Needs Assessment-Creation of AVCC

•Late 2005, Antelope Valley Partners for Health (AVPH) completed a Community Needs Assessment funded by Kaiser Permanente. It concluded that there was a critical need for expansion of primary care, prevention and education services for the uninsured and underinsured.

•Antelope Valley Community Clinic Advisory Board was created. Committee members included major providers, organizations & Community members: AV Hospital, Lancaster Community Hospital, High Desert Medical Group, Kaiser Permanente, County of Los Angeles and the Cities of Lancaster and Palmdale

•In March 2007 “Antelope Valley Community Clinic” a non-profit 501 (3) (3) was formed. The Board is 51% users of service.

•In August 2007 we received a start-up grant from Kaiser Permanent for $250,000

Main Community Health Center• We have signed a lease on a 15,000 sf building in

the Redevelopment area of Lancaster in a very high area of need.

• We have an option on an additional 25,000 sf that will serve as a “Community Health and Wellness Center”. We will have additional clinic and meeting/conference space & sub-let to other health and social service organization with similar mission.

• Rent from sub-lets will significantly reduce or eliminate our own rent and generate excess income that can then be used to provide additional care for the uninsured

Clinic Services• Primary care services by physicians and mid-level Nurse

Practitioners.

• Family, internal and pediatric medicine, obstetrics and gynecology. OB Clinic from AV Hospital is sub-letting

• Diagnostic laboratory and imaging services on-site and through AV Hospital and Lancaster Community Hospital in-kind support.

• Preventative health services including prenatal and perinatal services. Black Infant Health, Healthy Births, etc.

• Screening for breast and cervical cancer, well-child services, immunizations, disease management for diabetes, asthma and hypertension.

• Adult and Children's’ Dental program

Phase I- 15,000 sf Primary Clinic• This facility will serve as the primary space for

“clinical” or “medical” services. Antelope Valley Community Clinic, AV Hospital OB Clinic, Dental Services, Dispensary and Lab Draw. Parking for 2 mobile clinics in back.

• Estimated Construction Cost: $748,500

• Estimated Start-up/Operations: $365,000

• Estimated Opening: March-June 2009

Phase II-25,000 sf• This building will provide additional

clinical space, clinical administrative space, conference and training center for local area non-profits in the community. We will also sub-let to other non-profits to create a non-profit focal point in the community.
