissors to the

manufacturing of complicated surgical instruments, is remarkable. The companies in this industry

produce around 40,000 different instruments and equipment over different eras12

. Currently there are

around 10,000 different surgical goods used in different medical procedures being produced.

Around the world, there is no significant competition for the surgical instrument being produced at

Sialkot industrial cluster as no other country is producing such quality of surgical instruments in this

price range.

Multinational companies from all over the world are making joint ventures with Pakistani firms to

produce complicated surgical instruments. This shows the confidence of international companies in the

local skill. Despite all the good repute and quality production there is no local brand working

internationally with its own accepted brand name except in Middle East and UK where they got limited

success. There is dire need to channelize the efforts to brand internationally the products of this

cluster.

Despite the lack of indigenous Pakistan/Sialkot brand name, Pakistan is still faring well globally.

Following table shows the export trend of surgical equipment13

to the top ten trade destinations in the

world. The table shows that US is major importer of surgical instrument from Pakistan followed by

Germany and UK. Out of the total share of Pakistan's exports, almost 30 percent is exported to US as

compared to 13.2 percent for Germany and 10.5 percent for UK. US is the top importer of surgical

instruments from the world. During last five years, the growth rate of Pakistan's exports to US was 6

percent per annum followed by 2 percent to Germany and 6 percent to United Kingdom.

It is worth mentioning that during the last two years, the value of the export growth to USA, Mexico

and Czech Republic is positive while its value is negative for rest of the countries that shows

decreasing revenues from rest of the markets in the above table. There is need to focus on the negative

growth aspect of trade growth to ensure steady growth in business with existing international markets

for surgical instruments of Pakistan.

On the issue of trade barriers, there are no significant trade restrictions imposed by these top 10

importers except Mexico and Brazil who have imposed tariff rate of 4.7% and 9.3% respectively. It

shows a relatively supporting environment for Pakistani surgical instrument in these top 10 importing

countries from Pakistan.

Pakistan enjoys diversified market conditions for its surgical instrument industry as a whole. But as far

as volume of trade is concerned, it is focused on limited number of countries. The following picture

explains the diversification of trade by destination and volume of trade.

The above chart shows that all the importers of surgical instruments from Pakistan are also importing

from the world. They all diversify their choice in the world market. In the above chart it is evident that

during the period of last five years, four countries' (Austria, Singapore, UK and India) imports of

surgical instrument from Pakistan are decreasing as compared to their imports of surgical instrument

from the world. Over the same time period of time import share of Germany and USA increased as

compared to their imports from the world. This market situation demands an immediate probe into the

situation that why some of the top importers have reduced their imports from Pakistan despite the fact



that Pakistan has an advantage over the other countries in low price and high skills. If Pakistan is

losing ground because of price competitiveness, it implies that surgical instrument cluster requires

government institutional support that might be in terms of reduction of tariff on import of raw material

or competitive rebate on exports so that they can remain in competition with international firms. On

the other hand, if this cluster is losing its trade share because of quality standards then it has more

serious implications for the market. In that case there would be need of immediate policy intervention

to support this industry with latest technology and investment in R&D to compete world market.

Bold lines on the chart indicate strong share in exports of surgical instruments from Pakistan and

Partner country. The diagram indicates that surgical instrument cluster is not diversifying its trade

destinations as compared to its potential. The skill set possessed by labor at Sialkot surgical industrial

cluster is unique and they can compete all over the world but they are not diversifying despite the fact

that this cluster is underutilizing its production capacity. The reason could be the weak grounds of the

production process and certification of its products. By weak grounds we mean that they are working

for international buyer and not designing their own products. These firms are not entitled to provide

after sale service and designing rights14

. Due to this reason, the firms in this industrial cluster compete

with each other domestically to capture international orders at low price. This could be the reason in

case of Sialkot surgical industrial cluster that they are competing with each other at manufacturing

levels and not focusing on other markets to diversify their product and market.

The item wise export performance of Sialkot surgical cluster to its major importing country USA is

given in the following table. The major exports to US are comprised of Electro-medical apparatus

(electro-cardiographs, infra-red ray app), which covers 30 percent of Pakistan’s exports of surgical

instruments and around 19 percent of world imports. Over the period of last five years highest growth

is seen in the value of automatic regulating or controlling instruments and apparatus (58%) followed by

Instruments, apparatus & models, designed for demonstrational purposes (52%) in US.

Negative growth is also seen in orthopedic appliance (crutch/surgical belts & trusse) (-7%),

Hydrometers & similar floating instruments, thermometers (-1%), etc

There is need to focus trade strategies on the fast growing demand for surgical instruments in US as

this is one of the world's major importing countries of disposable surgical instruments.

Different policies regarding different markets are needed to capitalize in short period of time in the

world market. Country wise targeted intervention is required keeping in view the country wise

import trend and analyze the gap in their imports to capture maximum share out of that market.

General fit-to-all policy regarding export promotion might not help the export performance of this

cluster. Commodity wise micro level policy intervention is required to boost weaker areas of

exports in this cluster.

Issues in the Export of Surgical Instruments:

Despite the billion dollar exports to the world, Pakistani exports make only small share in the world

trade of surgical instruments. Due to high level of diversification in surgical instruments, Pakistan has

developed some special skill sets in four HS codes category (9018, 9004, 9001, and 9023) of surgical

instruments mentioned in the first column of above-mentioned table. Pakistan is currently competing

markets of developed countries like France, USA, and Germany etc. The price of exported surgical

instrument at Sialkot ranges between $1.5-2.515

as compared to US$ 0.35 in China. Despite this fact,

the price comparison of disposable surgical instruments of Pakistan shows that the prices of Pakistan-

made disposable surgical instruments are much lower than the prices of Germany and France. The

only problem with this cluster is the lack of innovation and diversification in the production process

and products. Furthermore this cluster needs to shift from low value instruments to high-tech

instruments to extend its share in the international market.

The international scenario for the trade of surgical instruments flows mostly from developing world to

developed world. Cheap labor and low cost of raw material are main attractions for the buyers and

manufacturers in the developing countries. Many commodities are produced at developing countries

and sold at developed world after the stamp of international brand on it. This is like a sale of a skill at

low cost due to poor market information and opportunities. Generally, innovations take place at

developed world and they sell those newly innovated commodities to third world at very high price.

Trade flows from less developed to developed countries at low price and reversal of this takes place at

very high price.

In case of Pakistan, many issues were raised against surgical instrument industry. The major blow was

the trade restrictions imposed on the surgical instrument cluster on the basis of child labor. ILO and

Surgical instrument manufacturer Association of Pakistan handled this situation by introducing a

comprehensive program to eradicate child labor from this industry. Currently as a result of this

program there is no child labor in the surgical instrument manufacturing units in Sialkot. Initially when

issue of child labor was raised by US agencies; SCCI and UNIDO made joint group to monitor this

problem on regular basis. This working group is still operational at Sialkot chamber of Commerce and

Industry effectively and has managed to remove Child Labor from Industry in Sialkot.

1.5.7 Socio economic dynamics of cluster

The production of Sialkot surgical cluster has many significant effects on the local economy of Sialkot.

The per capita income of Sialkot district is highest in Pakistan. It has manifold effects on the socio

economic horizons of the area. The production activity of this cluster has a spread of at least 15 square

miles and providing employment to the people through small medium and large size production units.

The outsourcing strategy adopted by cluster actors in 1970's had broadened the network of

opportunities for this area and home industry flourished that ultimately brought people to work. This

cluster is providing employment opportunities to almost 150,000 people with their subordinating

network of around 200,000-250,000 people. This has translated into enhanced living standards for the

people engaged in this cluster belonging to this region.

The industrialists from Sialkot have developed their own airport and a separate dry port. The SCCI has

invested in local infrastructure up gradation and development of basic infrastructure.

There is need to enhance social indicators of the city as literacy rate of Sialkot (76%)16

is less than

Lahore. Public health facilities cover around 65% of the population and unemployment rate of Sialkot

is 8 percent. There is need to develop educational setup at this city in such a manners that it supports

vocational trainings so that literate labor with basic skill can be provided to the manufacturing units in

Sialkot.

The training need of the industry is fulfilled by SIMTL and SMDC but due to lack of initiative at

aggregate level the main aim of these institutions is lacking behind its targets. Recently, TEVTA has

also developed setup at Sialkot for the provision of training to the unskilled labor in the fields of die

and mould making, Matching, heat treatment, plasma coating quality control, milling and turner etc.

The main problem is to create conducive environment for the new entrants to the market. There should

be scholarships for the unskilled, semiskilled and traditionally skilled youth to get fresh knowledge of

their trade. These trainings should also focus on the training of foremen, managers, engineers,

technical workers and quality enhancement staff.

1.6 ANALYSIS OF BUSINESS OPERATIONS

1.6.1 Cluster Production Operations

The production process of a surgical instrument is a detailed and complex process in which mix of

human and mechanical effort is involved. The process of making of a scissor involves almost 150

steps to reach the final stage. As this production cluster is ISO-9002 certified so they can neither

design nor provide after sale service but they can produce and sell in the world market.

The market of surgical goods making in Sialkot is very dynamic. As producers from all around the

globe approach this market for the making of their proposed designs, so local manufacturers have to be

up to date with the changing trends of the world.

The manufacturers in this cluster generally hire labor on permanent basis. There are small medium and

large-scale manufacturers operating in surgical goods cluster. Due to seasonal impact there is break of

30-40 days in the production process annually.

The raw material used in the process of making of surgical instruments requires quality material. As

most of the producers use mix of imported and domestic raw material to produce quality products, so

the local market and importers of the raw material need to be more efficient. The raw material is

generally available for the local producers but they were found complaining about the quality of both

imported and local raw materials.

1.6.2 Markets and Marketing strategies

The nature of business at Sialkot industrial cluster is demand driven. The production process at

surgical instrument cluster is generally a response to the supply orders from the international buyers.

The supply of these surgical instruments in the local market is limited. The main target of the domestic

producer is the export market. Major importers from USA, UK, EU countries, and Germany order

their desired designs of the surgical instruments at their own will.

Despite all the recognition and appraise from all around the world, surgical instrument cluster is still

working as a supporting organization for international brands. There is no Pakistani international brand

working anywhere in the world with its own name. No collective initiatives been taken by the cluster

organizations to do so. By registering their own logo, producers from this cluster will be able to

participate in international trade exhibitions and create their own identity. It will further upgrade status

of the industry.

The technology used in this cluster is not at par with the international technology. This cluster is

making semi final products but for the finishing touch they are lacking latest machines17. There are

several reasons for this phenomenon. Firstly, after the labor strike and unrest in 1970's most of the

production processes were outsourced to small industrial units. This change in the production strategy

developed specializations at small scale. These industrial units mainly do not aspire to adopt latest

innovations in the production process and hence remain producing using old technology. Secondly, the

skilled workers in this cluster are not very much educated and they find it difficult to switch to the

latest technologies.

The marketing strategy adopted by surgical instrument cluster is almost similar in nature. Some large

scale manufacturers have their offices in importing countries to get business and remain in touch with

the international importers. This practice is limited within this cluster. There is need to adopt this

strategy more by cluster members because it will not only enhance their knowledge about the market

but also they will also remain in touch with the current changes occurring in the medical instrument

making.

Personal contacts and visits is another way to get

business and market their product. In Surgical

instrument cluster this is most widely used

practice. Most of the visits of Pakistani producers

are to Germany because of its expertise in

Operating Room (OR) type of instruments. These

frequent visits to other countries enable them to

introduce their commodities and also help to

build confidence in business. This can bring long

term relationship between importers and

manufacturers.

Trade fairs and exhibitions are another important

source of marketing for surgical equipment

cluster. Large and medium scale producers

generally attend these exhibitions. The

affordability problem also arises here as small-

scale segment of the market cannot invest large

amount in the international exhibitions so they

remain at the market share of outsourced orders

from the local market. The actors in the market

also use electronic media and telephonic sources

to contact international buyers and get orders

from international market.

Sales channel is an important part of a business.

As surgical cluster is an export-oriented cluster so

its reliance on international sources is obvious.

The survey results shows that around 50% of the

firms are exporting to international buyers

through exporters, while 31% of the firms

directly sales in the market. The number of firms

using wholesalers and distributers is limited in surgical

instrument cluster.

To attract buyers from around the world most of the firms

in surgical cluster use electronic media/exhibitions (56%).

Broachers and social media are other most used channels

for the marketing of the products among the surveyed

firms. These results indicate that most of the business in

1.6.3 Value chain analysis of 5.5 Inch Forceps

The process of making of standard forceps passes many technical steps. Following are the steps

and their cost involve in each step:

After the production of forceps, the international profit margins as compared to local ex-factory

margins are very much different. At local levels, profit margin is around estimated value of 35% while

at international levels this margin is around 200-300% more than the local prices. If a production cost

of a standard forceps is Rs. 120 then after profit margin, the price would be around Rs.164. However,

the importer margin for this commodity might be around 50%-60% and the retail price of one forceps

reach is around Rs. 600-800.

This value chain analysis of a standard forceps clearly indicates the significant margin of profit goes to

international importer and the local producer is doing business at very low margins as compared to

international prices.

1.6.4 Technology used and Output

The technology employed in the production process of surgical instruments comprises of mixture of

local and imported equipment. Most of the companies use local made reverse engineered equipment to

upgrade their machinery18.

The workers in this cluster are mostly satisfied with the technology being

used in the production process. There is a unique mindset in this cluster that majority of producers do

not feel any significant pressure from producers all around the world and think that they have no

competitors. The point that they have in mind that the designs and quality that they are producing at

this lowest price none other can produce. So this market is competing in the world market not only

because of the unique product but also because of its price competitiveness.

The output from surgical industrial cluster is mainly produced with the help of machinery that has been

discarded by western companies and importers of equipment import these machines at low cost and sell

them to local producers. They replace their existing equipment with this second hand machines. This

cannot bring any change in the quality of the product for international competition. There is dire need

to assist this sector of the economy, financially or government should intervene by establishing

common facility centers (CFC) at Sialkot to overcome this gap in technology.

The mechanization process in surgical industry started in 1960's. Local producers adapted new

technology quickly and with the passage of time, production process shifted from handmade to

machine-made processes. The traditional skilled labor in this cluster was master of reverse engineering

and they started to produce these equipment locally and gradually this industry shifted to mechanical

processes.

In 1980s surgical industry shifted from manual forging to Drop Forging hammer. Later, this forging

hammer was designed and locally produced and provided to the local industry to improve their

production process. The equipment used in the production process at surgical instrument cluster is

undocumented as there are frequent changes in the instrument making tools, designs and requirements

of importers.

The quality standards are always the concerns of the importers. They impose strict conditions

regarding quality standards. There are many restrictions on the importers as well to ensure

improvements in management, technology and labor. To meet these issues surgical instruments cluster

adopted general standard quality checks to fulfill the requirements by the importer.

Food and Drug Administration of United States (FDA) first raised the issue of quality standards about

the usage of poor quality steel in the production of surgical instruments imported from Pakistan. The

steel used in the production of surgical instruments was not up to the standards of FDA. Initially FDA

gave warning to the manufacturers and in 1994 and took serious steps by imposing complete ban on the

imports from Pakistan. A more recent restriction included that for every product exported to US, the

product must adhere to ‘GOOD Manufacturing Practice’ (GMP) certification. This method ensures

quality system and good implementation of designing, labeling, installing, and servicing of surgical and

medical devices produced for human use.

After GMP the importer firms from Europe also implemented restrictions on Pakistani exporters to

have ISO certification before exporting to European Union. To help firms in attaining ISO certification

government of Pakistan provided every firm with subsidy of Rs. 200,00019

.

Chemical composition certificate for steel is another issue faced by the exporters to US. Certificate of

Europe (CE) is another quality assurance system for medical devices. The first deadline to meet the

standards of CE was 1998. The exports from Pakistan to EU were prohibited without the CE

certification20

.

The provision of latest technology especially in

the production of hi-tech medical instruments is

necessary. There is big gap between the

technological standards being followed at

international levels and at Sialkot surgical

industrial cluster. This cluster uses labor-

intensive traditional methods to produce surgical

instruments. Old or outdated technology causes

an increase in the cost of production and produce

relatively poor quality products. In order to make

this industrial cluster more competitive, there is

need to upgrade technology. In 2015, an

agreement was signed between SIMAP and

PCSIR to build common facility center

comprising of material and quality testing lab and

surgical instrument associations but very limited numbers of workers opt for training due to the reason

that they feel comfortable with their traditional equipment and techniques. At large scale

manufacturing units, these technical trainings are making some differences in designing, quality

enhancement, and efficiency of labor and output of the workers.

In 1962, through an ordinance, an Apprenticeship training programs was implemented to provide skill

to the unskilled labor force. Training cost was mutually born by firm and the government with different

ratios. As this was three-year training program firm share for the training was 50%, 60% and 70%21

of

the labor wages during year 1, 2 and 3 respectively.

This intervention was planned as different syllabus was also designed for the trainings but the outcome

was only 10-12 skilled workers annually in surgical instrument section. This was disturbing as demand

for the industry was much more than produced from his training program. This gap still prevails and

there is shortage of skilled labor in surgical instrument industry.

This cluster is lacking skilled labor especially in the field of grinding and polishing. Further areas in

which training can be initiated are given as follows:

attitude of the labor towards training is very weak. Lack of skilled labor is causing productivity slow

The most of the manufacturing units in this cluster are working at small scale and hence have little

investments and their financial need for further investment is low. But the medium and large scale

producers and suppliers in this cluster generally need finance to meet their international orders. The

branches of almost all the commercial banks are present in Sialkot. So the access to the financial

resource is relatively easy for the companies in the cluster. There are number of firms that avoid

interest based banking due to religious reasons.

1.6.7 Legal Issues Faced by Cluster

There are some serious concerns among the cluster members regarding slow process of sales tax refund

policy. This tax on exports was imposed in 2014. All the members view that this policy is blocking

their liquidity and hence adversely affecting the growth in the cluster. Market sources reveal that they

are waiting for the tax refund for the last three years but still they are not certain about its repayment.

This issue is more problematic for small firms that they already have limited amount of capital.

Government should take immediate steps to resolve these issues to support business especially at small

and medium scale of production. In budget for FY 2016-17, government has abolished this tax.

Due to the nature of employment in this cluster (contractual and daily basis workers) there is no social

security framework for these workers. There is need to design some rules to safeguard these workers

with social security laws. The large firms generally outsource their production process at medium and

small scale firms and they have minimum number of permanent employees. Due to this production

structure, most of the labor in surgical instrument cluster is vulnerable to any accident or other mishap.

There is need to provide this skilled labor a cover to secure them financially, physically and socially in

case of any accident.

Standardization is at third place as mentioned by the firms. The most important area R&D is given as

least attention by the firms.

Without the support from Government institution it is not possible for the exporters to expand their

business effectively and create new markets quickly. Unfortunately there is lack of coordination

among government and semi government institutions for the provision of business services in Sialkot

region to help and guide investors to expand exports.

Sialkot chamber of commerce and industry is an important source of guidance and business for the

local producers and exporters.

Institutional Support to the Surgical Instruments Industry

Following are the institutions that are supporting surgical instrument business in Sialkot:

a) Metal Industries Development Center (MIDC) British government in 1943 in Sialkot developed this center. The main aim of this center was

to support local industry and to fulfill local needs of surgical instruments.

From 1947-48, this center has been providing technical support to the local industry. This

b) Sialkot Dry Port Trust (SDPT)

Before 1980 most of the businesses from Sialkot export their shipment from Karachi and

Lahore. This was time taking process and caused delays in the supply of orders. SDTP was

established in 1984 to resolve these issues of the local exporters.

c) Surgical Instruments Manufacturer Association (SIMAP)

In 1958, to safeguard interests of the surgical instruments industry SIMAP was established. At

that time initial membership of the Association was 478. This association is still working

actively to manage issues of the cluster members.



During the survey, firms were also asked to provide their response against each facility provided at this

cluster by govt. Institutions or by Association. The above dig shows their response against each of

these categories. As shown by the above chart, general dissatisfaction prevails for all the categories

mentioned above. Most dissatisfaction recorded against the legal support provided to the firms while

research and development support was considered comparatively satisfactory as compared with other

1.9.1 Suppliers Power

Porter’s analysis for the surgical goods industrial cluster indicates that overall the supplier power in this

cluster is moderate. It shows that suppliers in this cluster have less ability to influence on the price of

the producer. As this is a large exporting sector, it has good number of suppliers that weakens the

power of the supplier to dictate its terms.

1.9.2 Buying Power

The buying power of the customer is between medium and strong range as they have influence on the

market as buyer and also as importer though they cannot effectively influence the prices. Each firm in

this cluster is dealing with limited number of clients that makes them each other's need. In this regard,

they focus on the expertise that their supplier possess and continue to do business on the existing terms.

Any fluctuation from buyer regarding basic terms of business can cause reduction in the business due

exports goes to USA. In this situation this market is vulnerable to any future possible

restriction by USA as it had been the case in the past. There is need to diversify the target

markets. The European region is another big market that can be focused for the exports of

surgical instruments. The reason for not being diversified may be due to the fact that almost

all the exporting firms in surgical cluster are producing for international brands/buyers, not for

international markets, so they can not deviate much from their requirements.

an agreement was signed between SIMAP and PCSIR to build common facility center

The target markets for surgical instruments is very much scattered in numbers but focused in

terms of exports. More than 50% of exports go to the international brands from top five

importer countries from Pakistan. But they still import very limited number from Pakistan as

compared to their domestic need. On the other hand, production capacity of the industry is

underutilized. It shows that there is a gap available at international market but producers are

not catching up with the situation. For the sake of expansion of business international

seminars and exhibitions and corner meeting can be initiated under the umbrella of

Government embassies in different countries especially in Europe, Middle East, Russia and

Tax procedures

The businessmen in this cluster were of the view that tax is not a problem but government

should simplify the process of taxation. There are many complications in the tax forms and

also in the types of taxes (commercial import duty, sales tax, income tax etc.). The

stakeholders were of the view that government should develop mechanism to combine all

taxes and charge them under one simple heading for the business men so that they can easily

understand and submit tax.

Situation of business associations/ Chamber of Commerce and Industry.

Sialkot chamber of commerce and industry and SIMAP regulating the issues of the surgical

instrument cluster since long. There is need to enhance the role of SIMAP to increase the

international business relations of small and medium scale business firms. As business at this

cluster is mainly outsourced by international companies, the association should focus on the

groups producing similar products in the cluster and support them with legal, technical and

R&D support to enhance production and quality at small level of groups of producers.

1.11 Future vision

The performance of surgical instrument industry has seen many highs and lows in the past. Sometimes

issues of internal unrest and laws have adversely affected the industry and sometimes the international

regulations have put this industry under pressure.



In the current world scenario this industry has bright future. The following are the key issues as well as

opportunities for the surgical instrument cluster in Sialkot:

There is need of some serious efforts to synchronize the production efforts within the cluster.

As the business in this cluster is mainly on the self-efforts and individual approaches, so even

small firms are also exporting to middle man at low prices to get market share. This leads to

decrease in the quality of production. Any decline in the quality standards would lead to loss

of market as a whole. So there is dire need to set minimum selling price for the export

commodities so that competition on quality product can be assured. The role of the

government is very important in this step. With the help of representatives of small, medium

and large scale producers, government can identify common regions and products being

produced at surgical cluster Sialkot and then develop some basic standards for the production

of these commodities with lower bar for the price level. Government should announce

incentives for small, medium and large firms who are willing to work in a proposed group.

These incentives can be tax free imports of raw material for specific period of time, electricity

at fixed or subsidized rate etc. This process will identify different groups in the industry with

There should be common pool for the R&D expenditure to hold joint research in the specific

can be done making a joint working group comprising of representatives of small, medium

and large size firms and focus on the important gaps in R&D and recommend interventions

with the help of government institutions.

The electricity costs is another big threat to the industry as use of generator in the production

process doubles the cost of input as compared to production from electricity.

The shortage of skilled worker in the industry is another area of concern. There is need to

focus on this issue by focusing and encouraging labor to get training from the available

resources. As the up-gradation in the manufacturing technology takes place there emerges the

need of a skilled labor also but it is hard to find efficient labor with required skill set. So this

area also needs attention of the concerned authorities (SIMAP, TEVTA, PSIC etc) to estimate

future gap in the demand and supply of skilled labor and design their future plan accordingly.

Also there is need to identify key future skill set required for the production of hi-tech

equipment and arrange resources for future endeavors.



There is need to utilize market up to its potential. This cluster is utilizing its 60% production

capacity. The reasons behind this underutilization of production capacity are shortage of

energy and lack of orders from international market. There is need to explore new markets for

the surgical instruments. For this purpose government can activate its offices abroad

(Embassies, commercial attaches etc) to identify potential markets for the quality products of

this cluster.

This cluster is producing for the world market for almost 100 years but still independent

identity of the product being produced at this industrial cluster is missing. There is need to

develop new brands to in the existing markets to get better share from the world market.

The market is missing initiative to compete in the world market. There is need to make joint

efforts (government and Local producers) to establish world class research center for the sake

of development of new brands in the field of surgical instruments. The making of a brand is

like making of an identity so without participation of senior and professional minds in

the process this is not possible to develop a world-class design. To do this Pakistani

medical experts from all over the world can be invited to help and support in the process of

designing of latest medical equipments. As mentioned above in the BOX above at pp

29, that since 2012 SIMAP is pursuing brand development efforts but due to shortage of funds

they are still far behind in making any significant progress in this field.

Diversification of the product is necessary for the better and secure business. If diversification

stops, the threat in the market increases. This diversification can be at small scale for example

an increase in the quality of raw material etc used in the production process. World market is

very dynamic and in the world, there are continuous developments in the process of making

and trade patterns. Hence, diversification in product (introduction of new equipments,

enhanced quality, different designs etc.) and marketing, both are required to remain in the

world trade business. The major issue with this cluster is the lack of innovation and

diversification in the production process and products. Furthermore this cluster needs to shift

from low value instruments to high-tech instruments to extend its share in the international

market.

Quality consultancy services to local businessmen are necessary to get better market for their

business. The education level of most of the small and medium size firms’ owners is mostly

semi-literate or illiterate. In such state of affairs, they can only get business with their

personal interaction at local evels and their access to international levels is blocked. In such

situation importance of business consultants is worthwhile. There is need to encourage and

develop consultancy services to the small and medium level business owners to expand their

business internationally. In this situation, the role of PSIC and SMEDA has become vital in

the recognition of issues of small-scale firms and provide them guidance according to market

situation. Seminars and awareness sessions can be organized on regular basis to convey them

with latest information regarding market trends and future needs of the market.

The firms in the industrial cluster should adapt the latest marketing strategies. As this is an

era of information technology and use of it can bring so many fruits for the business very

quickly. The issue of literacy is again causing hurdles for the systematic promotions of the

commodities being produced at surgical instrument cluster of Sialkot. There is need of

professional setups for the professional marketing of these instruments to get business directly

from international market. There is need to hire professional marketing agencies by SIMAP

for the promotion of surgical instruments as an industry.

There should be comprehensive plan of action regarding technological up-gradation of this

industrial cluster by engaging financial sector into framework. As need of the technical

equipment is different at different levels of industry (Small, medium and Large scale), so

multistage policies should be introduced for small, medium and large-scale firms in the

machinery that has been discarded by western companies and importers of equipment import

these machines at low cost and sell them to local producers. They replace their existing

equipment with this second hand machines. This cannot bring any change in the quality of the

product for international competition. This issue can be resolved by setting up common

facility centre with latest equipment and material testing lab.

Comparative study of similar cluster around the world and their marketing strategies with

special focus on the differences in their level of technology being used can also be launched to

study regional dynamics of the cluster.
